1.Efficacy and safety of paclitaxel liposome combined with cisplatin in the treatment of advanced non-small cell lung cancer

Objective To observe the effects and toxicity of paclitaxel liposome combined with cisplatin in the treatment of ad-vanced non-small cell lung cancer .Methods 80 cases of patients of advanced non-small cell lung cancer were randomly divided into study group and control group ,40 cases in each group .Study group was treated with paclitaxel liposome (135 mg/m2 ,IV drip 3 h , first days) ,control group received paclitaxel(135 mg/m2 ,IV drip 3 h ,first days) ,the two groups combined with cisplatin 75 mg/m2 ,IV drip ,for 3 days ,take 21 days as one cycle .Results The study group CR 0 .0% (0/40) ,PR 27 .5% (11/40) ,RR 27 .5% (11/40);control group CR 0 .0% (0/40) ,PR 25 .0% (10/40) ,PR 25 .0% (10/40) ,(P>0 .05) .The research group of rash ,muscle and joint pain ,peripheral neuropathy was significantly lower than that of the control group (P<0 .05) .The research group of progres-sion-free survival was 5 .3 months(95% CI 3 .713 -6 .408) ,the median survival period was 9 .3 months(95% CI 8 .337 -9 .907);control group of progression-free survival was 4 .6 months(95% CI 3 .187 -5 .547) ,the median survival period was 9 .4 months (95% CI 7 .787-11 .602) ,the two groups had no statistical significance (P>0 .05) .Conclusion Paclitaxel liposome combined with cisplatin in treatment of advanced non-small cell lung cancer clinical efficacy and long-term efficacy of taxol and quite ,but with less allergic reaction .
2.Clinical observations of maintenance chemotherapy by pemetrexed on advanced non-small-cell lung cancer

Objective To observe the clinical effect of maintenance chemotherapy by pemetrexed in advanced non-small-cell lung cancer. Methods 60 patients with advanced non-small-cell lung cancer (stage Ⅲb~ Ⅳ) were randomly divided into observation group and control group , all the patients showed complete or partial remission after receiving 4 cycles of induction chemotherapy. Observation group was given pemetrexed maintenance (500 mg/m2 on days 1 every 21 days) until first disease progression, control group was given best supportive care . Progression free survival ( PFS ) , overall survival ( OS ) and adverse reaction were observed . Results PFS and OS of observation group were significantly higher than control group, PFS was (7.3 ± 1.20) vs (3.5 ± 1.13) months, OS was (12.2 ± 1.14) vs (7.1 ± 1.23) months, the difference was statistically significant. The main side effects were fatigue and leucopenia which were tolerable. Conclusion Maintenance therapy with peme-trexed following initial therapy was feasible, and extended the PFS and OS, and shows a favorable toxicity profile.
3.Analysis of Volatile Chemical Constituents in Vinegar-Processed Schisandrae Sphenantherae Fructus and Vinegar-Processed Schisandrae Chinesis Fructus by HS-SPME-GC-MS

Objective:To analyze and compare the volatile components in vinegar-processed schisandrae sphenantherae fructus and vinegar-processed schisandrae chinesis fructus.Methods:The volatile components in vinegar-processed schisandrae sphenantherae fruc-tus and vinegar-processed schisandrae chinesis fructus were extracted by headspace solid phase-microextraction (HS-SPME) and quali-tatively analyzed by GC-MS.Results:Totally 20 kinds of constituents were identified from vinegar-processed schisandra sphenanthera fructus, which accounted for 99.55%of the total volatile components , and totally 21 kinds of constituents were identified from vinegar-processed schisandra sphenanthera fructus, which accounted for 99.90% of the total volatile components .Conclusion: The type and content of volatile components in vinegar-processed schisandrae sphenantherae fructus and vinegar-processed schisandrae chinesis fructus are quite different , and the study can provide scientific basis for the two traditional Chinese medicinal materials .

Background The mitochondrial Na+/Ca2+exchanger, NCLX, plays an important role in the balance between Ca2+influx and efflux across the mitochondrial inner membrane in endothelial cells. Mitochondrial metabolism is likely to be affected by the activity of NCLX because Ca2+activates several enzymes of the Krebs cycle. It is currently believed that mitochondria are not only centers of energy produc-tion but are also important sites of reactive oxygen species (ROS) generation and nucleotide-binding oligomerization domain receptor 3 (NLRP3) inflammasome activation. Methods&Results This study focused on NCLX function, in rat aortic endothelial cells (RAECs), induced by glucose. First, we detected an increase in NCLX expression in the endothelia of rats with diabetes mellitus, which was induced by an injection of streptozotocin. Next, colocalization of NCLX expression and mitochondria was detected using confocal analysis. Suppression of NCLX expression, using an siRNA construct (siNCLX), enhanced mitochondrial Ca2+influx and blocked efflux induced by glucose. Un-expectedly, silencing of NCLX expression induced increased ROS generation and NLRP3 inflammasome activation. Conclusions These findings suggest that NCLX affects glucose-dependent mitochondrial Ca2+signaling, thereby regulating ROS generation and NLRP3 in-flammasome activation in high glucose conditions. In the early stages of high glucose stimulation, NCLX expression increases to compensate in order to self-protect mitochondrial maintenance, stability, and function in endothelial cells.
5.The clinical, endoscopic and pathologic characteristics of eosinophilic gastroenteritis of 24 cases infants resulted from milk protein allergy

Objective By studying clinical features, treatment and prognosis of eosinophilic gastroenteritis of infants resulted from milk protein allergy an, to improve the diagnosis and treatment level of eosinophilic gastroenteritis. Methods 24 cases of infants which diagnosed eosinophilic gastroenteritis were chosen from June of 2010 to January of 2014 in children’s Hospital of XX province and By retrospective analysis clinical manifestations, endoscopic features, histopathology, treatment and prognosis of the 24 cases. Results The 24 cases who were vomiting, paroxysmal crying, abdominal distension (100.00%), which accompanied by haematemesis 23 cases (95.83%), 1case (4.17%) hematochezia, 17cases (70.83%) eczema, 21 cases (87.50%) mild to moderate anemia, 1 cases (4.17%) severe anemia, 19 cases (79.17%) the increasing of peripheral blood eosinophil cells, 8 cases (33.33%)the increasing of IgE of the serum and 4 cases (16.67%) the test of antibody of the Helicobacter pylori in Serum was positive; 3 cases (12.50%) were milk protein allergy by the detecting of food allergen-speciifc IgE antibodies, the endoscopic characteristics were hyperemia, edema, erosion, ulcer of gastric and duodenal mucosa. Among them, 24 cases (100.00%) were gastritis, 5 cases (20.83%) duodenitis and 1 cases (4.17%) duodenal ulcer. The histopathology of the 24 cases revealed that there were gastric or duodenal eosinophils infiltration (> 20/HPF) and were all associated with mast cell infiltration; By antisecretory, protection of the gastrointestinal mucosa and the obviating of milk protein had a satisfactory treatment effect, 24 cases of children with oral general formula milk test conifrmed that the milk protein allergy, The 3 cases of the patients were reviewed by 8~12 weeks after gastroscope, and the mucosa of the duodenum was smooth, Eosinophils were/HPF<8, mast cells were/HPF<5. Conclusion There are no speciifc clinical and endoscopic manifestations in eosinophilic gastroenteritis of infants resulted from milk protein allergy, gastrointestinal mucosa eosinophil inifltration and simultaneously are accompanied by abnormal mast cell inifltration;Mucosal type without the use of corticosteroids, through milk protein avoidance treatment can achieve satisfactory results, But definite diagnosis must rely on biopsy and eosinophils, combined with avoidance stimulation test of milk protein can further confirmed, But the excitation test should be at least 10 days of observation of children, and carefully recorded symptoms, so as not to delay the missed diagnosis of CMPA.
6.Susceptibility testing of Aspergillus biofilms against common antifungal drugs and a preliminary study on molecular mechanisms of antifungal drug resistance in Aspergillus biofilms

Objective To explore the susceptibility of Aspergillus biofilms to common antifungal drugs and molecular mechanisms of antifungal drug resistance of Aspergillus biofilms. Methods The susceptibility of planktonic spores of 22 clinical isolates of Aspergillus spp. to amphotericin B, caspofungin and voriconazole was evaluated by a standard broth microdilution method. Meanwhile, in vitro model of Aspergillus spp. biofilm was established for the 22 isolates, then the susceptibility of Aspergillus spp. biofilm to amphotericin B, caspofungin and voriconazole was evaluated by a method for antifungal susceptibility testing combined with colonmetric XTT-reduction assay. In addition, real-time reverse transcription (RT)-PCR was performed to determine the relative expressions of drug efflux pump genes and azole target enzyme genes during the formation of Aspergillus spp. biofilms. Results In terms of planktonic spores of Aspergillus spp., the- minimal inhibitory concentration (MIC) range was 0.5 to 2 mg/L for amphotericin B, 0.5 to 1 mg/L for voriconazole, and the minimal effective concentration (MEC) range was 0.125 to 0.25 mg/L for caspofungin. As far as Aspergillus spp.biofilms was concerned, the sessile minimum concentration required to inhibit the growth of 50% organisms (SMIC50) and 80% organisms (SMIC80) ranged from 2 to > 32 mg/L and from 8 to > 32 mg/L, respectively, for amphotericin B, from 32 to > 256 mg/L and from 256 to >256 mg/L respectively for caspofungin, from 4 to >256 mg/L and from 32 to > 256 mg/L, respectively for voriconazole. During the formation of Aspergillus spp.biofilms, no change was observed for the expression of any of the 7 tested drug efflux pump genes or azole target enzyme genes at 4 hours, while a significant increase was noted in the expression of AfuMDRl, CYP51B and CYP51A genes at 8 hours, as well as in the expression of AfuMDRl, AfuMDR2, AfuMDR4, CYP51A,CYP51B at 12, 16 and 24 hours. Of these genes, CYP51A showed the strongest increase in expression at the above 4 time points. The expression of AfuMDR3 and atrF experienced no significant change during the formation of Aspergillus spp. biofilms. Conclusions Compared to planktonic spores, Aspergillus spp. biofilms exhibit a decreased susceptibility to amphotericin B, caspofungin and voriconazole. After the formation of biofilms, the expression of drug efflux pump genes and azole target enzyme genes is elevated in Aspergillus spp.
7.Correlations between stroke-preventing knowledge,health relief and health behaviors in community hypertensive patients

Objectives To study the correlations between stroke-preventive knowledge,health belief and health behaviors in community hypertensive patients.Methods The questionnaire of SPKQ,CHBMS and HPLPⅡwere used to take the investigation among 94 hypertensive patients from a community hospital in Guangzhou.Results The total score on SPKQ was 62.70±18.39 and the average scores on CHBMS and HPLPⅡwere 3.51±0.24 and 2.48±0.37,respectively.The stroke-preventive knowledge was positively correlated with health belief,health motivation and self-efficacy(r=0.289,P<0.01;r=0.246,P<0.05;r=0.350 (P<0.01,respectively).The health motivation was positively correlated with health behaviors(r=0.304,P<0.01)and the seriousness negatively correlated with health behaviors(r=-0.279,P<0.01).Conclusion Medical staff should provide much more stroke education with community hypertensive patients and promote patients’health motivation and self-efficacy of health belief in stroke prevention,help patients understand stroke seriousness,establish and sustain healthy lifestyles.
8.Optimization of Forming Process of Luomai Granules by Response Surface Methodology

Objective:To optimize the forming process of Luomai granules. Methods: With the qualified granule yield and the moisture absorption as the indicators, the excipients were screened, and through the study of granulation effect, the proportion of excip-ients was optimized. With the normalized value of qualified granule yield, solubility, hygroscopicity and sensory evaluation as the eval-uation indices, and excipients times, ethanol concentration and amount of citric acid as the main influencing factors with five levels for each, the forming process of Luomai granules was optimized by central combination design-response surface methodology. Results:The option formula for Luomai granules was the follows:the excipients amount was 5-fold of the extract, 85% ethanol was used as the wet-ting agent, and citric acid content was 0. 3%. Conclusion:Optimized by the central combination design-response surface method, Lu-omai granules has such advantages as high qualified yield, low hygroscopicity, good solubility, soft taste and appropriate dosage, sug-gesting the optimized preparation process of Luomai granules is reasonable and feasible.
9.Involvement of pancreatic beta cell in pancreatic regeneration following experimental acute pancreatitis

Objective To investigate the role of pancreatic β cell on pancreatic regeneration following experimental acute pancreatitis.Methods Eighty-seven SD male rats were randomly divided into four groups:control group ( n =15 ),STZ group ( n =24),L-Arg group ( n =24 ),STZ + Arg group ( n =24).60 mg/kg of STZ was administrated by intraperitoneal injection to induce the diabetes model.2.5 g/kg body weight of LArg was administrated by intraperitoneal injection to induce the acute pancreatitis model.The rats were sacrificed 1,3,5,7 d later and the serum levels of amylase and glucose were measured.Relative pancreatic weight (pancreatic weight/body weight) were measured.Pancreatic tissue underwent routine pathologic examination,and the percentage of area of necrosis and tissue transformation was calculated.The expression of Reg4 and insulin was performed by immunofluorescence.Results Serum level of glucose significantly increased after STZ injection.After L-Arg injection,serum level of amylase significantly increased,and there was pancreatic tissue edema,necrosis,infiltration of inflammatory cells,which suggested the successful model induction.The percentage of area of necrosis in STZ + L-Arg group was (71.6 ± 6.0) % at the 3rd day,which were significantly higher than (42.3 ± 4.0 ) % in L-Arg group; the percentage of area of transformation was (45.6 ± 5.4) %,which were significantly lower than (78.5 ± 6.4) % in L-Arg group.Expression of Reg4 in pancreatic islets of STZ + L-Arg group was significantly lower than those in L-Arg group.Conclusions STZ impairs pancreatic β cells,aggravates pancreatic damage following L-arginine induced pancreatitis and inhibits pancreatic regeneration.
10.Investigation of stroke-preventing knowledge and influential factors in hypertensive patients from the communities of Guangzhou

Objective To investigate the current situations in stroke-prevention knowledge and influential factors in hypertensive patients from the communities of Guangzhou.Methods 94 hypertensive patients visiting the community health service centers in Guangzhou participated in the survey using stroke-preventing knowledge questionnaire.Results The total score on the stroke-preventing knowledge was(62.70±18.39).By stepwise regression analysis,those female and well-educated scored higher. The score on monitoring the blood pressure was the lowest(43.09±25.77).There were mistakes in medication,physical exercises and living habits.Conclusions The hypertensive patients in the communities in Guangzhou lack of stroke-preventing knowledge.In the stroke-preventing education,those male patients as well as those with less education deserves more attention.
