OBJECTIVE To explore study a method for rapid detection of bacterial infection in clinic to diagnose septicemia early.METHODS 16S rRNA gene of ten bacterial species was amplified with PCR,by using human genome DNA,HBV-DNA and Candida albicans as comparison.The sensitivity test was done by the method of gradual dilution of Escherichia coli.RESULTS The bacterial species were amplified and the products were 371 bp,but human genome DNA,HBV-DNA and C.albicans showed no amplification products.Sensitivity test showed that it could detect as low as 1.5?104/L of E.coli.CONCLUSIONS The method is rapid and highly specific and sensitive in detecting the existence of bacterial 16S rRNA gene.
2.The Expression of Cyclooxygenase-2 in Several Epidermal Tumors

Objective To explore the expression of cyclooxygenase-2 (COX-2) in different epidermal tumors and its significance. Method The expression of COX-2 was detected by immunohistochemical staining of skin specimens from 10 cases of basal cell carcinoma (BCC), 8 squamous cell carcinoma (SCC), 8 Bowen′s disease (BD), 12 seborrheic keratosis (SK) and 10 normal volunteers. Results Compared with normal epidermal cells, COX-2 expression was significantly increased in BCC, SCC and BD cells. Especially, COX-2 expression in SCC was the most intensive. COX-2 expression in SK cells was similar to that of normal epidermal cells. Conclusion The up-regulation of COX-2 expression may play a potential role in the pathogenesis of epidermal tumors.

Objective To evaluate the safety and efficacy of ex vivo ureteroscopy (ExURS) as means of rendering a donated kidney stone-free in a living related renal transplantation.Methods Clinical data were analysed of ExURS as means of rendering a donated kidney stone-free in a living related renal transplantation and relative literature was reviewed.The ECT results showed that GFR of left and right kidney was 38.7 and 42.3 ml/min respectively.The donor underwent a left laparoscopic donor nephrectomy.Immediately after cold perfusion,ExURS was performed with 4 ℃ ice-cold saline irrigation.Basket extraction and holmium laser lithotripsy was performed.Calculi were fragmented with pneumatic intracorporeal lithotripsy and fragments were removed with forceps.F6 indwelling ureteral stents were kept during transplantation.Urine flowed out immediately after reperfusion of the allograft and the distal ureter appeared edema 2 min later.Routine ureter-bladder wall anti-reflux replantation was done after the resection of the edema part.Results Pyeloscopy was successfully performed.A total of 2 calculi,diameter 8,12 mm,were visualized in donor kidney.The ex vivo treatment time was 30 nin.The warm and cold ischenia time was 60s and 50 min,respectively.There were no intraoperative complications.At a follow-up at 8 months,there was no recurrent calculi formation in the recipient and donor.Conclusion ExURS is technically feasible to render a stone-bearing kidney stone free without compromising ureteral integrity or renal allograft function.
4.Comparison of choroid thickness in healthy pregant women and non-pregant women

Objective:To compare and observe the changes in choroidal thickness between healthy pregnant women and healthy non-pregnant women.Methods:A prospective clinical study. From January 2019 to August 2019, healthy pregnant women (pregnant women group) and healthy non-pregnant women age-matched were enrolled during the same period (the normal group) in the obstetrics of Zhuji People's Hospital. All patients were enrolled with their right eyes. Frequency-domain OCT-enhanced depth imaging technology was used to measure the subfoveal macular and 1000 μm above, below, nasal, and temporal choroidal thickness and foveal retinal thickness (CMT). The choroidal thickness and CMT of the pregnant women group and the normal group were compared by t test, and the choroidal thickness and CMT of the normal group and the eyes of different gestational weeks were compared by one-way analysis of variance. Results:The pregnant women group and the normal group included 161 patients (161 eyes) and 40 patients (40 eyes). According to the different gestational weeks, the pregnant women were divided into the first trimester group, the second trimester group, and the third trimester group, with 47 patients (47 eyes), 66 patients (66 eyes), and 48 patients (48 eyes) respectively. There was no significant difference in age, axial length, intraocular pressure, and CMT between the different gestational week groups and the normal group ( F=1.433, 1.558, 0.416, 2.288; P>0.05). The subfoveal choroidal thickness (SFCT) of the pregnant women group and normal group were 317.7±73.9 μm and 279.7±44.1 μm, respectively, and the difference was statistically significant ( t=3.113, P=0.002). Compared with the normal group, the choroid at the upper, lower, nasal, and temporal sides of the pregnant group 1000 μm from the fovea was thickened. The difference between the upper, nasal and temporal sides was statistically significant ( t=2.699, 3.474, 2.595; P<0.05). The SFCT of the eyes in the first trimester group, the middle group, and the late group were 305.8±72.3, 327.7±69.8, 315.8±80.5 μm, respectively. Compared with the normal group, the difference was statistically significant ( F=4.180, P=0.007). Pairwise comparison between the two groups, the second trimester group was significantly different from the normal group ( P=0.003). There was no significant difference among the first trimester group、the third trimester group and the other groups ( P>0.05). Conclusion:The choroidal thickness of pregnant women is thicker than normal, and the choroidal thickness in the second trimester reaches the maximum value; while the macular CMT during pregnancy has no significant change.
5.Synthesis and anti-tumor activity of Z-Gly-Pro-OH-podophyllotoxin derivatives

Ten novel podophyllotoxin derivatives (IIIa-IIIi and IV) were synthesized by three-step reactions using podophyllotoxin and N-benzyloxycarbonyl glycine-L-proline as raw material. The structures of the target compounds were confirmed by 1H NMR, 13C NMR and MS. MTT method was used to test anti-tumor activity of the target compounds on HepG2, THP-1, HeLa and MCF-7 cells. The results showed that all the target compounds had inhibitory activity against HepG2, THP-1, HeLa and MCF-7 cells, and the inhibitory activity of IIIa on HepG2 cells was the most prominent with an IC50 value of 0.58 nmol/L. The binding mode of compound IIIa and FAPα was studied by molecular docking. Compound IIIa could bind to multiple sites of FAPα enzyme.
6.Diagnostic value of contrast-enhanced ultrasonography in preoperative Borrmann classification of gastric cancer.

OBJECTIVETo investigate the diagnostic value of contrast-enhanced ultrasonography (CEUS) in preoperative Borrmann classification of gastric cancer.
METHODSAsulfur hexafluonde-filled microbubble ultrasound contrast agent and continuous real-time imaging technique of contrast pulse sequencing were used. Two hundred and eighty-five patients with gastric cancer confirmed by biopsies who received preoperative CEUS examination were involved in this study. CEUS results were compared with postoperative pathological findings.
RESULTSThe accuracy rate of CEUS in determining the Borrmann classification of gastric cancer was 92.3%(263/285). The accuracy rates of BorrmannI(, II(, III(, IIII(, and IIIII( were 100%(12/12), 90.6%(77/85), 92.6%(126/136), 95.7%(45/47), and 60.0%(3/5) respectively.
CONCLUSIONCEUS is a useful diagnostic method for preoperative Borrmann classification of gastric cancer.

Objective To assess the prognosis and effect on renal function of pediatric urolithiasis caused by melamine-contaminated milk powder (PUMMP) in a long-term follow-up.Methods One hundred and two of 8335 children (≤ 6-year-old) with history of consuming melamine-contaminated milk powder screened in our hospital were followed up for eighteen months after diagnosis. Urinary system ultrasonography, urinalysis, urinary microprotein profiles [microalbumin (ALBU), immunoglobulin G (IgG), and N-acetyl-β-D-glucosidase (NAG)], urinary melamine and cyanuric acid were examined in the first visit and at the end of follow-up. Results Follow-up was completed in 91 children and the stone was excreted in 82 children (90.1%).Stones less than 5 mm in diameter were most vulnerable to discharge, and stones larger than 10 mm could not be expelled without interventions. At the end of follow-up, no melamine or cyanuric acid was found in the urine samples of 74 patients. Urinalysis showed that incidences of proteinuria, microscopic hematuria and leukocyturia were 0%, 5.1% and 2.0%, which were significant different from those in the first visit (Pproteinutria=0.123, Phemnatuna=0.038 and Pleukocyhuris=0.005).Urinary microprotein profiles revealed that some children whose urinalysis was normal still presented glomerular and renal tubular injury and the abnormal rates were 8.8% and 12.1%respectively. The glomerular injury was mainly related to persistent stone, male and younger.Conclusions 90.1% of children with PUMMP passes urinary stones at the end of follow-up.Stone size is the major risk factor of discharge. No melamine or cyanuric acid is found in the urine of children. After eighteen months, glomerular and renal tubular injury is still found in some patients. Further follow-up is necessary.

Chest trauma is one of the most common injuries. Venous thromboembolism (VTE) as a common complication of chest trauma seriously affects the quality of patients′ life and even leads to death. Although there are some consensus and guidelines on the prevention and treatment of VTE at home and abroad, the current literatures lack specificity considering the diagnosis, treatment and prevention of VTE in patients with chest trauma have their own characteristics, especially for those with blunt trauma. Accordingly, China Chest Injury Research Society and editorial board of Chinese Journal of Traumatology organized relevant domestic experts to jointly formulate the Chinese expert consensus on the diagnosis, treatment and prevention of chest trauma venous thromboembolism associated with chest trauma (2022 version). This consensus provides expert recommendations of different levels as academic guidance in terms of the characteristics, clinical manifestations, risk assessment, diagnosis, treatment, and prevention of chest trauma-related VTE, so as to offer a reference for clinical application.
