Objective To assess the value of ultrasonic elastography in the diagnosis of special type breast cancers. Methods Routine ultrasonograms, color Doppler flow images and elasticity images of 13 patients with special type breast cancer confirmed surgical pathologically were retrospectively analyzed. Results The score of mucinous carcinoma, inflammatory breast cancer (1 patient), lactation breast cancer, invasive micropapillary carcinoma, tubular carcinoma and invasive lobular carcinoma (2 patients) were 4-5. The score of 1 patient of invasive lobular carcinoma was 1 and 1 patient of lactation breast cancer was 3. The characteristics of color Doppler flow imaging of majority patients were varies, while no obvious lesion was observed in Paget's disease. Conclusion Ultrasonic elastography is helpful to the diagnosis of inflammatory breast cancer, lactation breast cancer, invasive micropapillary carcinoma of the breast, papillary carcinoma and tubular carcinoma of the breast. Early diagnosis of Paget's disease still depends on pathology.

Objective To study the clinical characteristics, treatment and prognosis of infective endocarditis in children. Methords Clinical data from 83 patients of infective endocarditis admitted from 1998 to 2012 were retrospectively analyzed. Results In a total of 83 patients, there were 53 males and 30 females, and the average age was 6.8±4.6 years. The main clinical characteristics were fever (77.1%) and mild to moderate anemia (71.1%). The C-reaction protein (67.5%), erythrocyte sedimen-tation rate (60.2%), and white blood cell (47.0%) were elevated. Twenty (24.1%) patients had embolism. Blood culture was pos-itive in 56 (67.5%) cases with bacteria mainly being Gram-positive and Streptococcus and Staphylococcus accounted for 89.3%. Vancomycin and other sensitive antibiotics were effective. Neoplasm was detected in 68 cases (82%) by transthoracic echocar-diograerphy. Fifty-ifve (66.2%) patients underwent cardio surgery. Seven patients (8.4%) died. Conclusion In recent years, the distribution of pathogenic bacteria in infective endocarditis had changed. Streptococcus mitis and Staphylococcus aureus has become a major pathogens and need to be treated by vancomycin and other sensitive antibiotics. The detection rate of neoplasm is higher by echocardiography.

Objective To evaluate the impact of PET/CT on the clinical therapeutical strategy of patients with non-small cell lung cancer (NSCLC). Methods Sixty patients with histologically confirmed NSCLC supposed to accept radical radiotherapy or surgical operation were firsthy divided into group A (stage Ⅰ-Ⅱ) and group B (stage Ⅲ) according to CT findings, and the individualized treatment strategy was designed, then restage and compare with different stage and management with initial treatment strategy by PET/CT scanning. Results ①PET/CT scanning restaged the TNM stage in 53.33% (32/60) patients, including 3 of T stage, 23 of N stage and 9 of M stage. Changes of stage occurred in 65.85% (27/41) patients with adenocarcinoma and in 26.32% (5/19) with squamous carcinoma (P＜0.05), while in 34.29 (12/35) with central and 57.14% (20/35) with peripheral NSCLCs (P＜0.05). ②PET/CT scanning resulted in alteration of the managements of NSCLC patients. The management changed from radical into palliative strategy with PET/CT information in 9 (15.00%) patients. In 25 patients who were supposed to accept surgical operation, the treatment changed to radical radiotherapy in 8 (8/25, 32.00%) and to palliative radiotherapy in 5 (20.00%) patients. In 35 patients who were supposed to accept radiotherapy, surgical operation or palliative radiotherapy was finally chosen in 5.71% (2/35) and 11.43% (4/35) patients, respectively. Conclusion PET/CT images can make significant alteration to clinical stage and treatment plan in patients with NSCLC.

Objective To study the effect of telbivudine (TBV) on the ratio of CD4+ CD25+CD127low/-regulatory T cell (Treg) of HBeAg-positive chronic hepatitis B (CHB). Methods The ratios of Treg before and after treatment for 12, 24, 36 and 48 weeks with TBV of thirty-five patients with HBeAg-positive CHB were detected. Results Ratios of Treg were (12.55 ± 1.78)%, (10.42 ± 1.04)%, (9.55 ± 0.74)%, (8.78 ± 0.67)%, (8.74 ± 0.71)%respectively before and after treatment of 12, 24, 36 and 48 weeks and all the values were lower than its former one (P < 0.05), except the 48 weeks′. Conclusions The ratio of Treg of HBeAg (+) CHB decreases after treatment with TBV.

[Objective] To evaluate the effect of fast rehabilitation nursing pathway in patients after artificial hip replacement.[Methods] 86 patients underwent artificial hip replacement were randomly divided into the control group and the experimental group with 43 cases in each group.The control group received routine care,while the experimental group was given fast rehabilitation nursing pathway.Anxiety and pain at 48h after operation were evaluated,and hip joint function(Harris score)was evaluated six months after operation.[Results] Compared with the control group,anxiety and pain score at 48h after operation decreased markedly,and Harris score six months after operation increased significantly.[Conclusions] Fast rehabilitation nursing pathway can reduce both the short-term and long-term efficacy of patients after artificial hip replacement.

Objective To observe the effect of virtual reality robotic hand on hand motor function and activities of daily living of pa-tients after stroke. Methods From June, 2015 to June, 2016, 32 patients with hand motor dysfunction were assigned into experimental group (n=16) and control group (n=16). The experimental group received training with virtual reality robotic hand and hand based rehabilitation, while the control group received hand based rehabilitation only, for four weeks. They were evaluated with Fugl-Meyer Assessment (FMA) of fingers and wrists and modified Barthel index (MBI) before and after treatment. Results The total score and the scores of items of FMA and MBI improved after treatment in the experimental group (Z>3.45 or t>3.45, P<0.01). The total score and the scores of the finger, wrist of FMA, and the scores of the eating, dressing and grooming of MBI improved in the control group (Z>2.07 or t>4.18, P<0.05). The total scores and scores of the items of FMA and MBI improved more in the experimental group than in the control group (Z>2.14 or t>3.20, P<0.05). Conclusion Virtual reality robotic hand training can promote the recovery of hand function and activities of daily living in patients af-ter stroke.

Objective To investigate the clinical effect of non-steroidal anti-inflammatory drugs and αl-adrenergic antagonist on treating patients with ureteral stones.Methods A total of 128 patients with ureteral colic due to ureteral stones were randomly divided into control and treatment group,and 64 cases in each group.Both two groups were treated with tamsulosin 0.4 mg oral,intravenous injection of saline 1000 mL Patients in treatment group were received intramuscular non-steroidal anti-inflammatory drugs(10 mg) and patients in control group were received pethidine hydrochloride(10 mg).Ultrasound exam were performed after 6-8 h to evaluate the stone expulsive rate.Extracorporeal shock wave lithotripsy and other treatment were underwent when the stone was still not discharge.Results The effective rate of analgesia was 96.9％ (62/64) in treatment group and 100％ (64/64) in control group(x2 =2.03,P =0.50).The stone expulsion rate in treatment group was 28.1％ (18/64),higher than that in control group(12.5％ (8/64),x2 =4.83,P =0.05).Conclusion It is effective to relive ureteral cohc with non-steroidal anti-inflammatory drugs only,and it is better than pethidine in promote stone expulsion when they both used with α1-adrenergic antagonist.
