1.The application of the sevoflurane anesthesia via laryngeal mask airway (LMA)composited block in groin ;area in pediatric hernia surgery

Objective To compare the security between the sevoflurane anesthesia via laryngeal mask airway (LMA) composited nerve block in groin area and ketamine anesthesia in the pediatric hemioplasty.Methods 80 children (aged 2 -6,weight 10 -25kg,ASAⅠ -Ⅱ)with high ligation of hernia sac were divided into two groups according to the random number table method:group S -sevoflurane combined with inguinal region block under laryngeal airway ventilation;group K -ketamine combined with caudal block.Group S was inhalated with 6% -8% sevoflurane for foundation anesthesia,and then followed with vein induction:fentanyl 2μg/kg and propofol 2mg/kg.The laryngeal mask was placed when the temporomandibular joint was relaxed.Inguinal region was blocked by the surgeon after disinfect and spread towels.Anesthesia was maintained by 3% to 4% sevoflurane with mechanical ventilation interop-eration.Group K was adopted the intramuscular ketamine (5mg/kg)for basic anesthesia and then used modified sacral canal blocking.Ketamine 1mg/kg was administered before skin incision.Additional ketamine 0.5mg/kg was intravenously injected when it was appeared with body move,choking cough,etc.The values of MAP and HR in the two groups were recorded respectively at the time point of 1min before skin cutting (T0 ),1min after skin cutting (T1 ),5min after skin cutting (T2 ),the end of operation (T3 )and awaken time (T4 ).The body movement was observed intraoperation,the number of SpO2≤95% and awaken restlessness,the recovery time and residence time in anesthesia recovery room were recorded.Results The values of MAP in group S at time points of T1 ,T2 ,T3 were (77.6 ±6.2)mmHg,(77.8 ±6.4)mmHg,(76.5 ±4.7)mmHg respectively,and the values of MAP in group K at time points of T1 ,T2 ,T3 were (86.9 ±8.1)mmHg,(88.1 ±5.3)mmHg,(86.4 ±6.5)mmHg respectively.The t values at time points of T1 ,T2 ,T3 were 5.766,7.893,7.806 respectively when the group S compared with group K(P <0.0001),the group S was superior to group K.The values of HR in group S at time points of T1 ,T2 ,T3 were (121.3 ± 9.6)times/min,(121.9 ±8.4)times/min,(120.3 ±7.8)times/min respectively,and the values of HR in group K at time points of T1 ,T2 ,T3 were (138.6 ±9.4)times/min,(136.5 ±7.5)times/min,(128.7 ±6.9)times/min respec-tively.The t values at time points of T1 ,T2 ,T3 were 8.144,8.200,5.101 respectively(P <0.0001),the group S was superior to group K.The MAP and HR were more smoothly in group S than in group K.The recovery time in group S and K was (7.5 ±3.4)min and (16.7 ±5.5)min respectively(t =8.99,P <0.0001),it was shorter in group S than in group K.The residence time in anesthesia recovery room was (15.4 ±4.2)min and (23.7 ±6.3)min respectively (t =6.93,P <0.0001),it was shorter in group S than in group K.Intraoperative body movements was 3 cases and 15 cases respectively in group S and group K(χ2 =10.32,P <0.001);Cases of SpO2 ≤95% was 4 and 12 respec-tively in group S and group K(χ2 =10.32,P <0.001),the occurrence of body movements and SpO2 ≤95% cases was lower in group S than in group K.Conclusion The sevoflurane anesthesia via LMA composited nerve block in groin area has the advantages of faster induction,faster recovery,the hemodynamics was more stable,adverse reactions was fewer and so on,it can be safely used in pediatric anesthesia.
2.Changes and significance of B cell specific monoclonal murine leukemia virus integration site -1 and helper T cell-related cytokine in children with immune thrombocytopenia

Objective To analyze the functional changes and the clinical significance of B cell specific mono-clonal murine leukemia virus integration site -1 (Bmi -1 )and Th1 /Th2 cells in children with newly diagnosed im-mune thrombocytopenia(ITP)by testing the mRNA expressions of Bmi -1,helper T cell -related cytokine interferon (IFN)-γand interleukin(IL)-4 in children with newly diagnosed ITP.Methods Thirty -six cases of patients with newly diagnosed ITP in the experimental group came from the inpatient and outpatient children admitted to the Depart-ment of Pediatrics of the First Affiliated Hospital of Xinxiang Medical University from April to December 201 3.In the control group,26 cases of children requiring selective operation were admitted to the Department of Pediatric Surgery during the same period.The mRNA expressions of Bmi -1,IFN -γand IL -4 in the peripheral blood lymphocytes were detected by means of the reverse transcription -polymerase chain reaction(RT -PCR)method,and were analyzed and compared by t test and linear correlation analysis.Results (1 )The mRNA expressions of Bmi -1,IFN -γand IL -4 in peripheral blood lymphocytes in the experimental group were 2.63 ±0.54,3.84 ±0.43 and 1 .44 ±0.39,respec-tively;while the mRNA expressions of Bmi -1,IFN -γand IL -4 in the peripheral blood lymphocytes in the control group were 3.91 ±0.92,2.88 ±0.57 and 1 .87 ±0.34,respectively.The levels of IFN -γof the experimental group were significantly higher than those of the control group (P <0.001 )and the levels of Bmi -1 and IL -4 in the experi-mental group were significantly lower than those in the control group (all P <0.001 ).(2)The mRNA expressions be-tween IFN -γand IL -4 in the peripheral blood lymphocytes in the experimental group were in negative correlation (r =-0.667,P <0.001 ).The mRNA expressions between IL -4 and Bmi -1 in the same group were in a positive correlation (r =0.776,P <0.001 ).There were no correlation in the mRNA expressions between IFN -γand Bmi -1 (r =-0.206,P >0.05).Conclusions Bmi -1 may be involved in the pathogenesis of ITP by regulating Th cell, and Th cell dysfunction may occur in the children with ITP,and the disproportion between Th1 and Th2 may be due to the advantages of Th1 .
3.DNA Typing of Sporothrix schenckii by Analysis of Ribosomal-DNA Regions

Objectives To identify genotypes of 31 Sporothrix schenckii (S.schenckii) strains by Southern blotting and to explore the relationship between genotypes and geographic distributions and clinical manifestations.Methods Total DNA was extracted by cetyltriethyl ammonium bromide (CTAB).The polymorphisms were detected by hybridization of ApaⅠ-digested S.schenckii genomic DNA with a probe amplified from the small-subunit rDNA and adjacent internal transcribed spacer (ITS) regions.The band patterns manifested by Southern blotting were employed to investigate genotypes of 31 strains of S.schenckii collected from five different areas in China.Results Of 31 strains of S.schenckii,15 individual patterns (DNA Type A-O) were recognized.Type A to C accounted for 51.61% of all strains.Conclusion The Southern blotting provides a highly sensitive and reliable means for DNA typing of S.schenckii.It is also found that there is an obvious correlation between DNA patterns and different geographic distribution and clinical manifestations.
4.Hypolipidemic activity of total phenylpropanoid glycosides fromLigustrum robustum(Roxb.) Blume and its mechanisms on AMPK pathway

Aim To evaluate the hypolipidemic effect of total phenylpropanoid glycoside extracted from Ligustrum robustum(Roxb.)Blume(CNTG)and its mecha-nisms.Methods The 60 hamsters were randomly divided into six groups,namely the control group,the model group,the positive control group(fenofibrate,150 mg·kg-1),the high(1 200 mg·kg-1),medium(600 mg·kg-1)and low(300 mg·kg-1)doses of CNTG groups.Only the control group was given control diet and other groups received high-fat diet.The changes of serum lipid were measured and analyzed in 1st week to ensure the successful establishment of the model.The drugs were administered daily for four weeks and the concentrations of lipids were determined in the 2nd week,3rd week and 4th week respectively.Quantitative real-time PCR and Western blot were used to assay the mRNA and protein expression of related signaling enzymes and proteins.Results Compared with the model group,the concentrations of serum TG,TC,LDL-C(P<0.05,P<0.01)and hepatic TG,TC(P<0.01)were effectively reduced in hamsters in CNTG-treated groups.Mechanism research found that CNTG increased the levels of phospho-AMPKα,LKB1 and phospho-LKB1 in liver(P<0.05).Conclusion CNTG prevents hyperlipidemia via activation of hepatic LKB1-AMPK pathway.
5.Tissue distribution and excretion of Hylotelephin

Objective To investigate tissue distribution of Hylotelephin in Beagle dogs and excretion of Hylotelephin in rats. Methods A high performance liquid chromatography (HPLC) with UV detection was developed to study the concentrations of Hylotelephin in biological samples, taking anthracene as an internal standard, Benzoyl chloride as the pre-column derivatization of Hylotelephin and methanol-water as the mobile phase. Results After a single intravenous dose of 10.6 mg/kg Hylotelephin in beagle dogs, parent drug was widely distributed to virtually all tissues in 5 min and the concentration of Hylotelephin in most tissues at 90 min were lower than those at 5 min obviously. Hylotelephin was mainly distributed in kidney, liver and spleen, secondly in heart, lung and intestine. The parent drug concentration in kidney, liver and spleen was similar to that in blood at the same time point. After a single intravenous dose of 36 mg/kg Hylotelephin in rats, the excretion of the parent drug in urine, feces and bile amounted to 88.45%, 0.61% and 1.08% of the dose, respectively. The parent drug excretion amounted to 67% of the dose in 3 h. Conclusion Hylotelephin was distributed and eliminated in Beagle dogs rapidly. It was mostly distributed in kidney, liver, spleen and plasma. The parent drug excretion was 88% via urine.
6.Effectiveness of a comprehensive intervention for childhood obesity on self-efficacy, weight perception, and behavior change

Objective:
To evaluate the effectiveness of a comprehensive intervention for childhood obesity on self-efficacy, weight perception, and stage of behavior change, and to provide a scientific evidence for childhood obesity interventions.
Methods:
The study was a cluster randomized controlled trial. Twelve primary schools and 1 889 children (aged 7-11 years) from Dongcheng District in Beijing were recruited in September, 2013, and were administered a comprehensive intervention for childhood obesity. Six schools (930) were randomly allocated to the intervention group, and six schools (959) were randomly allocated to the control group. A face-to-face questionnaire survey was carried out, R 4.0.3 software was used to analyze the data, and multiple linear regression model was used to analyze the intervention effect.
Results:
After the intervention, mean (standard deviation) scores of self-efficacy among children in the intervention and control groups were (4.4±0.9) and (4.2±1.1), respectively. After the intervention, the proportion of children in the intervention group who underestimated or overestimated their body weight was 28.4% and 11.3 %, respectively, while the figures in the control were 37.1% and 6.9%; compared with the control, the risk of underestimating body weight decreased among children in the intervention group( OR=0.64, 95%CI=0.52-0.80, P <0.01), while the risk of weight overestimation increased( OR=1.79, 95%CI=1.26-2.54, P <0.01). After the intervention, the proportion of children in the contemplation or action stage of behaviour change was 65.3% and 83.6%, while the figures in the control group were 59.8% and 69.8 %, respectively; in comparison with the control group, more children in the intervention group were in comtemplation ( OR= 1.28 , 95%CI=1.03-1.59, P =0.02) or action stage of behaviour change( OR=2.59, 95%CI=2.04-3.27, P <0.01).
Conclusion
The comprehensive intervention for childhood obesity improved self-efficacy, weight perception, and facilitated behavior change, but may increase the risk of weight overestimation.
7.Identification of a novel SLC26A4 mutation in a child with enlarge vestibular aqueduct syndrome.

OBJECTIVETo analyze mutations of SLC26A4 gene and explore their origins for a patient with enlarge vestibuar aqueduct syndrome.
METHODSClinical data and peripheral venous blood samples were collected from the patient and her parents. Genome DNA was extracted from the peripheral blood. All of the 21 exons of the SLC26A4 gene were amplified with PCR and subjected to directly sequencing.
RESULTSThe patient was found to have carried two mutant alleles of the SLC26A4 gene, namely c.1522A to G and c.1229C to T, which were inherited from her father and mother, respectively.
CONCLUSIONSLC26A4 c.1522A to G is likely to be a pathogenic mutation. Above results may facilitate genetic counseling and prenatal diagnosis for this family.

8.Third investigation and analysis of quality control situation of intensive care unit in traditional Chinese medicine hospitals in Sichuan Province

Objective To evaluate the present development and status of quality control for intensive care unit (ICU) in Sichuan Provincial traditional Chinese medicine (TCM) hospitals including integrated traditional Chinese and western medicine hospitals and ethnic hospitals, and to provide practical references for improving the service quality of ICU. Methods Supervisory Group of Sichuan Provincial Critical Care Medicine Quality Control Center of TCM was established in September 2018. From September 8th to 17th, 2018, according to the Scoring Criteria of Quality Control and Supervision Project of TCM for Critical Care Medicine, a 10-day quality control professional guidance was hand out to TCM hospitals with independent ICU in Sichuan Province. The service level of different aspects of hospital quality control was evaluated and ranked from equipment and resource support, medical team, service capacity and level, ward quality, completion of critical care core indicators, completion of quality control of TCM, development of new technologies, diagnosis and treatment schemes for dominant diseases. Results There were 52 TCM hospitals across the province that had an ICU. Thirty-three hospitals were third-class (63.5%), while the rest 19 hospitals were second-class (36.5%). Province-level, city-level and county-level hospitals were accounted for 9.6% (5/52), 38.5% (20/52), and 51.9% (27/52), respectively. Average bed ratio of ICU was 1.8%. Doctor-bed and guard-bed ratios were 0.71∶1 and 2.0∶1, respectively. The average annual admission rate of patients and the average daily admission rate of beds were higher, which were basically 1%. Ward quality was high; the incidence of nosocomial infection was controlled below 10%. Compliance rate of septic shock bundle treatment was high. The incidences of ventilator-associated pneumonia (VAP), catheter-related bloodstream infection (CRBSI) and catheter-associated urinary tract infection (CAUTI) were 0.45%, 0.22%, and 0.30%, respectively. Participation rate of TCM was about 83.4%. Average number of new technologies was about 4.4. Average number of disease schemes was about 2.62. Conclusions ICU of Sichuan Provincial TCM hospitals reaches the standard level in service capacity and level, ward quality, critical medicine quality control, and participation rate of TCM treatment. Improvements are required for other prospects, including department scale, medical personnel allocation, new technical development, diagnosis and treatment schemes of dominant diseases.
10.Determination of 27 volatile organic compounds in exhaled breath by thermal desorption-gas chromatography-mass spectrometry

Background Volatile organic compounds (VOCs) in exhaled breath are closely associated with respiratory diseases and are linked to various metabolic reactions in the human body. A quantitative analytical method can provide technical support for studying VOCs related to various diseases. Objective To establish a thermal desorption-gas chromatography-mass spectrometry (TD-GC-MS) method for the determination of 27 VOCs in exhaled breath. Methods VOCs in exhaled breath were collected using a Bio-VOC sampler and enriched with Tenax TA thermal desorption tubes before TD-GC-MS analysis. Standards were collected using thermal desorption tubes and optimized for thermal desorption conditions as well as chromatographic and mass spectrometric conditions: The separation of the 27 VOCs was achieved by an optimized temperature program, the improvement of sensitivity by optimizing quantitative ions, and the increase of VOCs desorption efficiency by optimizing thermal desorption time and temperature. Limit of detection, limit of quantification, accuracy, precision, and stability of the proposed method were investigated by spiking with a blank gas bag, and exhaled breath samples from 20 healthy individuals were collected for an application study of the proposed method. Results The thermal desorption temperature was 280 ℃, and desorption time was 6 min. A VF-624ms chromatographic column was selected for the separation of target substances. The initial temperature of heating program was 35 ℃, maintained for 1 min, and then increased to 100 ℃ at a heating rate of 3 ℃·min−1 for 1 min, followed by increasing to 210 ℃ at a heating rate of 28 ℃·min−1 for 5 min. A quantitative analysis was conducted with a single ion monitoring (SIM) mode. Under these conditions, the 27 VOCs showed good linear relationships in their respective concentration ranges and the correlation coefficients were higher than 0.9990. The limits of detection of the method were in the range of 0.01-0.13 nmol·mol−1, the limits of quantification were in the range of 0.02-0.44 nmol·mol−1, and the spiked recoveries were in the range of 80.1%-120.5%, with intra-batch and inter-batch precision ≤ 18.8% and 17.9% respectively. All substances can be stored at room temperature (23-28 °C) for 7 d and at 4 °C for 14 d. The proposed method was applied to exhaled breath samples from 20 subjects with detection rates≥ 80% (except for trans-2-pentene and decane) and a concentration range of 0.00-465.50 nmol·mol−1. Conclusion The established TD-GC-MS method for quantification of VOCs in exhaled breath is characterized by high sensitivity and good accuracy, and is suitable for quantitative determination of VOCs in exhaled breath, which can provide technical support for the study of exhaled breath VOCs.
