2.Epidemiological characteristics of rotavirus infection in infants with diarrhea in Guilin area

Objective To investigate the epidemiologic features of infant rotavirus(RV) diarrhea in Guilin area .Methods The detection of group A rotavirus were performed on 1 300 patients from Guilin 181st hospital by using immune colloidal gold method , and the epidemiological analysis were done according to the detection results .Results In the 1 300 patients ,132 were found rotavir‐us positive ,the total positive rate was 10 .15% .The positive rate of >1-2 years old group was the highest(23 .08% ) ,compared with other age groups(P<0 .05) .In November ,December and January ,the positive rates were 24 .14% ,20 .00% ,23 .53% ,respec‐tively .Compared with the positive rate in other months ,there was statistically significant difference(P<0 .05) .Conclusion Infants of 6 months to less than 2 years old in Guilin have higher detection rate of RV infection ,and the infection peak appears in autumn and w inter .
3.Ankle-foot orthosis improves walking ability of hemiplegic patients:a Meta-analysis

BACKGROUND:Studies have shown that ankle-foot orthosis can increase the feedback on the input information from receptors in the skin of the foot and leg to improve the ankle joint position sense, and promote brain function reorganization.
OBJECTIVE:To systematical y evaluate the effect of ankle-foot orthosis on the improvement of walking in hemiplegic patients.
METHODS:The Chinese Biomedical Literature Database, CNKI, WanFang Data and VIP database were searched for reports of randomized control ed trials of ankle-foot orthosis to improve walking ability in hemiplegic patients, from the date of establishment of each database to June 2013. The randomized control ed trials which met the criteria were included for the Meta-analysis.
RESULTS AND CONCLUSION:A total of 9 randomized control ed trials involving 456 patients were included. Meta-analysis showed that, compared with conventional treatment and drug therapy, ankle foot orthosis via the continuous treatment shows certain advantages to improve lower extremity motor function in hemiplegic patients, life skil s and 10-meter maximum walking speed. Due to a limited number of included documents, the remaining indicators such as walking speed, stride difference and balance function were only for appropriate descriptive analysis. The results suggested that, by improving abnormal gait, walking speed, stride frequency, gait cycle, space asymmetry, ankle muscle spasms and balancing, the ankle-foot orthosis could achieve the goal of improving walking function. Ankle-foot orthoses could not be confirmed to exert the role in the fol owing indicators, including time asymmetry, double support phase prolongation and stride length. This evidence shows that ankle-foot orthoses in hemiplegic patients may promote recovery of motor function of the lower limbs and activities of daily living to a certain extent, but the more high-quality, multi-center randomized control ed trials with large samples are necessary.
4.Does resting energy expenditure increase in children with newly diagnosed solid malignant tumor?

Objective To gather observational data on resting energy expenditure(REE)of children with newly diagnosed malignant solid tumor, and to examine factors that might be relevant to their energy expenditure change. Methods It is a retrospective study. Data from sixty-one patients with newly diagnosed malignant tumor admitted to department of pediatric surgery in Xinhua Hospital were reviewed in this study. Resting energy expenditure was measured by indirect calorimetry. Patients with benign tumor admitted during the same period were used as the control group. Results The difference of percentage of patients with normal, decreased, or increased REE between benign and malignant groups was not significant(χ2 = 0.466, P = 0.792). Patients with one or more gastrointestinal symptoms had higher percent of Pre-REE compared with patients without any gastrointestinal symptoms, but no significant difference existed(119.8±80.4 vs 91.1±27.2, Z = 0.746, P = 0.456). Different tumor types were associated with different effects on REE. There was no significant difference in REE levels between patients with decreased diet intake and those without(Z = 0.528, P = 0.598). Conclusions It is confirmed that not all patients with malignant tumor have an increased REE. Moreover, the tumor site might be an independent factor affecting patients' REE level.
5.Exploration of case-teaching method on emergency medicine in universities of traditional Chinese medicine

It is very necessary to apply case-teaching method to Emergency Medicine in Universities of TCM. To establish the case-teaching system,we ourselves have written teaching materials,trained teachers, and designed all sorts of the medical records. Moreover, we have deployed other methods and principles to put this teaching into practice, used clinical skill practising as a essential supplement of teaching effect. Comparing with the traditional teaching method, it is clear that case-teaching method could make students' learning initiative and positivity improved significantly, their test scores promoted obviously. Thus, carrying out case-teaching method in Emergency Medicine could advance students capability of acquisition of knowledge and their clinical thinking .
6.Quantification of reduced glutathione by analyzing glutathine-S-transferase reaction process taking into account of product inhibition

Objective To investigate the reliability of the integrated method for kinetic assay of substrate in the presence of product inhibition using glutathione-S-transferase(GST) as model.Methods Purified GST from pig liver was used to catalyze the conjugation of reduced glutathione(GSH) to 1-chloro-2,4-dinitrobenzene(CDNB)(final concentration at 1.0mmol/L),and reaction curve was monitored by product absorbance at 340nm.Maximal product absorbance after the completion of reaction was predicted by the integrated method.Results The optimization of product inhibition constant conferred to resistance to the variation of residual substrate concentration on the estimation of maximal product absorbance.This integrated method for kinetic substrate assay was also resistant to common source of errors.The recovery for extra GSH in rat liver homogenate was above 98% with linear response ranged from 2.0?mol/L to 90 ?mol/L.The concentration of GSH in rat liver was consistent to previous reports.Conclusion The integrated method is valid for kinetic assay of substrate when there is product inhibition,and it exhibits some universality as a kinetic method for enzymatic analysis with obvious advantages.
7.Identification and analysis of arsenic interactors byEscherichia coli proteome microarray

Objective · To globally study the influence of arsenite to the various biological pathways of Escherichia coli as a model organism.Methods · The protein-arsenite interactions was globally studied based on a proteome microarray constructed by 4256 affinity-purified Escherichia coli proteins. The functions of interacting proteins and their network were then analyzed by bioinformatics. Results · 91 proteins that remarkably interact with arsenic were successfully identified. Bioinformatics analysis found that most of the proteins possess catalytic activityand are involved in various biosynthesis and cellular metabolism pathways. The interactions of arsenic with proteins encoded by malY, cfa and hypF genes were further validated by Western blotting, which proves the results of proteome microarray reliable. Conclusion · Arsenite interacts with a variety of enzymes ofEscherichia coli and can greatly affect its biological metabolism.
8.Protective effect of aqueous extract from dioscorea nipponica Makino against carbon tetrachloride-induced acute liver injury in mice via TLR4/MyD88 signal pathway

Objective To investigate the effect of the aqueous extracts from Dioscorea nipponica Makino (AEDN) against the carbon tetrachloride (CCl4)-induced mice acute liver injury by regulating TLR4/MyD88 signal pathway.Methods 60 mice were randomly divided into control group, model group, low, medium and high dose AEDN groups according to radom number table with 10 mice in each group. Mice in low, medium and high dose AEDN groups were adiminstrated with 50, 100 and 200 mg/kg AEDN, in control and model groups were adiminstrated with solvent once a day for 7 consecutive days. Two hours after the last administration, mice were intraperitoneal injected with with 0.3% CCl4 olive oil solution to induce acute liver injury model, except for the mice in control group. Twenty-four hours after injection, the expressions of TLR4, MyD88 and NF-κB in liver tissue were evaluated by Western blot, mRNA levels were evaluated by PCR, and the AST and ALT levels in serum were also detected.Results Compared with model group, the serum AST (98.00 ± 17.75 U/L, 57.49 ± 9.66 U/L, 39.60 ± 9.49 U/Lvs. 113.40 ± 9.71 U/L) and ALT levels (76.00 ± 14.73 U/L, 50.70 ± 9.35 U/L, 35.25 ± 9.93 U/Lvs. 95.42 ± 11.64 U/L) were significantly decreased in low, medium and high dose AEDN groups (P<0.01); MyD88 (0.67 ± 0.21vs. 1.74 ± 0.42), NF-κB p65 (0.51 ± 0.09vs. 1.76 ± 0.31) and TLR4 (0.97 ± 0.25vs. 2.99 ± 0.72) levels were down-regulated in high dose AEDN group (P<0.01); the mRNA levels of IL-6 (2.22 ± 0.25, 1.76 ± 0.31vs. 5.20 ± 0.60), IL-1β (1.96 ± 0.35, 1.47 ± 0.23vs. 7.37 ± 0.99)、TNF-α (2.06 ± 0.25, 1.34 ± 0.33vs. 2.98 ± 0.50) in medium and high dose AEDN groups significantly decresed (P<0.01).Conclusions The AEDN has protective effect against CCl4-induced acute liver injury in mice via adjusting TLR4/MyD88 signal pathway.
9.Values of serum VEGF, CEA, CA19-9 and SF for predicting the efficacy to chemotherapy in advanced gastric cancer

Objective To investigate the values of serum vascular endothelial growth factor (VEGF) ,carcinoembryonic antigen (CEA) ,carbohydrate antigen 19-9 (CA19-9) and ferritin (SF) determination in assessing the efficacy of chemotherapy in patients with advanced gastric cancer .Methods Wedetermined the concentrations of serum VEGF ,CEA ,CA19-9 and SF for 85 patients with advanced gastric cancer ,before and after 1 cycles of chemotherapy and evaluated the the effect of chemotherapy after 2 cycle of chemotherapy .Results In effective group ,serum concentrations of VEGF ,CEA ,CA19-9 and SF significantly decreased after chem-otherapy cycle(P< 0 .05) ,serum VEGF and CEA concentrations significantly increased in progress group (P< 0 .05) ,serum VEGF ,CEA ,CA19-9 and SF concentrations in stable group and serum CA19-9 and SF concentrations in progress group did not change significantly(P> 0 .05) ,compared with those before chemotherapy cycle .Multiple regression analysis showed that serum CEA ,VEGF ,CA19-9 concentration changes correlated with chemotherapy efficiency ,while serum SF didn′t(t=1 .58 ,P=0 .119) . Conclusion Serum CEA ,VEGF ,CA19-9 concentration changes associated with gastric cancer chemotherapy efficiency ,which help predict the efficiency of chemotherapy .
10.Investigation and Analysis of Shortage Situation of Essential Medicines and Exploration of Intervention Mea-sures in Changchun

OBJECTIVE:To provide reference for solving the shortage of national essential medicines in Changchun. METH-ODS:According to preliminary work,a questionnaire was conducted to investigate and statistically analyze the shortage situation of essential medicines in 22 higher than secondary and 50 basic medical and health institutions in Changchun;ABC analysis was used to analyze the national essential medicines in shortage;the bid results and shortage causes of the primary and secondary nation-al essential medicines in shortage were inquired. RESULTS:Totally 72 questionnaire were send out,and 72 were recycled with ef-fective recovery of 100%. The investigation results showed there were 19 shortage breeds(22 dosage forms),which lost the bid-ding,and 99 breeds(111 dosage forms)of national essential medicines in shortage,which won the bidding,including 85 kinds of chemical medicines and biological products,14 kinds of Chinese patent medicine in Changchun. Cardiovascular system drugs shows the largest number in the bid chemical medicines and biological products,and the Chinese patent medicine were mainly Fu-zheng agent and Quyu agent(internal medicine). Results of ABC analysis showed,among the 111 bid shortage dosage forms,17 were primary and 23 were secondary. The main causes for them were price raising in raw materials,lack of production,no produc-ing in manufacturers and purchase price higher than winning bids. CONCLUSIONS:Low accessibility of raw material drugs,low enthusiasm to production,lower drug price than a reasonable level,and small clinical requirements can lead to the shortage of na-tional essential medicines. It is suggested to intensify in supervising the raw materials’marketing,raising the enthusiasm of manu-facturers,perfecting the system of drug bidding,developing fixed-point production and building drug reserve system to guarantee the production and supply of national essential medicines,and satisfy clinical demands.
