1.Clinical effect study of acupuncture therapy combining with biofeedback therapy on senile chronic insomnia patients

Objective To study the clinical effect of acupuncture therapy combining with biofeedback therapy on senile chronic insomnia patients. Methods 150 cases of senile chronic insomnia were divided into 3groups randomly: group A received acupuncture therapy only, group B received biofeedback therapy only, and group C received both therapies. The scores of Pittsburgh Sleep Quality Index (PSQI) ,sleep efficacy,sleep latency, total sleep time and the scores of Hamilton Anxiety Scale (HAMA) and Hamilton Depression Scale (HAMD-17) were assessed before the treatment and at the end of the 4th and 8th week during the treatment.Results After 8 weeks treatment, the scores of PSQI, sleep efficacy, sleep latency, total sleep time and the scores of HAMA and HAMD-17 of group C((7.92 ±2.59)score,(82.52 ±8.93)% ,(24.06 ±8.23)minutes,(413.75± 42.41) minutes, (9.63 ± 3.75) score, (10.10 ± 3.27) score) were better than that of group A (( 9.51 ± 2.92)score, (79.06 ± 10.70) %, (33.16 ± 11.31) minutes, (373.47 ± 40.65) minutes, (15.08 ± 4.20) score, (14.33±± 3.56) score) and group B (( 11.46 ± 3.75) score, (68.85 ± 12.34) %, (33.65 ± 11. 38) minutes, (281.88 ±38.02) minutes, (11.63 ± 4.15) score, (12.08 ± 4.08) score) significantly (P < 0.01). Conclusion The therapy of acupuncture combining with biofeedback is benefit to improve the sleep quality of senile chronic insomnia patients,meanwhile the anxiety and depression associated with insomnia can be improved.
2.Effect of postural reduction and balloon dilation on correction of vertebral height by percutaneous balloon kyphoplasty

Objective To explore the effect of postural reduction and balloon dilation on the correction of vertebral height following balloon percutaneous kyphoplasty (PKP) for osteoporotic vertebral coinpressive fracture (OVCF).Methods From June 2012 to December 2013,47 OVCF patients involving 57 fractured thoracolumbar vertebrae with intact posterior wall underwent PKP.In all patients postural reduction was performed under general anesthesia.The percentages of vertebral height loss before and after balloon dilation were compared.The curative effects were evaluated by comparing preoperative and postoperative visual analogue scale(VAS) scores and cobb angles.Results Compared with preoperation,the vertebral heights of anterior and middle columns were significantly improved after postural reduction,balloon dilation and PKP (P < 0.05),but there was no significant improvement before and after balloon dilation (P >0.05).The postoperative average cobb angle (22.8°± 8.1°) was significantly decreased compared with the preoperative one (22.8° ± 8.1°) (P < 0.05).The VAS scores at preoperation,one day postoperation and the final follow-up were respectively 8.5 ± 0.4,3.4 ± 0.2 and 3.1 ± 0.3.The postoperative ones were significantly lower than the preoperative one (F =7.518,P =0.006).Conclusions In PKP,postural reduction under general anesthesia plays an important role,but balloon dilation may exert an insignificant effect on restoration of vertebral height.
3.Development of portable medicine coffer for medical support

To develop a portable medicine coffer for medical support in order to standardize the management of special medicine. The coffer with double-fingerprint cryptography and internal mechanical lock was com-posed of a body and a cap. The coffer had the body made of high-intensity plastics or light alloy, monitor made of high-intensity abrasion-resistant glass as well as rechargeable lithium battery. The coffer eliminated the deficiencies of traditional medicine chest. The portable coffer standardizes the management of special medicine for medical support.
4.Observation on the effect of decompression in the treatment of large jaw cystic lesions

Objective To investigate the clinical efficacy and safety of decompression in the treatment of large jaw cystic lesions.Methods 42 patients suffered from large jaw cystic lesions were randomly divided into two groups,21 patients in the control group performed the traditional curettage surgery;21 patients in the experiment group performed the decompression surgery.The intraoperative blood loss,operative time,the incidence rate of postoperative pain and postoperative infection,follow-up recurrence rate were compared between the two groups.Results The intraoperative blood loss,operative time,postoperative pain and postoperative infection rates of the experiment group were significantly better than the control group,the differences between two groups were significant(P < 0.05).The recurrence rate of the control group and the experiment group was 24.0%,4.8%.The recurrence rate of the experiment group was significantly lower than the control group (P < 0.05).Conclusion Compared to the traditional curettage surgery,decompression surgery can reduce blood loss,shorter operative time,reduce postoperative infection,reduce the risk of recurrence,then improve the clinical efficacy.
5.Diagnostic utility of neutrophil CD64 and C-reactive protein as biomarkers of burn sepsis

Objective To investigate the utility of combined test of neutrophil CD64 and C-reactive protein(CRP)for early diag-nosis of burn sepsis.Methods A prospective study was conducted in 76 hospitalized burn patients,including three groups:burn sepsis(n=27),local infection(n=31)and controls without bacterial infection (n=18)based on their diagnosis.Blood samples were collected within 24-48 h after hospital admission for analysis of neutrophil CD64,CRP,white blood cell count (WBC), neutrophils percentage (N)by flow cytometry,and bacterial culture.These parameters were re-evaluated for the patients in sepsis group 7 and 14 days after antibiotic therapy.Results Neutrophil CD64,CRP,WBC and N were significantly higher in sepsis patients than the corresponding values in the patients with local infection or control patients (P < 0.01 ).Neutrophil CD64 and CRP in the patients with local infection were significantly higher than those in the control patients without bacterial infection (P < 0.01).Neutrophil CD64 was positively correlated to CRP.Neutrophil CD64 and CRP decreased 7 days after an-tibiotic therapy,but the difference was not statistically significant compared to the pretreatment levels.Neutrophil CD64 and CRP decreased significantly 14 days after antibiotic therapy compared to the pretreatment levels (P < 0.01),but still higher than the levels in the control group.The receiver operating characteristic (ROC)curve of CD64 + CRP combined test,CD64, CRP,WBC,N for detecting bacterial infection showed an area under curve (AUC)of 0.952,0.923,0.904,0.756 and 0.728,respectively.Conclusions Neutrophil CD64 is useful in early diagnosis of burn sepsis.The expression of neutrophil CD64 does not have significant difference between gram-positive and gram-negative bacterial infections.Combined test of neutrophil CD64 and CRP can improve the efficiency for diagnosis of burn sepsis.
6.Study on modified guar gum microsphere as a protein drug carrier.

A biodegradable modified guar gum microsphere was prepared for the first time by ionic gelation of the guar gum derivative containing quarternary ammonium group with tripolyphosphate at room temperature in the absence of emulsifying agent or organic solvent. Its average particle diameter was about 140 microm and the particle size had a narrow and normal gauss distribution. From the loading experiment of bovine serum album (BSA) with various concentrations, it was found that the encapsulation efficiency is more than 80%. By the investigation of in vitro release from the BSA-loaded microsphere, it was found that the BSA had a continuous release for more than 6 hours and the release percentage was affected by the initial concentration of the BSA and temperature.

7.Cephalometric analysis of the soft tissue profile in Hunan Han adults with normal occlusion.

OBJECTIVE:
To analyze the characteristics soft tissue profile in Hunan Han adults with normal occlusion and to explore the differences of the soft tissue profile in gender, region and race.
METHODS:
Lateral cephalometric radiographs were performed for 323 volunteers (164 females and 159 males) from Hunan Province. The digital cephalometric radiographs were imported into the WinCeph8.0 measurement and analysis system in a same computer. According to the Holdaway analysis method, the mean and standard deviation were calculated. Then the results were compared to the data from Liaoning, Nepalese, Japanese, North American Caucasians.
RESULTS:
The normal value of cephalometric facial soft tissue profile in Han population adults with normal occlusion in Hunan Province were obtained. There were significant differences in subnasale to H line (Sn-H), upper lip tension, lower lip sulcus depth (Si-PgsLs), and soft tissue chin thickness (Pg-Pgs) between males and females (all P<0.05). Compared with Liaoning Province, significant regional differences in soft tissue facial angle (FH-NsPgs), upper lip sulcus depth (Ss-Ls), Sn-H, lower lip thickness, upper lip thickness, H angle, lower lip to H line (Li-PgsLs), Si-PgsLs, and Pg-Pgs were found in Hunan Province (all P<0.05). Compared with the Japan, significant regional differences in FH-NsPgs, nose prominence (Sn-Pn), lower lip thickness, upper lip tensity, H angle, Li-PgsLs, Si-PgsLs, and Pg-Pgs were found in Hunan Province (all P<0.05). Compared with the Nepalese, significant regional differences in Sn-Pn, Ss-Ls, Sn-H, skeletal profile convexity (A-NPs), basic upper lip thickness, upper lip tensity, H angle, Li-PgsLs and Pg-Pgs were found in Hunan Province (all P<0.05). Compared with the North America, significant regional differences in Sn-Pn, Ss-Ls, Sn-H, A-NPs, basic upper lip thickness, upper lip tensity, H angle, Li-PgsLs, Si-PgsLs and Pg-Pgs were found in Hunan Province (all P<0.05).
CONCLUSION
There are both similarities and differences in facial soft tissue profile between males and females with normal occlusion in Hunan Province. The characteristics of facial soft tissue profile in Hunan Province are different from that in Liaoning area, Nepal, Japan and North America.

8.Strategies of online assessment method of medical education based on SWOT analysis

9.Study on computed tomography features of nasal septum cellule and its clinical significance.

OBJECTIVE:
To investigate the features of nasal septum cellule in computed tomographic (CT) images and its clinical significance.
METHOD:
CT scans data of nasal septum in 173 patients were randomly obtained from January 2001 to June 2005. Prevalence and clinical features were summarized in the data of 19 patients with nasal septum cellule retrospectively.
RESULT:
(1) Nineteen cases with nasal septum cellule were found in 173 patients. (2) All nasal septum cellule of 19 cases located in perpendicular plate of the ethmoid bone, in which 8 cases located in upper part of nasal septum and 11 located in middle. (3) There were totally seven patients with nasal diseases related to nasal septum cellule, in which 3 cases with inflammation, 2 cases with bone fracture, 1 case with cholesterol granuloma, 1 case with mucocele.
CONCLUSION
Nasal septum cellule is an anatomic variation of nasal septum bone, and its features can provide further understanding of some diseases related to nasal septum cellule.

10.Free graft of vascularized iliac bone flap based on deep iliac circumflex vessels for the treatment of os-teonecrosis of the femoral head

To investigate the feasibility and clinical efficacy of free graft of vascularized iliac bone flap based on deep iliac circumflex vessels for the treatment of osteonecrosis of femoral head (ONFH). Methods Twelve cases of ONFH(13 hips) received free graft of vascularized iliac bone flap based on deep iliac circumflex ves-sels from April, 2016 to July, 2017.The average Harris score was (74.7±6.9) in the second stage and (68.6±9.2) in the third stage. After debriding the necrosis bone of the femoral head, the contralateral vascularized iliac bone flap had been harvested, and then implanted into the femoral head. The deep iliac circumflex artery and its accompanying vein were anastomosed with the transverse branch (or ascending branch) of the lateral circumflex femoral artery and con-comitant vein respectively. The herringbone brace was used for hip fixation for 3 weeks after operation. X-ray exami-nation (1, 3, 6, 9 and 12 months after operation, respectively) and Harris hip score (6 and 12 months after operation, respectively) were performed to evaluate the recovery results of the femoral head. Scores were recorded and analyzed by paired t-test.The difference was considered to statistically significant if P<0.05. Results The patients were fol-lowed-up for 15(12-20)months. The iliac bone flap of 12 patients (13 hips) healed well. There was no necrosis and collapse in 12 hips, except 1 femoral head collapsed slightly because of weight loading too early. The average Harris score was (91.6±4.5) of the second stage and (84.8±6.1) of the third stage. Compared with scores before the operation, the difference was statistically significant (P<0.05). Conclusion Free vascularized iliac bone grafting based on deep iliac circumflex vessels is an ideal treatment for ONFH head in middle and advanced stage.
