1.Influence of mother's treatment for syphilis on the incidence of congenital syphilis in newborn babies

Objective To explore the influence of mother's treatment for syphilis on the incidence of congenital syphilis in newborn babies.Methods 40 cases of neonatal congenital syphilis,according to mother different treatment methods,were divided into regular treatment group (20 cases),mother without regular treatment group (10 cases),mother with no treatment group (10 cases).The incidence of neonatal congenital syphilis was compare.Results The neonatal congenital syphilis could affect multiple systems,manifested as skin damage,pathological jaundice,hepatosplenomegaly,edema,anemia,thrombocy to penia,bone injury,cerebrospinal fluid abnormalities et al.The incidence rate of neonatal congenital syphilis in regular treatment group was 10%,which was significantly lower than normal treatment 40% and untreatment 50% (x2 =11.2,P < 0.05 ; x2 =20.4,P < 0.01).The neonatal congenital syphilis incidence rate was not statistically significant between the informal group and untreatment group (x2 =0.68,P > 0.05) ;30 cases were cured (75 %),improved in 7 cases (17%) ;3 cases died (7 %).Conclusion The prevention and treatment of syphilis infection in mothers are the key point to prevent neonatal congenital syphilis.
2.Expression and clinical significance of MGMT,Ki-67 and P53 in human gliomas

Objective To detect the expression of DNA repair enzyme O6-methylguanine DNA methyl-transferase(MGMT),cell proliferation-related nuclear protein(Ki-67)and P53 protein in gliomas,and inves-tigate the relationship and clinical significance among them and gloma grade. Methods 61 cases of brain glio-ma specimes and 16 cases of internal decompression of brain trauma were used to detect the expression of MGMT,Ki-67 and P53 by using immunohistochemical SP method. Results MGMT protein expression (19. 67%∶0,χ2 =3. 729,P=0. 062),Ki-67 protein expression(39. 34% ∶0,χ2 =5. 722,P=0. 016)and P53 protein expression(27. 87% ∶0,χ2 =9. 146,P=0. 002)showed significant differences in gliomas com-pared to normal brain tissues,the expression of Ki-67 was significantly higher in high-grade gliomas(Ⅲ-Ⅳ) than that in low-grade gliomas(Ⅰ-Ⅱ)(14 ∶10,χ2 =11. 718,P =0. 001). No significant difference was found between the MGMT and P53. Conclusion MGMT protein can be used as a biomarker in gliomas detec-tion;Ki-67 has a positive correlation with tumor grade,and can be used as a reference indicator of pathological grade;P53 protein expression may be used as a potential target for the treatment of gliomas.
3.The de-escalation therapy combined bronchoalveolar lavage in treatment of acute stroke complicate pulmonary infection

Objective To evaluate the effect of the de-escalation therapy of antimicrobial agents combined bronchoalveolar lavage on the treatment of acute stroke complicate pulmonary infection.Methods 80 patients of acute stroke complicated pulmonary infection were randomized into two groups.The method of the de-escalation therapy of antimicrobial agents combined bronchoalveolar lavage was used in therapeutic group(n=40)while the patients in control group(n=42)received routine therapy of antimicrobial agents.The effective rate,the bacteria1 eradication rat,the average time of pneumonia absorption and the average time NICU stay between two groups were compared and analyzed.Results The effective rate and the bacteria1 eradication rat in therapeutic group were significantly higher than those in control group(P
4.Invasion and metastasis inhibition of Foxp3 on ovarian cancer cells Skov3 and its mechanism

Objective:To investigate the effect of Foxp3 on the invasion and metastasis of the ovarian cancer cells Skov 3 and the related mechanism.Methods: Plasmid overexpressing Foxp 3 gene was constructed and transfected into Skov 3 cells using liposome.Proliferation,invasion,RT-PCR and Western blot analysis were done to observe the expression of Foxp 3 gene in Skov3 cells and the invasion and metastasis ability.Also,MMP-2 and MMP-9 gene expression was detected.Results:Overexpression of Foxp3 was confirmed in Skov3 cells after Foxp3 plasmid transfection.Compared with the empty vector group , overexpression of Foxp3 gene significantly inhibited the migration and invasion of Skov 3 cells.Meanwhile , the expression level of MMP-2 and MMP-9 genes were decreased.Conclusion:Upregulation of FoxP3 gene expression can inhibit cell proliferation ,invasion and migration in ovarian cancer Skov3 cells through downregulating MMP-2 and MMP-9 gene expression.This study provided experimental basis for further research on Foxp3 gene and gene therapy for ovarian cancer.
5.Analysis of antibiotic resistance of Klebsiella pneumoniae and Escherichia coli in old patients with lower respiratory tract infections

Objective To investigate antibiotic resistance of Klebsiella pneumoniae and Escherichia coli in old patients with lower respiratory tract infections. Methods Kirby Bauer agar diffusion method was used to evaluate the drug sensitivity in 240 strains of Klebsiella pneumoniae and Escherichia coli isolated from patients with lower respiratory tract infection. Phenotypic confirmatory test recommended by NCCLS1999 was used to detect extended spectrum beta lactamases(ESBLs). Results The resistant rates of Klebsiella pneumoniae and Escherichia coli to 14 antibiotics in old patients and in non old patients with lower respiratory tract infections were amoxicillin 93 2% vs 87 3%, piperacillin 57 1% and 42 9%, cefuroxime 51 4% and 33 3%, cefotaxime 40 1% and 17 5%, ceftazidime 13 6% and 3 2%, ceftriaxone 39 0% and 17 5%, cefoperazone 37 3% and 15 9%, cefepime 10 2% and 3 2%, amikacin 47 5% and 34 9%, ciprofloxacin 54 2% and 38 1%, imipenem 0, cefoperazone/sulbactam 0, piperacillin/tazobactam 1 1% vs 0, and cefmetazole 9 6% and 4 8% respectively. Out of 240 clinical strains of Klebsiella pneumoniae and Escherichia coli, 78(32 5%) were considered ESBLs producers by phenotypic confirmatory test. The prevalence of ESBLs in old patients was 38 4%, which was much higher than that in non old patients(15 9%). The resistant rate of ESBLs producing strains to imipinem, cefoperazone/sulbactam, piperacillin/tazobactam and cefmetazole was the lowest, being 0, 0, 2 6% and 12 8%. Conclusions The resistant rates of Klebsiella pneumoniae and Escherichia coli to most antibiotics and the prevalence of ESBLs in old patients with lower respiratory tract infection were higher than that in non old patients. Imipinem, cefoperazone/sulbactam, piperacillin/tazobactam and cefmetazole were the effective antibiotics to infections caused by ESBLs producing strains.
6.Effect of fluvastatin on expression of connective tissue growth factor in the myocardium of diabetic rats

Objective To investigate the effects of fluvastatin on expression of connective tissue growth factor (CTGF)in the myocardium of diabetic rats. Methods Thirty-two Sprague-Dawley rats were randomized into four groups: normal control, untreated and STZ-induced diabetic rats, and diabetic rats treated with low-dose and high-dose fluvastatin.After 12 weeks′ intervention, body weight as well as the weights of both whole heart and its left ventricle were measured to calculate the ratio of heart weight to body weight (H/B) and the left ventricle mass index (LVMI).The mRNA expression of CTGF, transforming growth factor β1 (TGF-β1), fibronectin (FN),collagen type Ⅲ (ColⅢ) in the myocardium were detected by RT-PCR. Results Compared with the normal control group, the untreated diabetic group showed the increased (all P<0.05) H/B, LVMI and mRNA expressions of CTGF, TGF-β1, FN, and Col Ⅲ in the myocardium. In addition to the effective regulation of lipid metabolism, fluvastatin treatment could obviously reduce the increment of H/B and LVMI(P<0.05~0.01), and suppress the mRNA expressions of CTGF, TGF-β1, FN, and Col Ⅲ in the myocardium of diabetic rats(P<0.01).The down-regulation of CTGF was more significant than that of TGF-β1.All these effects were in dose dependent way. Conclusions Fluvastatin inhibits extracellular matrix accumulation in the myocardium of diabetic rat.Down-regulating the overexpression of CTGF in diabetics is an underlying mechanism of fluvastatin in the cardiac protection.
7.Application of Plasmic Endotoxin Detection in Treatment of Sepsis

Objective To estimate the clinical application of plasmic endotoxin detection in the treatment of sepsis.Methods 36 septic patients admitted in our intensive care unit were observed prospectively.The endotoxin, white blood cell, C-reaction protein and procalcitonin were detected.After that, the relationships between endotoxin and white blood cell, C-reaction protein, procalcitonin, gastrointestinal dysfunction respectively were analyzed.Re-sults The mortality and the endotoxin level were both higher in patients with septic shock than those in the septic pa-tients without septic shock ( P<0.05) .There were no significantly statistical differences between endotoxin concen-tration and gastrointestinal dysfunction, white blood cells, or C-reaction protein respectively (P>0.05).However, there was statistically difference between endotoxin concentration in plasma and procalcitonin ( r =0.462, P =0.039) .Conclusion The risk of poor prognosis is larger while shock occurs in the septic patients.Endotoxin is re-lated to shock and serum procalcitonin.
8.EXPRESSION OF YB-1 IN ENDOMETRIOSIS

Objective To investigate the expression and significance of YB -1 in serum and endometrial tis-sues in patients with endometriosis ( EMs ) for taking YB -1 EMs as an index of early diagnosis and treatment . Methods ELISAwas adopted to detect YB -1 expressions in serum of EMs group and control group .The expression of YB-1 in endometrial tissue in the EMs group and control group was detected by immunohistochemistry SP method . Results The average concentration of YB -1 in serum of EMs group (14.65 ±3.91 ug/L) was higherthan thatin the control group (9.64 ±2.37 ug/L), with statistically significant differences (p<0.05).The results of immuno-histochemistry showed that the expression of YB -1 was moderately positive in endometria of patients with EMs . Weakly positive expressions of YB -1 were detected in a fewcases of the control group .There are significant differ-ences in the positive expression rates of YB -1 between two groups ( p <0.05).Conclusion The expression of YB-1 in serum and endometria of patients with EMs is higher .YB-1 can not only be an intracellular marker , but also expected to become an ideal serological marker .
9.Domestic retrospective analysis of pulmonary cryptococcosis in the recent 22 years

Objective To learn the domestic investigation into pulmonary cryptococcosis in the 22 years.Methods We retrieved the literature about pulmonary cryptococcosis included in CMCC,and summarized the clinical data of 67 patients suffering from pulmonary cryptococcosis.Results The clinical situation and radiological manifestation of pulmonary cryptococcosis were diverse and nonspecific.Cryptococcal meningitis was the commonest complication(31.34%).Almost all patients with pulmonary cryptococcosis were once misdiagnosed.Final diagnosis mainly depended on the smear,culture or pathological examination of various clinical specimens.Therapeutic tool involved resection with operation and the application of anti-fungi drugs.Conclusion To increase the cognition to the disease is the key to increase the final diagnosis rate.Therapeutical effect of anti-fungi drugs should be emphasized,especially the application in preoperation and postoperation can decrease the dissemination of cryptococcus after operation,which might result in cryptococcal meningitis.
10.The diagnosis and surgical treatment of hepatolithiasis associated with cholangiocarcinoma

Objective To explore into the relationship between the hepatolithiasis and the intrahepatic cholangiocarcinoma, and summarize the experience of early diagnosis and surgical treatment of these disease. Methods A retrospective clinical analysis was made in 28 cases of hepatolithiasis complicated by intrahepatic cholangiocarcinoma from September 1998 to December 2008. Results It was found that the incidence of cholangiocarcinoma in hepatolithiasis was 5.9%. The correct rate of preoperative ultrasonic diagnosis was 57.15% ( 16/28 ) ,and that of CT diagnosis was 46.43 % ( 13/28), and that of MRCP diagnosis was 40.00% (6/15). 12 cases were radically resected,12 cases were treated by palliative therapy, and 4 cases were examined only with biopsy, and all cases were followed up. The 1-,2-,and 3-year survival rate in radical surgery were 83.34% ,58.34% ,25.00% respectively. And the 1-year,and 2-year survival rate in palliative surgery group were 66.67% ,8.33% respectively. And all the cases examined only with biosy died in 6 months after the biosy. Conclusion Cholangiocarcinoma is related to hepatolithiasis. In patients of hepatolithiasis who were older than 40 years and have a long history of recurrent cholangitis, weight-loss in a short period, progressive jaundice, or intractable abdominal pain,the possibility of accompanying cholangiocarcinoma should be considered. The key of improving the therapeutic effectiveness was early diagnosis ,early treatment and striving for radical operation.
