Objective To investigate the risk factors of stroke-associated infection (SAI)and their correlation with immunodepression.Methods One hundred fourteen consecutive patients with acute ischemic stroke admitted with 24 hours after symptom onset were enrolled.They were divided into infection (n =45) and non-infection (n =69) groups according to whether they had infection or not within 7 days after admission.Automatic blood analyzer and flow cytometry were used to measure the peripheral blood lymphocyte count and T lymphocyte subsets on the day of admission.Univariate analysis and multivariate logistic regression analysis were used to analyze the related risk factors for SAI.Results There were statistical significances in vomiting,disturbance of consciousness,dysphagia,indwelling gastric tube,indwelling catheter on admission,and the National Institutes of Health Stroke Scale (NIHSS) score 0-3 days after admission between the infection and non-infection groups.Further more,the numbers of peripheral blood lymphocytes,CD3 + and CD4 + T lymphocytes in the infection group were significantly lower than those in the non-infection group.Multivariate logistic regression analysis showed that dysphagia,the increased NIHSS score 3 days after admission,and the decreased numbers of CD4 + T lymphocytes were the independent risk factors for SAI.Conclusions The T lymphocyte immune function decreased in patients with SAI.It was the result of the combined action of various factors.Targeted prevention and treatment should be taken to reduce the occurrence of SAI.

Objective To understand the job stressors, coping style and general self-efficacy of emergency nurses and relationship of them. Methods A cross-sectional study involving 83 emergency nurses were conducted through the Chinese Nurses′ Job Stress Sources Scale, Trait Coping Style Questionnaire, General Self-efficacy Scale. Results Job stress among nurses in emergency department was caused by heavy workloads and time distribution (2.88±0.82), issue of patient care (2.78±0.59). The score of emergency nurses′coping propensity was 3.77, indicating more use of positive coping. Negative coping scored higher than the norm (26.99±4.76 vs.21.25±7.41), the difference was significant (t=10.290, P<0.01). The average score of general self-efficacy was 2.40 ± 0.48, lower than the norm (2.86±0.52), the difference was significant (t=-8.07, P<0.01). Active coping style scores were negatively correlated with job stress (r =-0.294, P < 0.05), negative coping style scores were positively correlated with job stress (r =0.239, P<0.05). General self-efficacy and positive coping scores were positively correlated (r=0.270, P<0.05). Conclusions The job stress among emergency nurses was at high level, self-efficacy was not high, but they still have a positive behavior patterns. Nurse managers should take advantage of the positive psychological characteristics of nurses and give reasonable support and guidance, in order to enhance the confidence of nurses and promote self-efficacy, optimize coping, improve the physical and mental health in emergency nurses.

Background and purpose:Gastric cancer impacts human health seriously. Accurate preoperative assessment of T staging and metastatic lymph nodes of gastric cancer was beneifcial to patients’ treatment options and their prognosis. The purpose of this study was to evaluate the diagnostic performance of diffusion-weighted magnetic resonance imaging for preoperative assessment of T staging and metastatic lymph nodes in patients with gastric cancer. Methods:This study selected 35 gastric cancer patients met the criteria for this prospective study. They all underwent gastric 3.0 T MRI+DWI imaging scan. These patients’ T stage and metastatic lymph nodes were evaluated before the surgery, with the reference of post-operative histopathological ifndings. Kappa consistency test was used to assess the consistency of T staging between the two methods. This study analyzed short axis diameter, long axis diameter and apparent diffusion coeffcient (ADC) values of lymph nodes, relative of muscle’s ADC (rADCm) values (rADCm=ADC values of lymph nodes/ADC values of right erector spinae), and relative of primary tumor’s ADC (rADCp) values (rADCp=ADC values of lymph nodes/ADC values of primary tumor) on MRI image. Independent samples test was used to assess the difference between metastatic lymph nodes and benign lymph nodes. The receiver operating characteristic(ROC) curve was generated to evaluate the accuracy of diffusion-weighted magnetic resonance imaging (MRI) for preoperative assessment of metastatic lymph nodes of gastric cancer.Results:The accuracy of diffusion-weighted MRI for T stages was 77.14%, 75% for T1, 100% for T2, 76.47% for T3 and 75.00% for T4 and respectively. There were statistically signiifcant differences in the long axis diameter, the short axis diameter, ADC value, rADCm and rADCp between metastatic lymph nodes and benign lymph nodes (P<0.05). The area under the ROC curve of rADCm was greater than thats of other criteria, so rADCm was the most signiifcant parameter. The best discriminative cut-off value of long axis, short axis, ADC value, rADCm value and rADCp value were 9.55 mm, 6.05 mm, 0.934×10-3mm2/s, 0.60 and 1.083, respectively. The sensitivity and speciifcity were 59.00% and 73.10%, 59.00% and 69.80%, 82.60% and 88.50%, 83.70% and 84.60%, 78.20% and 80.80%, respectively.Conclusion:Diffusion-weighted MRI has great signiifcance for preoperative assessment of T staging and metastatic lymph nodes of gastric cancer.

Objective To investigate the effects of Yindan Xinnaotong Capsule on the osteoponin(OPN)expression in myocardium tissue of the rats after acute myocardial infarction(AMI)and to clarify the mechanism of Yindan Xinnaotong Capsule in alienating rat AMI.Methods 90 Wistar rats were used to establish AMI models by ligating of the left anterior descending coronary artery.The AMI model rats were randomly divided into AMI model group, highdoseofYindan(16g·kg-1·d-1)group,lowdoseofYindan(0.08g·kg-1·d-1)group,positivedrug control captopril(5 mg·kg-1 ·d-1 )group(n=12);at the same time sham operation group(n=10)was set up, the rats in sham operation group was treated with wearing without ligation. All the rats were administrated for 4 weeks,then the myocardium tissue samples were obtained.The histological changes of myocardium tissue were observed by HE and Masson staining;the DNA fragments of apoptotic cells were detected by TUNEL staining and the apoptotic index(AI)was calculated.The expression of OPN mRNA in non-infarction area was measured by RT-PCR.Results Compared with sham operation group,the AI of the rats in model group was significantly increased (P<0.05);compared with model group,the AI of the rats in Yindan groups and captopril group were markedly decreased(P<0.01 );compared with captopril group, the AI of the rats in high dose of Yindan group was significantly decreased(P<0.05).Compared with sham operation group,the expression level of OPN mRNA in non-infarction area of the rats in model group was significantly increased(P<0.01);compared with model group, the expression levels of OPN mRNA in non-infarction area of the rats in Yindan groups and captopril group were also decreased significantly(P<0.01);compared with captopril group,the expression level of OPN mRNA non-infarction area of the rats in high dose of Yindan group was significantly decreased(P<0.05).Conclusion Yindan Xinnaotong Capsule could obviously alleviate the apoptosis of myocardium tissue of the rats after acute myocardial infarction and decrease the expression of OPN mRNA in non-infarction area of left ventricle, which indicates that Yindan Xinnaotong Capsule may protect myocardium tissue through decreasing the OPN mRNA expression.

ABSTRACT:Objective To observe whether N-acetyl-seryl-aspartyl-lysyl-proline (Ac-SDKP)can inhibit rat silicotic fibrosis by regulating stimulatory G proteinα(Gαs)/inhibitory G proteinα(Gαi)signal.Methods Male Wistar rats were randomly divided into three groups (n=1 0 ):control 1 6-w group,silicosis 1 6-w group,and Ac-SDKP pre-treatment group.The pathological changes of the lung tissue was observed by HE staining;the expressions ofα-smooth muscle actin (α-SMA),Collagen Ⅰ,fibronectin (Fn),Gαs,Gαi2 ,Gαi3 and cAMP were detected by Western blot.Immunofluorescence was performed on lung tissue sections to detect the coexpression ofα-SMA/Gαi3 . Results Within silicosis 16-w group,HE staining showed that the silicotic nodule volume increased,nodule fusion and the formation of interstitial fibrosis could be seen,and cell fibrous nodules were visible.Immunofluorescence staining showed the enhanced coexpression ofα-SMA/Gαi3 in fibrosis area.Compared with those in control group, the expressions ofα-SMA,Collagen Ⅰ,Fn,Gαi2 and Gαi3 significantly increased in silicosis 1 6-w group,but the expressions of Gαs and cAMP decreased.Compared with silicosis 1 6-w group,Ac-SDKP pre-treatment group had alleviated lung injury and decreased coexpression ofα-SMA/Gαi3 .The expressions ofα-SMA,Collagen Ⅰ,Fn,Gαi2 and Gαi3 protein significantly decreased in Ac-SDKP pre-treatment group,while the expressions of Gαs and cAMP increased obviously.Conclusion Ac-SDKP can regulate the expressions of Gαs and Gαi and promote the formation of cAMP,thus playing an effective role against silicotic fibrosis.

Salmonella enterica serovar Choleraesuis strain C500 is a live attenuated vaccine that has been widely used in Chi‐na for over 50 years to prevent piglet paratyphoid .However ,as C500 is obtained by chemical methods ,the genetic background of this strain remained unclear .In this study ,we compared the genomic differences between the virulent reference strain C 78‐2 and C500 by suppression subtractive hybridization combined with the mirror orientation selection method (MOS‐SSH ) .Six genes (asr ,ydgF ,ydgD ,ydgE ,rpoS ,and ptsG) were lost in C500 strain .Using real‐time PCR analysis ,we demonstrated that the genes regulated by rpoS ,a vital transcriptional regulator playing an important role in Salmonella infection ,were downregulated in C500 .Additionally ,the virulence of the rpoS mutant strain C78‐2ΔrpoS was 100 000 times lower than the parental strain in BALB/c mice .So loss of rpoS gene is the major factor leading to the attenuation of C500 strain .

Objective To observe the curative effect of low molecular heparin for treating secondary high coagulation state in the patients with nephrotic syndrome(NS) .Methods Total 87 cases of NS in our hospital were divided into the conventional treat‐ment group (n=42) and the low molecular heparin treatment group (n=45) .The routine treatment group was given the prednisone treatment and the low molecular heparin treatment group was treated by low molecular heparin combined with prednisone .The re‐lated indicators of blood coagulation before and after treatment were detected and the clinical curative effects in two groups were an‐alyzed .Results The coagulation related indicators in the conventional treatment group had no statistically significant difference be‐tween before and after treatment (P>0 .05) ,the prothrombin time(PT) and activated partial thrombin time(APTT) after treat‐ment in the low molecular heparin treatment group were significantly extended compared with before treatment ,while the concen‐trations of D‐dimer and fibrinogen were significantly decreased and the concentration of antithrombin Ⅲ was markedly increased compared with before treatment ,showing statistically significant differences between the two groups (P<0 .05);the patients of the low molecular heparin group patients had no bleeding after treatment .Conclusion Low molecular heparin combined with predni‐sone can reduce the secondary high condensation state in NS without bleeding and has a significantly clinical effect .

Objective:To investigate clinical efficacy of tunneled subcutaneous pedicle flaps in the repair of ipsilateral nasal alar defects.Methods:A total of 20 patients with nasal alar defects after excision of basal cell carcinoma, which were repaired with ipsilateral tunneled subcutaneous pedicle flaps, were retrospectively collected from January 2018 to January 2020. There were 11 male patients and 9 female patients, with an average age of 60.6 years (range, 48 - 76 years) . According to clinical manifestations, the basal cell carcinoma lesions could be classified into 3 types: nodular-ulcerative type (13 cases) , superficial type (4 cases) and pigmented type (3 cases) , and the skin lesions varied from 0.4 cm × 0.5 cm to 0.9 cm × 1.1 cm in size. All the patients received extended surgical resection of basal cell carcinoma, and nasal alar defects were repaired with tunneled subcutaneous pedicle flaps, with the size of flaps ranging from 0.7 cm × 0.8 cm to 1.2 cm × 1.4 cm.Results:All flaps survived successfully, the nasal alar defects were completely repaired, and all wounds healed primarily. During the postoperative follow-up of 6 - 24 months, neither recurrence of tumors nor obvious scar hyperplasia occurred on the nasal ala, and nasal morphology and appearance were favorable.Conclusion:The tunneled subcutaneous pedicle flap can be used for the repair of ipsilateral nasal alar defects following excision of tumors or other lesions, resulting in favorable therapeutic outcomes and aesthetic appearance.

Objective:To evaluate the relationship between Na + leak current channel (NALCN) in hippocampal dentate gyrus (DG) region and cognitive function in mice. Methods:Thirty-nine male wild type C57BL/6 mice, aged 6-8 weeks, weighing 18-22 g, were studied.Three mice were sacrificed to verify the expression of NALCN co-localized with neuronal nuclear antigen (NeuN) in the hippocampal DG region through immunofluorescence technique.The remaining 36 mice were divided into 2 groups ( n=18 each) by the random number table method: control group (group C) and NALCN gene knockout group (group KO). NALCN-shRNA virus was injected in hippocampal DG region in group KO, and scrambled-shRNA virus was injected in group C. Three weeks after virus injection, behavioral tests (Y maze test and open field test) were performed, then the animals were sacrificed, and the hippocampal tissues were removed for determination of the expression of NALCN protein and mRNA using Western blot and real-time polymerase chain reaction. Results:NALCN and NeuN colocalized a lot on the same neuron in the hippocampal DG region of mice, and NALCN was widely expressed in the hippocampal DG region.Compared with group C, the expression of NALCN protein and mRNA was significantly down-regulated, the times of entering the new arm were reduced, the duration of staying at the new arm was shortened ( P<0.05), and no significant change was found in the parameters mentioned above in the open field test in group KO ( P>0.05). Conclusions:NALCN in the hippocampal DG region is involved in the regulation of cognitive function in mice, and the down-regulation of NALCN may lead to cognitive decline.

Objective To explore the effect of regional synergistic treatment system on the treatment time and short-term prognosis of patients with ST-segment elevation myocardial infarction (STEMI).Methods A retrospective analysis of the clinical data of STEMI patients who admitted to emergency center of Suzhou Kowloon Hospital Affiliated to Shanghai Jiaotong University School of Medicine and underwent primary percutaneous coronary intervention (PPCI) from January 2013 to January 2017 were conducted. All patients were divided into two groups, group A was the patients who underwent the PPCI before the establishment of the acute chest pain area co-treatment system (from January 2013 to December 2014), and group B was the patients who received the treatment after the establishment of the area co-treatment system (from January 2015 to January 2017). The length of time from onset of symptoms to the balloon dilatation (S2B), the length of time from the first medical contact to the balloon dilatation (FMC2B), the length of time from entering the gate of hospital to the balloon dilatation (D2B), and the incidence of 90-day end point events (including heart failure, all-cause death, and other related adverse events) were collected. The relations of the establishment of the acute chest pain area co-treatment system and the incidence of 90-day end point events were analyzed by multivariable Logistic regression analysis.Results Among the 221 enrolled patients with STEMI, 83 patients were in group A and 138 patients were in group B respectively. Compared with group A, S2B time [minutes: 180 (140, 210) vs. 201 (154, 225)], FMC2B time [minutes: 89 (78, 100) vs. 94 (83, 107)] and D2B time [minutes: 66 (62, 70) vs. 85 (72, 99)] were significantly shortened in group B (allP < 0.05), the incidence of 90-day end point events were significantly decreased (heart failure:20.3％ vs. 32.5％, all-cause death: 1.4％ vs. 7.2％, other related adverse events: 23.2％ vs. 36.1％, allP < 0.05). It was shown by multivariable Logistic regression analysis that the establishment of the acute chest pain area co-treatment system could lower the incidence of 90-day end point events [heart failure: odds ratio (OR) = 1.904, 95％ confidence interval (95％CI) = 0.968-1.004, P = 0.048; all-cause death:OR = 11.724, 95％CI = 0.955-1.048,P = 0.013; other related adverse events:OR = 1.925, 95％CI = 1.049-3.530,P = 0.034].Conclusion The construction of regional synergistic treatment system can shorten the emergency treatment time of STEMI patients and reduce the incidence of 90-day end point events including heart failure and death.
