1. Chaiqinchengqi decoction alleviates liver injury during severe acute pancreatitis in mice via inhibition of tlr4/nf-kb p65 pathway

Aim To explore the protective mechanisms of Chaiqinchengqi decoction (CQCQD) on liver injury during severe acute pancreatitis ( SAP) based on TLR4/NF-kB p65 pathway. Methods Thirty-six KM mice were randomly divided into control group (Control) , SAP and SAP + CQCQD (treatment group) (n = 12). The mice in SAP group were injected with 20% L-arginine intraperitoneal^ (3.3 g • kg"1, 2 times, 1 h interval). The mice in SAP + CQCQD group were intragastrically administered with Chaiqinchengqi decoction (19 g • kg-1 • d"1) following induction of SAP. Histopathologic^ changes of pancreas and liver were observed 72 hours after model establishment. The endotoxin in serum was tested, and the TLR4 and p-NF-kb p65 expression, as well as some inflammatory cytokines in liver was detected. Re-suits Compared to control group, the pathological damages of the pancreas and liver in SAP group were aggravated, serum endotoxin increased, the expression of TLR4 and p-NF-kB p65 in liver increased, and IL-6, TNF-a and MIP-1 a mRNA in liver were significantly elevated. Compared to SAP group, the intervention of CQCQD partially relieved the pathological damages of the pancreas and liver, reduced the endotoxin in serum , and decreased the expression of TLR4 and p-NF-kB p65, as well as IL-6, TNF-a and MIP-1 a mRNA in the liver. Conclusion CQCQD appears to protect liver injury during SAP via inhibiting TLR4/NF-kB p65 activation, then decreasing cytokines and chemo-kines.
2.Effects of liver-smoothing and Spleen-srengthening Formula on Serum Sex Hormone and Expression of Estrogen Receptors (ER) and Progesterone Receptors (PR) of Rats with Hyperplasia of Mammary Gland

Objective To investigate the prevention and treatment mechanism of liver-smoothing and spleen-srengthening formula on hyperplasia of mammary gland,we study the effects of it on morphology,histology,serum sex hormone and its receptor of rats with hyperplasia of mammary gland.Methods Sixty female Wistar rats(SPF)were randomized into 5 groups:control group,disease model group,the tamoxifen group,small dose of medicine group and large-dose drug group.Estradiol Benzoate and Progesterone were used to establish animal model of breast hyperplasia.Rats in different groups were respectively maintained with Tamoxifen,small and large dose of liver-smoothing and spleen-srengthening formula.Then changes of morphology,histology,serum sex hormone and its receptor were obtained.Results Liver-smoothing and spleen-srengthening formula could reduce the number of breast lobular and the expansion degree of adenovirus catheter.It also narrowed the acinar cavity.In serum of rats with breast hyperplasia,Estradiol(E2)and PRL were declined ;Progesterone(P)and Testosterone(T)were increased;FSH and LH were partly increased.Positive expression degree of ER and PR were inhibited obviously.Conclusion Liver-smoothing and spleen-srengthening formula can reduce not only the content of estrogen receptors(ER)and progesterone receptors(PR)but also their sensitivity to estrogen of rats with breast hyperplasia by regulating the serum sex hormone.Our studies suggest that it can obviously inhibit hyperplasia of mammary gland.
3.Mutation analysis in RECQL4 gene in a patient with Rothmund-Thomson syndrome

Objective To detect the mutations in RECQL4 gene in a Chinese patient with Rothmund- Thomson syndrome (RTS). Methods Blood samples were collected from a sporadic patient with RTS, his unaffected parents and 30 unrelated population-matched controls. DNA was extracted, and all the coding sequences of RECQL4 gene were amplified by PCR. Direct sequencing was performed with the amplicons to detect the possible mutations in these subjects. Results Two mutations, i.e., IVS11-1G > A and 3401 A >T, which resulted in a premature termination codon at amino acid 560, were found in the RECQL4 gene of the patient. His father was heterozygous for IVS11-1G > A, and his mother for 3401 A>T. Meanwhile, neither of the two mutations were observed in 30 unrelated normal control individuals. Conclusion Two mutations, including IVS11-1G>A and 3401 A>T are present in the RECQL4 gene of the sporadic patient with RTS.
4.Research progress on relationship between brain metastasis from lung cancer and ex-tracellular vesicles

Extracellular vesicles (EVs) are vesicles like body of phospholipid bilayer membrane, which are able to mediate the transfer of genetic material. There are receptors, proteins and nucleic acids in the body, with carrying tumor genetic material, regulating tumor microenvironment, promoting tumor angiogenesis and mediating tumor cell metastasis. At present, it is found that there is a close re-lationship between the secretion and the metastasis of lung cancer. The main aspects of the brain metastasis of the lung cancer medi-ated by the exocrine body include the regulation of the microenvironment of the brain, the destruction of the blood-brain barrier, and the regulation of tumor cell pathology. The study on the relationship between the tumor and the metastasis of lung cancer may pro-vide more molecular targets for the development, diagnosis and treatment of lung cancer.

6.Research Progress in Experimental Methods for the Evaluation of Adhesion Property of Bioadhesive Drug Delivery Systems in vitro

Bioadhesive delivery systems with sustained release characteristics can promote the absorption and increase the bio-availability of drugs by prolonging the residence time on the applied site. The evaluation of adhesion property of bioadhesive drug deliv-ery systems is important because the sustained release effect and the absorption are closely related with the adhesion. The methods for the evaluation of the adhesive properties of bioadhesive drug delivery systems in vitro were summarized in the article, including determi-ning the minimum peel force, measuring the residues on mucosa surface, determining the retention time on mucosal surface, measuring the swelling and the viscosity of fluid and so on, and the operation of some methods were described simply as well.
7.Analysis of distribution and antibiotic resistance of 233 positive results of blood cultures in neonatal intensive care unit

Objective To investigate the pathogens distribution and their drug resistance of blood culture-positive neonates with sepsis in neonatal intensive care unit(NICU) and to provide evidence for clinically reasonable use of antibiotics.Methods One thousand four hundred and fifty neonates admitted to our NICU of Affiliated Hospital of Guangdong Medical College from Jan 2009 to Jul 2011 were enrolled.The blood samples from 1 450 neonates were cultured and isolated,and all of the isolates and their resistances were analyzed retrospectively.Results A total of 9 species and 233 pathogenic strains were isolated from 1 450 specimens,and the positive rate was 16.1%.Among the 233 strains,93 strains were gram-positive cocci,103 strains were gram-negative bacilli,and 37 strains were fungus.Among gram-positive cocci,the most dominant organism was coagulase negative staphylococcus,the next was Staphylococcus aureus;while among gram-negative bacilli,Ktebsiella pneumoniae were the most frequently isolated,the next were Escherichia coli and Acinetobacter baumannii,and among the fungus,monilia were the most isolated.The resistance rates of gram-positive cocci to vancomycin,linezolid and teicoplanin were high.Moreover,the antibiotic resistance rates to penicillin,oxazocilline and the β-lactamase were more than 95%.Gram-negative bacilli were sensitive to levofloxacin,imipenem and meropenem.A majority of fungus was Monilia,which was sensitive to antifungal.Conclusion Coagulase negative staphylococcus,Klebsiella pneumoniae and Escherichia coli are the the most common pathogens of neonatal sepsis in NICU,and have high drug resistance.Clinicians should select sensitive drug treatment according to bacterial identification and susceptibility testing.Besides,mycotic infection cannot be ignored and merit our attention.
8.Small diameter graft shunts combined with pericardial devascularization for the treatment of bleeding esophagogastric varices

Objective To study the effect of small diameter graft (0.8 cm) splenocaval or mesocaval shunts combined with pericardial devascularization in the treatment of portal hypertensive variceal bleeding. Methods Splenocaval shunts were performed in 14 patients and mesocaval shunts were done in 24 patients, in combination with pericardial devascularization. Results The average decrease of free portal pressure was 6.6±1.2 cm. There was no significant changes in liver function postoperatively (P>0.05). Platelet counts and leukocyte counts were back to normal in splenocaval shunt patients postoperatively (P< 0.05). Operative mortality was 3%. Pyrexia developed in 4 patients, intractable ascites in 1 patient, chylons ascites in 1 patient, hepatic encephalopathy in 1 patient, intraabdominal infection in 1 patient and stress ulceration in 1 patient. All patients recovered after expectant treatment except one who died from severe intraabdominal infection. 35 patients received follow-up between 6 months and 3 years, total effective rate was 89%, 2 patients died from recurrent variceal bleeding, the shunt potency rate was 80% in 1 year and 75% in 3 years. Esophagogastric varices disappeared or alleviated as shown by endoscopy in 25 patients on 6 months postoperatively. Conclusions Small diameter portosystemic graft shunts combined with poricardial devascularization is an effective therapy for bleeding esophagogastric varices with a low rate of hepatic encephalopathy. Splenocaval shunt alleviates hypersplenism concurrently.

10.Formula and Preparation of Cantharidin Chitosan Bioadhesive Microspheres by Spray Drying

Objective: To optimize the formula and preparation technology of cantharidin chitosan bioadhesive microspheres.Methods: The microspheres were prepared by a spray drying method.A single-factor experiment was used to study the effect of chitosan with different molecular weight on the gastric mucosa adhesion rate were also studied, and the effects of different drug loading ratio, concentration of chitosan acetic acid solution and speed of peristaltic pump on the loading rate were also studied.The cantharidin entrapment efficiency as the studying index, a response surface method was used to further optimize the formula.Results: The best formula and preparation process of cantharidin chitosan bioadhesive microspheres were as follows: the concentration ratio of antharidin to chitosan was 19.83% , the concentration of chitosan acetic acid solution was 0.77% , and the peristaltic pump flow rate was 9.225 ml·min-1.With the best formula, the entrapment efficiency of cantharidin chitosan bioadhesive microspheres was 90.14%.Conclusion: The microsphere preparation by the spray drying method is stable and repeatable.Cantharidin chitosan bioadhesive microspheres have high entrapment efficiency and promising biological adhesion.
