2.Cytogenetic and molecular aberrations of multiple myeloma patients: a single-center study in Singapore.

BACKGROUNDMuch is known about the cytogenetic lesions that characterize multiple myeloma (MM) patients from the USA, Europe, and East Asia. However, little has been published about the disease among Southeast Asians. The aim of this study was to determine the chromosomal abnormalities of MM patients in our Singapore population.
METHODSForty-five newly-diagnosed, morphologically confirmed patients comprising 18 males and 27 females, aged 46 - 84 years (median 65 years) were investigated by karyotyping and fluorescence in situ hybridization (FISH). FISH employing standard panel probes and 1p36/1q21 and 6q21/15q22 probes was performed on diagnostic bone marrow samples.
RESULTSThirty-four cases (75.6%) had karyotypic abnormalities. Including FISH, a total detection rate of 91.1% was attained. Numerical and complex structural aberrations were common to both hyperdiploid and non-hyperdiploid patients. Numerical gains of several recurring chromosomes were frequent among hyperdiploid patients while structural rearrangements of several chromosomes including 8q24.1 and 14q32 characterized non-hyperdiploid patients. With FISH, immunoglobulin heavy chain (IGH) gene rearrangements, especially fibroblast growth factor receptor 3 (FGFR3)/IGH and RB1 deletion/monosomy 13 were the most common abnormalities (43.4%). Amplification 1q21 was 10 times more frequent (42.5%) than del(1p36) and del(6q21).
CONCLUSIONSWe have successfully reported the comprehensive cytogenetic profiling of a cohort of newly-diagnosed myeloma patients in our population. This study indicates that the genetic and cytogenetic abnormalities, and their frequencies, in our study group are generally similar to other populations.

3.Systemic review of global case reports on ankle pseudo aneurysm: Analysis of epidemiology, clinical presentation, diagnosis and treatment

Introduction: Pseudo aneurysm of the ankle is a rare
presentation and the management is complex due to the
complex anatomy. The aim of this review is to examine the
epidemiology, etiology, diagnostic modalities used and
management. This is the first systematic review of this topic
in literature.
Methods: We performed a systematic review in multiple
databases (including PubMed, MEDLINE, EMBASE, and
Scopus) from 1966 to May 18, 2019, to identify all case
reports and case series describing patients with ankle
pseudo aneurysm. This systematic review was performed in
accordance with the Preferred Reporting Items for a
Systematic Review and Meta-analysis (PRISMA). Our
inclusion criteria included patients with ankle pseudo
aneurysm of any age. We excluded reports in which the
individual level data is not available. Patients demographic
(gender, age), clinical characteristics (precipitating event,
duration), diagnostic modality and treatment were recorded.
Result: There were in total 23 case reports identified on
ankle pseudo aneurysm from 1966 until 2018. Among these
twenty-three reports, 16 (70%) were male and seven (30%)
patients were female. Age distribution showed higher
number of reports among young adults, 15 patients (65%).
Based on our systematic review trauma (48%), arthroscope
(48%) and arthrodesis (4%) were the etiologies described in
all these case reports. Ultrasound duplex and CT Angiogram
has been used as a single modality in three reports each. In
fifteen patients (65%) combination of imaging has been used
for diagnosis. Anterior tibial artery is the most commonly
injured vessel among the reported cases, comprised of 14
(61%) patients. Among these arthroscopes were the highest
reported precipitating events, 9 (64%), followed by trauma in
four patients (29%) and arthrodesis in one patient (7%).
Treatment modalities described in all previous reports were
excision and ligation, 10 (42%); excision of sac and primary
repair, 4 (17%); excision of sac followed by reversed
saphenous venous graft repair, 2 (8%); US guided
compression, 2 (8%); US guided thrombin injection, 4 (17%);
stenting, 1 (4%) and coiling, 1 (4%).
Conclusion: Ankle pseudoaneurysm is mostly preventable
by detailed initial assessment following trauma or careful
approach during arthroscope. Evolving diagnostic modality
and treatment has shed some light into noninvasive
management of pseudo aneurysm of ankle.
4.The Need for a Well-Organized, Video-Assisted Asthma Education Program at Korean Primary Care Clinics.

BACKGROUND: The purpose of this study was to assess the effect of our new video-assisted asthma education program on patients' knowledge regarding asthma and asthma control. METHODS: Adult asthmatics who were diagnosed by primary care physicians and followed for at least 1 year were educated via smart devices and pamphlets. The education sessions were carried out three times at 2-week intervals. Each education period lasted at most 5 minutes. The effectiveness was then evaluated using questionnaires and an asthma control test (ACT). RESULTS: The study enrolled 144 patients (mean age, 56.7±16.7 years). Half of the patients had not been taught how to use their inhalers. After participating in the education program, the participants' understanding of asthma improved significantly across all six items of a questionnaire assessing their general knowledge of asthma. The proportion of patients who made errors while manipulating their inhalers was reduced to less than 10%. The ACT score increased from 16.6±4.6 to 20.0±3.9 (p<0.001). The number of asthmatics whose ACT score was at least 20 increased from 45 (33.3%) to 93 (65.3%) (p<0.001). The magnitude of improvement in the ACT score did not differ between patients who received an education session at least three times within 1 year and those who had not. The majority of patients agreed to the need for an education program (95.8%) and showed a willingness to pay an additional cost for the education (81.9%). CONCLUSION: This study indicated that our newly developed education program would become an effective component of asthma management in primary care clinics.
