1.The retention of fiber periodontal splint with Periocline treatment of severe periodontal disease curative effect analysis

Objective To investigate the clinical effect of periodontal fiber retention periodontal splint and Periocline combined treatment for severe tooth. Methods the control group was given routine retention of fiber periodontal splint treatment, study group in retention of fiber periodontal splint treatment combined with periocline. The changes of PLI, BI, PD and AL before and after treatment in two groups of patients with severe periodontal disease were recorded. The data were input into SPSS software and given statistical analysis. Results The detection of two groups of PLI, BI, PD, AL and other indicators of no significant difference between before treatment; after treatment of different options of group AL, PD decreased than the control group (P<0.05); PLI of the study group after treatment, BI decreased significantly than before (P<0.05), the control group on the index no obvious change than before. Conclusion The application of retention of fiber periodontal splint combined with Periocline treatment of severe periodontal disease can significantly improve the curative effect, is conducive to the protection of the quality of life.
2.Detection of anti-glutamic acid receptor 1 antibody in serum of epilepsy children

BACKGROUND: The recent researches show that there are various kinds of autoantibodies in epilepsy patients such as anti-nuclear antibodies, anticardiolipin antibodies,anti-?2 glucoprotein antibodies and anti-glutamic acid decarboxylase antibodies,which indicate that some types of epilepsy are mediated by autoimmune mechanism.OBJECTIVE: To detect the level of anti-glutamic acid receptor (GluR1)antibody in epilepsy children.DESIGN: A controlled trial.SETTING: Medical College of Dalian University, General Hospital of Jilin Chemistry Group and Sanchong University, Japan.PARTICIPANTS: Totally 56 cases of epilepsy children were from Maternal and Child Health in Qingdao City including 30 males and 26 females and aged from 3 months to 13 years.While 48 health examinees and 12 patients with brain tumor from General Hospital of Jilin Chemistry Group were taken as controls, including 38 males and 12 females and aged from 6months to 17 years.Their guardians were all informed of the detection index and consented to join the study.METHODS: Polystyrene plates were coated with GluR1 polypeptide antigen,and optimal conditions of enzyme linked immunosorbent assay (ELISA) for GluRI antibody was ascertained with chessboard titration.MAIN OUTCOME MEASURES: The GluR1 antibody level was detected with ELISA.RESULTS:The concentration of coating GluR1 polypeptide antigen was20 mg/L; The dilution of serum and horseradish peroxidase sheep anti-hu-man (HRP-SaH) IgG were 1:50 and 1:1 500 respectively. Intra-assay andinter-assay coefficientof variability were 4.48%-8.80% and 11.18%-16.60% respectively; The specific absorption test demonstrated that the ab-sorbance value of positive serum decreased 2.4 times after absorption ofGluR1 polypeptide compared with that before absorption.There was obvi-ous higher positive rate of GluR1 antibody in serum of epilepsy childrenthan control group (40%, 5%, P<0.01).CONCLUSION:①ELISA is specific and stable for GluR1 antibody in serum. ②There is the possible autoimmune response of GluR1 antibody in serum of epilepsy children.
3.NURSING OBSERVATION ON THE ANAL EXHAUST EFFECT IN PATIENTS TREATED WITH WARM WATER FOOT BATH AFTER LAPAROSCOPIC SURGERY

Objective To observe the anal exhaust effect in patients treated with warm water foot bath after laparoscopic surgery.Methods 90 cases after laparoscopic surgery including renal cancer , renal cysts, epinephrine-tumor patients were randomly divided into A , B and C group.Nursing measures were given at 6 hours after operation. A group were treated with warm water feet bath ; B group received abdominal ring massage ; And C group were control group.Time of anal exhaust postoperatively was observed among the 3 groups.Results Anal exhaust time after oper-ation in A group was average (24.34 ±0.28) hours, which was significantly shorter than in B group (33.07 ±0.18) h and C group (37.90 ±0.19) h (p <0.01).Conclusion Warm water foot bath can promote the anal exsufflation rapidly, effectively after laparoscopic surgery in Urology patients , and has great significance helpful to early recovery .
4.Effects of acute mechanical stretch on the expression of mechanosensitive potassium channel TREK-1 in rat left ventricle.

To explore the role of mechanosensitive potassium channel TREK-1, Western blot analysis was used to investigate the expression changes of TREK-1 in left ventricle in acute mechanically stretched heart. Forty Wistar rats were randomly divided into 8 groups (n=5 in each group), subject to single Langendorff perfusion for 0, 30, 60, 120 min and acute mechanical stretch for 0, 30, 60, 120 min respectively. With Langendorff apparatus, an acute mechanically stretched heart model was established. There was no significant difference in the expression of TREK-1 among single Langendorff perfusion groups (P>0.05). As compared to non-stretched Langendorff-perfused heart, only the expression of TREK-1 in acute mechanically stretched heart (120 min) was greatly increased (P<0.05). This result suggested that some course of mechanical stretch could up-regulate the expression of TREK-1 in left ventricle. TREK-1 might play an important role in mechanoelectric feedback, so it could reduce the occurrence of arrhythmia that was induced by extra mechanical stretch.

5.Dynamic monitoring of the neutrophil/lymphocyte ratio could predict the prognosis of patients with ;bloodstream infection

Objective To explore the value of dynamic monitoring of the neutrophils/lymphocyte ratio ( NLR ) in peripheral blood for the prognosis of patients with bloodstream infection ( BSI ). Methods A retrospective study was conducted. 205 patients who were≥18 years old, their length of hospital stay>24 hours, and they were treated in the China-Japanese Friendship Hospital from January 2013 to October 2014 were enrolled. According to the 28-day survival, the patients were divided into survival group ( n=160 ) and death group ( n=45 ). The white blood cell ( WBC ), neutrophils count ( NEU ), neutrophils ratio ( Neut%), lymphocyte count ( LYM ), lymphocyte ratio ( Lym%), and NLR in peripheral blood were recorded at 1, 3, 7 days after admission. Receiver-operating characteristic curve ( ROC ) was plotted for evaluating the value of these factors on the 28-day prognosis, and logistic regression analysis was used to evaluate the risk factors for predicting the outcome. Results ①On the 1st day, WBC, NEU, Neut%, NLR, and procalcitonin ( PCT ) in the death group were significantly higher than those in the survival group [ WBC (×109/L ):15.28±8.23 vs. 11.58±6.55, NEU (×109/L ):13.34±7.53 vs. 10.03±5.31, Neut%:0.886±0.076 vs. 0.845±0.102, NLR:21.20 ( 13.10, 28.80 ) vs. 12.08 ( 6.81, 20.47 ), PCT (μg/L ):3.13 ( 0.85, 10.12 ) vs. 1.34 ( 0.36, 5.81 ), P<0.05 or P<0.01 ], while hemoglobin ( Hb ), platelet count ( PLT ), albumin ( ALB ) content were significantly lower than those of the survival group [ Hb ( g/L ):86.09±19.83 vs. 107.89±22.82, PLT (×109/L ):157.51±117.81 vs. 195.44±97.28, ALB ( g/L ):24.11±6.94 vs. 31.99±6.89, P<0.05 or P<0.01 ]. On the 3rd day and 7th day, WBC, NEU and NLR in the death group were significantly higher than those of the survival group [ WBC (×109/L ):16.61±10.25 vs. 8.91±4.93, 16.05±9.46 vs. 8.79±4.45; NEU (×109/L ): 14.15±9.98 vs. 6.97±4.64, 14.36±9.03 vs. 6.59±4.07; NLR: 24.13 ( 8.49, 38.26 ) vs. 5.52 ( 3.58, 8.87 ), 17.74 ( 10.74, 32.85 ) vs. 4.35 ( 2.78, 7.27 ), all P<0.01 ], and the LYM and Lym%were significantly lower than those in the survival group [ LYM (×109/L ):0.61 ( 0.38, 1.04 ) vs. 1.05 ( 0.78, 1.43 ), 0.69 ( 0.35, 0.92 ) vs. 1.37 ( 0.93, 1.76 );Lym%:0.039 ( 0.024, 0.101 ) vs. 0.135 ( 0.094, 0.186 ), 0.056 ( 0.033, 0.082 ) vs. 0.170 ( 0.108, 0.237 ), all P<0.01 ].②It was shown by ROC curve that the maximum area under the ROC curve ( AUC ) of WBC, NEU, Neut%, LYM, Lym%, and NLR about prognosis of BSI were observed on 7 days ( 0.777, 0.819, 0.905, 0.755, 0.880, 0.887 ). Based on Neut%>0.855 on the 7th day as a predictor of cut-off value of death in 28 days, the sensitivity was 78.8%, specificity 89.1%, respectively. When Lym%<0.088 on the 7th day as a predictor of cut-off value of death on 28 days, the sensitivity was 89.5%, and specificity was 83.9%. When NLR>10.34 on the 7th day as a predictor of cut-off value of death in 28 days, the sensitivity was 81.8%, and specificity was 91.0%.③Survival analysis showed that the 28-day survival rate in the patients with 7-day NLR<10.34 was significantly higher than that in those with 7-day NLR>10.34 ( 95.0%vs. 34.1%,χ2=82.650, P=0.000 ).④It was shown by multi-factor logistic regression analysis that the levels of 1-day Hb and 7-day NLR were the independent prognostic predictors of 28-day mortality [ Hb: odds ratio ( OR ) = 0.946, 95% confidence interval ( 95%CI ) = 0.913-0.981, P = 0.003; 7-day NLR:OR=34.941, 95%CI=8.728-139.884, P=0.000 ]. Conclusions The trend of changes in NEU, LYM and NLR as shown by repeated routine blood examinations contributes to prediction of the outcome of patients with BSI. The levels of 1-day Hb and 7-day NLR are the independent prognostic predictors for 28-day mortality.
6.Genotyping of E.coli and klebsieua pneumoniae producing extended spectrum ?-lactamases in Jilin area of China

Objective To genotype strains producing ESBLs in Escherichia coil and Klebsieua pneumoniae to provide references for the clinical application of drugs.Methods PCR was used to determine the genotype of the ESBLs.Results The percentage of TEM type,SHV type and NO-TEM-NO-SHV type in Escheroichia coil was 80.4%,7.8% and 11.8%,in Klebsieua pneumoniae was 78.1%,71.9% and 25.0%.Most of Klebsieua pneumoniae produced more than two Extended Spectrum ?-lactamases.Conclusion The difference in the distribution of genotypes in different regions is confirmed.
7.The establishment of immunochromatographic tests trips with colloidal gold for early and rapid diagnostic of hemorrhagic fever with renal syndrome

Objective To establish an early,simple and rapid colloidal gold strip method for the detection of IgM against hantavirus nucleocapsid protein in patients with hemorrhagic fever with renal syndrome(HFRS).Methods Purified recombinant Hantavirus nucleoprotein(rNP)was labeled by colloidal gold particles and then sprayed and fixed on fiberglass membrane as the combination pad.Anti-Human IgM(μ-chain specific)antibody produced in goat was fixed in the detection area,and mouse antihantavirus antibody was fixed in the quality control area.Both of them were on nitrocellous membrane strip in tandem.Together with a specimen pad ahead.The conbination pad and the nitrocellous membrane were assembled into a test strip.The colloidal gold strip assay was compared with ELISA for evaluation of specificity and sensitivity.Results The colloidal gold strip tests showed positive in the serum samples from 50 cases of HFRS which was clinically diagnosed and then verified by ELISA within 10-15 minutes.Whereas 30 serum samples of healthy donors have tested negative.Conclusions Our new colloidal gold immunochromatographic test strip method was well concordant with the ELISA assay,but the former was more raoid and simole.It could be used primary medical services.
8.Diagnostic value of Lugol staining for detecting early esophageal cancer and precancerous lesions

Objective To study the role of Lugol staining for detecting early esophageal cancer and precancerous lesions.Methods 2% Lugol solution was adopted to stain esophageal mucosa in 45 cases with esophageal suspicious lesions,observed and multiple biopsies were taken for pathological study.Results Thirty-nine cases were light or non-stained among the 45 cases with Lugol staining.Which revealed 8 esophageal cancers(5 early cancer,3 advanced cancer),5 Barrett esophagus,,11 mild-moderate dysplasias.The rate of detecting early esophageal cancer and precancerous lesions with Lugol staining was 46.7%.Conclusion Lugol staining is helpful for diagnosis of early esophageal cancer and precancerous lesions,and it is simple in operating.The role of Lugol staining in detecting early esophageal cancer and precancerous lesions has important clinical value.
9.Clinicopathologic analysis of spiradenoma: 25 case reports

Objective To analyze clinicopathologic features and malignant transformation of spiradenoma (SA).Methods Clinicopathologic data on 25 cases of SA were retrospectively analyzed.An immunohistochemical study was conducted on 5 of the 25 patients.Clinicopathologic features of SA were summarized based on clinicopathologic data,immunohistochemical results and relevant literature.Results The average age of these patients was 40 years.Lesions were acquired and solitary in 24 cases (96%,24/25),and occurred most frequently in the extremities.The diameter of tumors was less than 2 cm in 20 cases (87%,20/23).Twenty-three cases were benign SA with various histological manifestations,including 15 cases (65%,15/23) of typical SA,8 cases (35%,8/23) of cylindroma.VarTing numbers of mitotic figures were observed in different cases.The immunohistochemical study showed epithelial and myoepithelial differentiation.Two cases of SA experienced malignant transformation into poorlydifferentiated basal cell adenocarcinoma and well-differentiated adenocarcinoma respectively.Lymphocytes decreased significantly in number or disappeared in the malignant area,and both the malignant tumors exceeded 2 cm in diameter.Conclusions SA is a kind of neoplasm with distinct histological characteristics and nonspecific clinical manifestations.Most SA cases have a benign clinical course,but malignant transformation should be considered in some cases with longterm clinical course and large size.
10.Setup error of Orfit versus vacuum bag in radiotherapy for cervical cancer

Objective To compare the setup errors of the negative pressure vacuum air cushion (vacuum bag) and the Orfit body foam fixator (Orfit frame) in radiotherapy for cervical cancer.Methods A total of 40 patients receiving three-dimensional radiotherapy for cervical cancer were enrolled in this study and equally and randomly divided into vacuum bag group and Orfit frame group.And the two groups were divided into Orfit-1 group, Orfit-2 group, vacuum-1 group, and vacuum-2 group according to the treatment course.The Orfit-1 group and vacuum-1 group were the data in the first 12 treatments, while the Orfit-2 group and vacuum-2 group were the data in the following 13 treatments.A cone-beam computed tomography scan was performed before each treatment to analyze setup error and then the body position was corrected to start the treatment.Comparison of continuous data between groups was made by paired t-test, while comparison of categorical data was made by chi-square test.Results There was a significant difference in the setup error in y-axis direction between the Orfit-1 group and the Orfit-2 group (P=0.003) and the setup error in r-axis direction between the vacuum-1 group and the vacuum-2 group (P=0.013).There were no significant differences in the setup errors in four directions (x-axis, y-axis, z-axis, and r-axis) between the Orfit-1 group and the vacuum-1 group (P>0.05).There were significant differences in the setup errors in y-axis and z-axis directions between the Orfit-2 group and the vacuum-2 group (P=0.007;P=0.001).Conclusions The Orfit frame and the vacuum bag have their own advantages and disadvantages in the fixation of body position in radiotherapy for cervical cancer.The setup error can be improved by long vacuum bags, ultrasound bladder capacity scanner, image-guided radiotherapy, or sectional radiotherapy plan.
