1.Clinical Significance of HBV Molecular Genotypes and Quantitative Serology Detection in Patients with Chronic Hepatitis B

Objective To characterize the molecular genotypes and quantitative serological biomarker, and to explore their clinical significance in patients with chronic hepatitis B ( CHB) . Methods A total of 210 CHB patients were enrolled and HBV DNA loading, HBV molecular genotypes, quantitative serological biomarker, and ALT were measured. Patients were grouped according to the natural phases of HBV infection:HBeAg (+) immune tolerance, immune clearance, HBeAg (-) low replicative and reactivation phase. The correlation was analyzed between molecular genotypes and serological biomarkers, HBsAg and other biomarkers. Results Subgenotypes B2 with high level of HBV DNA and C2 with middle level of HBV DNA were found to be most prevalent. The positive rates were 59. 0%(B2) and 25. 2%(C2). Patients genotype with B showing more potent viral activity. Subgenotype B2 was sinificantly corelated with HB-sAg, HbeAg, and ALT levels(P < 0. 01), but there was no statistical difference for subgenotype C2. There was statistical difference be-tween positive rates of subgenotypes(P>0. 05), but HBsAg level was found to be significantly correlated with HBV DNA, HBeAg, ALT levels during some phases of CHB. Conclusion Molecular genotyping and quantitative serological biomarkers detection might be helpful for earlier prediction of the long-term disease outcomes in patients with CHB.
2.The prevalence rate of mild cognitive impairment and its related factors in community elderly in Hangzhou City

Objective To understand the prevalence rate of mild cognitive impairment (MCI) and its related factors in community elderly so as to provide evidence for the community intervention. Methods A cross-sectional study was performed in a random sampling of 925 elderly people. Mini-mental state examination (MMSE), activities of daily living (ADL) scale and questionnaire were used for face-to-face interview. Chi-square test and logistic regression analysis were applied in this study. Results The prevalence rate of MCI was 21.1% in elderly people. Female, high age, illiteracy, loss of spouse, lack of exercise, no professional activitives and not doing some work for the descendants were the risk factors for MCI. Conclusions The aging and the low educational level are the risk factors for MCI in the elderly. High self-care ability scores are the protective factors for MCI.
3.Measurement of serum interleukin-2,soluble interleukin-2 receptor and interleukin-6 of renal allograft recipients

Sequential monitoring of the levels of serum interleukin-2(IL-2),soluble interleukin-2 receptor(sIL-2R)and interleukin-6(IL-6)were conducted in 60 patients for 2 months after renal transplantation.The results showed that the levels of serum IL-2,sIL-2R and IL-6 were increased significantly several days prior to the clinical diagnosis in the patients with acute rejection,which were much higher than those in CsA-induced nephrotoxicity group.The levels of IL-2,slL-2R and IL-6 in the patients with rejection sensible to methylprednisolone came down to the pre-rejection levels several days after the treatment.It was concluded that sequential monitoring of serum IL-2,sIL-2R and IL-6 of renal allograft recipients are helpful for the early diagnosis and differential diagnosis of acute rejection as well as the evaluation of methylprednisolone in the treatment of antirejection.

Objective To investigate the Correlation between Plasma Total Homocysteine and Copper in Patients with coronary artery disease complicated with diabetes mellitus. Methods 31 pateints with coronary artery disease complicated with diabetes mellitus were studied. 36 age-matched and gender-matched patients with coronary artery disease not complicated with diabetes mellitus and 30 normal subjects were served as control groups. The concentrations of copper and total homocysteine was determined and compared. Objective To investigate the association of Lipoprotein(a) concentrations and patients with aortic dissection.Methods 27 pateints with aortic dissection were studied. Sixty age-matched and gender-matched patients with acute myocardial infarction and fifty normal subjects were served as control groups. The left ventricular (LV) myocardial remodeling process that occurs in various settings of congestive heart failure (CHF) had historically been attributed to intrinsic changes in the cardiac myocyte. However, it is now recognized that important changes also occur within the extracellular matrix (ECM) of the myocardium, contributing to the remodeling process. Matrix metalloproteinases are a family of proteolytic enzymes responsible for myocardial extracellular protein degradation.Several MMP species identified within the human myocardium may be dysregulated in congestive heart failure (CHF). The purpose of the present review is to present a brief overview of the matrix metalloproteinases (MMPs) within the myocardium that likely contributes to myocardial remodeling,to examine the results from basic studies to explore relationship with respect to MMPs activation and congestive heart failure.-
7.Enterococcus faecium: Its Clinical Distribution and Drug Resistance

OBJECTIVE To investigate the clinical distribution and antibiotic resistance of Enterococcus faecium in our hospital and provide the reference for the clinical treatment. METHODS The statistical method was used to analyze the status of drug resistance of 519 E. faecium strains which were isolated form our hospital from Jun 2006 to May 2008. RESULTS Among 519 isolated E. faecium strains, most common sources of specimens were urine (34.9%), sputum(26.8%) and feces (18.3%); E. faecium infection mainly distributed at ICU, respiratiory wards and EICU; E. faecium was resistant to multiple antibiotics. The drug-resistant rates to vancomycin and teicoplanin were 6.2% and 3.2%, respectively. CONCLUSIONS E. faecium could cause various infection, and has a high antibiotic resistance which is difficult to treat, we should be paid high clinical attention.
8.Surveillance on MIC of Antibiotic Resistance of Meticillin-resistant Staphylococcus aureus

OBJECTIVE To review and analyze the change in the MICs of vancomycin,teicoplanin and linezolid in meticillin-resistant Staphylococcus aureus(MRSA) strains isolated in our hospital from 2003 to 2007. METHODS The MICs of vancomycin,teicoplanin and linezolid were tested by Etest method on a sample of randomly selected MRSA strains. RESULTS The incidences of MRSA increased from 52.2% in 2003 to 74.5% in 2007.MIC of vancomycin increased from 1.85 ?g/ml in 2003 to 2.15 ?g/ml in 2007,and teicoplanin MIC geometric mean increased even more markedly from 1.28 ?g/ml in 2003 to 2.07 ?g/ml in 2007.The linezolid MIC remained almost unchanged. CONCLUSIONS The incidences of MRSA were increasing from 2003 to 2007.There is a upward trend in MIC of glycopeptide over the years,in which the increase for teicoplanin is higher than others two.
9.Functional results of lung volume reduction surgery in 43 patients with severe chronic obstructive pulmonary emphysema

Objective To evaluate the outcome of lung volume reduction surgery (LVRS)under video-assisted thoracoscopy in patients with severe chronic obstructive pulmonary emphysema.Methods The follow-up date of 68 patients with severe emphysema underweut LVRS by video-assisted thoracoscopy were analyzed retrospectively betweeu June 2002 to June 2012.Mean FEV1.0 、FCV 、RV、PO2 、PCO2 、mean 6MWT were assessed before their surgery and 6 months,1-year and 2-years after their surgery respectively.Results Compared with their preoperative results,Lung function,arterial blood gasanalysis and 6-minute walk distance (6-MWD) increased outstanging.Conclusion LVRS by VATS provides significant clinical and physiological improvement for the majority of patients with severe chronic obstructive pulmonary emphysema.
10.Living condition and healthcare demand of elderly adults in Hangzhou

Objective To find out the living condition of the elderly adults and their needs for community bealthcare services so as to provide evidence-based guidance for our govemment. Methods A total of 925 participants aged > 60 years were personally interviewed for questionnaire and ADL scale evaluation. Chi-square test and Kmskal-Wallis H test were used for data analysis. Results The subjects showed relatively lower education level and the illiteracy rate was high. Their marriage was stable and a decreased divorce rate was found. The economic situation was satisfactory. Their life satisfaction was farbeyond the health satisfaction. Most of the elderly did not live with their offspring,although 21.8% could not take care of themselves. The prevalence rate of chronic diseases was 81.5% ,which resulted in more demands for social activities, health consultation, rehabilitation nursing, and home healtheare services in the community. Conclusion They need to pay more attention to physical and mental health of the elderly. A community healthcare system that could provide services in an-all-round way,including medical care, should be established and improved.
