2.Neuroprotective mechanisms of histone deacetylase inhibitors in ischemic stroke

Histone deacetylases (HDACs) catalyze the deacetylation of histone and nonhistone proteins. They play important roles in regulating chromatin remodeling and gene transcription,and that aberrant chromatin remodeling and transcriptional dysregulation are associated with many neurodegenerative diseases. Preclinical studies have suggested that histone deacetylases inhibitors (HDACIs) can target diverse pathophysiologies of ischemic stroke,not only reduce neuronal damage and infarct volume,but also promote the neuronal plasticity and functional recovery after ischemia. This article mainly reviews the neuroprotective mechanisms of HDACIs in ischemic stroke.
3.Influence of Xuebij ing in production of NO and expressions of iNOS and NF-κB induced by LPS in vascular endothelial cells

Objective To investigate the protective effects of Xuebijing(XBJ)on the injury of vascular endothelial cells(VEC)induced by lipopolysaccharide (LPS),and to study the mechanisms of the production of nitric oxide (NO)and the expressions of inducible nitric oxide sytnhase (iNOS)and signal transduction under XBJ intervention condition.Methods The cultured VEC were divided into control group, LPS (1 mg · L-1 )group, LPS (1 mg·L-1)+XBJ(25 g·L-1)group,LPS(1 mg·L-1)+pyrrolidine dithiocarbamate(PDTC,20μmol·L-1) group;XBJ and PDTC were administrated 1 h before incubation of with LPS.Western blotting method was used to detect the expressions of iNOS and NF-κB p65 protein.The level of NO in the supernatant was measured by Griess reagent.Results Comparaed with control group,the NO level and the expression levels of iNOS protein and NF-κB p65 protein in VEC in LPS group were significantly increased(P<0.01).Compared with LPS group,the NO level and the expression levels of iNOS protein and NF-κB p65 protein in VEC in LPS+XBJ group were significantly decreased (P<0.05 or P<0.01);the NO level and the expression levels of NF-κB p65 protein and iNOS protein in VEC in LPS+PDTC group were significantly decreased(P<0.05 or P<0.01).There were no significant differences of the NO levels and the expression levels of iNOS protein between LPS+XBJ group and LPS +PDTC group (P>0.05),but the expression level of NF-κB p65 protein in LPS+PDTC group was lower than that in LPS+XBJ group(P<0.05).Conclusion XBJ can inhibit the production of NO and the expression of iNOS protein in VEC;its mechanism may be related to inhibiting the activation of NF-κB to control inflammation.
4.Curcumin downregulation of MGMT expression to enhance the chemosensitivity of temozolomide on maligant glioblastoma

Objective To explore the roles of curcumin in regulating high level O6-methylguanine-DNA methyltransferase (MGMT) expression and the chemosensitivity of gliomas cells to temozolomide (TMZ) in malignant gliomas.Methods The expressions of MGMT were detected in C6 and U87 cell lines and primary malignant gliomas cell by qRT-PCR.The cell proliferation was analyzed after added in signal curcumin,signal TMZ,curcumin combined with TMZ in cells by CCK-8 assay.The cell apoptosis rate was detected by flow cytometry.Results Compared with single curcumin (C6:0.64 ±0.03;U87:0.63 ±0.06;primary cell:0.51 ±0.07) and single TMZ (C6:0.53 ±0.06;U87:0.51 ±0.04;primary cell:0.79 ±0.03),curcumin combined with TMZ (C6:0.14 ±0.01;U87:0.12 ±0.03;primary cell:0.29 ±0.02) could significantly reduce the expression of MGMT in C6,U87 cell lines and primary cell line (C6:F=23.675,P=0.006;U87:F=29.021,P=0.001;primary cell:F=25.534,P=0.001).The combination of curcumin and TMZ could significantly inhibit the cells proliferation,decrease the halfinhibition concentration (IC5o) value (C6:F=6.731,P=0.012;U87:F=17.321,P=0.008;primary cell:F=18.857,P =0.007).The apoptosis rate of the cells was significantly increased after the combined action of curcumin and TMZ (C5:F=25.871,P=0.001;U87:F=6.847,P=0.009;primary cell:F=36.641,P=0.000).Conclusion Synergistic effect of curcumin and TMZ can reduce the expression of MGMT and increase the chemosensitivity of TMZ to malignant glioma,and provide new strategy and methods for the treatment of malignant glioma.
5.Clinical study of Alpha lipoic acid injection combined mecobalamin and prostaglandin E on treating patients with diabetic neuropathy

Objective To investigate the clinical effect and safety of alpha lipoic acid injection combined mecobalamin and prostaglandin E on type 2 diabetic patients complicated with diabetic peripheral neuropathy(DPN).Methods One hundred and sixty type 2 diabetic patients complicated with DPN in the General Hospital of Benxi Iron and Steel Group Corporation from Jan.2011 to Dec.2012 were randomly divided into the treatment group(n =80) and the control group (n =80).Three cases of the treatment group and 5 cases of the control group discharged early from the study because of their own reasons.There were 77 cases in the treatment group and 75 cases in the control group.On the basis of controlling blood glucose,patients in the two groups were given 500 μg mecobalamin combined with intramuscular injection once two days,as well as prostaglandinE 10 μg injection once a day.Patients in treatment group were added with 600 mg alpha lipoic acid for intravenous injection once a day for 10-14 days.Total symptom score (TSS),nerve conduction velocity,satisfaction and adverse reactions were evaluated before and after treatment.Results TSS score,tingling score,burning sensation score and hypoesthesia score,numb score in treatment group were (3.5 ± 2.5),(1.1 ± 0.4),(0.9 ± 0.7),(1.3 ± 0.4),(1.3 ± 0.9),significantly lower than those in control group (4.3 ± 2.1,t =2.11,P <0.05;1.5 ±0.5,t =1.86,P<0.05;1.3 ±0.5,t =1.83,P <0.05;1.7 ±0.5,t =1.87,P <0.05; 1.9 ± 0.4,t =1.91,P < 0.05).The median nerve conduction velocity,peroneal nerve motor conduction velocity,median nerve sensory conduction velocity,common peroneal nerve sensory conduction velocity of patients in treatment group were (53.6 ± 1.4) m/s,(49.6 ± 1.1) m/s,(47.3 ± 1.1) m/s,(48.2 ± 1.9) m/s,lower than those in control group((48.5 ±2.7) m/s,t =-4.94,P <0.05;(43.9 ±2.1) m/s,t =-5.36,P <0.05; (41.6 ± 1.8) m/s,t =-5.09,P <0.05;(43.2 ±2.5) m/s,t =-4.27,P < 0.05,P <0.05).In the treatment group,97.4% (75/77) physicians and 92.2% (71/77) patients were satisfied with treatment effect,while in the control group,84.0% (63/75) physicians and 78.7% (59/75) patients were satisfied with treatment effect.During the study periods,there were 3 cases with facial flushing and 1 cases of dizziness in the treatment group,and 2 cases of facial flushing and 1 cases of dizziness in the control group.All adverse reactions were spontaneous remission without any special treatment.Conclusion Alpha lipoic acid intravenous drip is effective in term of treating type 2 diabetic peripheral neuropathy,and with high safety.
6.Detection and clinical significance of immune function in lung cancer patients

Objective To study the variety and significance of immune function in lung cancer patients.Methods 68 pretherapy lung cancer patients and 20 healthy volunteers with peripheral blood samples by flow cytometry.Results The proportion of CD+3T cells,CD+4T cells and the ratio of CD4/CD8 decreased in lung cancer patients,while the proportion of CD+8T cells and natural killer cells increased.Serum levels of TNF-α,IL-4,IL-6,IL-10 were significantly increased in patients compared with healthy controls,while the ratio of Th1/Th2 was decreased.(2)The natural killer cells level was negatively correlated with the CD3,CD4,CD8 levels respectively.The CD4 level was negatively correlated with the levels of IL-6,IL-10 respectively.(3)No statistically significant relationship was found between the peripheral blood lymphocyte,Th1-Th2 cytokines and pathological type,lymph node metastasis,distant metastasis.Conclusion Immune suppression was very common in lung cancer patients,particularly cell mediated immunity suppressed markedly.The proportion and function of CD+4T cells decreased significantly.which may resulted from the Th1,Th2 imbalance.Natural killer cells and CD+8T cells increased significantly,but could not prevent tumor development,which may resulted from these factors that suppress immune response in tumor microenvironment.
7.Correlation between endothelial microparticles and interleukin-6 and C-reactive protein in patients with coronary heart disease

Objective To investigate alterations of endothelial microparticles(EMPs) levels in patients with coronary heart dis‐ease(CHD) ,and to evaluate the relationship between EMPs and interleukin‐6(IL‐6) and C‐reactive protein(CRP) .Methods A to‐tal of 38 patients with CHD were divided into myocardial infarction(MI) group(13 cases) and unstable angina(UA) group(13 ca‐ses) and stable angina(SA) group(12 cases) ,and 12 healthy subjects were enrolled as control group .Levels of EMPs were meas‐ured by using flow cytometry technique .Concentration of IL‐6 and CRP were determined by using enzyme linked immunosorbent assay and special protein analysis respectively .Results Compared with SA group and control group ,levels of EMPs were signifi‐cantly increased in MI group and UA group(P<0 .05) .Levels of EMPs in patients with CHD were significantly correlated with IL‐6 and CRP ,with correlation coefficient of 0 .79 and 0 .50 respectively(P<0 .05) .Conclusion EMPs could contribute to monitoring and evaluating the degree of endothelial cells injury in patients with CHD as a laboratory indicator .EMPs might enhance the vascu‐lar inflammation in patients with CHD ,and further accelerate the development of CHD .
8.Analyiss of lymph nodes metastasis in papillary thyroid carcinoma and its surgical approaches

Objective To investigate the characteristics of lymph nodes metastasis in papillary thyroid carcinoma (PTC) and to discuss surgical approach.Methods All patients underwent total thyroidectomy,central lymph nodes and lateral lymph nodes dissection in Department of Thyroid Surgery,the First Hospital of the Jilin University,from Dec.2011 to Dec.2012.Results With the increase of the number of positive central lymph nodes,lateral cervical lymph node metastasis rate increased as well and accompanied multi region metastasis trend.In 102 cases of lateral positive cervical lymph node patients,55 cases were in level Ⅱ,accounting for 53.92%,62 cases were in level Ⅲ,accounting for 60.78%,76 cases were in level Ⅳ,accounting for 74.51%,and 17 cases were in level V,accounting for 16.67%.In lateral lymph nodes metastasis,we found level Ⅳ was the most vulnerable area,followed by level Ⅲ,level Ⅱ and level Ⅴ.Conclusions Prophylactic central lymph node dissection should be performed in PTC patients.Patients with central lymph node metastasis especially with the number of positive lymph nodes > 3 should be performed ipsilateral level Ⅱ-Ⅴ lymph node dissection.

10.One case report of neonatalCandida arthritis

ObjectiveTo explore the clinical characteristics, diagnosis and treatment of neonatalCandida arthritis. Methods The clinical data from one case of neonatalCandida arthritis with swelling and pain in multiple joints as the ifrst symp-toms were reviewed.Results Boy born at 31 weeks' gestational age had the birth weight of 2100 g. The patients was admitted to neonatal intensive care unit after birth and treated with antibiotics for a long time due to infection. At 24 days after birth, blood culture showed the growth ofCandida albicans. At 29 days after birth, the bilateral knee joints showed swelling. TheCandida arthritis was diagnosed through X ray and joint-cavity lfuid culture. The patient was treated with Fluconazole for 13 weeks. When discharged, the joints swelling had been disappeared, while the muscle tension of the double upper limbs was signiifcantly weak. The shoulders had visible slight activities independently. The ifngers, wrists and elbows had less independent activities. The muscle tension of the double lower limbs was slight and the independent activity was more than upper limbs, but the extend was smaller than normal. The palmar grasp relfex was weak. The 30 months follow-up showed that each joint had even length, no abnormality and normal activities. The language and intelligence were normal.Conclusions NeonatalCandida arthritis is mostly combined with or secondary to systemic disseminated candidiasis, and the priority therapy is antifungal drugs. Surgical operation can be considered if necessary, which can improve the prognosis.
