1.Progress in stenotrophomonas maltophilia

Stenotrophomonas maltophilia(SMA) is a emerging opportunistic pathogen that can cause difficult-to-treat infections and exhibits significant degrees of poorly understood multidrug resistance,especially in irnmunocompromised patients.SMA is not an inherent virulent pathogen,but it can colonize in respiratorytract epithelial cells and surfaces of medical devices make it an important colonizer in hospitalized patients.Management of SMA infection is hampered because of its high-level intrinsic resistance to many antibiotics and the increasing occurrence of acquired resistance to the first-line drug such as co-trimoxazole.The prevention and treatment of SMA infection attract more and more attention in recent years.
2.Application of flexible fiberoptic bronchoscope used in central lung cancer diagnosis

Objective:To investigate the diagnostic value of fiber bronchoscopy for central lung cancer.Methods: The clinical data of 137 patients with central lung cancer were retrospectively analyzed. The diagnostic coincidence rate and the performance of the bronchoscopy were analyzed and compared with the traditional CT imaging examination.Results: The coincidence rate of diagnosis(90.51%) was higher than that of CT (75.91%) (x2=10.450,P<0.05). The diagnostic coincidence rate of flexible fiberoptic bronchoscope of squamous cell carcinoma, undifferentiated carcinoma and adenocarcinoma was 92.86%%, 92.68% and 66.67% respectively. The most common type of the fibrous branch was hyperplasia (61.31%) by flexible fiberoptic bronchoscope.Conclusion:In the diagnosis of central lung cancer, the fiber bronchoscopy has important value of MRI and CT can not be replaced.

4.Effect of fluoxetine on the viability and lipopolysaccharide-induced tumor necrosis factor-α release of primary cultured rat astrocytes

ObjectiveTo investigate the effects of fluoxetine on viability and lipopolysaccharide (LPS)-induced TNF-α release of primary cultured rat astrocytes.MethodsThe cells were plated on 96-well tissue culture plates and treated with (5,10,20,40) μM fluoxetine for 24 hours,MTT method was used to measure the cell viability.The cells were plated on 48-well tissue culture plates,treated sequentially with (5,10,20,40) μM fluoxetine and 1 μg/ml LPS,and enzymatic linked immunosorbent assay(ELISA) was used to detect the TNF-α level of the cell supematant.ResultsCompared to viability of the control group( OD value:0.20 ± 0.017 ),fluoxetine at the dose of 20 μM,40 μM increased the viability of astrocytes ( OD value:0.23 ± 0.013,0.24 ± 0.012 ) (P <0.05,P<0.01 ).Treatment with LPS for 24 hours,the level of TNF-α was significantly increased ( 53.84 ±24.84) pg/ml compared to the control group( 8.00 ± 10.87)pg/ml (P < 0.01 ),fluoxetine at a dose of 10 μM can suppressed LPS-induced TNF-α release from astrocytes ( 28.85 ± 3.36 ) pg/ml (P < 0.05 ).ConclusionFluoxetine can increase astrocytes viability and suppress LPS-induced TNF-α release from astrocytes.
5.Clinical implication of thyroid peroxidase antibody detection

Thyroid peroxidase (TPO) , originally described as thyroid microsomal antigen, is present on the apical surface of thyroid follicular cells and is an antigen involved in cell mediated cytotoxicity. TPO evokes high-affinity, IgG-class autoantibodies (TPOAbs) and TPO-specific T cells that are markers of thyroid infiltration or implicated in thyroid destruction, respectively. Enzyme-linked immunosorbent assay is used most frequently for TPOAb detection and quantification. The other conditions associated with TPOAbs include pernicious anemia, connective tissue disorders, diabetes, inflammatory bowel disease, mood disorders, and fertility-related problems such as miscarriage, infertility, in vitro fertilization failure, pre-term delivery, and postpartum thyroiditis. The detection of TPOAhs is recommended in the investigation of goitre, diagnosis of Graves' disease and Hashimoto's thyroiditis, and the prediction of risk of developing hypothyroidism during subclinical thyroid disease.
6.Electroacupuncture applied at the Sanyinjiao point can relieve stress incontinence by up-regulating the expression of spinal NMDA and α 2 adrenergic receptors

Objective:To explore the effect of electroacupuncture on the expression of N-methyl-D-aspartic acid (NMDA) in the spine and the activity of α 2 adrenergic receptors in rats with stress urinary incontinence (SUI). Methods:In all, 48 adult female rats were randomly divided into a sham operation group, a model group and an acupuncture group. The sham operation group received sham vaginal distention. Both the model and acupuncture groups had SUI induced using endovaginal balloon dilatation. The former group then had no further intervention, while the latter was given acupuncture on the sanyinjiao point for one week. Urodynamic testing and leak point pressure (LPP) determination were then conducted with all three groups, as well as urethral sphincter electromyography. The expression of NMDA in the L 6-S 1 spinal segments using the polymerase chain reaction and α 2 adrenergic receptor activity was analyzed using western blotting. Results:No significant differences in the urodynamic variables were observed among the three groups. However, compared with the sham operation group, a significant decrease in the average LPP and the frequency and amplitude of the external urethral sphincter electromyography was observed in the model group. Moreover, compared with the model group, the acupuncture group showed significantly increased average LPP and higher electromyographic frequency and amplitude. The average mRNA and protein expression of NMDA in the spine were also significantly higher in the acupuncture group, as was the average α 2 receptor activity. Conclusions:Vaginal distention is an effective method of establishing urinary incontinence in rats. Electroacupuncture can not only improve the ability to store urine, it also increases urination, which may be related to the up-regulation of the expression of NMDA and α 2 adrenergic receptors.
7.The effect of early exercise intervention on deep venous thrombosis after joint replacement

Objective To evaluate the effect of early exercise intervention on deep venous thrombosis (DVT) after joint replacement.Methods 100 patients with joint replacement were divided into observation group including 50 cases and control group including 50 cases by random number table method.Early exercise intervention was used in observation group and usual nursing was used in control group.The hemodynamic index (femoral vein blood flow peak velocity and average velocity) and DVT incidence were compared between the two groups.Results After the intervention for 3 days,the femoral vein blood flow peak velocity and average velocity of observation group respectively were (43.5±4.1) cm/s and (27.3±3.2) cm/s,those of control group respectively were(36.6±3.7) cm/s and (20.2±2.8) cm/s.The difference between the two groups was statistically significant (t=2.987,2.895,P < 0.05).After the intervention for 7 days,the femoral vein blood flow peak velocity and average velocity of observation group respectively were (56.1±4.7) cm/s and (32.1±3.9) cm/s,those of control group respectively were (41.3±3.9) cm/s and (22.4±4.3) cm/s.The femoral vein blood flow peak velocity and average velocity of observation group significantly higher than control group(t=3.675,2.929,P < 0.05).The DVT incidence of control group and observation group respectively were 40.0%(20/50) and 14.0%(7/50),showed significant differences between the two groups(x2=8.574,P=0.003).Conclusions Using the early exercise intervention on patients with joint replacement can effectively improve the lower limb venous blood flow velocity and prevent DVT.
8.Effect of fast track surgery on functional recovery of knee joint replacement

Objective To study the effect of fast tract surgery (FTS) used in patients undergoing total knee arthroplasty (TKA). Methods For the experimental group, 341 inpatients with TKA in the Joint Surgery Department of Shandong Provincial Hospital from June 2014 to June 2015 were treated with FTS nursing measures. For the control group, 355 inpatients with TKA of the same department from January 2013 to May 2014 were treated with conventional nursing measures. The effects of FTS nursing measures, the parameters of patients′ postoperative complications, average hospitalization days and the function recovery of knee joint were evaluated respectively. Results After implementing the FTS nursing measures , the incidence of deep venous thrombosis of lower limbs reduced from 8.45%(30/355) to 2.35%(8/341) (χ2=2.340, P<0.05). The average hospitalization days treated without/with FTS nursing measures were:unilateral knee (14.49±3.62) days (without FTS) vs. (11.95±4.53) days (with FTS), bilateral knees (15.80± 3.01) days (without FTS) vs. (13.19±4.08) days (with FTS)(t=3.166, P<0.05). Two weeks after surgery, the HSS scores in the experimental group was 65.72 ± 5.54, while the HSS scores in the control group was 52.43 ± 7.32 (t=2.452, P < 0.05). Three months after surgery, the HSS scores in the experimental group was 88.72 ± 7.10, while the HSS scores in the control group was 72.14 ± 8.73 (t=2.528, P < 0.05). Conclusions FTS nursing measures could significantly reduce the incidence of deep venous thrombosis of lower limbs, shorten the hospitalization days of inpatients, and promote the functional recovery of knees.

Objective To study the chemical constituents of the heartwood of Trachycarpus fortunei.Methods Compounds were isolated and purified by silica gel column , Sephadex LH-20 chromatography and recrystallization .Their structures were elucidated by physicochemical properties and spectral data .Results Ten compounds were isolated from the 95%ethanol extract of the heartwood of T.fortunei and identified as 1,3-dihydroxyanthraquinone (1),mollugin(2), 4-hydroxy-2, 6-dimethoxy-benzoic acid ( 3 ), 5-hydroxy-2-hydroxymethyl-γ-pyranone ( 4 ), 3, 5-dihydroxy-2-methyl-γ-pyranone(5),3,5-dihydroxy-γ-pyranone (6),daucosterol (7),4-hydroxybenzoic acid (8),pyrocatechol (9),β-sitosterol (10).Conclusion Compounds 1-7 have been obtained from this plant for the first time ,and 1-6 from the genus for the first time.The screening results of antitumor activity in vitro show that the half inhibitory concentration IC 50 of compound 2 for human osteosarcoma cells U20S and human neuroblastoma cells SY 5Y is 18.10 and 26.59 μg/ml respectively. Compound 5 can inhibit the growth of human breast cancer cells MCF-7 with a half inhibitory concentration IC 50 of 37.31μg/ml.
10.Determination of Osthol and Berberine in Kangfuyan Effervescent Suppository by RP-HPLC

OBJECTIVE: To establishes an HPLC method for determination of the contents of Osthol and Berberine in Kangfuyan effervescent suppository. METHODS: HPLC was carried out on a column of Diamonsil (250 mm?4.6 mm,5 ?m) with mobile phase consisted of acetonitrile-0.2% phosphate acid (52∶48,Lauryl sulfonic acid sodium 0.1 g was contained in every 100 mL) with flow rate at 1 mL?min-1.The detection wavelength was set at 322 nm and the column temperature was set at room temperature. RESULTS: The linear range was 0.019 8~0.396 0 ?g(r=0.999 97) for Osthole versus 0.056 65~1.133 00 ?g(r=0.999 99) for Berberine. The average recoveries of Osthol and Berberine were 101.06% (RSD=1.10%) and 101.88%(RSD=0.98%), respectively. CONCLUSION: This method is simple, accurate and reproducible, and applicable for the quality control of Kangfuyan effervescent suppository.
