1.Survey of nursing service needs and satisfaction in gynecological outpatient undergoing surgery

Objective To investigate the nursing service needs of gynecological outpatient undergoing surgery as well as the level of satisfaction.Methods Eighty-eight patients undergoing surgery in the outpatient department of our hospital from March 2011 to September 2012 involved in the survey by a self-designed care service needs and satisfaction scale to investigate their needs for nursing service at pre-,intra-,and post-operation as well as their level of satisfaction with nursing service.Results The total score on nursing service needs was(3.39±0.35),falling between those on the needs and the strong needs.The needs at pre-,intra-, and post-operation were multifold.The total score on satisfaction with nursing service was(3.34±0.48),falling between those on the moderate satisfaction and great satisfaction,among which they were most unsatisfied with waiting longer than 30 mins preoperatively. Conclusions The outpatients for obstetric and gynecological consultation have needs of different aspects and the hospitals can generally meet their needs.The pertinent nursing service should be done based on the preoperative,intraoperative and postoperative needs for care service so as to improve their satisfaction.

3.Clinical Observation on Therapeutic Effect of Breviscapine Injection on Migraine

OBJECTIVE:To observe the therapeutic effect of Breviscapine injection in treating migraine. METHODS: 81 patients with migraine were randomly divided into two groups: 42 in treatment group were treated with Breviscapine injection 20 mL added to 5% Glucose injection 300 mL iv gtt q.d, and 39 in control group with Flunarizine 10 mg po quaque nocte. The treatment lasted for 2 courses (1 course of treatment lasted 14 days) with an interval of 15 days between the two courses. The patients were followed for more than 6 months. The curative effects between the 2 groups were compared, and the variations in hemorheological indexes of the 2 groups before and after treatment were observed. 4.Effects of tolterodine on detrusor hyperreflexia for patients with spinal cord injury

ObjectiveTo evaluate the efficacy and tolerance of tolterodine in treating detrusor hyperreflexia in patients with spinal cord injury.Methods50 patients of spinal cord injury patients with detrusor hyperreflexia were involved in this study.The data from voiding diary were evaluated 7 days before and after the patients being treated with tolterodine 2 mg twice daily.ResultsAfter 12 weeks of treatment,the functional bladder volume increased from(114.0±44.3) ml to(207.7±54.9) ml,the mean frequency of micturition decreased from(8.9±2.7) /d to(4.6±2.5) /d,the mean volume of incontinence decreased from(646.7±348.9) ml to(426.6±291.3) ml,the mean frequency of incontinence decreased from(7.8±3.1) /d to(4.8±3.2) /d,which were all statistically different before and after treatment.5 patients felt dry mouth,but they could tolerated it.ConclusionTolterodine is an effective and well tolerable antimuscarinic agent for treating detrusor hyperreflexia after spinal cord injury.
5.Time-related Expression of IL-6、IL-8 and TNF-α Following Explosive Injury to Rabbit’s Chest

Objective To explore the relationship between the expression changes of cytokines, inter-leukin-6(IL-6), interleukin-8(IL-8), and tumor necrosis factor-α (TNF-α), and the wound time following explosive injury to rabbit’s chest. Methods The rabbit’s model of explosive injury was established. The expression levels of IL-6、 IL-8 and TNF-α in the plasma were detected by ELISA method at different wound time (0.5-12 h). Results The level of IL-6 increased at 3 h after wounding and reached peak at 6 h. The level of IL-8 increased at 1 h and reached peak at 6 h. The level of TNF-α increased at 0.5 h and reached peak at 3 h. Conclusion IL-6, IL-8 and TNF-α have a time-related expression after explo-sive injury.

7.Implantation of artificial urinary sphincter for treatment of trauma-induced genuine stress incontinence (report of 2 cases and review of literature)

Objective To evaluate the therapeutic effect of implantation of artificial urinary sphincter (AUS) for treatment of patients with trauma-induced genuine stress incontinence. Methods Two patients (1 male and 1 female) with genuine stress incontinence due to urethral rupture underwent implantation of AUS device (AMS 800).In the male case,the cuff of the AUS device was used to wrap the bulbar urethra.The reservoir was placed into the retropubic space.The controllable pump was implanted into the right scrotum. In the female case,the cuff was used to wrap the neck of bladder.The reservoir was placed into the retropubic space. The pump was implanted into the right subcutaneous labium majus.The literature was reviewed to show the indication,efficacy,complication and durability of implantation of AUS device. Results After operation the male patient was followed up for 17 months,and he achieved complete continence.The female was followed up for 14 months,and she had significant improvement in continence with changing 1 or 2 small pads every day.There were no infection, erosion, urethral atrophy and mechanical failure during follow-up.The review of the literature showed that in patients with implantation of AUS the mean of continence rate was 85.5%;the incidence rates of infection,erosion,urethral atrophy and mechanical failure were 6.1%,7.5%,6.1% and 16.7%,respectively. Conclusions The implantation of AUS is an effective,reliable method for patients with trauma-induced genuine stress incontinence.
8.Botulinum-A toxin injection into detrusor to treat detrusor hyperreflexia and neurogenic incontinence in patients with spinal cord injury

Objective To evaluate the effects of Botulinum-A toxin (BTX-A) injection into detrusor to treat detrusor hyperreflexia and neurogenic incontinence in the patients with spinal cord injury (SCI). Methods A total of 31 patients with SCI(mean age,30 years;male 20,female 11) were included into the study.All the patients underwent urodynamic examination and voiding diary was recorded.300 U of BTX-A was dissolved in 15 ml of saline,and the solution of BTX-A was injected into 30 different points in detrusor using a flexible cystoscopic needle.The evaluation for the effects and follow-up included voiding diary,urodynamic testing and observation of adverse and toxic effects. Results After the first injection,29 of the 31 patients had symptom relief within 3 to 14 days;4 cases received the second injection;4 cases were treated in combination with anticholinergic medication;and 2 cases had no improvement and,therefore,received other treatments.The mean follow-up was 8.5 months. After 3-week treatment of BTX-A,the mean frequency of incontinence decreased from 14.2 to 2.5 times per day.The mean volume of intermittent catheterization (IC) increased from 124 to 495 ml each time.Urodynamic data showed that mean cystometric bladder storage volume increased from 133 to 475 ml,mean maximum storage detrusor pressure decreased from 62.7 to 17.1 cmH 2O(1 cmH 2O=0.098 kPa).No adverse and toxic effect was observed. Conclusions Our initial experience suggests that Botulinum-A toxin injection into detrusor to treat detrusor hyperreflexia and neurogenic incontinence in patients with SCI may be an effective,safe,feasible and micro-invasive treatment choice.However,it is necessary to observe its long term outcome.
9.Application of evidence-based medicine in the department of neurology

Objective To explore ways of enhancing the therapeutic level of the neurology department and reducing the economic burdens of patients. Methods Relevant materials were collected by means of evidence based medicine to optimize therapeutic schedules, with cerebral infarction being the pitching in point. Results Prior to the adoption of evidence based medicine, the average length of stay, the effective rate of treatment and the expenses for single entity diseases were respectively 17.83?13.44, 89.91% and 4 550.70 yuan, whereas after the adoption of evidence based medicine, they were respectively 15.38?11.37, 94.16% and 3 942.88 yuan. Conclusion Evidence based management is a means of scientific management. Its application in the department of neurology has produced a marked effect and it is worthy to be spread.
10.Application of Trospium Chloride in Treatment of Overactive Bladder (review)

Trospium chloride is an anticholinergic agent with predominantly peripheral nonselective antimuscarinic activity and water solubility. It has potential therapeutic value in decreasing detrusor contracivity and tonity, so as to increase predominantly mean maximal bladder capacity and compliance. Multicenter studys showed that the tolerability and efficacy of trospium chloride was as same as oxybutynin and better than toterodine. Adverse events of trospium chloride is the same as other anticholinergic agents, however its specially constitutional feature so as not to have toxicity to central nervous system. Trospium chloride has potential therapeutic value in the neurourological rehabilitation.
