1.Effect of phosphorylating-p38 mitogen-activated protein kinase on restructuration of extracellular matrix of diabetic glomeruli

Objective To study the expression and significance of the p38 mitogen-activated protein kinase (MAPK) and cAMP-responsive element binding protein (CREB 1) in experimental diabetic mellitus rats, and their relationship with extracllular matrix (ECM) restructuration. Methods Sixty male Wistar rats, with their kidneys removed surgically, were divided in two groups: control group (n=30) and diabetic group (n=30). Diabetic model was reproduced by intraperitoneal injection of Streptozocin (STZ, 65mg/kg). Six rats of each group were sacrificed at 1, 2, 4, 8 and 12weeks after STZ injection. Immunohistochemistry was used to assess the expression of p-p38 MAPK and p-CREB 1 in both control and diabetic groups. Western blot was performed to determine the activity of p-p38 MAPK and p-CREB 1 in both groups. Semiquantitative competitive reverse transcription polymerase chain reaction (PCR) was performed to detect the expression of mRNA of p38 MAPK, CREB 1 and fibronectin (FN). Results Compared with the control group, expression of p-p38 MAPK in diabetic glomeruli was significantly enhanced at 1, 2, 4, and 8 weeks, then it lowered at 12 weeks. P-CREB 1 was increased at 2, 4, 8 weeks, also decreased at 12 weeks. Elevation of FN mRNA began after 4 weeks in diabetic rats. Conclusions Glomerular p38 MAPK activity was increased in early stage of diabetes. This activated p38 MAPK pathway in diabetic glomeruli may in part play a role in the pathogenesis of extracellular matrix restructuring.
2.Ultrastructural localization of atrial natriuretic peptide-synthesizing cells in rat gastric mucosa and the correlation of atrial natriuretic peptide-synthesizing cells to microvessel density

BACKGROUND: Previous studies have confirmed that atrial natriuretic peptide (ANP) exists in different regions of rat gastric mucosa in different densities, and ANP can inhibit the spontaneous contraction of gastric smooth muscles in rat, guinea pig and human.OBJECTIVE: To investigate the ultrastructural localization of ANP-synthesizing cells and the correlation between ANP-synthesizing cells and microvessel density in rat gastric mucosa.DESIGN: Single sampling study.SETTING: Affiliated Hospital of Chengde Medical College. MATERIALS: Eighteen adult male Wistar rats of clean grade, weighing 300-350 g, were used in this study.METHODS: This study was conducted in the Laboratory of Chinese Materia Medica Immunology, Chengde Medical College from October 2004 to July 2007. The ultrastructural localization of ANP-synthesizing cells in rat gastric mucosa was performed by postembedding immunoelectron microscopy technique. The area density of ANP-synthesizing cell distribution was calculated by histochemical technique (saffron solution and toluidine solution staining included). Microvessels of rat gastric mucosa were revealed distinctively by tannic acid-ferric chloride staining method and scanning electron microscope technique.MAIN OUTCOME MEASURES:①The ultrastructural localization of ANP-synthesizing cells in rat gastric mucosa.②The distribution characteristics of gastric microvessels in different regions of rat gastric mucosa.③The distribution characteristics of ANP-synthesizing cells in rat gastric mucosa.④Correlation between ANP-synthesizing cell distribution and microvessel density.RESULTS: ① Ultrastructural localization of ANP-synthesizing cells: It was confirmed that ANP-cells were the enterochrochromaffin (EC) cells. ② Observation of microvessels: The microvessels were stained successfully by tannic acid-ferric chloride and scanning electron microscope technology. Microvessels of gastric mucous were in wriggled way and cut in different cross-sections. They could be clearly observed in distinct three-dimensions. The microvessel density was the largest in the basal glands of rat gastric mucous. ③ Distribution of ANP-synthesizing cells: It was identified that EC cell was only a chromaffin cell in the rat gastric mucosa. The chromaffin granules (brown granules) were localized in the cytoplasm of EC cells. Negative staining for chromaffin granules was detected in the submucosa and smooth muscle. EC cells had different distribution densities in different regions. The density order of EC cells was gastric cardiac region > gastric pyloric region > gastric fundic region in mucosal layer. ④ Correlation between ANP-synthesizing cells and microvessel density: Correlation analysis showed that there was a positive correlation between ANP-synthesizing cells and microvessel density in the gastric cardiac region(r = 0.53, P < 0.05, n=18), and there was a negative correlation between above-mentioned two indices in the gastric pyloric region(r = 0.38, P > 0.05 , n=18)and in the gastric fundic region(r = -0.29, P > 0.05, n=18).CONCLUSION: ANP-synthesizing cells are the EC cells of gastric mucosa. There is a positive correlation between ANP-synthesizing cells and microvessels in the rat gastric cardiac region, while there is a negative correlation between ANP-synthesizing cells and microvessels in the rat gastric pyloric region and in the gastric fundic region.
3.Epidemiological status and risk factor analysis of female breast disease in Tongzhou district of Beijing

Objective An epidemiological study was conducted to investigate the prevalence of female breast disease and risk factors in Tongzhou District of Beijing,providing objective data to prophylaxis of the disease in this area.Methods A questionnaire survey was done in 1 575 females aged 18-60 years in Tongzhou district of Beijing by a cluster random sampling method.Clinical breast examination,near- infrared scanning,colour ultrasound,X-ray and other adjunctive examinations were performed in these subjects for diagnosis of the disease.The subjects were divided into breast disease group and normal control group.And then a case-control study was conducted for risk factor analysis of female breast disease.Results Breast disease was found in 303 of 1 575 females,most in 30-50 years,the incidence being 19.2%.Breast hyperplasia was found in 270 females,breast fibroma in 5,other benign breast disease in 25,and breast cancer in 3,Breast disease was found related to education level,menopause time,history of breast hyperplasia and annoyance.Conclusions Female breast disease is related to age,education and menopause.History of breast hyperplasia and negative emotion are risk factora of the disease.Attention should be paid to breast health care and healthy life style,decreasing incidence of the disease.
4.Acute myeloid leukemia after liver transplantation

BACKGROUND:Acute myeloid leukemia after liver transplantation is a rare complication with high mortality. OBJECTIVE:To study the clinical features of acute myeloid leukemia after liver transplantation. METHODS:One case of acute promyelocytic leukemia after liver transplantation was reported, and literatures were reviewed. RESULTS AND CONCLUSION:Case presentation was post-odontectomy bleeding with associative abnormal coagulation test at 85 months after liver transplantation. Routine blood test, bone marrow test and chromosome analysis and examination diagnosed as acute promyelocytic leukemia and promyelocytic leukemia/RARαpositive chimeric gene, and clearly diagnosed as acute promyelocytic leukemia combined with disseminated intravascular coagulation. The patient received fresh frozen plasma transfusion to correct the abnormal coagulation, and then received induction chemotherapy with retinoic acid, arsenic trioxide and daunorubicin to obtain bone marrow complete remission. The patient was treated with daunorubicin combined with cytarabine and mitoxantrone combined with cytarabine regimens after remission induction to consolidate the chemotherapy for two courses of treatment, and then subsequently subjected to arsenous acid chemotherapy, and the bone marrow was sustained for remission. During chemotherapy, the dose and type of immunosuppressive agents were adjusted, and the patient had stable liver function without serious infection or complications. The results indicate that acute myeloid leukemia is common and often occurs after liver transplantation. When the patient displays hematological abnormality, acute promyelocytic leukemia should be considered. Early diagnosis and treatment can reduce the mortality.
5.Endoscopic and pathologic findings in acute graft versus host disease after allogeneic bone marrow transplantation

Objective To analyze the endoscopic and pathologic characters of acute graft versus host disease(aGVHD) after aUogeneie bone marrow transplantation.Methods The endoscopic and pathologic findings of 4 patients with acute aGVHD were retrospectively analyzed.Results The clinical manifestations of 4 patients were abdominal pain and diarrhea occurred 21~57 days after bone marrow transplantation.The eolonoseopy detected mucosa edema,erosion and multiple ulcers.The pathological findings included epithelial necrosis accompanied with infiltration of lympbocytes and plasmacytes,no cytomegalovirus was found in biopsies,and aGVHD was diagnosed.Three patients recovered after the treatment with corticosteroids,while the other one died.Conclusion The intestine is involved in aGVHD after allogeneie bone marrow transplantation,and the diagnosis depends on eolonoscopy and biopsy.
6.The clinical value of needle localization biopsy technique in clinical nonpalpable breast disease

Objective To study the clinical value of mammography and ultrasound needle localization biopsy technique in clinical nonpalpable breast disease.Methods 148 patients with negative clinical palpation were operated only because of the mammography or ultrasound findings of lesions.They were undergone localized excision biopsy and derived definite pathological diagnosis.By histopathology diagnosis,benign and malignant lesions,lesion size,complete resection rate were statistically analyzed.Results 21 cases in 148 cases were malignant lesions(14.2%),which were all early breast cancer.The other 127 cases were benign breast disease(85.8%).147 cases were successfully resected at the first time and 1 case was successfully resected at the second time.Conclusion Mammography and Ultrasound fine needle localized biopsy in nonpalpable breast disease diagnosis and treatment has great significance,which can raise early diagnosis rate of breast cancer without mass,and this method is simple and easy for promotion.
7.Clinical application of wire guided localization of breast biopsy under X-ray guidance

Objective To study the diagnostic and treatment value of wire guided localization of breast biopsy for non-palpable breast lesions.Methods The clinical data of 314 cases of non-palpable breast lesion resection operation in our hospital were retrospectively analyzed.The accuracy rate of resection and postoperative pathological examination results were analyzed.Results All lesions were positioned accurately and completely resected,48 cases in 314 cases were breast cancer(15.3 %),in which 42 cases were early breast cancer which were confirmed by pathology (87.5%).Conclusion Wire guided localization of breast biopsy has high accuracy,low cost,which is useful for the diagnosis of early breast cancer,there is a certain value in clinical application.

9.The relationship between phosphorylation at Ser3 of cofilin1 and Taxol resistance of ovarian cancer in elderly female patients

Objectives To analyze the relationship between phosphorylation of serine-3 of cofilin1 and Taxol resistance of ovarian cancer and to evaluate the prognostic value of cofilin1 phosphorylation in estimating ovarian cancer survival using clinical samples.Methods Wild type (WT) plasmids with over-expression of wild cofilin1,S3L plasmids with over-expression of cofilin1 and inhibited phosphorylation at serine-3,and siRNA-C plasmids with down-regulation of cofilin1 were constructed using molecular biology techniques.After the SKOV3 and SK-TR30 cell-lines were transfected with these plasmids,changes in biological characteristics and drug resistance after instant transfection were compared.Fifty-one elderly female patients with microscopically confirmed ovarian cancer,who had received chemotherapy with Taxol and cis-platinum (TC)after surgery,including 30 chemo-sensitive and 21 chemo-resistant cases,were recruited in this study.Immunohistochemical methods were used to detect the expression of cofilinl and phosphorylated cofilin in tissue sections from the two groups.Progression free survival(PFS)was analyzed.Results Compared with the control group,the proportion of cells increased at the G0/G1 phase(P=0.034)and decreased at the S phase (P=0.031),and the apoptosis rate decreased(P=0.020),in SKOV3 cells with high expression of cofilin1.However,these characteristics disappeared when the wild type was replaced with the inactive mutant S3L.When cofilin1 expression was down-regulated in SK-TR30 cells,the proportion of cells at the G0/G1 phase decreased(P=0.020).The expression of cofilin1 was detected in 56.7 % (17/30)and 57.1% (12/21),respectively,sections of ovarian tumor tissues of the chemo-sensitive and chemoresistant groups,and there was no statistic difference (x2=0.332,P =0.943)between the two groups.The expression of phosphorylated cofilin was much higher in the chemo-resistant group (85.7% or 18/21)than in the chemo-sensitive group (53.3% or 16/30),and the difference was statistically significant(x2=5.829,P=0.016).The higher expression of phosphorylated cofilin was also correlated with shorter PFS(x2 =21.440,P<0.01,95% CI:0.068-0.883),especially in the chemo-sensitive group.Conclusions Serine-3 phosphorylation status of cofilin 1 is associated with paclitaxel resistance in ovarian cancer,but the underlying mechanisms of regulation need further investigation.
10.Effects of valsartan on the expression of STAT1 and STAT3 in glomerular mesangial cells under high concentration of glucose

Aim To investigate the effects of valsartan on activation of signal transducers and activators of transcription 1,3 in glomerular mesangial cells(GMCs) under high concentration of glucose.Methods we used high concentration glucose and valsartan to stimulate the cultured rat GMCs in vitro.The protein expressions of signal transducer and activatior of transcription 1,3(STAT1,STAT3),p-STAT1 and p-STAT3 were observed by Western blot.The protein synthesis of TGF-?_1,fibronectin and type IV collagen in the supernatants of the GMCs were detected by enzyme-linked immunoadsorbent assay(ELISA) and radioimmunoassay.TGF-?_1 mRNA was measured by reverse transcription and polymerase chain reaction(RT-PCR).Results Compared with low glucose control group,the expressions of p-STAT1,p-STAT3 and TGF?_1 mRNA were significantly increased in GMCs under high concentration glucose medium and there observed the high concentration of TGF-?_1,fibronectin and type IV collagen in the supernatants.The expression levels of p-STAT1,p-STAT3 and TGF-?_1 mRNA were significantly lower in the valsartan group than in those the high concentration glucose group.The concentration of TGF-?_1,fibronectin and type IV collagen in the supernatants in the valsartan group were lower than that in the high concentration glucose control group.Conclusion Valsartan can inhibit overproduction of TGF-?_1 and ECM proteins in GMCs under high concentration of glucose,partly by regulating the phosphorylation of STAT1 and STAT3.
