To explore the relation between the gene polymorphism of type 2 diabetes patients and their response to sulphonylurea(SU) and accomplish the personal therapy,the range of the SU pharmacogenomics should includes genes involved in the process of SU metabolism and acting,other pathological or physiological process of type 2diabetes also should be investigated,the genes recoding sulphonylurea receptor 1(SUR1),inwardly rectifying K+ channels 6.2(Kir6.2),CYP2C9 and insulin receptor substrate 1(IRS1) are the most important.

Objective To observe the clinical effects of anti-hepatitis B virus with telbivudine combined with adefovir dipivoxil.Methods 82 cases of patients diagnosed with chronic hepatitis B from January 2014 to February 2016, were randomly divided into two groups, the control group were treated with adefovir ester treatment, the observation group were treated with telbivudine combined with adefovir dipivoxil treatment, patients were followed up and recorded Child-Pugh score, aspartate aminotransferase ALT, total bilirubin TBIL, aspartate aminotransferase AST, albumin ALB, HBV-DNA negative rate, HbeAg seroconversion rate case, the use of statistical methods for data analysis.Results After treatment, the observation group Child-Pugh score, ALT values were(4.01±0.79)points,(37.19±4.82)U/L, were better than the control group(5.46±1.16)points,(61.49±9.78)U/L, and the differences were statistically significant (P<0.05).Observation group after treatment TBIL, AST, ALB, respectively(34.91±5.49)μmol/L,(56.49±5.28)U/L,(45.51±4.24)g/L, were better than the control group(61.62±10.06)μmol/L,(78.64±8.16)U/L,(38.76±3.21)g/L, and the differences were statistically significant (P<0.05).The observation group after treatment for HBV-DNA negative rate and HbeAg seroconversion rate(82.93%, 58.54%)respectively were higher(58.54%, 29.27%), and the difference was significant (P<0.05).Conclusion Telbivudine combined with adefovir dipivoxil anti-HBV effect is good, better than medication alone is worthy of further research and application.

ObjectiveTo search the appropriate experimental conditions for using green fluores-cent protein (GFP) as a reporter gene in tumor gene therapy. Methods The plasmids carrying mu-tated GFP gene were transfected into the eukaryotic cells to observe transient gene expression. Thesestudies were conducted to 1. directly determine GFP expression and express stability in the COS- 7cells, 2. compare the transfection efficiency of two plasmids pcDNA3 - EGFP, pSVKa - S65T with dif-ferent promoters in different tumor cell lines, 3. determine two genes expression in a single cell usingLacZ cotransfected with GFP by FACS. ResultsFluorescence could be detected in intact viable cellsunder different sets of conditions. The expression of GFP might last two weeks or more and the ex-pressed fluorescence was stable. The transfection rate of pcDNA3 - EGFP expressed was different inthree tumor cell lines examined. But pSVK3 - GFP expressed similarly in four tumor cell lines exam-ined. FACS showed the probability of two genes entering a single cell is above >85% at the ratio 1:4.ConclusionThe above data indicate that the GFP can be visualized continuously and directly for geneexpression in living cells. GFP may also be used for quicklly selecting cells carrying a target gene.

Objective To investigate the association between thyroid autoimmunity (TAI) and miscarriage.Methods A systematic review of both case-control and cohort studies on the association between TAI and miscarriage published before March 2009 was performed.All the literatures were abstracted based on the defined selection criteria by two independent investigators.Publication bias was tested by funnel plot.The odd ratios of case-control studies and relative risks of cohort studies were pooled respectively,depending on the result of heterogeneity test among the individual studies.The software Review Manager (version 4.3.1) was used for meta analysis.Results Electronic search yielded 23 articles,4 of which were reviews and were excluded.The remaining 19 clinical studies met the selection criteria,of which 7 were case-control studies and 12 cohort studies.A clear association between the presence of thyroid autoantibodies (TA) and miscarriage was found with a pooled odds ratio of 2.72 (95% CI 1.27-5.80,P = 0.01) in 7 case-control studies,and a pooled relative risk of 2.41 (95% CI 1.96-2.96,P＜0.01) in 12 cohort studies.Women with TA were found to have slightly older age (age difference,1.29 years,95% CI 0.43-2.16,P = 0.003) and raised TSH levels (TSH difference,0.61 mIU/L,95% CI 0.51-0.71,P＜0.01) compared with those without TA.Conclusion TAI is significantly associated with miscarriage,which can be interpreted as the direct involvement of TA.Alternatively,the association can be partially explained by the slightly older age in women with TA compared with those without.A third possibility is mild thyroid failure,as TSH levels in TA-positive but euthyroid women are higher than in TA-negative women.

Objective To investigate MRI features and its clinical value in the pigmented nodular synovitis (PVNS) of the knee joint in order to improve the accuracy of diagnosis of the disease.Methods MRI signs of 9 cases with PVNS of knee joint confirmed by operation and pathology were analyzed retrospectively.Results In 9 patients, joint effusion and marginal bone erosion were found.Diffuse thickening of the synovial membrane, multiple soft tissue nodules showed hypointense on T1WI and mixed hypointense and hyperointense on T2WI,were revealed on MRI.Conclusion MRI features of PVNS of the knee joint are characteristic, which reflects the pathological basis and helps to improve the diagnostic accuracy of the disease.

Objective To study the curative effect and prognosis of 60 cases of anthracycline resistant breast cancer patients treated with docetaxel and vinorelbine Changchun.Methods 60 cases of patients with anthracene admissions ring resistant breast cancer From January, 2013 to October, 2016, in our hospital were researched.The control group were treated with docetaxel plus cisplatin, The observation group was treated with Changchun Red Sea combined with cisplatin, Two groups of patients with adverse reactions after treatment, quality of life scores, including physical function, psychological function, social function, mental function, treatment efficacy.Results After treatment, Incidence of adverse reactions in Observation group was less than the control group26.66%(8/30)vs 53.33%(16/30); Physical function, psychological function, social function, mental function score in Observation group (67.38 ±10.27、123.29 ±26.30、61.26 ±12.37、48.03 ±16.04) score were less than the control group (50.27 ±9.26、98.08 ±21.36、42.19 ±10.28、30.27 ±9.67) score(P<0.05).Remission rate of observation group 90.00% (27/30) was statistically higher than that in the control group 63.33%(19/30)( P<0.05).Conclusion Based on 60 cases of anthracycline resistant breast cancer were treated with vinorelbine and Changchun after docetaxel treatment of Changchun vinorelbine docetaxel treatment effect is better , can reduce the adverse reactions, is conducive to the prognosis of patients.

Objective To assess accuracy and safety of ultrasonography guided or sonography X-ray guided percutaneous neddle biopsy(US-PCNB or CT-PCNB) for diagnosis of pulmonary masses.Methods Seventy one patients with radiographic evidences of pulmonary mass underwent US-PCNB or CT-PCNB.The diagnostic results of US-PCNB or CT-PCNB were confirmed by surgery or by follow-up examinations.Results The accuracy rate for diagnosis of pulmonary malignancy US-PCNB and CT-PCNB was 96 2% vs 94 7%.Complications occurred 3 87% vs 5 26%.Conclusions There was no statistically differences between US-PCNB and CT-PCNB.US-PCNB suitable for pulmonary tumors of around type.CT-PCNB suitable for pulmonary tumors of central type.

Objective To study the clinical features and differential diagnosis of hairy-cell leukemia variant (HCL-V).Methods A case with HCL-V was reported and the literatures were reviewed.Results The patient had splenomegaly for twenty years and a history of recurrent pulmonary infection.His blood routine test showed a high white blood cell count and abnormal high proportion of lymphocytes.Peripheral smear and bone marrow smear both showed significantly higher proportion of lymphocytes,part of which had soma jagged,prominent nucleoli and villous/hairy cytoplasmic projections.His hairy leukemic cells expressed CD1 1c,CD19,CD20,CD22,and had variable expression of FMC-7,CD103 and lambda,but not CD5,CD23 and CD25.Transmission electron microscope showed many monocytes with villous exist in peripheral blood.Conclusions HCL-V is a rare and an indolent form of a small,mature,B-cell leukemia,based on the clinical,peripheral smear,bone marrow smear,flow cytometric analysis and transmission electron microscopy features,a diagnosis of HCL-V is confirmed.The differential diagnosis should always include splenic marginal zone B-cell lymphoma and HCL-C,because they have different clinical and biological features.
