1.Analyze the practice results of digital subtraction angiography composite operating room hundred grade purification

Objective To discuss the practice effect about the waiting area in the hospital building of the daping hosptial ,the third military medical university to be reformed into digital subtraction angiography (DSA) composite operating room .Methods We reasonably choiced the form of purify conditioning ,medical C arm ,smallpox air supply ,and the efficient screening device layout at the end of smallpox air supply .Results The main parameters of the measured results met the national standards of indexes .The fan frequency was 48 Hz running at hundred grade ,and its operation energy consumption was about 25 kw/h ;the fan frequency was 35 Hz running at ten thousand grade ,and its operation energy consumption was about 12 kw/h;the conversion time was very short between hundred grade and ten thousand grade running ,about 10 minutes .Conclusion On the condition that the building environ-ment is not fit to install hanging medical C arm ,we can thorough choose floor type C arm ,adopt “leakage resistance type clean air supply smallpox” ,reasonably modify the form of purify air conditioning to design or modify a hundred grade operating room meet-ing requirement .
2.Treatment of hepatocellular carcinoma with bile duct tumor thrombus

Objective To study the treatment of hepatocellular carcinoma(HCC) complicated with bile duct thrombus.Methods Restrospective study was performed on the treatment of 34 cases of HCC complicated with bile duct tumor thrombus from January 1995 to December 2002 admitted to our hospital.Results Among the 34 cases, one patient had no treatment,6 cases received PTCD and 27 cases received operation.In surgical treatment patients,the postoperative 30 days mortality was 22.2%(6/27),the morbidity was 55.6%(15/27).In 16 patients who received hepatectomy with thrombectomy and biliary duct drainage, the survival time was 1~27 months with a median survival time of 16.5 months.Conclusion The prognosis of HCC complicated with bile duct tumor thrombus is poor. Early diagnosis and aggressive operative are the key points to improving the survival time of patients.
3.Effects of Recombinant Human Growth Hormone on Hypoalbuminemia in Cirrhotic Rats after Partial Hepatectomy

Objective To investigate the effects of rhGH on hypoalbuminemia in cirrhotic rats after partial hepatectomy. Methods Thirty rats were randomly divided into normal control group ( n =6), liver cirrhosis group (LC group, n =6), liver cirrhosis and hepatectomy group (LCH group, n =6), PN (parenteral nutrition) group ( n =6, given PN after hepatectomy) and rhGH+PN group ( n =6,given rhGH and PN after hepatectomy). Liver function and blood glucose were measured. The expression of ALB mRNA was detected by RT PCR. Liver Ki67 immunohistochemistry was studied. Results Compared with PN group, serum ALP was lower; serum ALB and blood glucose were higher in rhGH+PN group. The expression of hepatic ALB mRNA was higher, and hepatic Ki67 labeling index was higher as well in this group. Conclusion rhGH can improve hypoalbuminemia after partial hepatectomy in cirrhotic rats with partial hepatectomy.
4.Comparison of Dissolubility of Lansoprazole Enteric- coated Tablets With That of Enteric- coated Capsules

OBJECTIVE: To study the method of measuring the in vitro dissolubility of lansoprazole enteric- coated tablets and to find out the difference between the dissolubility of lansoprazole enteric- coated tablets and that of lansoprazole enteric- coated capsules.METHODS: To establish a measuring method of the dissolubility of lansoprazole enteric- coated tablets and compare the dissolubility of lansoprazole enteric- coated tablets with that of lansoprazole enteric- coated capsules and to acquire the dissolving parameters for variance analysis.RESULTS: (1)The accumulative dissolving quantity of the both preparations exceeded 80% in 30 minutes.(2)The comparison of Weibull parameters,T50,Td and m,between each other in group A showed no significant differences(P>0.05).It was the same for those of group B.(3)The parameters,T50 and Td,of group A,in comparison with those of group B,showed significant differences(P<0.001).The times needed for dissoiving 50% and 63.2% of quantity in group B were both shorter than those in group A.The m parameters in different groups had no significant differences(P>0.05).There was no significant difference in slope of equation between two groups.CONCLUSION: This measuring method of the in vitro dissolubility of lansoprazole enteric- coated tablets is simple,convenient,accurate,stable,feasible and good in repetition.It is concluded that there is significant difference between the dissolubility of the two dosage forms of enteric- coated preparation of lansoprazole.
5.Effects of branched-chain amino acids-enriched early parenteral and enteral nutrition on the liver function and serum aminograms in cirrhotic rats after partial hepatectomy

Objective To evaluate the effects of branched-chain amino acids-enriched early parenteral and enteral nutrition on the liver function and serum aminograms in cirrhotic rats after partial hepatectomy. Methods In this prospective randomized controlled study, 24 cirrhotic rats, induced by thioacetamide, were randomized into three groups: enteral nutrition (EN) group, EN + branched-chain amino acid (BCAA) group, and parenteral nutrition (PN) + BCAA group. After receiving partial hepatectomy, rats in all three groups were nutritionally supported with equal amount of calorie and nitrogen contents from the 1st postoperative day ( PO day 1 ) to PO day 5. On PO day 6, parameters including body weight, liver functions, prealbumin, transferring, and serum aminograms were measured or determined, and the level of liver albumin mRNA was detected by reversal transcription-polymerase chain reaction and morphological examinations such as HE staining and immunohistochemical staining, which were assessed by index of Ki67 protein index. Results Body weight was significantly decreased in all three groups on PO day 6 (P <0.05 ). Compared with EN + BCAA group, serum aspartate aminotransferase, alanine aminotransferase, and alkaline phosphatase after partial hepatectomy were significantly higher in PN + BCAA group (P <0.05 ). Serum alkaline phosphatase level was significantly higher in PN + BCAA group than in EN group ( P <0. 05). The level of prealbumin was significantly lower in PN + BCAA group when compared with EN group or EN +BCAA group ( both P < 0. 05 ), although no such significant difference was noted in terms of transferrin ( P >0. 05 ). The levels of leucine and isoleucine elevated while those of tyrosine, phenylalanine, arginine and tryptophan declined in PN + BCAA group or EN + BCAA group when compared with EN group ( P < 0. 05 ). Aminograms were not significantly different between EN + BCAA group and PN + BCAA group ( P > 0. 05 ). Levels of total amino acid and aromatic amino acid (AAA) were significantly lower while BCAA and ratio between BCAA and AAA (BCAA/AAA) were significantly higher in PN + BCAA group or EN + BCAA group than in EN group (P < 0. 05 ).Significantly lower level of albumin mRNA and index of Ki67 were observed in PN + BCAA group than in EN group or EN + BCAA group (P < 0.05 ) on PO day 6. Conclusions BCAA-enriched EN or PN reverses amino acid disequilibrium and restores BCAA/AAA in cirrhotic rats after partial hepatectomy. Compared with PN, EN is superior in improving postoperative liver function, promoting protein synthesis, and speed up tissue regeneration in the postoperative liver. However, it still can not restore serum albumin in a short term.

A series of novel biodegradable and bionic functional polymers, PLLA-PC-PLLA, were synthesized using L-lactide ring-opening polymerization by L-a-Glycerophosphorylcholine (PC) from nature source. The hydrophilicity of the polymers was investigated. The results made known that, as PC group was brought into the backbone of PLLA, PLLA-PC-PLLA gained much better hydrophilicity than did PLLA, and polar phosphatidylcholines probably transferred to the sample surface in aqueous environment. The relative growth ratios of ECV304 cells to the lixivium of all PLLA-PC-PLLA were higher than 84% in 5 d culture. The cells adhesion of ECV304 on the films of PLLA-PC-PLLA lagged as compared to that on PLLA, but they could proliferate and cover the films in total. The difference between PLLA-PC-PLLA and PLLA was due to the existence of PC group. Thus, PLLA-PC-PLLA, the same as PLLA, are not cytotoxic, and ECV304 can attach and proliferate on them. PLLA-PC-PLLA have potential applications in the fields of tissue engineering and drug delivery system.

7.Self-assembled monolayers as model systems to study the relation between biocompatibity and surface chemistry of biomaterials.

The surface properties of biomaterials are essentially important to their biocompatibility. The complexity of surface composition and structure of biomaterials bring out the problem that it is difficult to make fully clear how the surface chemical properties and the structures of biomaterials control the biological reactions between the surfaces and proteins and/or cells. The structure of self-assembled monolayers (SAMs) is well established and SAMs have the characteristics of which a variety of functional groups and molecules can be introduced, either before or after the monolayer is formed, and diversified spectroscopy monitoring can be used to characterize SAMs and changes after their interactions with proteins or cells. Thus, SAMs are suitable model substrates for the study of the relationship between the surface chemical properties and biocompatibility of biomaterials. This paper reviews the researches on SAMs as models to study the absorption of proteins, cell adhesion and proliferation on materials, and the influences of both surface chemical functional groups and motion of molecular chains on hemocompatibility of biomaterials.

8.Clonal expansion T cells identified in acute monoblastic leukemia by CDR3 size analysis of TCR V beta repertoire using RT-PCR and genescan.

OBJECTIVETo investigate the distribution and clonality of T cell receptor (TCR) V beta repertoire in patients with acute monoblastic leukemia (AML-M5).
METHODSExpression of the TCR V beta repertoire was analyzed using reverse transcription-polymerase chain reaction (RT-PCR), which amplified the complementarity determining region 3 of 24 TCR V beta genes in peripheral blood from 9 cases with acute myclogenous leukemia subtype 5 or acute monoblastic leukemia (AML-M5). PCR products were further studied by genescan analysis to identify T cell clonality.
RESULTSExpression of 1-10 V beta subfamilies was found in samples from 9 patients. Genescan analysis showed that some V beta subfamily products from 8 of 9 cases contained an oligoclonal peak. Oligoclonal T cells of the V beta 2 subfamily could be found in 6 patients with AML-M5.
CONCLUSIONST cell clonality expansion was found in AML-M5 cases and were tendentious in the V beta 2 subfamily, suggesting a the specific immune response for leukemia cell (M5) associated antigen and may display antileukemia activity.

9.Study on the distribution and clonal expansion of TCR Vβ subfamily in peripheral T cells after infusing mesenchymal stem cells in patients with chronic GVHD

Objective To investigate the distribution of TCR Vβ genealogy and clonal expansion in peripheral blood after infusing mesenchymal stem cells (MSC) in patients with chronic GVHD. Methods The complementarity determining region 3 (CDR3) of 24 TCR Vβ subfamily genes in peripheral blood mononuclear cell from 1 case with cGVHD after allogeneic hematopoietic stem cell transplantation (Allo-HSCT),who were treated with infusing MSC,were amplified using RT-PCR. The blood samples were taken at the first and the fifth day after 1st infusion; and the first day,the 10 th day and the 20 th day after the second infusion of MSC,as well as the MSC infused as control . The products were labelled by fluorescein and then analyzed the CDR3 size with gene scan technique to determine the clonality of T cells. Results There were no expression of TCR Vβ subfamily with the MSC infused and after the 1st day of the first infusion of MSC. Then 3,10,14,10 Vβ subfamilies clones are appeared at the other time points,of which were polyclone and oligoclone predominately. In the same time,the manifestations of cCVHD have been abated. Conclusion MSC played a certain role in reviving the immune function of the patients after Allo-HSCT and mitigating the disease of chronic GVHD. Lineage analysis of TCR Vβ subfamily showed some predominant expression.
10.Effect of curcumin on the expression of p-STAT3 and IκB in db/db mice.

OBJECTIVE:
To determine the effect of curcumin on diabetic nephropathy in db/db mice and its possible mechanism.
METHODS:
Ten female db/db mice were randomly divided into 2 groups: one was treated with curcumin at 200 mg/(kg.d) and the other was a placebo group. Five age-matched db/m mice were grouped as the controls. In the curcumin group, curcumin was administered to db/db mice for 18 weeks. At the end of the experiment, the blood glucose and albumin were measured, and the kidney tissue sections were stained with PAS to observe the pathological changes. The expression of collagen IV and FN in the kidney was detected by immunohitochemistry staining. Western blot was used to detect the phosphorylation of signal transducer and activator of transcription 3 (STAT3) and IκB in the kidney.
RESULTS:
Compared with db/m mice, the weight and blood glucose of db/db mice were markedly increased, accompanied with heavy proteinuria, glomerulus hypertrophy, mesangial area expansion, thickening of basement membrane and ECM deposition. The phosphorylation of STAT3 was upregulated and the degradation of IκB was increased. Compared with the db/db mice, curcumin significantly decreased the urinary albumin, inhibited the phosphorylation of STAT3 and the degradation of IκB, and reduced the expression of collagen IV and FN in the kidney.
CONCLUSION
Curcumin can obviously decrease albuminuria and attenuate glomerular sclerosis in diabetic db/db mice by inhibiting phosphorylation of STAT3 and degradation of IκB.
