1.Influence of empowerment management on nursing quality in blood pnrification center

ObjectiveTo analyze the effect of empowerment management for nursing risk in blood purification center.Methods16 nurses were chosen and nursing risk were authorized to six management groups,each group was respectively responsible for their share of nursing work.Before the implementation of empowerment management for nursing risk,100 patients undergoing maintenance hemodialysis were chosen,and after the implementation another 100 patients were also selected.The nursing effect was compared before and after the implementation.ResultsAfter the implementation,the score of nursing quality and patients' satisfaction degree were significantly improved.ConclusionsThe effective empowerment management for nursing risk can raise management effect of nurses continuously,and stir up the initiative and creativity of nursing staff,lower nursing risk occurrence.It is an effective management method that promotes nursing quality comprehensively.
2.Correlation study of serum hs-CRP, UA level and the correlation of disease in elderly acute myocardial infarction patients

Objective To investigate correlation study of serum hs-CRP,UA level and the correlation of dis-ease in elderly acute myocardial infarction patients , and provide a reference basis for clinical judgment for patients with illness development and prognosis prediction .Methods 40 aged patients with acute myocardial infarction pa-tients were selected as the research object ,and protionally random according to time admission the same period ran-domly selected were 40 cases of elderly patients with unstable angina pectoris ,40 cases of elderly patients with stable angina,40 cases of clinic medical normal healthy old people for controlled object .The levels of serum hs-CRP,UA and correlation was analyzed.Results The serum hsCRP,UA in the myocardial infarction group were (34.67 ±12.18)mg/L, (412.46 ±68.93)μmol/L,which in the unstable angina pectoris group were (17.21 ±8.39) mg/L,(274.19 ± 46.62)μmol/L,which in stable angina pectoris group were (5.25 ±1.40) mg/L,(297.41 ±37.43)μmol/L,which in the health group(4.96 ±1.24)mg/L,(279.64 ±34.78)μmol/L,the differences among the myocardial infarction group,unstable angina pectoris,stable angina pectoris group had statistical significance (P<0.05), the differences between myocardial infarction group and health group had statistical significance (P<0.05);Myocardial infarction patients prognosis group hs-CRP,UA group was obviously higher than that of the prognosis was good ,compared with statistical significance difference ( P <0.05 );In myocardial infarction group , unstable angina pectoris group , the hsCRP was associated with positive correlation exists between UA (P<0.05),stable angina pectoris group no correla-tion(P>0.05);Different prognosis in patients with myocardial infarction hsCRP is associated with UA has positive correlation between ( P<0.05) .Conclusion Elderly acute myocardial infarction patients serum hs-CRP,UA obviously increased ,and illness development correlation ,a high level of hs-CRP,UA patients often indicates poor prognosis .

4.The analysis of 38 cases of ECG changes after PCI

ObjectiveTo explore the dynamic changes of electrocardiography (ECG) in patients with coronary heart disease(CHD) after the percutaneous coronary intervention(PCI). MethodsTo compare and analyze the regularity of the changes of 12-lead ECG on 38 cases in times,including one week,three weeks,one month,two and three months after the PCI. ResultsCompared with the ECG before the operation, the occurrences of the low-lying or the bidirectional T-wave were significantly increased,and lasting to 2 ~ 3 weeks after tie operation(5.0% and 9.6%va2.6% ,P < 0.05). The ST segment depression( > 1.0mm) and inverted T-wave in the 3 weeks and 2 months after the operation were significantly increased(5.0% ,5.7% vs 2.6% ,P < 0.05 ; 18.0% and 25.2% vs 16.2% ,P <0.01 ). After that,the cases of ST segment depression were reduced to the level before PCI,and there was a significant reduction in the cases of the inverted T-wave ( P < 0.01 ). ConclusionsIn the ECG of the patients with CHD after PCI, there were always early one-off abnormal Q-wave, changes of T-wave (inverted or low-lying, bidirectional)and ST segment depressions,etc. After that ,the changes above would turn back to the level before the operation.
5.Fungus Infection in Clinics: Survey and Analysis

OBJECTIVE To inquire into the risk factors of uremia patients with fungus infection and search for the effective methods to prevent and control it.METHODS All the cases of the 82 uremia patients with fungus infection in the clinic of our hospital were analyzed.RESULTS The sites of the fungus infection were intestines and stomach,blood vessel,respiratory tract,oral cavity,reproductive tract,skin soft tissue and urinary tract.The common fungus infection was Candida albicans,about 60.61 per cent.The risk factors of uremia patients with fungus infection were the old age,several underlying diseases,hypoimmunity,not using medicine properly during the treatment and invasive operation.CONCLUSIONS The effective methods to prevent and control fungus infection are as the follows: to improve the staff′s awareness of the danger of fungus infection,be strict to the rules of hospital isolation and sterilization,and be try to reduce invasive operation.We should strengthen the cleaning of the wards and the crew's hands,strengthen the monitoring of quality and pay more attention to the primary treatment.We should use the medicine properly,especially the use of antibiotics,immune suppressants and hormones,and pay attention to result of culture from the microbiological laboratory.We should cure the underlying diseases actively and improve the suffers′ immunity function.

7.Molecular Cloning of a Protein Antigen Gene of Cysticercus cellulosae

Objective To immunoscreen one protein antigen gene from a cDNA library of Cysticercus cellulosae . Methods A cDNA library of C. cellulosae was constructed after cDNA was synthesized, and immunoscreened using rabbit anti C. cellulosae polyclone antibody. The gene structure and its possible function were analyzed by comparing with the sequences available in the GenBank, after the insert of positive clone was subcloned to pBluescript SK plasmid and the nucleotide sequence of the cDNA was determined by dideoxynucleotide chain termination method using a Taq DyeDeoxy Terminator Cycle Sequencing Kit. The amino acid sequence was deduced from nucleotide sequence using GENETYX software. Homological search of the nucleotide sequences was done using BLAST in GenBank. Results and Conclusion A cDNA clone of 1 320 bp was isolated. The clone contained one open reading frame composed of 1 260 bp encoding 420 amino acids, in which two potential glycosylation sites were found. The partial nucleotide sequence of the gene fragment showed high homology with the essential spectrin gene of Caenorhabditis elegans and the erythrocyte surface antigen gene of Plasmodium falciparum , when the gene fragment was homologically analyzed in GenBank.
8.Expression of GRHL-3 in diffuse large B-cell lymphoma and its clinical significance

Objective To study the expression of GRHL-3 in diffuse large B-cell lymphoma (DLBCL) tissues and its clinical significance.Methods One hundred and sixty-eight pathology paraffin-embedded diffuse large B-cell lymphomas tissues were collected from January 2006 to September 2011.Immunohistochemistry was used to detect the expression of GRHL-3 protein in the above tissues.Results The positive expression rate of GRHL-3 protein in the GCB type tissues was higher than that in the non-GCB type tissues [84.87 %(101/119) vs 14.29 % (7/49), P < 0.01].Further analysis indicated that in the non-GCB type tissues,the positive expression rate of GRHL-3 in the latter stage group was significantly higher than that in the early stage group [90.00 % (63/70) vs 77.56 % (38/49), P < 0.01].The positive expression rate of GRHL-3 in the lactatede hydrogenase increased group was significantly higher than that in the normal lactated hydrogenase [91.67 % (77/84) vs 68.57 % (24/35), P < 0.01].The positive expression rate of GRHL-3 in the extranodal involvement status ≥ 2 group was significantly higher than that in the extranodal involvement status 0-1 group [96.29 % (26/27) vs 81.52 % (75/92), P < 0.05].The positive expression rate of GRHL-3 in the IPI score 4-5 group was significantly higher than that in the IPI score 0-1 group [91.30 % (65/69) vs 66.67 % (18/27), P < 0.01] and IPI score 2-3 group [91.30 % (65/69) vs 79.96 % (18/23), P < 0.05].However, the expression of GRHL-3 had no correlation with the gender, age, and performance status of DLBCL.Conclusion The positive expression rate of GRHL-3 protein in the GCB type tissues is higher than that in the non-GCB type tissues.The positive expression rate of GRHL-3 in the DLBCL is correlated with the Ann Arbor stage, lactate dehydrogenase, extranodal involvement status and IPI score.
9.Clinical effects of selective laser trabeculoplasty in the treatment of ocular hypertension and different course of primary open angle glaucoma

Objective To compare the efficacy of selective laser trabeculoplasty (SLT) on the intraocular pressure (IOP) reduction in the treatment of ocular hypertension (OHT),early and moderate primary open angle glaucoma (POAG).Methods Non-randomized controlled study was conducted in 120 patients (120 eyes),who were divided into 3 groups according to the patients' condition,including OHT group with 40 eyes of 40 patients,early-stage POAG group (40 eyes of 40 patients) and medium-stage POAG group (40 eyes of 40 patients),aged rang from 16 to 79 years.SLT was performed on the 360° trabecular meshwork using 532 nm frequency-double Q-switched Nd:YAG laser (Selecta duet),with the pulse of 3 ns and spot diameter of 400 nm.The initial energy was 0.6 m J,which decreased at 0.1 mJ interval till champagne bubble appeared.The following-up was postoperative 1 day,1 week,1 month,3 months and 6 months,respectively,and changes in IOP before and after SLT was recorded and analyzed statistically.Resuits Postoperative IOP at different time points was significantly lower than that of preoperation in the three groups,with significant difference (all P < 0.05).There was also significant difference in IOP of the each group at different time points after the operation (all P < 0.05).The preoperative baseline IOP of OHT group,early-stage POAG group and medium-stage POAG group was (25.95 ± 1.80) nmHg (1 kPa =7.5 mmHg),(24.66 ± 1.55) mmHg and (25.62 ± 2.67) mmHg,respectively,approaching significant difference (P <0.05).After eliminating the influence of baseline IOP,intergroup differences of postoperative IOP at each time point in the three groups were statistically significant (all P < 0.05),and intragroup differences were also significant (all P < 0.05).When compared with corrected baseline IOP,the reduction of IOP was the largest in OHT group after SLT,followed by the early-stage POAG group,and the lowest was in the medium-stage POAG group.Conclusion SLT has a good effect on IOP reduction for OHT and POAG,and the treatment effect of SLT declines successively according to the severity of POAG.
10.Distribution and drug sensitivity test of bacteria of patients on chronic rhinosinusitis with or without nasal polyps.

OBJECTIVE:
To study the distribution and drug sensitivity test of bacteria of patients on chronic rhinosinusitis with or without nasal polyps.
METHOD:
The purulent discharges were collected from sinus of 175 patients with chronic rhinosinusitis with or without nasal polyps during endoscopic sinus surgery. The results of germiculture and drug sensitivity test were analyzed.
RESULT:
From 175 specimens, 118 (67%) showed positive results in germiculture. Among them, 79 strains of gram positive bacteria and 39 strains of gram negative bacteria were detected. Staphylococcus epidermidis, Staphylococcus aureus and Staphylococcus haemolyticus were the most common pathogens in gram positive bacteria. The most common pathogens of gram negative bacteria were P. Aeruginosa, Enterobacter aerogenes, Enterobacter cloacae. The sensitive antibiotic on gram positive bacteria were amikacin, Daptomycin, Linezolid, vancomycin, teicoplanin, amoxicillin and clavulanate potassium, cefuroxime, respectively. The sensitive antibiotics on Gram negative bacteria were amikacin, Cefoperazone/sulbactam and imipenem, ceftazidime ceftazidime, aztreonam, levofloxacin, respectively.
CONCLUSION
Bacterial infection was common happened in the sinus cavity of patients with chronic rhinosinusitis with or without nasal polyps. Gram positive bacteria were the main pathogenic bacteria and gram positive bacteria and gram negative bacteria have great differences in the sensitivity of antibiotics. For patients with chronic rhinosinusitis, the using of antibiotics should depend on the drug sensitivity test.
