1.Studies on fingerprint of bidens bipinnata L.by HPLC

Objective The fingerprint method based on high-performance liquid chromatography for the quality control of bidens bipinnata L were firstly described and optimized. Methods HPLC system was used to obtain the fingerprint spectrum of bidens bipinnata L. The chromatography condition was ODS C18 column (4. 6mm×255mm,25μm),mobile phase Methanol-0. 2% HAc,and flow speed 1. 0ml ? min-1, detection wavelength 363nm.Results In this experiment,the standard fingerprint had been established and the similitude degree of 10 batches bidens bipinnata L. has also been obtained(0.941~0.999). Conclusion The method was accurate,reliable,good reproducibility, strong characteristic and could be used for the standard of general judgement and the quality estimation about the bidens bipinnata L.
2.Operations and Application Experience of Perpendicular Acupuncture at the Back-Shu Points of the Five Zang-organs

The Back-Shu points of the five Zang-organs are a group of points where qi and blood pour into the back and lumbar regions. Acupuncture at these points has a wide indication scope. However, it’s reported that internal organs were punctured in some cases because of improper control of needling depth, angle, and intensity during acupuncture. We used perpendicular needling instead of conventional oblique needling during the acupuncture of the Back-Shu points of the five Zang-organs. We found that when patients were completely relaxed, acupoints were correctly selected, the needling depth, the way of holding the needle, and inserting methods were well controlled, perpendicular needling of the Back-Shu points of the five Zang-organs can produce stronger needling sensation, and it’s easier to operate, safer, and more effective compared to oblique needling.
3.A COMPARITIVE STUDY ON IODINE NUTRITIONAL STATUS OF STUDENTS IN ZHOUSHAN ISLANDS

Objective: To measure the iodine nutritional status of students in Zhoushan Islands and evaluate the advantages and disadvantages of iodized salt prophylaxis. Methods: A comparative study was carried out in 12 classes which were selected from Dinghai(iodized salt district) and Daishan(non-iodized salt district) of Zhoushan Islands by random sampling. Mann-Whitney test was used to compare the urinary iodine concentration and dietary iodine intake of the two groups. Spearman correlation was used to look for the correlation between urinary iodine concentration and dietary iodine intake in the two groups. Ordinal regression was used to analyze the factors that affect the urinary iodine concentration. Poisson regression was used to analyze the risk factors of goiter.Results: The urinary iodine concentration in non-iodized salt district was 129 ?g/L,which was lower than 169 ?g/L in iodized salt district(u=6.302,P=0.000) , whereas the daily iodine intake in the two groups was 105 ?g/d and 184 ?g/d respectively, the difference was also significant(u=4.710,P=0.000). The influence of sex and iodized-salt intake was significant in the final regression model (P0.05). 4.Expression and significance of E-cadherin and matrix metallopeptidase 7 in basal-like and Her-2 over-expressing breast carcinoma

Objective To investigate the expression of E-cadherin and matrix metallopeptidase 7(MMP-7)and their significance in basal-like and Her-2 over-expressing breast carcinoma. Methods The protein expression of E-cadherin and MMP-7 were analyzed by immunohistochemical method in 20 cases of basal-like breast carcinoma and 21 cases of Her-2 over-expressing breast carcinoma.Statistical method was used to analyze the clinicopathological performance.Patients were followed up from 6 to 60 months.Results The ratio of basal-like breast carcinoma and invasive ductal carcinoma was 3.4%(20/501).the mean age was 49 years.The mean size of basal-like breast carcinoma(2.1 cm)was smaller than that of Her2 over-expressing breast carcinoma(3.0 cm)(P<0.05).The 5-year survival rate of patients with basal-like breast carcinoma was obviously(40.0%) lower than that of Her-2 over-expressing breast carcinoma (71.4%)(P<0.05),though,the 3-year survival rate was similar between the two groups.The protein expression of E-cadherin was down-regulated and even deficient in basal-like breast carcinoma(P<0.05).The protein expression of MMP-7 was higher in basal-1ike breast carcinoma than that in Her-2 overexpressing breast carcinoma(P<0.05).Conclusion Down-regulated expression of E-cadherin and upregulated expression of MMP-7 may be one of the mechanisms leading to high metastasis of basal-like breast carcinoma.
5.Effects of Mirror Visual Feedback and Electromyographic Biofeedback on Upper Extremity Function in Hemiplegics after Stroke

Objective To investigate the effects of mirror visual feedback (MVF) and electromyographic biofeedback (EMGBF) on upper extremity function in hemiplegic patients after stroke based on task-oriented training. Methods 90 patients with hempiplegia after stroke were randomly divided into control group (n=30), EMGBF group (n=30) and MVF group (n=30). All patients accepted routine rehabilitation and task-oriented training once a day for 8 weeks. The EMGBF group also accepted EMGBF, and the MVF group accepted MVF in addition. They were assessed with Fugl-Meyer Assessment (FMA) and the Upper Extremity Function Test (UEFT), and their integrated electromyogram (iEMG) of affected upper extremities were recorded before and after treatment. Results All the groups improved in scores of FMA and UEFT, as well as the iEMG after treatment (P<0.05), and ranked as the MVF group, the EMGBF group and the control group from improving more to less (P<0.05). Conclusion Mirror visual feedback combined with electromyographic biofeedback may further promote the recovery of upper limb function in patients with hemiplegia after stroke based on task-oriented training.

Methods ArticularchondrocyteswereisolatedfromSprague-Dawley rats, and their phenotype was determined by toluidine blue and immunocytochemical staining. The GAG content of cell culture supernatant was deter-mined by dimethyl-methylene blue spectrophotometric assay, while Hyp content by chloramine T assay. ELISA assay was used to measure MMP-2 , TIMP-2 content. Furthermore, the ERK1/2, p38 MAPK phos-phorylation protein expression levels were tested by Westernblotassay.Results ASIC1acontributedto the effect of GAG, Hyp and TIMP-2 levels reduction induced by extracellular acidification, while the effect of MMP-2 was weaker. Moreover, ASIC1a could in-crease the ERK1/2 , p38 MAPK phosphorylation pro-teinexpressionlevels.Conclusion ASIC1acould regulate rat articular chondrocytes matrix turnover via ERK1/2 and p38 MAPK signaling pathway, and there-by inhibit the rat articular cartilage damage induced by acidosis.
7.Establishment of a visualized detection method of Sendai virus by reverse transcription loop-mediated isothermal amplification

Objective To establish a simple and sensitive detection method of Sendai virus ( SeV ) by reverse transcription loop-mediated isothermal amplification ( RT-LAMP) technique. Methods According to the published Gen-Bank sequences (DQ219803. 1), six pairs of primers were designed targeting the conserved region of SeV. The amplifica-tion products were detected with a LAMP real-time Turbidimeter. (LA-302). Through optimizing the LAMP primers and re-action conditions, a rapid and specific detection method of SeV was established. Meanwhile, the amplified products were colored by fluorescence detection reagent after completion of the reaction, so that the amplification could be visualized and detected by naked eyes. Then, methodological evaluation of the RT-LAMP was tested. Results The method of RT-LAMP showed a highly efficient amplification for SeV viral target gene which was performed at 63℃ for 60 min with the LAMP re-al-time Turbidimeter (LA-302). The detection limit was 2. 1 TCID50, 100 times higher than that of RT-PCR, and no cross-reaction with other RNA and DNA viruses of mice was observed. The results of SeV LAMP reaction was visualized and the tube could be directly observed by naked eyes with the addition of fluorescence detection reagent. The results were consist-ent with the results detected by real-time tubidimeter. 92 clinical samples were detected byRT- LAMP, RT-PCR and indi- rect ELISA, and the coincidence rate was 100%. Conclusions This established SeV RT-LAMP detection method is fast, specific, highly sensitive,easy to perform under simple conditions, and is suitable for rapid detection of Sendai viirus.
8.Disposal of a COVID-19 outbreak caused by imported case of an Omicron variant strain

Abstract: Objective To retrospectively analyze the investigation and disposal of the COVID-19 outbreak caused by the transmission of the Omicron variant in infected imported cases, and provide basis for COVID-19 outbreak management. Methods The description epidemiological method was used to describe the COVID-19 outbreak in Sanya City from March 31 to April 15, 2022. The propagation chain was mapped and the experience gained and shortcomings identified in emergency responses were analyzed. Results The outbreak resulted in 95 reported locally transmitted COVID-19 cases with a incubation period M(P25, P75) of 4 (3, 5) d. In the 95 cases, the proportion of cases detected through close contact screening, centralized isolation, community screening, control area screening, active treatment (examination), and key population screening were 33.68%, 22.11%, 18.95%, 12.63%, 6.32%, 4.21% and 2.11%, respectively. The epidemic spread for 6 generations, causing 5 clusters of outbreaks and 12 cases of cluster disease. The epidemic affected 12 villages/neighborhood committees, 1 bar, 1 hospital, 1 small clinic, 1 farmer's market, 1 large shopping mall and 1 restaurant in 2 districts of Sanya City. The result of gene sequencing was Omicron variant BA.1.1. Through the immediate launch of emergency plans, nucleic acid and antigen testing, controlling close contact between infected persons and close contacts, suspending indoor business sites, central urban control, and temporary suspension, COVID-19 was controlled within 16 days. Conclusions The transmission chain of this outbreak was clear and was caused by imported cases. Strengthening the management of the pass, doing a good job in information sharing and docking, timely screening for cases, screening, pushing, controlling high-risk groups, and implementing comprehensive control measures, can effectively prevent the spread of the epidemic, providing a reference for the control of epidemic situations in relevant scenarios.
9.Expression and significance of immune checkpoint B7-homolog 4 in endometrial cancer

Objective:To investigate the expression of B7 homolog 4 (B7-H4) and its clinical significance in endometrial cancer.Methods:A total of 833 patients with endometrial cancer admitted to Peking Union Medical College Hospital, Chinese Academy of Medical Sciences from 2009 to 2019, were enrolled. The expression of B7-H4, mismatch repair (MMR), p53, programmed cell death ligand 1 (PD-L1) protein, and CD 8+ T lymphocyte density in endometrial cancer tissues were detected by the EnVision two-step method of immunohistochemical staining. First-generation sequencing (Sanger method) was used to determine molecular subtyping of endometrial cancer. The χ2 test was used to compare the differences in positive expression rate of B7-H4 protein in endometrial cancer tissues with different clinicopathological features and molecular subtyping, PD-L1 protein expression, and CD 8+ T lymphocyte density. Survival analyses [including recurrence-free survival (RFS) and disease-specific survival (DSS)] were performed for 664 patients with follow-up time≥3 months, with a median follow-up time of 31 months (range: 4-121 months), and the Cox proportional hazards regression model was used to analyze the relevant factors affecting the prognosis of patients with endometrial cancer. Results:(1) The median age of 833 patients was 58 years (range: 25-88 years); pathological type: 595 with endometrioid carcinoma, 238 with non-endometrioid carcinoma; surgical-pathological staging: 542 cases at stage Ⅰ, 38 cases at stage Ⅱ, 173 cases at stage Ⅲ, and 45 cases at stage Ⅳ. Molecular subtyping was performed in 590 patients, including 50 with POLE mutation, 163 with mismatch repair defect (MMR-d) type, 246 with nospecific molecular change (NSMP) type, and 131 with p53 mutation subtype. (2) B7-H4 protein was expressed with brownish-yellow stainind in the cell membrane and cytoplasm of endometrial carcinoma, and the positivity rate of B7-H4 protein was 71.5% (596/833). The positivity rates of B7-H4 protein among patients with different age, surgical-pathological stage, tumor grade, pathological type, depth of muscular invasion, presence or absence of lymphovascular space invasion, and molecular subtype were significantly different (all P<0.05). The positivity rates of B7-H4 protein among patients with different PD-L1 protein expression and CD 8+ T lymphocyte density were not significantly different ( P>0.05). The 5-year RFS (83.9%) and DSS (87.3%) of B7-H4 protein-positive patients had an increasing trend compared with the 5-year RFS (77.2%) and DSS (78.1%) of B7-H4 protein-negative patients, but there were not statistically significant differences ( P=0.053, P=0.083). (3) Univariate analysis showed that the 5-year RFS and DSS of patients with different age, tumor grade, surgical-pathological stage, pathological type, depth of muscular invasion, lymphovascular space invasion, and molecular subtype were significantly different (all P<0.01). There were no significant differences in 5-year RFS ( P=0.184, P=0.113) and DSS ( P=0.549, P=0.247) among patients with different CD 8+ T lymphocyte density and PD-L1 protein expression. Further analysis according to molecular subtype, the results of CD 8+ T lymphocyte density and PD-L1 protein expression showed that the 5-year RFS and DSS of B7-H4 protein-positive patients were higher than those of B7-H4 protein-negative patients with NSMP subtype, low density of CD 8+ T lymphocyte and PD-L1 protein-negative endometrial carcinoma (all P<0.05), however, there was no significant difference in 5-year DSS between B7-H4 protein-positive patients and B7-H4 protein-negative patients with PD-L1 protein-negative endometrial cancer ( P=0.060). Multivariate analysis showed that positive expression of B7-H4 protein was an independent factor for 5-year RFS ( HR=0.27, 95% CI: 0.09-0.78, P=0.016) and DSS ( HR=0.16, 95% CI: 0.05-0.58, P=0.005) in patients with NSMP subtype endometrial carcinoma. In patients with low-density CD 8+ T lymphocytes endometrial cancer, positive expression of B7-H4 protein was an independent factor for 5-year RFS ( HR=0.45, 95% CI: 0.26-0.80, P=0.006), but it was not an independent factor for 5-year DSS. In patients with PD-L1 protein-negative endometrial cancer, B7-H4 protein was not an independent factor for 5-year RFS. Conclusion:B7-H4 protein expressed highly in endometrial carcinoma tissues, and its high expression is closely related to clinicopathological features, molecular subtype of p53 mutant and NSMP, and the favorable prognosis of patients with low density of CD 8+ T lymphocyte immunophenotype endometrial carcinoma.
10.Clinicopathological and molecular characterization of high-grade endometrial carcinoma with POLE mutation: a single center study

Objective:
The molecular classification system of endometrial carcinoma (EC) in ‘The Cancer Genome Atlas’ is widely acknowledged for its prognostic utility. Subsequently, more simplified classification system that incorporate DNA polymerase epsilon (POLE) exonuclease domain mutations, mismatch repair deficiencies (MMRd), and abnormal p53 (P53abn) has also demonstrated its clinical utility. These classifications helped identifying a ‘POLE ultramutated’ (POLEmut) category of patients, most of whom show excellent prognoses despite having high-grade ECs. We aimed to investigate the clinicopathological and molecular characteristics of high-grade ECs with POLEmut.
Methods:
We investigated 414 patients with high-grade ECs (including endometrioid carcinomas grade 3, serous carcinomas, clear cell carcinomas, mixed carcinomas, undifferentiated and dedifferentiated carcinomas, and carcinosarcomas) by sequencing and immunohistochemical staining.
Results:
Forty-three tumors (10.4%) were classified as POLEmut, including 2 with new, possibly pathogenic POLE mutations at P286C and L424V. These patients had very good prognoses except for 1 with stage IV disease and residual tumor. Eleven patients in this group also had P53abn and 4 had MMRd; molecular analysis revealed that patients with synchronous POLE pathogenic mutation and other mutations had a POLEmut or MMRd phenotype; survival analysis found no difference in prognosis between these patient categories. The prognoses of patients in the POLEmut EC group were not significantly influenced by treatment or risk category.
Conclusions
Patients with high-grade EC exhibiting POLEmut have very good clinical outcomes, and should be identified urgently in daily work owing to their conflicting morphology. Our findings also provide guidance on subclassifying ECs with poor histological appearance.
