2.Analysis of the increase of bone marrow prolymphocytes after complete remission in children with acute lymphoblastic leukemia

Objective To investigate the clinical significance of bone marrow prolymphocytes increasing range from 5 % to 15 % after complete remission (CR) in children with acute lymphoblastic leukemia provide a prognosis index.Methods The cases of the children with acute lymphoblastic leukemia were analyzed retrospectively.They were divided into three groups,namely A group (N ≤ 5 %),B group (5 % < N ≤ 10 %),C group (10 % < N ≤ 15 %) according to the bone marrow lymphoblast percentage,and their relapse rates were analysed.Results After the CR,the appearance of bone marrow prolymphocytes slightly increased in children with acute lymphoblastic leukemia accounted for 40.54 % (30/74).When the bone marrow prolymphocytes increased to 5 %< N≤ 10 %,the difference of relapse rates [21.05 % (4/19)] had no statistically significant compared with the negative control group [15.91% (7/44)] (P =0.895),when they increased to 10 % < N ≤ 15 %,the difference of relapse rates [54.54 % (6/11)] had statistically significant compared with the negative control group (P =0.014).The difference of incidence rates of this slightly increasing between children' s B-ALL and T-ALL had no statistically significant (P =0.078).Conclusions The bone marrow prolymphocytes increase slightly (5 % < N ≤ 10 %) after CR in children with acute lymphoblastic leukemia might be the normal bone marrow B-lineage lymphocytes' reactive hyperplasia and the prognosis is relatively well.When the bone marrow prolymphocytes increase to 10 % < N≤ 15 %,these prolymphocytes are most likely to be leukemia cells and indicate the possibility of relapse.
3.Effect of family doctor 's contract service combined with drug therapy on quality of life of hypertensive patients

Objective To explore the effect of family doctor contracting service combined with drug therapy on the quality of life of patients with hypertension. Methods Selected in May 2016 to May 2017 in a city in a district of three community health service center clinics diagnosed with hypertension in patients with 840 cases, were given amlodipine besylate tablets treatment, and the implementation of conventional health management And community intervention. According to whether or not to accept the family doctor contract service is divided into observation group (a total of 435 cases) and the control group (a total of 405 cases). Health status questionnaire (SF-36) was used to assess the quality of life before and after the intervention, and the blood pressure control and quality of life improvement were compared between the two groups before and after treatment.Results There was no significant difference in the blood pressure level between the two groups before treatment. The systolic and diastolic blood pressure in the observation group were better than those in the control group (P<0.05). There were no significant differences in physiological function, physiological function, physical pain, general health, mental health, energy, social function, affective function and total score of SF-36 before treatment. The scores of the observation group were significantly higher than those of the control group before and after treatment. The difference was statistically significant (P<0.05). Conclusion Family doctor contract service combined with drug therapy can effectively control the patient's blood pressure levels, improve the quality of life.
4.Study on Pureparation Techniques of Jiawei Sini Granules

Objective To optimize the conditions of Jiawei Sini granules by inspecting the macroporous resin adsorption refined methods.Methods Orthogonal design was used with saikosaponin A,paeoniflorin,hesperidin and glycyrrhizin as indicators determined by HPLC.Results The AB-8 macroporous resin,the biggest per gram of resin adsorption capacity were saikosaponin A 2.8 mg,paeoniflorin 62.8 mg,hesperidin 49.6 mg,glycyrrhizin 84.9 mg,with the concentration of durg 0.5 g/mL,the resin column diameter-height ratio of 1∶7 and adsorption velocity of 2(BV/h).Conclusion Macroporous resin adsorption method can be used for refining Jiawei Sini Granules.
5.Yiyuan Shengjing Decoction Treat Similar Climacteric Syndrome of Breast Cancer after Treated with Tamoxifen

Objective] To observe the clinical effect of Yiyuan Shengjing Decoction on similar climacteric syndrome of breast cancer after treated with tamoxifen. [Method] Choose 60 cases of breast cancer, randomly divide them into control group 30 cases and treatment group 30 cases, the control one were treated with tamoxifen and oryzanol, the treatment one, with tamoxifen and Yiyuan Shengjing Decoction. After 4w, evaluate the efficacy, compare their clinical symptoms score, hormone levels and endometrium thickness, and calculate the clinical relieving rate. [Result] After treatment, both groups clinical symptoms were relieved more than before treatment, with difference of statistical meaning; comparing on clinical symptoms scores, the treatment was much less than control one, with statistical meaning; on clinical relieving rates, the treatment group reached 83.33%, while 26.67% for control one, both had difference of statistical meaning. The comparison of reproductive hormone levels and endometrium thickness, the difference had no statistical meaning. [Conclusion] Yiyuan Shengjing Decoction treating breast cancer similar climacteric syndrome after treated with tamoxifen had marked cure effect, without influencing reproductive hormone level and endometrium thickness.
6.Relationship between polymorphisms of DNA repair gene XRCC1 and susceptibility to lung cancer

Purpose:The current study analyzed the polymorp hisms of DNA repair gene XRCC1 and relationship between genetic variations and s usceptibility to lung cancer Methods:A case-control study of 111 patients with lung cancer a nd 210 normal residents as controls was conducted to detect XRCC1 polymorphisms at Arg399Gln loci . Genotypes were analyzed by PCR-based restriction fragment le ngth polymorphism(RLFP) techniques. Results:XRCC1 codon 399 heterozygous genotype Arg/Gln might hav e weaker protection effect on squamous cell carcinoma and reduce the risk of lun g cancer on smokers. Homozygous genotype Gln/Gln of XRCC1 codon 399 might increa se the risk of lung cancer and have synergistic effect with smoking. Conclusions:The results demonstrated that genetic polymorphism of XRCC1 DNA repair gene might contribute to the susceptibility to lung cancer a nd have synergistic effect with smoking.
7.Catastrophic health expenditure for poor and low-income rural residents under New Rural Co-operative Medical Scheme

Objective:To evaluate the catastrophic health expenditure for poor and lower-income rural residents and the ability of New Rural Cooperative Medical Scheme( NRCMS) to alleviate poverty. Methods:We selected Zhe-jiang, Hubei, and Chongqing provinces as sample areas and obtained 1661 questionnaires through a field survey. We calculated the out-of-pocket expenditure per year, incidence, average gap, relative gap, and concentration index of catastrophic health expenditure for participating households. Results:Both before and after compensation, incidence, average gap, and relative gap of catastrophic health payment for poverty group were the highest, followed by low-in-come group. After compensation, the concentration index decreased. This means that catastrophic health expenditure tends to be focused more on families with financial difficulties. Conclusion:To protect poor and low-income rural res-idents from catastrophic health expenditure, we should promote critical illness insurance, improve the medical assis-tance system, implement payment reform, and improve the NRCMS.
8.Evaluation of the sedative and hypnotic effects of H1208.

This study is to investigate the sedative and hypnotic effects of a novel compound H1208. The sedative activity of H1208 was investigated by recording the spontaneous locomotor activity of mice. The hypnotic property was evaluated by the latency and duration of sleep (loss of righting reflex) in mice and the effect of hypnotics on sleep pattern of electroencephalogram were studied in conscious, freely moving mice with chronically implanted electrodes. The brain monoamine neurotransmitters levels in mice were measured by high performance liquid chromatography-electrochemical detection. The spontaneous locomotor activity was decreased by 56.7% and 80.2% in H1208 (5 and 25 mg x kg(-1), ip) treated mice, respectively. The loss of righting reflex was directly induced in mice after H1208 (60 mg x kg(-1), ip) administration. The non-rapid eye movement sleep increased significantly by 131% and 259%, respectively, within 3 hours after H1208 (30 and 60 mg x kg(-1), ip) administration. However, the rapid eye movement sleep decreased significantly. The contents of DA in the striatum and cortex and 5-HT in the cortex decreased significantly. These results demonstrated that H1208 has potent sedative and hypnotic effects, which may be closely related to the decreased contents of DA and 5-HT in mouse brain.
9.Evaluation of the improvement effect of humanistic care on cognitive function impairment and living ability of patients with Alzheimer's disease

Objective To explore the improvement effect of humanistic care on cognitive function impairment and living ability of patients with Alzheimer's disease. Methods 76 patients with Alzheimer's disease from January 2009 to May 2011 were taken as the research object.They were randomly divided into the control group and the observation group with 38 cases in each group.The control group was given conventional procedures for nursing care,and the observation group used humanistic nursing care as the guidance.The MMSE scale score,living ability index and family satisfaction degree before nursing and 1 month and 3 months after nursing for the two groups were taken for statistical comparison. Results The proportion of patients whose MMSE scale≥ 21 points and living ability score ranging from71 to 95 1 month and 3 months after care were higher than those of the control group.The satisfaction degree of patients in the observation group was also higher than the control group. Conclusions Humanistic care can significantly improve the cognitive dysfunction and living ability of patients with Alzheimer's disease,besides,family members of patients are more satisfied with this nursing model.
10.Does resting energy expenditure increase in children with newly diagnosed solid malignant tumor?

Objective To gather observational data on resting energy expenditure(REE)of children with newly diagnosed malignant solid tumor, and to examine factors that might be relevant to their energy expenditure change. Methods It is a retrospective study. Data from sixty-one patients with newly diagnosed malignant tumor admitted to department of pediatric surgery in Xinhua Hospital were reviewed in this study. Resting energy expenditure was measured by indirect calorimetry. Patients with benign tumor admitted during the same period were used as the control group. Results The difference of percentage of patients with normal, decreased, or increased REE between benign and malignant groups was not significant(χ2 = 0.466, P = 0.792). Patients with one or more gastrointestinal symptoms had higher percent of Pre-REE compared with patients without any gastrointestinal symptoms, but no significant difference existed(119.8±80.4 vs 91.1±27.2, Z = 0.746, P = 0.456). Different tumor types were associated with different effects on REE. There was no significant difference in REE levels between patients with decreased diet intake and those without(Z = 0.528, P = 0.598). Conclusions It is confirmed that not all patients with malignant tumor have an increased REE. Moreover, the tumor site might be an independent factor affecting patients' REE level.
