1.Antimicrobial resistance profile of the clinical isolates in Fuzhou First People's Hospital, Jiangxi during 2015

Objective To investigate the antimicrobial resistance profile of clinical isolates in Fuzhou First People's Hospital during 2015. Methods? The?bacterial?isolates?were?identified?using?ATB?Reader?semi-automatic?equipment?via?colorimetric?method.?The antimicrobial susceptibility was tested by disk diffusion method. The results were interpreted according to the CLSI breakpoints issued in 2015. All data were analyzed by WHONET 5.5 software. Results A total of 1215 non-duplicate clinical isolates were collected during 2015, including gram negative bacteria (70.6%) and gram positive bacteria (29.4%). The prevalence of MRSA was 50.0 % in S. aureus and the prevalence of MRCNS was 84.1 % in coagulase-negative Staphylococcus. Most MRSA strains (80.3?%)?were?resistant?to?erythromycin,?tetracycline?and?norfloxacin.?No?staphylococcal?isolate?was?found?resistant?to?vancomycin?or teicoplanin. The prevalence of ESBLs was 52.5% in E. coli and 34.7% in K. pneumoniae. Ten strains of Enterobacteriaceae were resistant to carbapenems. Less than10% of the P. aeruginosa isolates were resistant to amikacin, cefoperazone-sulbactam, meropenem and tobramycin. About 2.4% and 51.8% of the Acinetobacter isolates were resistant to polymyxin E and cefoperazone-sulbactam, respectively, while more than 75% of these strains were resistant to other commonly used antimicrobial agents. Conclusions Most of the clinical isolates are gram-negative bacilli in our hospital during 2015. Antimicrobial resistance remains an important clinical concern, which should be addressed seriously.

3.Diagnostic value of 3D-ultrasound for cord entanglement in monoamniotic twins

Objective To discuss the diagnostic value of 3D-ultrasound for cord entanglement in monoam-niotic twins. Methods From November 2013 to March 2016,a total of 65 monoamniotic twinsunderwent routine obstetric ultrasonography to screen fetal abnormalities in our hospital. The cord was meticulously observedusing a systematic continuous sequence approach. Fetuses who showed cord entanglement by 2-D ultrasound or 3-D flow Doppler were all recruited. The gestational weeks,fetal malformation,ultrasonic image feature of cord entangle-ment,and clinicaloutcome were recorded and analyzed retrospectively. Results 4 monoamniotic twins showed cord entanglement. Three-dimensional color Doppler clearly delineated thefeatures of entanglement. At following-up of 8 fetuses,3 fetuses were found intrauterine fetal death and 3 fetuses with structural abnormalities except for 2 well infants.Conclusion Cord entanglement may threaten the safety of fetusesin monoamniotic pregnancies.In aid of 2-D ultrasound or 3-D flow Doppler in monoamniotic twins can help to find it in time ,monitor it carefully and provide evidence for obstetric treatment .
4.Inhibitory effect of ophiopogonin-B on adhesion, invasion and migration of A549 cells in vitro

Aim To explore the inhibitory effects of ophiopogonin-B (OP-B ) on cell adhesion,invasion and migration in non-small cell lung cancer (NSCLC) A549 cells in vitro and its possible mechanism.Meth-ods Cell adhesion assay and transwell chamber assay were used to detect the ability of cell adhesion,migra-tion and invasion.qRT-PCR was used to detect the mRNA levels of MMP-2 and 9 .Meanwhile ,Western blot assay was performed to determine the protein levels of MMP-2/9 and p-Akt.Results OP-B significantly inhibited the ability of cell adhesion,invasion and mi-gration in A549 cells at the concentration of 10 μmol· L-1 (P<0.01 ).Meanwhile,it inhibited the mRNA and protein levels of MMP-2 and MMP-9 and down-regulated the phosphorylation of Akt (P <0.0 1 ). Conclusion OP-B inhibits cell adhesion,invasion and migration in A549 cells through down-regulation of the mRNA and protein expression of MMP-2/9 ,and the inhibitory effect on the expression of p-Akt.
5.Effects of mechanical ventilation on cell apoptosis in lung tissue of rat with acute lung injury

Objective To investigate the effects of different tidal volumes and positive end expiratory pressures on cell apoptosis in lung tissue of rats with acute lung injury.Methods Forty healthy male Sprague-Dawley rats were randomly(random number)divided into five groups,namely low tidal volume group(LV,VT 8 mL/kg),high tidal volume group(HV,VT 30 mL/kg),low tidal volume group with PEEP 2cmH2O(LV2P,VT 8 mL/kg,PEEP 2 cmH2O),low tidal volume group and PEEP 5cmH2O (LV5P,VT 8 mL/kg,PEEP 5 cmH2O)and low tidal volume group and PEEP 8 cmH2O(LV8P,VT 8 mL/kg,PEEP 8 cmH2O).After intravenous administration of oleic acid(OA,0.1 mL/kg),the rat model of acute lung injury was made.Mechanical ventilation was employed in rats of the experiment groups.Rats were sacrificed and their whole lungs were taken after mechanical ventilation for 2 hours.The transferase d-UTP end labeling assay(TUNEL)was used to define the extent and distribution of apoptotic cells in bronchus and lung tissues.The level of caspase-3 protein was determined by immunohistochemistry.Results The apoptotic cells on both alveolar septum and bronchial epithelium obviously increased with high level of caaspase-3 protein in HV group.The number of apoptotic cells obviously decreased with decrease in caspase-3 protein after PEEP ventilation.These changes were more significant in LV5P than those in other groups(P < 0.01).Conclusions The mechanical ventilation with low tidal volume and PEEP produces protective effects on lung from injury.The cell apoptosis plays an important role in the course of VILI.
6.Preparation of a deactivation vaccine against Aeromonas hydrophila and research about its immue effect in the north of Henan

Objective:Selected virulence factors more than and high virulent Aeromonas hydrophia strain made into inactivated vaccine,to study the immunization effect of inactivated vaccine.Methods:Crucian were vaccinated with formalin-killed vaccine via in-traperitoneal injection.Controls were injected with the same volumes of saline.Then the antibody titres, histopathology and relative percent survival were analyzed from samples of both groups.Results: The antibody in the indirect agglutination reaction could be detected in vaccinated fish once a weeks after immunization and reached highest level 6 weeks after immunization.The histopathology analysis indicated that the vaccine had a good protective effect on crucian target organs.Vaccinated fish showed 100%relative percent survival and the immune period would be 6 month.Conclusion:The vaccine in this study has a significant protective effect on crucian and may be used as effective fish vaccines against bacterial septicemia.
7.Clinical features of late-onset neuromyelitis optica spectrum disorders

Objective To explore the clinical features of late-onset neuromyelitis optica spectrum disorders (LON?MOSD). Methods A retrospective analysis was performed to evaluate 61 patients with LONMOSD admitted to our hospital from January 2010 to May 2015. Results (1) The median age at onset was 57 (53, 63) years, male/female was 1∶3.7. Thirty-two patients (52.5%) had transverse myelitis (TM) and 16 patients (26.2%) had optic neuritis (ON) at the disease onset. Fifty-one patients (83.6%) experienced recurrent attacks. Forty patients (65.6%) showed abnormal brain magnetic resonance imag?ing (MRI). Spinal cord MRI showed more frequently present in thoracic regions (39.3%). (2) There were no significant differ?ences in clinical features between AQP-4 seropositive and seronegative groups. (3) By Spearman analysis, it was obvious that EDSS scores at acute phase and remission were positively correlated to AQP-4 antibody levels (rs=0.389, P<0.01;rs=0.380, P<0.01). But there was no correlation between the mean segments of spinal cord and relapse rates with AQP-4 anti?body levels (rs=0.146, P>0.05;rs=0.096, P>0.05). Conclusion LONMOSD patients are more prone to present with TM at onset and have more lesions in thoracic spinal cord and brain. The AQP-4 antibody titres can indicate the severity of disease in acute phase.
8.Method and significance of ultrasonographic assessment of the uterine weight before pelvic floor reconstruction

Objective To investigate the accuracy of ultrasonographic assessment of the total uterine weight and the feasibility of using this method in the pelvic floor reconstruction.Methods Firstly,81 cases with hysterectomy due to benign uterine diseases or uterine prolapse were studied.The preoperative dimensions and gravities of corpus uteri and cervix were calculated by formulas,and were then compared with the postoperative measurements.Subsequently,46 cases with pelvic floor reconstruction and preserved the uterus were subjected to retrospectively analysis of uterine measurement parameters.Results There were no statistically differences between the preoperative and postoperative diameters of corpus uteri and cervix (P > 0.05),and no statistically differences between the weight of corpus uteri and cervix estimated by the formulas [(87±55),(32±6) g] and the true weight [(88±57),(33±6) g; P>0.05].In 46 cases that underwent the pelvic floor reconstruction by transvaginal mesh repair and preserved the uterus,42 cases were successful treated and the average weight of total uterus was (49 ± 13) g (95%CI:39.90-49.88 g) ; the 4 relapsed cases were treated with hysterectomy and the weight of total uterus were 85.24,82.69,92.67 and 120.06 g which were consistent with the weights estimated by the formulas (87.36,82.00,90.88,123.12 g ; all P > 0.05).Conclusions The uterine weight might be a significant factor for uterus preservation in pelvic floor reconstruction,while ultrasonographic assessment can accurately estimate the uterine weight preoperatively.All these raised the feasibility of assessing uterine weight preoperatively in pelvic floor reconstruction.
9.Curved planar reformation of the facial nerve canal with multislice spiral CT

Objective To study the methods of delineating the whole length of bilateral facial nerve canals in one image. Methods High resolution computed tomography of the temporal bone was performed in 60 cases (120 ears) by Philips Mx8000 multislice spiral CT. Parameters: 120 kV, 200~250 mAs, Collimation: 0.5 mm, Pitch: 0.875, Scan time: 0.75 s/ring, Matrix: 512?512, Reformation interval: 0 1~0.2 mm, Reformation matrix: 1 024?1 024. Curved planar reformation(CPR)images were prepared along the facial nerve canal in the axial plane, and in the coronal and sagittal plane of multiplanar reconstruction(MPR). In the axial plane, a reference line was traced following the facial nerve canal from the internal auditory meatus, through the labyrinthine segment, the tympanic segment up to the second genu and mastoid segment. Another two protocols of curved reformatting were adopted: (a) a curved line was delineated along the facial nerve canal on the coronal reformatted image; (b) a curved line was drawn along the facial nerve canal on the sagittal reformatted image. The reference lines were carefully revised and moved exactly to the center of each segment of the facial nerve canal. For displaying bilateral facial nerve canals in one image, one reference line should be drawn along bilateral facial nerve canals. Results In 56 cases of 60 CPR, images in the axial plane, and coronal plane of MPR could show the unilateral or bilateral facial nerve canals clearly. The result of CPR of bilateral facial nerve canals in sagittal plane of MPR was unsatisfactory. The image on one side was often clear, but just part of it could be showed on the other. So the left and right facial nerve canals should be reformed separately. In 4 cases, CPR was unsatisfactory. In 1 of them the labyrinthine and tympanic segment had breaks because the patient′s head shook during the scanning. In 3 of them the facial nerve canals were showed unsatisfactorily because of the inexact position of head during the scanning. Conclusion The unilateral or bilateral facial nerve canals could be showed in one image by CPR. Combining the images of the high resolution CT in axial scan and MPR of the temporal bone, it would be helpful in the diagnosis of the lesions of facial nerve canal.
10.Perinatal diagnosis and treatment of congenital bronchogenic cysts in infants

Objective To discuss the diagnosis and early intervention treatment of fetal congenital broncho-genic cysts based on the cases reviewed. Methods The clinical features of 7 infants presenting bronchogenic cysts diagnosed antenatally from January 2013 to May 2014 in Guangdong Women and Children's Hospital and Health In-stitute were reviewed retrospectively. Pathology,the prenatal diagnosis and treatment of bronchogenic cysts experience were summarized combined with CT after birth and surgery. Results Based on the prenatal diagnosis of fetal and postnatal CT and surgical pathology,a total of 7 cases with congenital bronchial cysts were diagnosed. Of which 4 ca-ses were suggestive of congenital cystsic adenomatoid malformation by prenatal diagnosis,and the other 3 cases had fetal bronchial cysts by prenatal diagnosis,antenatal diagnosis was accurate in 42. 9%(3 / 7 cases). CT examinations were taken in 7 cases after birth,and the cyst excision was performed on them with surgery thoracic approach;the average age at surgery was(5. 3 ± 1. 7)months. Four cases had simple bronchial cystss,2 cases with congenital cystsic adenomatoid malformation,1 case with congenital pulmonary sequestration. The accuracy of CT diagnosis was 85. 7%(6 / 7 cases). All surgical treatment was effective. Conclusions Fetal bronchial cysts is always associated with the other presence of lung congenital malformations. Prenatal diagnosis of congenital bronchial cysts is difficult. Prenatal diagnosis is difficult to exclude congenital cystsic adenomatoid malformation. To avoid symptoms like oppres-sion,infection,prenatal diagnosis combined with CT examination after birth and early treatment are necessary and reliable clinically.
