1.Screening for NGF Active Chinese Medicinal Herbs and Their Active Components

It has been revealed that nerve growth factor (NGF ) deficiency is the main cause of some kinds of neuron degenerative diseases such as Alzheimer's disease, and NGF administration can protect neuronal function decay and promote nerve regeneration. Moreover,NGF actions are mediated by its receptors. In the present study, 125I-NGF receptor binding assay was used as a model for the screening of NGF receptor active components from 14 Chinese medicinal herbs. It was found that a component from the root of Achyranthes bidentata BI. possesses such inhibitory activity with IC50 = 6. 18? 3.43 ?g/ml.
2.Diagnosis and treatment of adrenocorticotropic hormone-independent macronodular adrenocortical hyperplasia

Twelve cases of adrenocorticotropic hormone-independent macronodular adrenocortical hyperplasia were retrospectively reviewed.All cases had physical signs and biochemical evidence of Cushing syndrome. Urinary free cortisone(UFC)levels were not suppressed by high dose and low dose dexanethasone suppression tests,and plasma adrenocorticotropic hormone levels were low in all patients.CT scan showed bilateral enlarged adrenal glands in all cases,and histopathologic examination confirmed bilateral adrenal macronodular hyperplasia. The follow-up revealed that 4 patients had a normal blood pressure 8y after bilateral adrenalectomy. In 8 cases the blood pressure was rebound 3y after single side adrenalectomy,6 of whom had medical treatment; 2 of whom had contralateral adrenalectomy and the blood pressure returned to normal afterwards.There was no Nelson syndrome in all cases.
3.Effect of lanthanum chloride on the proliferation and expressions of cyclin D1 and cyclin dependent kinase 4 in rat hepatocellular carcinoma CBRH-7919 cells

BACKGROUND: It has been proved that the rare earth compounds can obviously restrain the growth of tumor in vitro and in vivo.OBJECTIVE: To check the effect of chlorinated lanthanum (LaCl3) on the expressions of cyclin D1 and cyclin dependent kinase 4 (CDK4) of hepatocellular carcinoma cells.DESIGN: Controlled observation.SETTING: Department of Nephropathy, First Clinical Hospital, Jilin University. MATERIALS: The experiment was carried out in the cell culture laboratory (province-level) of Department of Histology and Embryology, Jilin University from July 2002 to June 2003. Wells of the microwell plate seeded with CBRH-7919 cells were randomly divided into 4 groups according to the dose of LaCl3: control, 0.01, 0.1, 1.0 mmol/L LaCl3. And each group was further divided into three subgroups according to the culture time: 1, 3, 5 days, with 6 wells for each.METHODS: The growth of CBRH-7919 cells was monitored following treated with 0.01, 0.1, 1.0 mmol/L LaCl3 for 1, 3, 5 days in vitro, respectively. ① Absorbance(A) of each well was measured by MTT assay.② The changes of distribution of cells into different phases of cell cycle were detected by flow cytometry. ③ Cyclin D1 and CDK4 expression was detected by immunocytochemical analysis.MAIN OUTCOME MEASURES: The expression of cyclin D1 and CDK4 in CBRH-7919 cells of different doses of LaCl3 at different time.RESULTS: Effect of LaCl3 on the growth of CBRH-7919: On days 3 and 5, the A value of each well in the 0.1 and 1.0 mmol/L LaCl3 groups was respectively significantly lower than that in the control group (day 3: 1.140±0:070, 0.706±0.092,1.461±0.087; day 5: 1.888±0.020,0.625±0.037,2.544±0.032, respectively) (P < 0.05,0.001 ). ②Effect of LaCl3 on the percentage of CBRH-7919 cells at G0/G1 stage: On days 1, 3, and 5, the percentage of CBRH-7919 cells at G0/G1 stage in the 1.0 mol/L LaCl3 group was respectively significantly higher than that in the blank control group [(60.70±0.20)%, (39.49±0.67)%; (61.66±0.97)%, (45.56±1.00)%; (69.92±0.18)%, (49.24±0.27)%, (P < 0.01,0.001 )]; On day 5, the percentage of CBRH-7919 cells at G0/G1 stage in the 0.1 mol/L LaCl3 group was respectively significantly higher than that in the blank control group[(58.88±0.73)%, (49.24±0.27)%, P < 0.05]. ③ Effect of LaCl3 on the expressions of cyclin D1 and CDK4 of CBRH-7919 cells: On day 1, the expressions of cyclin D1 and CDK4 of CBRH-7919 cells in the 0.1 and 1.0 mmol/L LaCl3 groups were respectively lower than those in the blank control group (expression of cyclin D1: 562±35,453±22,860±82;expression of CDK4: 705±84,680±28,762±16, P < 0.05, 0.01 and 0.001).CONCLUSION: LaCl3 inhibited the growth of CBRH-7919 cells correlating down-regulating cyclin D1 and CDK4 as reflected on the cell cycle arrest of CBRH-7919 cells from G1 stage to S stage.
4.Application of endoscopic miniprobe ultrasonography in diagnosis and therapy of gastrointestinal submucosal tumors

Objective To evaluate the diagnostic value of endoscopic miniprobe ultrasonography (MPS) and endoscopic therapies under the guidance of MPS in gastrointestinal submucosal tumors. Methods Patients with submucosal tumors of lower digestive tract were given MPS under colonoscope. Endoscopic therapy or surgery had been taken according to different deriving layers of submucosal tumors. The ultimate diagnosis was confirmed by histology. Results Twenty patients with submucosal tumors were diagnosed with MPS under colonoscope, including 9 patients with stromal tumors, 5 patients with lipoma, 3 patients with car-cinoid, 2 patients with abscess,1 patient with intestines pneumatosis. Four patients with benign stromal tumors derived from muscularis mueosa and 2 patients with carcinoid were resected by endoscopy. Two patients with abscess and 1 patient with intestines pneumatosis were puncture under endoscopy. Three patients with benign stromal tumors from muscularis propria, 2 patients with malignant stromal tumors, 2 patients with lipama and 1 patient with careinoid were resected surgically. 88.24% submucosal tumors diagnosed by MPS were identified by pathology. Three cases were followed-up by endoscopy. Conclusions Different layers of gastrointestinal tract with submucosal tumors can be distinguished clearly by MPS leading to definite diagnosis of submucesal tumors. MPS can play an important role in selecting treatment procedures.
5.Role of TPSA, F/T and PSAD in diagnosis of prostate cancer

Objective To investigate the diagnostic value of serum TPSA, the ratio of free PSA(FPSA)and TPSA(F/T) and PSAD in prostate cancer(PCa).Methods The serum TPSA and PSAD levels and the ratio of F/T of 467 patients with BPH,116 patients with PCa and 50 healthy men as normal control were determined, analyzed and compared. Results Serum TPSA of PCa (53.26±33.10) were significantly higher than that of BPH (8.12±9.70) (P <0.01), and normal control (1.51 ±1.17) (P <0.01); Serum PSAD of PCa[(1.59±1.46) ng·ml-1·cm-3] were significantly higher than that of BPH [(0.14±0.17) ng·ml-1·cm-3] (P <0.01),and normal control [(0.08±0.07) ng·ml-1·cm-3] (P <0.01); F/T of PCa (0.22±0.16) was significantly lower than that of BPH (0.27±0.15) and normal control (0.36±0.14) (P <0.01). When TPSA was limited specificity at 4 ~10 ng/ml, F/T of PCa (0.18±0.13) were significantly lower than that of BPH (0.27±0.14), (P <0.05), PSAD of Pca [(0.21 ±0.07) ng· ml -1· cm -3] were significantly higher than that of BPH [(0.11 ±0.06) ng· ml-1· cm-3], (P <0.001). When F/T was 0.16, PSAD used 0.15 as the critical value, the sensitivity, specificity of diagnostic value and positive prediction value were 81.6 %, 78.2 %, 96.1% and 53.8 %, 76.9 %, 97.9 %, respectively;And the diagnostic efficiency was the highest. ConclusionF/T and PSAD have valuable role as the diagnose marker of PCa. When PSA level between 4 ng/ml and 10 ng/ml, F/T ratio and PSAD are of significant value to differentiate PCa from BPH patients.
6.Clinical analysis of adrenal ganglioneuroma: a report of 21 cases

The clinical data of 21 cases of adrenal ganglioneuroma treated in our hospital from July 1988 to August 2008 were reviewed. The clinical manifestations were: lower back uncomfortableness (9 cases), paroxysmal dizziness (4 cases), central obesity ( 1 case) and 7 cases were discovered incidentally.Five cases were diagnosed pre-operationally and remaining 16 cases were diagnosed post-operationally by pathology. All patients completely recovered after surgery except one, who still had dizziness with high blood pressure. Ultrasonography, CT and MRI examinations are the main diagnostic methods for adrenal ganglioneuroma, and it is confirmed by histopathology, surgical removal of the tumor is the main therapy for the disease.
7.Relationship of operation time point with therapeutic effect in central cervical spinal cord injury syndrome

Objective To evaluate the correlation of interval from injury to surgery with outcome for central cervical spinal cord injury syndrome.Methods A retrospective analysis was done for 31 patients with central cervical spinal cord injury syndrome treated surgically between March 2007 and March 2011.According to the timing of surgical intervention,the patients were divided into early operative group (<7 days from injury,n =14) and late operative group (≥7 days from injury,n =17).Japanese Orthopedic Association (JOA) score on admission,at postoperative half-year and at final follow-up were recorded and compared between the two groups.JOA recovery rate was determined and analyzed for correlation with operation time point.Results Mean follow-up was (16.1 ± 4.5) months (range,12-25 months).All patients had different degree of neurological recovery.JOA recovery rate in early operative group was apparently better than that in late operative group at final follow-up (P < 0.01).JOA recovery rate at follow-up was negatively correlated with intervals between injury and surgery (r =-0.763,P < 0.01).Conclusion For patients with central cervical spinal cord injury syndrome,time from injury to surgery is negatively correlated with outcome,indicating that early surgery may be associated with better outcomes.
8.Structural bone graft for treatment of complicated distal radius fractures and bone defects

ObjectiveTo investigate the curative effect of structural bone graft in treatment of high energy injury-induced complicated distal radius fractures combined with articular surface collapse and comminuted epiphyseal bone defect.MethodsThe study involved 38 patients with distal radius fractures (AO classification, type C3) treated from January 2007 to October 2010.The patients were treated with temporary Kirschner wire fixation to recover the wrist articular surface, distal radius length and palmar tilt angle at the greatest possibility.According to the form of the bone defects, the iliac bone taken from three layers of the cortical bone was embedded in the subchondral bone to support the bone graft, and a small amount of cancellous bone graft was collected to fill the articular surface collapse as much as possible for anatomical reduction of the articular surface, and anatomical form of the distal radius was reconstructed.Modified Sarmiento Gartland and Werley evaluation systems were used to evaluate the articular function recovery.ResultsThe follow-up lasted for 1-3.2 years, which showed no complications such as plate rupture or iatrogenic nerve blood vessels injuries.The fracture healing time was 15-22 weeks (mean 18.3 weeks).The wrist articular function in some patients obtained obvious improvement through functional rehabilitation training.According to the modified Gartland and Werley function evaluation systems, the curative effects reached excellence rate of 87%.During follow-up, there were two patients with high progressive loss in the height of the distal radius and two with articular surface collapse and severe carpal joint flexion and extension dysfunction.ConclusionsThe high energy injury-induced distal radius fractures with articular surface collapse and epiphyseal bone defect need recovery of the wrist articular surface, distal radius length and palmar tilt angle.Structural bone graft can effectively support the grafted bone and maintain the flatness of the radiocarpal articular surface, restore the anatomy of the distal radius and facilitate the functional recovery of the wrist.

Objective To explore effects of monitored anesthesia care (MAC) on gastrointestinal endoscopy.Methods All patients who underwent gastrointestinal endoscopy in Peking University People's Hospital from September 2010 to September 2015 were analyzed retrospectively and divided into two groups:the MAC group and control group by usage of MAC.Success rates of removal of foreign bodies by upper gastrointestinal endoscopy were compared between two groups during this period.Detection rates of ectopia of gastric mucosa in upper esophagus were compared between two groups from September 2014 to September 2015 when esophagogastroduodenoscopy was performed.The monthly cases of gastrointestinal endoscopies were compared between the two groups,which were separately collected from one MAC operating table and one routine operating table from September 2014 to September 2015.Results The success rate of removal of foreign bodies in the MAC group was higher than that in the control group (100.0% VS 89.7%,x2 =11.737,P=0.001).The detection rate of ectopia of gastric mucosa in upper esophagus in the MAC group was higher than that in the control group(4.3% VS 1.7%,x2 =58.751,P<0.001).The monthly cases of gastrointestinal endoscopies in the MAC group was higher than that in the control group(350.2±36.2 cases VS 213.2±27.9 cases,t =17.591,P<0.001).Conclusion The use of MAC for gastrointestinal endoscopy improves operating success rate,accuracy and operating time.MAC in gastrointestinal endoscopy can be recommended.
