1.Diagnosis and Treatment of Subactue Thyroiditis

Objective To investigate the cause, the clinical manifestation, the diagnosis and the treatment of subacute thyroiditis. Methods The medical history, clinical characteristics, laboratory data, nuclear medicine tests and treatment of 76 cases of subactue thyroiditis were analyzed. Results Subacute thyroiditis was more common in women than men, and 76 3% cases with subacute thyroiditis occurred during 30~49 years old. Almost all cases were characterized by enlargement of the thyroid with pain and tenderness, 49 cases of which showed that radioiodine uptake was low with increased serum thyroid hormone levels. 25 cases were treated with prednisone, 23 cases were treated with intrathyroid injection of dexamethasone, 23 cases were treated with intrathyroid injection of dexamethasone and triamcinolone, all recovered well, and 5 cases underwent operation incorrectly. Conclusions Confirmation of subacute thyroiditis should put emphasis on a complete medical history, physical examination and necessary tests. Nuclear medicine tests play an important role in diagnosis of subacute thyroiditis, and the diagnostic drug treatment or fine needle aspiration biopsy may be performed in suspected cases. Glucocorticoid is the best medicine for treating this kind of disease. There is no need for operation therapy of subacute thyroiditis.

Objective To assess the efficacy and feasibility of neoadjuvant therapy of TPF regimen including docetaxel (TAX), cisplatin (DDP) and 5-fluorouracil (5-Fu) combined with concurrent DDP and radiotherapy (RT) in patients with local advanced nasopharyngeal carcinoma (NPC). Methods From April 2008 to May 2009, 40 patients with newly diagnosed UICC stage Ⅲ orⅣ local advanced NPC were enrolled. Patients were randomly assigned to group A(DDP every 3 weeks) and group B(DDP every week). Two cycles of induction chemotherapy with TAX 60 mg/m2 dl, DDP 60 mg/m3 dl and 5-Fu 600 mg/m2 dl-5 were given on a 3-weekly cycle, followed by RT and chemotherapy(group A: DDP 80 mg/m2 every 3 weeks for 2 times; group B: DDP 30 mg/m2 weekly for 6 times). Two-dimension conformal RT technique with 68-72 Gy/(34-36) fractions for 7 weeks was administered to the nasopharynx and 60-66 Gy/(30-33) fractions for 6-6.5 weeks to the node-positive area. Results 38 patients (78 Cycles) were evaluable for efficacy and toxicity. One patient in each group was excluded due to toxicity. 17 (17/19) patients of group A finished 2 cycles of planed DDP chemotherapy, while only 10 (10/19) patients of group B completed 6 weeks of planed DDP chemotherapy, 4 completed 5 weeks, 4 completed 4 weeks and 1 completed 2 weeks. Response to neoadjuvant TPF was as follows: 4 patients (10.5 %) achieved complete response(CR), 27(71.1%) achieved partial response(PR) and 7 (18.4 %) achieved stable disease (SD), so the overall response (CR+PR) rate was 81.6 %. After RT, 32 patients (84.2 %) achieved CR, 5 (13.2 %) PR and 1 (2.6 %) SD, so the overall response rate was 97.4 %. Conclusion TPF induction chemotherapy followed by concurrent DDP and RT is an effective regimen in the treatment of advanced NPC. Concurrent DDP chemotherapy on a 3-weekly cycle is recommended. Further study should be made to investigate how to increase the dose intensity of chemotherapy.
4.Clinical analysis of 18 cases of drug hypersensitivity syndrome

Objective To explore the clinical features of drug hypersensitivity syndrome(DHS). Method Retrospective analysis of 18 cases of DHS was made in detail including clinical situation, laboratory examination, treatment instance,duration and prognosis. Results The earliest symptoms of all patients were skin rash and fever, which occurred in 2 to 8 weeks after the administration of the drugs, accompanied with superficial lymphadenopathy in 13 patients, liver dysfunction in 14 patients, renal impairment in 11 patients, leukocytosis in 13 patients and eosinophilia in 10 patients. The skin lesions were multiform. The common eruption was purpura-like erythemas, accompanied with swelling and repeated desquamation. The suspected drugs were as follows: allopurinol in 11 cases, carbamazepine in 4 cases, phenytoin in 2 cases and minocycline in 1 case. Corticosteroid was the first line of treatment Conclusions DHS is a clinical syndrome to have characteristic symptom and a relatively severe disease. It is crucial to discontinue the suspected drugs promptly and to start sufficient dose of corticosteroid.
5.Study on Acinetobacter baumannii plasmid with 3 types of beta-lactamase genes in a burn ward.

OBJECTIVETo study the transferrable character of Acinetobacter baumannii (AB) plasmids with 3 types of beta-lactamase gene.
METHODSThe plasmid of multi-drug resistant AB (donor) isolated from burn wound were transferred to E. coil ATCC25922 (receptor) through conjugation, and drug sensitivity was also observed. Drug-resistant gene and stability of filial generation and zygote were analyzed by PCR.
RESULTSThe drug-resistance of donor plasmids to Sulfamethoxazole, Ampicillin, Cefalotin, Cefpodoxime, Cefuroxime, Imipenem/Cilastatin and Ampicillin/Sulbactam, and three types of beta-lactamase gene were transferred to the receptor, and were also stably transmitted for passages. The minimum inhibitor concentration of receptor to Sulfamethoxazole was > 2 mg/L after conjugation with donor, and inhibitory character could be transferred to next generation.
CONCLUSIONbla(TEM-1), bla(PER-1) and bla(OXA-23) genes carried in the plasmid of AB can be transferred through conjugation and stably transmitted for passages, and it is one of the molecular mechanisms for AB with multi-drug resistance after burn infections.

6.Calcium citrate: a new biomaterial that can enhance bone formation in situ.

OBJECTIVETo investigate the effect of a new biomaterial combining calcium citrate and recombinant human bone morphogenetic protein-2 (rhBMP-2) on bone regeneration in a bone defect rabbit model.
METHODSTotally 30 male New Zealand white rabbits were randomly and equally divided into calcium citrate-rhBMP-2 (CC-rhBMP-2) group and rhBMP-2 only group. Two 10 mm-long and 5 mm-deep bone defects were respectively created in the left and right femoral condyles of the rabbits. Subsequently 5 pellets of calcium citrate (10 mg) combined with rhBMP-2 (2 mg) or rhBMP-2 alone were implanted into the bone defects and compressed with cotton swab. Bone granules were obtained at 2, 4 and 6 weeks after procedure and received histological analysis. LSD t-test and a subsequent t-test were adopted for statistical analysis.
RESULTSHistomorphometric analysis revealed newly formed bones, and calcium citrate has been absorbed in the treatment group. The percent of newly formed bone area in femoral condyle in control group and CC-rhBMP-2 group was respectively 31.73%+/-1.26% vs 48.21%+/-2.37% at 2 weeks; 43.40%+/-1.65% vs 57.32%+/-1.47% at 4 weeks, and 51.32%+/-7.80% vs 66.74%+/-4.05% at 6 weeks (P less than 0.05 for all). At 2 weeks, mature cancellous bone was observed to be already formed in the treatment group.
CONCLUSIONFrom this study, it can be concluded that calcium citrate combined with rhBMP-2 signifcantly enhances bone regeneration in bone defects. This synthetic gelatin matrix stimulates formation of new bone and bone marrow in the defect areas by releasing calcium ions.
