1.Analysis of postoperative life quality of children patients with congenital heart disease in China and abroad

Objective To investigate the current status of postoperative life quality of 2-to 12-yearold children with congenital heart disease,to compare results between 2-to 4-year-old,5-to 7-year-old and 8-to12-year-old groups.Meanwhile,compare the results with foreign researches.Methods Pedsol cardiac module was adopted to investigate 255 children patients with letters in a certain heart center in Shanghai.The survey included heart problems and treatment,perceived physical appearance,treatment anxiety,cognitive problems and communication.2-to 4-year-old group was reported by parents and others were reported by both children and parents.The results underwent analysis and were compared with data of other countries.Results There existed statistical difference among three groups.Compared with foreign researches,quality of life of Chinese children was lower except heart problems and treatment.Conclusions We should provide suitable health care and family centered nursing according to physical and psychological development of children to improve children's quality of life.
2.Cooling and heating before breast cancer surgery inhibits metastasis and improves the postoperative immune response

Objective To study the effect of cold and heat treatment on metastasis and the peripheral blood immune response in patients with breast cancer. Methods A total of 124 breast cancer patients were randomly di-vided into a hot and cold therapy group and a control group, each of 62. Both groups were given conventional surgical resections, and the hot and cold therapy group was given cooling and heating physical therapy for one month before the operation. The efficacy of the treatment and the patients′ spiritual state were compared after the treatment. The effect on lung metastases was evaluated using tomographic images of the chest, and the changes in immune cell levels in the peripheral blood were detected using flow cytometry. Results After the treatment, the remission rate (54.8%), spiritual life ratings on the Karnofsky scale and the area of lung cancer nodule transfer were significantly better in the hot and cold therapy group than in the control group. Twelve weeks after the treatment, the percentage of MDSCs in the control group patients was 72.5%, significantly higher than the 5.3% of the therapy group. The CD4+and CD8+ T cells were 2.2% and 1.8%, significantly lower than the 26.7% and 13.9% of the control group. Con-clusion Cold and hot therapy before a mastectomy can effectively inhibit the metastasis of breast cancer and promote patients′physical rehabilitation and spiritual life. It may regulate the microenvironment of their immunosuppression and stimulate antitumor immunity.
3.Color Doppler blood flow sonography used to detect local recurrence in patients undergoing Miles procedure for rectal carcinoma

Objective To evaluate color Doppler sonography in the diagnosis of local recurrence in patients undergoing Miles procedure for rectal carcinoma.[WT5”HZ] Methods Transperineal and transsacral color Doppler blood flow sonography was performed on 51 patients presenting presacral masses after Miles procedure for rectal carcinoma. Blood flow richness and the blood flow dynamics indexes e.g. peak systolic velocity (PSV), diastolic velocity (EDV),resistive index (RI), and pulsatility indexs (PI) were recorded and analyzed. 4.Investigation for the expressions of Skp2 and P27 in normal and tumor salivary tissues by tissue chip technique

Objective:To study the expressions of Skp2 and P27 in the normal and tumor tissues of salivary glands,and then to evaluate their significance in the development and progression of salivary tumor.Methods:SP immunohistochemical method was used to evaluate the expression of Skp2 and P27 in the normal and tumor tissues of salivary glands. Results:The rates of Skp2 and P27 expression were neither significantly different between polymorphic adenoma and basal cell adenomas, nor significantly different between adenoid cystic carcinoma and mucoepidemoid carcinoma. The rates of Skp2 positive were significantly different between normal salivary glands and benign salivary tumor, as well as between normal salivary glands and malignant salivary tumor. The rates of Skp2 and P27 positive were significantly different between normal salivary glands and malignant salivary tumor.Conclusion:Skp2 overexpression and decreased expression of P27 might play an important role in the development and progression of salivary tumor, overexpression of Skp2 protein may lead to increase of P27 degradation.
5.MRI diagnosis of cerebral glioma involving corpus callosum and its operational pathology

Objective To investigate the MRI manifestations of cerebral glioma involving corpus callosum,and to enhance the ability to diagnosis and differential diagnosis.Methods Thirty four cases of cerebral glioma involving corpus callosum under went plain and Gd DTPA enhanced MR scans.The MRi findings of these gliomas were compared with operation and pathology.Results Gerebral gliomas involving corpus callosum presented as long T 1 and long T 2 homogeneous or inhomogeneous signal intensity and obvious mass effect.The MRI findings of Gd DTPA enhanced MR scans were obvious enhancement,slight enhancement or no enhancement accouding to different pathological kinds of gliomas."Butterfly signs" and "semi butterfly signs" were the reflections when gliomas of corpus callosum invaded two sides or one side of cerebral hemisphere.These two signs were significant markings in the diagnosis of cerebral gliomas in corpus callosum region.Conclusions Cerebral gliomas involving corpus callosum are intracranial tumors with special location.MRI has significant clinical value in the diagnosis and differential diagnosis of this kinds of tumour.
6.Effect of Telmisartan on carotid arterial intima-media thickness in patients with essential hypertension

Objective To investigate inhibitory effect of Telmisartan on carotid arterial intima-media thickness in patients with essential hypertension. Methods 104 patients with mild or mid essential hypertension were randomly divided into three groups, which include telmisartan group( telmisartan 80 mg/d PO), ramipril group( ramipril 5 mg/d PO), and control group( other anti-hypertension agents). Blood pressure was monitored during treatment. Carotid arterial intima-media thickness(CIMT) were measured in all patients at beginning and patients who had the 12 months course. Results There were 91 patients who had the 12 months course,which include telmisartan group33 cases,ramipril group28 cases, and control 30 cases. The CIMT was significantly decreased in telmisartan group and ramipril group ( all P < 0. 05 ), and not changed in control while blood pressure fall effectively. In ramipril, group5 cases were ceased the course because of severe cough. Conclusion Both telmisartan and ramipril could decrease blood pressure and CIMT effectively, and there were less side effects in telmisartan group.
7.Efficacy on hemiplegic spasticity treated with plum blossom needle tapping therapy at the key points and Bobath therapy: a randomized controlled trial.

OBJECTIVETo evaluate the efficacy on hemiplegic spasticity after cerebral infarction treated with plum blossom needle tapping therapy at the key points and Bobath therapy.
METHODSEighty patients were collected, in compliance with the inclusive criteria of hemiplegic spasticity after cerebral infarction, and randomized into an observation group and a control group, 40 cases in each one. In the control group, Bobath manipulation therapy was adopted to relieve spasticity and the treatment of 8 weeks was required. In the observation group, on the basis of the treatment as the control group, the tapping therapy with plum blossom needle was applied to the key points, named Jianyu (LI 15), Jianliao (LI 14), Jianzhen (SI 9), Hegu (LI 4), Chengfu (BL 36), Zusanli (ST 36), Xiyangguan (GB 33), etc. The treatment was given for 15 min each time, once a day. Before treatment, after 4 and 8 weeks of treatment, the Fugl-Meyer assessment (FMA) and Barthel index (BI) were adopted to evaluate the motor function of the extremity and the activity of daily life in the patients of the two groups separately. The modified Ashworth scale was used to evaluate the effect of anti-spasticity.
RESULTSIn 4 and 8 weeks of treatment, FMA: scores and BI scores were all significantly increased as compared with those before treatment in the two groups: (both P<0. 05). The results in 8 weeks of treatment in the observation group were significantly better than those in the control group (all P<0. 05). In 4 and 8 weeks of treatment, the scores of spasticity state were improved as compared with those before treatment in the patients of the two groups (all P<0. 05). The result in 8 weeks of treatment in the observation group was significantly better than that in the control group (P<0. 05). In 8 weeks of treatment, the total effective rate of anti-spasticity was 90. 0% (36/40) in the observation group, better than 75. 0% (30/40) in the control group (P<0. 05).
CONCLUSIONThe tapping therapy with plum blossom needle at the key points combined with Bobath therapy effectively relieves hemiplegic spasticity in the patients of cerebral infarction and improves the motor function of extremity and the activity of daily life.

9.Concomitant malformations and chromosomal abnormalities in prenatally diagnosed congenital diaphragmatic hernia

Objective To analyze the concomitant malforrnations,chromosomal abnormalities and outcomes in prenatally diagnosed congenital diaphragmatic hernia (CDH) cases. Methods Cases of fetal CDH,prenatally identified in the First Affiliated Hospital of Sun Yat-sen University from January 2002 to November 2008,were recruited.The concomitant realformations,chromosomal abnormalities and outcomes of fetal CDlH were analyzed.Fisher's exact test was applied. Results During the study period,71 CDH cases were identified including 62(87.3%) left-sided CDH and 9 (12.7%) right-sided ones.Among the 71 CDH fetuses,38(53.5%)were isolated CDH.33 (46.5%)were complicated with other realformations(complex CDH),including 18(54.5 0A) cardiovascular defects,10 (30.3%)central nervous system abnormalities,9(27.2 0A)genitourinary abnormalities and others.Fetal karyotying was performed in 19 out of the 71 CDH fetuses.among which 12 were isolated CDH cases with normal karyotype,and 4 of the rest 7(4/7)complex CDH cases with chromosomal abnormalities showing a significant differenee compared to the isolated CDH (P.0.009).Sixty-five pregnancies were terminated including all complex CDH(n=33)and 32 isolated CDH.The rest 6 isolated CDH fetuses were term delivered and 5 of them survived after repair of diaphragmatic hernia and one died after birth. Conclusions Left-sided CDH are more common than right-sided ones. Approximately half of the CDH cases are complicated with other malformations,especially cardiovascular abnormalities.The risk of chromosomal abnormalities increases in complex CDH and is relatively low in isolated CDH.The influence of surgical procedure on the prognosis of CDH has not yet determined.
10.The effect of synthetic rehabilitation therapy for the stroke patients with cognitive deficits

Objective To explore the effect of the synthetic rehabilitation therapy for the stroke patients with cognitive deficits,and the improvement of the patients'motor function and independent living ability.Methods The synthetic rehabilitation therapy was carried out in 136 stroke patients with cognitive deficits.The synthetic rehabilitation therapy including :acupuncture,hyperbaric oxygen therapy ,cognitive training and physical therapy etc.The patients'cognitive function,motor function and independent living ability were evaluated and analyzed statistically before and after the treatment.Results the patients'cognitive function,motor function and independent living ability are significantly higher after the treatment (P<0.01).Conduston The synthetic rehabilitation therapy can significantly improve the cognitive function of the patients with cognitive deficits,and it can improve their motor function and independent living ability.
