1.Effect of partial internal sphincterotomy on the pain after cutting seton treatment

Objective To study the influence of partial internal sphincterotomy on the postoperative pain after cutting seton treatment in patients with perianal infection. Methods A controlled study was carried out on 40 cases of cutting seton insertion with (Group A) or without (Group B) partial internal sphincterotomy. The subjects reported their postoperative pain at defecation and at rest respectively using a visual analogue scale (VAS). The intensity of pain between the two groups was compared. Results All the 40 patients were cured, without recurrence or fecal incontinence. During the period of cutting seton treatment, the pain scores at defecation and at rest were lower in the Group A than in the Group B ( P 0.05). Conclusions Internal sphincterotomy can significantly relieve anal pain after cutting seton treatment, being an effective method.
2.Studies on immunomodulatory effects of methanol extracts from Chinese algae

Objective:To study the immunomodulatory effect of extracts from Chinese marine algae, methanol extracts of thirty-nine species were assayed.Methods:The proliferation of T,B lymphocyte and cytotoxic was measured by MTT assay.Results:Among the methanol extracts of algae, 16 showed well concentration-dependent immunomodulatory effects, 6 have exhibited fine dual-immunomodulatory effects at the doses of 1-100 ?g/ml.Conclusion:These results suggest Chinese algae have different immunomodulatory effects and merit further investigation.
3.Values of fractional exhaled nitric oxide plus serum procalcitonin and C-reactive protein in the evaluation of asthma

A total of 60 patients with an acute attack of asthma were studied.On presentation,fractional exhaled nitric oxide (FeNO) and the serum concentrations of procalcitonin (PCT) and C-reactive protein (CRP) were measured.And sputum culture was also performed.The patients were re-evaluated while returning to their clinical remission states.They were classified into 2 groups:patients with bacterial infection (group A) and those with nonbacterial infection (group B).The levels of FeNO were significantly higher in patients with acute exacerbation than those in remission.No difference existed between groups A and B ( P > 0.05 ).The levels of PCT and CRP of group A with acute exacerbation were significantly higher than those of group B( all P <0.05).While in remission,the levels of PCT and CRP decreased significantly in group A ( P < 0.05 ) ; But compared with exacerbation,the levels of PCT and CRP showed no change in group B (P >0.05).And no differences existed between two groups while in remission (P > 0.05 ).An elevation of FeNO indicates the acute exacerbation of asthma.And the increased serum levels of PCT and CRP are associated with bacterial infection.
4.Silencing AP-2α induces chemoresistance to oxaliplatin of HCT-116 cell in colorectal carcinoma HCT-116 cells

Objective To investigate the effect of AP-2α on the chemoresistance to oxaliplatin of colorectal carcinoma cell and its related mechanism.Methods Plasmid of GV102-AP-2α-RNAi (experimental group) and control plasmid GV102-NC (negative control group) were transfected into HCT-116 using Lipofectamine 2000 respectively.The AP-2α expression levels of mRNA and protein of experimental group,control group and HCT-116 blank group were detected by qRT-PCR and Western blot.Cell proliferation assay was performed using the CCK-8 and the apoptosis assays were preformed with Annexin V-PE Apoptosis Kit.Results The AP-2α expression levels of mRNA and protein both decreased after transfection of AP-2α-RNAi plasmid,moreover,the effect produced by subsequence 1 was the most significant.After treatment by oxaliplatin,AP-2α protein levels increased with time while mRNA did not change significantly.Western blot results suggested that the level of AP-2α protein in experimental group which was maintained in oxaliplatin was lower than the negative control group.CCK-8 results suggested that cell proliferation ability was significantly higher for the experimental group maintained in oxaliplatin [(88±3) %] than the negative control group maintained in oxaliplatin [(57±3) %] and the blank group maintained in oxaliplatin [(73t4) %].Flow cytometry showed that the apoptosis rate of the experimental group maintained in oxaliplatin [(15.07±1.20) %] was lower than the control group maintained in oxaliplatin [(24.93±0.90) %] and the blank group maintained in oxaliplatin [(23.71±1.32) %].Conclusion AP-2α may be related to the sensitivity of colon cancer cells to oxaliplatin.

6.Gene diagnostic method for primary immunodeficiency disease based on targeted panel sequencing

Objective · To design and build a high-throughput sequencing approach based on targeted panel sequencing (TPS) using for the primary immunodeficiency disease (PID) diagnosis. Methods · By reviewing the literature and querying the relevant databases to determine the known diseasecausing genes of PID, capture probes using for the TPS were designed and customized for all exons and flanking sequences of these genes. A child suspected with PID was diagnosed by the customized TPS. Results · The PID sequencing panel contains a total of 100 known pathogenic genes. The sequencing data of the patient has 16414298 reads. The average coverage depth is 157 X, 98.35% of the target region sequencing depth is greater than 20 X, and 99.97% of the target region sequencing depth is greater than 1 X. Finally, a heterozygous nonsense mutation was found in the exon 2 of the CXCR4 gene (c.1000C>T, p.Arg334*) in the child. The results of Sanger sequencing confirmed the variation in the child and showed that his parents were wildtype at the corresponding sites, indicating the mutation is de novo. Conclusion · This study established a high-throughput sequencing diagnostic approach for PID, with which a case of WHIM syndrome was successfully diagnosed.
7.Gemcitabine and cisplatin in the treatment of advanced liver cancer

Purpose:To study the response rate and adverse reactions of gemcitabine and cisplatin in the treatment of advanced liver cancer and the disease related symptoms improvement(DRSI). Methods:16 patients of advanced liver cancer were treated from Dec. 1999 to Dec. 2001.Gemcitabine was used intravenously by infusion for 30 min with the dose from 1 000 mg/m 2 to 1 250 mg/m 2 on day 1,8,cisplatin was infused intravenously with a dose of 25 mg/m 2 on day 1,2,3.Twenty one days were counted as one cycle. After 2 cycles of the treatment the response rate and adverse reaction and clinical symptoms were evaluated. Results:Among these 16 patients,there was no complete response,PR 4(25%),MR 6(37.5%),SD 4(25%),PD 2(12.5%),and clinical benefit response(CR+PR+MR+SD)14(87.5%). DRSI could be observed in 31.3%(5/16) for 1 cycle and 68.8%(11/16) for 2 cycles.The main adverse reactions were hematologic toxicity(grade 3/4),which included leucopenia 15.4%(6/39),anemia 12.8%(5/39),thrombocytopenia 25.6%(10/39),and nonhematologic toxicity was mild.Conclusions:Combination of gemcitabine with cisplatin was an effective and new chemotherapy for advanced liver cancer,it has high DRSI and mild adverse reactions.
8.Effect of fluid therapy on skin microcirculatory perfusion during induction of general anesthesia in patients undergoing major abdominal surgery

Objective To investigate the changes in skin microcirculatory perfusion during induction of general anesthesia and the effects oftwo fluid therapy regimens in patients undergoing abdominal surgery.Methods Thirty-six ASA Ⅰ or Ⅱ patients aged 18-64 yr scheduled for elective major abdominal surgery were randomized to receive either 6% hydroxyetlayl starch(130/0.4)7 ml/kg(HES group,n=18)or lactated Ringer's solution 7 ml/kg(RL group,n=18)for compensatory intravascular volultne expansion(CVE)before tracheal intubation.Meanwhile both groups received continuous intravenous infusion of RL at a of 8 ml·kg~(-1)·h~(-1).Tracheal intubation was performed at 40 min after the onset of infusion.Anesthesia was maintained with with sevoflurane,remifentanil and rocuronium.Operation was started at 20 min after tracheal intubation.The microcirculatory perfusion was measured on forehead skin by using Doppler perfusion imaging system(LDPI)PI)at the onset of fluid infusion(T_0,baseline),the end of endotracheal intubation(T_1)and the onset of skin incision(T_2).Rwsults The MAP,HR,blood gases and body temperature were within the normal during the experiment and there was no significant difference between the 2 groups.The skin microcirculatory perfusion and CVP at T_1 were significantly higher in group HES than in group RL(P<0.05 or 0.01).Compared with the baseline value at T_0,the skin microcirculatory perfusion at T_1 was significantly increased in group HES(P<0.01),but there was no significant change in the skin microcirculatory perfusion at T_1 in group RL(P>0.05),the skin microcirculatory perfusion at T_2 was singificantly decreased in both groups(P<0.01),and CVP and PaO_2/FiO_2 at T_(1.2) were significantly increased,while Hb at T_(1.2) was significantly decreased in both groups(P<0.05).The skin microcirculatory perfusion in both groups was significantly lower at T_2 than at T_1(P<0.01).Conclusion The infusion of 6% HES 130/0.4 can improve the skin microcirculatory perfusion and the effect is better than that of RL during induction of general anesthesia in patients seheduled for abdominal surgery.
9.An Experimental Study on the Release of Endotoxin from A. Baumannii Induced by Antibiotics Biapenem,Meropenem and Levofloxacin

Objective To study mechanisms of lipopolysaccharide (LPS) release from Acinetobacter baumannii (A. baumannii) induced by biapenem (BIPM), meropenem (MEM) and levofloxacin (LVFX). Methods A. baumannii was ex-posed to different concentrations of BIPM, MEM, LVFX, and ceftazidime (CAZ) at different time points. The LPS release was measured by tachypleus amebocyte lysate (TAL) test. Quantities of bacteria exposing to 0.5 MIC antibiotics were measured. The morphology of bacteria treated with different antibiotics for 7-hour was evaluated by light microscope. Results The LPS releases of A. baumannii treated with four kinds of antibiotics were significantly less than those of control group. In the later period, LPS releases of A. baumannii exposed to the high concentration of antibiotics tended to smooth. The cell quanti-ties of bacteria were significantly lower in treatment group than that of control group. The cells of A. baumannii treated with CAZ and LVFX showed long filamentous shape. The bacteria exposed to MEM and BIPM showed obviously globular. At the same time, some bacterial cells were induced to lysis rapidly by BIPM. Conclusion The level of LPS release is influenced by kinds of antibiotics, concentration and incubation period. All kinds of antibiotics could induce morphological changes of A. baumannii, which may be related to the different mechanisms that bacteria induce the different levels of LPS release.
10.Reliability of mannitol for fluid responsiveness test in patients undergoing intracranial surgery

Objective To evaluate the reliability of mannitol for fluid responsiveness test in the patients undergoing intracranial surgery.Methods Sixty-two ASA physical status Ⅰ or Ⅱ patients,aged 18-64 yr,with body mass index of 18-25 kg/m2,scheduled for elective intracranial surgery,were enrolled in the study.The patients were mechanically ventilated after induction of anesthesia.The radial artery and central vein were cannulated,and FloTracTM/VigileoTM system was connected for stroke volume variation monitoring.Before infusion of mannitol,effective circulating blood volume was confirmed according to stroke volume variation.20% mannitol 250 ml was infused over 20 min starting from onset of craniotomy.The fluid responsiveness test was recorded at the end of mannitol infusion.Results The sensitivity of fluid responsiveness test was 43%,and the specificity of fluid responsiveness test was 44%.Conclusion Mannitol can not be used for fluid responsiveness test in the patients undergoing intracranial surgery.
