1.Prognosis and changes of myocardial intersitium before and after ventricular assist device application

Objective To investigate the relation between the changes d myocardial collagen content before and after ventricu-lar assist device (LVAD) application and the prognosis in dilated cardiomyopathy. Methods Patients was divided to two groups ac-cording to the prognosis (weaning or transplantation) after LVAD application. Left ventricular samples were collected from the patients (weaning group n= 10, transplanted group n=8) at the time the LVADs were implanted, and again during cardiac transplantation ( n = 8 ). The content of neutral salt soluble collagen (NSC) and acid soluble collagen (ASC) were measured by Sired collagen assay and total collagen and insoluble collagen (ISC) by quantification of hydroxyproline. Moreover, protein quantification of each collagen fraction was performed simultaneously. Results Before LVAD implantation all collagen fraction and their protein contents were higher in the weaning group than that in the transplanted group ( P <0.05), but the difference in the collagen fraction disappeared after LVAD support. Comparison of the pre- and post-LVAD subgroups of the transplanted group showed an increase of NSC and its protein content and total hydroxyproline after LVAD support (P<0.05). Conclusion The significances of myocardial intersititial remodeling in DCM are not identical. Some myocardial collagen and protein content increases may be related to good prognosis. The mechanism of LVAD improving cardiac function may be related to the changes of myocardial intersitial collagen.

Aromatase inhibitors(AIs) are standard therpy for postmenopausal women with estrogen responsive breast cancers.By inhibiting the aromatase enzyme,causing decreases in endogenous estrogens,the treatment of Als is responsible for lower bone mineral density(BMD) and increased fractures.Therefore,early recognition, prevention,and/or treatment of AI-induced bone loss is needed.Zoledronic acid is specific inhibitors of osteoclasts and extensively used in bone metastasis patients.Recently, there are several trials evaluating the use of in- travenous zoledronic acid as prevention and treatment of AI-induced bone loss in postmenopausal women with breast cancer.In this article, we aim to review the use of zoledronic acid in this population including the response and safety.
5.Pre-surgical evaluation of the cerebral tumor in the left language related areas by functional MRI

Objective To evaluate the application of combination of BOLD-fMRI and diffusion tensor tractography (DTT) in pre-operative evaluation of cerebral tumors located at the left language related areas.Methods A non-vocal button pressing semantic judging paradigm was developed and validated in 10 right-handed volunteers at 3 T.After validation, this protocol combined with DTI were applied to 15 patients with left cerebral tumor prior to surgical resection, and 3 of them had aphasia fMRI data analysis was on subject-specific basis by one-sampled t-test.The distance from the tumor to Broca area and pre-central "hand-knot" area were measured separately.Functional language laterality index (LI) was calculated by taking out Broca area and Wernicke area.Three dimensional architecture of frontal lobe white matter fibers, especially arcuate fasciculus, were visualized using diffusion tensor tractography on Volume-one software.The images demonstrating relationship among tumor, language activation areas and white matter fibers were reviewed by neurosurgeons as part of pre-operative planning.One year after the operation, patients were followed up with MRI and language function test Results The non-vocal semantic judging paradigm successfully detect Broca area, Wernicke area and pre-central "hand-knot" area In 12 of 15 patients, the relationship of Broca area and pre-central motor area to the left brain tumor in language related areas was identified, which make the pre-operative neurosurgical plan applicable to minimize the disruption of language and motor.8 patients had the left language dominant hemisphere, 3 patients with the right language dominant hemisphere and 1 patient with bilateral dominance.The other 3 patients' fMRI data were corrupted by patients' motion.Diffusion tensor images were corrupted by motion in 1 patient but demonstrated the impact of tumor on left arcuate fasciculus in 14 patients.Diffusion tensor tractography showed disruption of left arcuate fasciculus in 3 patients, deviation/deformation in 6 cases and unaffected by tumor in the remaining 5 cases.Pre-operative review of language activation maps and diffusion tensor tractography of arcuate fasciculus influenced the surgical approach in all 12 patients.Post-operatively, one patient died accidentally, the remaining 14 patients showed no tumor recurrence after one year follow-up.No patient showed deterioration of language function, though 3 patients who had aphasia pre-operation improved slightly.Conclusion The combination of fMRI and diffusion tensor tractography is helpful for identifying the relationship of brain tumors with language cortex and white matter fibers, which is important for sparing language function during individually surgical plan.
6.The clinical analysis of hypoxic spells on patients with tetralogy of fallot

Objective To explore the prophylaxis and treatment of hypoxic spells on patients with TOF.Methods Retrospectively analysed and compared the case history of the inpatients with TOP in our hospital,to analyse the epilepticus cause of hypoxic spells and study the hypoxia mechanism and summarize the prevention and control measures.Results 4(17%)cases of TOF died after hypoxic spells,the epilepticus cause of hypoxic spells included suckling,early getting up,crying,standing abruptly after stool,scorching climate,infection fever and anemia,the hypoxia mechanism was likely to the increase of infundibulum cardiac muscle contraction,the systemic circulation resistance drop suddenly,hypezventilation,high-viscosity syndrome.Conclusion To control the epilepticus cause and perform operative treatment early can reduce the hypoxic spells.The prevention and control measures were strengthening the live nurse,disconnected oxygen inhalation,oral beta-blocker therapy and venons injection 5% sodium bicarbonate.Early operative treatment was the best redical treatment.
7.The clinical analysis of 18 children with neurogenic pulmonary edema

Objective To investigate the etiology clinical diagnosis and treatment of pediatric neurogenic pulmonary edema(NPE).Methods The clinical data of 18 hospitalized children with NPE were retrospectively analyzed.Results 18 cases of NPE children with traumatic brain injury 2 cases( 11.1% ),cervical spinal cord injury 1 case(5.5% ) and,subarachnoid hemorrhage 2 cases( 11.1% ),and epilepticus states 2 cases( 11.1% ),non-HFMD caused encephalitis 3 cases( 16.6% ),NPE caused hand,foot and mouth disease 8 cases(45.6% ),survival 6 cases (33.3%) and 12 patients died (66.7%)in 18 hospitalized children with NPE.Conclusion The common cause of pediatric NPE was central nervous system infections and poor prognosis,early diagnosis and early treatment could improve the efficacy and prognosis.
8.Effects of Intermittent Hypoxic Exposure on the Production of Erythrocyte and Some Haematopoietic Factor in Rats with Exercise-Induced Anemia

9.Distribution of Nosocomial Infection Pathogens and Analysis of Drug-Resistance in Intensive Care Unit

Objective To understand the distribution of nosocomial infection pathogens and drug - resistance in intensive care unit of our hospital for providing the guidance of clinical rational administration and preventing the hospital infection. Methods Pathogenic bacteria were isolated from the patients who suffered from nosocomial infection in intensive care unit from January of 2008 to December of 2008. They were tested by microbe VITEK and drug - sensitive reagent. Rate of drug resistance of the pathogenic bacteria was analyzed. Results Gram - negative bacilli( G- ) accounted for 47.67% of the isolated pathogenic bacteria,and most were Acinetobacter baumannii (21.65%) , Pscudomonas aeruginosa( 8.00% ) , Stenotropham onasm altophilia(6.33% ) and Klebsiella pneumoniae (4.00%). Grampositive cocci ( G~+ ) accounted for 9.56 % , and most were Staphylococcus aureus (5.56%) and Enterococcus faecium ( 1.44 % ). The rnycetes occupied the 42. 78% of the pathogens. The main pathogenic bacteria were Candida albicans (24. 44% ) and Candida albicans ( 10.89% ). The rate of drug resistance of Gram - negative bacilli( G~- ) was high as a whole,while the rate of mycetes was low. Conclusion Enhance monitoring on pathogenic bacteria distribution and drug resistance analyses of nosocomial infection in ICU could benefit for the guide of clinical rational administration and depressing multidrug - resistant bacteria.
