1.Evaluation of the Effectiveness of Bio-Safety Theory Training before Laboratory Medicine Internship

Objective To evaluate the effectiveness of bio-safety theory intensive training before laboratory medicine intern-ship.Methods Collected 220 students of three universities in human before laboratory medicine internship,according to dif-ferent university,and the different grade of the same university,and the same class grouping pre-and post-the bio-safety in-tensive training,the obj ects of study in accordance with the training scheme to do on-site questionnaire survey by secret way in three universities,comparing the effectiveness pre-and post-training.Results Three universities had no difference before training of laboratory medicine students (χ2=0.081~3.135,P>0.05).However,in the same university,pre-training,the basic concepts and microbiological hazards assessment could be better grasped (respectively,P=0.000,0.000).Post-train-ing,the basic concepts and bio-safety operation specification could be promoted (respectively,P=0.000,0.002).In the same class between pre-and post-training,test results had no difference (χ2=0.096~2.408,P>0.05)except personal protec-tive equipmentin 2008 grade students,while in 2009 grade test performance was improved (χ2=4.821~12.27,P<0.05) except the basic concepts.Conclusion Bio-safety intensive training has good effects to operation skills,according to the spe-cific situation of students targeted training,which made students real benefit,and ensure laboratory medicine students’labo-ratory safety.
2.Alkylated-chitosan preparation and its hemostatic effect

BACKGROUND:18-Alkylated-chitosan has been proved to exhibit a good coagulation capacity through hydrophobic modification. OBJECTIVE:To improve the adhesion between the material and the wound through the introduction of alkylated-chitosan molecules. METHODS:The alkyl group was introduced in the chitosan by potentiodynamic method to prepare the alkylated-chitosan with different substitution degrees (7%,16%, 26%and 40%), and then their structure, viscosity, porosity, water absorption, contact angle and in vitro coagulation capacity were detected. Thirty New Zealand rabbits were enrol ed, modeled into the left femoral artery hemorrhage, and randomized into six groups, including four experimental groups, positive control and control groups, fol owed by treated with alkylated-chitosan sponges with different degrees of substitution, absorbable hemostatic gauze and chitosan sponge, respectively. Afterwards, the blood loss and clotting time were observed. RESULTS AND CONCLUISON:With the degree of substitution increased, the viscosity of alkylated-chitosan increased firstly and then decreased, the porosity and water absorption decreased gradual y, and the contact angle on a rise. Compared with chitosan, different alkylated-chitosans showed a better henostatic effect in vitro, especial y the alkylated-chitosan with 16%substitution degree. The blood loss and clotting time in the experimental groups except that the alkylated-chitosan with 7%substitution degree were significantly lower than those in the control group (P<0.05);the bold loss in the alkylated-chitosan with 40%substitution degree group was significantly lower than that in the positive control group (P<0.05). These findings suggest that the alkylated-chitosan with 40%substitution degree exhibits the best hemostatic effect in vivo.

Chronic kidney disease (CKD) is becoming a global social problem. It is important to slow down the progression of CKD for economic and social concerns. In recent years, it has been found that colon is one of the vital organs which produce uremic toxins. And enterogenous uremic toxins are closely related to the prognosis of CKD. Theory of gut-kidney axis for the slowdown of CKD progression was raised by foreign scholars and became the research hot spot. Colon therapy with traditional Chinese medicine (TCM) has been widely used in clinical practice and is believed to slow down the progression of CKD by numerous clinical reports. However, low re-search quality and ambiguous results limited its further application. Under the guidance of senior TCM Professor Huang Chunlin, who emphasized the method of draining turbidity through bowels in the management of CKD, from the Nephrology Center, Guangdong Provincial Hospital of Chinese Medicine, as well as the modern theory of gut-kidney axis, we had carried out a series of exploratory researches which will provide data and methodology support for further confirmatory studies and improve its effectiveness.
4.Impacts of workplace violence and professional identity on nurse work engagement in Class Ⅲ Grade A hospitals

Objective? To understand the influence of workplace violence and professional identity on nurses' work engagement, so as to provide guidance for clinical nursing management. Methods? From July to September in 2018, nurses from 4 ClassⅢ Grade A hospitals in Wenzhou city were selected as subjects randomly. The General Data Questionnaire, the Workplace Violence Scale, Professional Identity Scale and the Work Engagement Scale were used in the study. A total of 880 questionnaires were distributed and 862 valid ones were retrieved with the effective recovery rate of 97.95%. Results? In the 862 participant nurses, 247 (28.65%) nurses suffered from zero frequency workplace violence, 354 (41.07%) nurses suffered from low frequency workplace violence, 202(23.43%) nurses suffered from intermediate frequency workplace violence, 59(6.84%) nurses suffered from high frequency workplace violence. The total score of professional identity was (110.89±19.26) points;the total score of work engagement was (59.72±8.95) points;workplace violence scores were negatively correlated with the nurses' work engagement (P<0.05);the total score of nurses' professional identity and the scores of all dimensions were positively correlated with the scores of work engagement (P<0.05);Multiple linear regression analysis showed, nurse professional identity, workplace violence, professional title, working duration and education are the main influencing factors of their work engagement (P< 0.01). Conclusions? The nurses' work engagement needs to be improved, and professional identity, workplace violence, professional title, length of work and education are the main influencing factors. It is suggested that clinical nursing managers should take measures to reduce the frequency of workplace violence, improve the working environment for the nurses so as to improve their working engagement level and ensure the quality of clinical nursing service.

Pretreatment of the cells with 1,25VD3 obviously lowered 400 U/L HDM-induced TSLP expressions (P<0.05), but 1,25VD3 added along with HDM in the cells did not produce significant effects on TSLP expressions (P=0.58).
CONCLUSIONBoth 1,25VD3 and HDM can induce TSLP expression and release in 16HBE cells, but pretreatment with 1,25VD3 can decrease HDM-augmented TSLP expression in the cells.

8.Identification of compound heterozygous variants of F12 gene in a pedigree affected with inherited coagulation factor XII deficiency.

OBJECTIVE:
To explore the molecular pathogenesis for a pedigree affected with hereditary coagulation factor XII (FXII) deficiency.
METHODS:
Potential variant of the F12 gene was analyzed by PCR and Sanger sequencing. Expression plasmids were constructed by site-directed mutagenesis based on the wild-type and transiently transfected into 293T cells. FXII:C and FXII:Ag of the expression products were determined in the supernatant and cell lysate. Western blotting was used to verify the identify of the protein.
RESULTS:
Gene sequencing revealed that the proband has carried 46TT genetype and heterozygous p.Glu502Lys variants in exon 13, and a heterozygous p.Gly542Ser variant in exon 14 of the F12 gene. Transfection experiment suggested that the FXII:C and FXII:Ag of p.Glu502Lys variant in the supernatant were 28% and 24%, compared with the wild-type (100%) and FXII:Ag of cell lysates was 39% compared to the wild-type (100%). The FXII:C and FXII:Ag of p. Gly542Ser variant in the supernatant were 32% and 17% and the FXII:Ag of cell lysates was 59%.
CONCLUSION
The 46TT genetype, p.Glu502Lys and p.Gly542Ser variants of the F12 gene probably underlie the low FXII level in the proband. As shown by in vitro experiment, the p.Glu502Lys and p.Gly542Ser variants can both inhibit the synthesis and secrection of the FXII protein.

9.Clinicopathological features and myeloid differentiation factor 88 L265P gene mutation in patients with diffuse large B-cell lymphoma of central nervous system

Objective:To analyze the clinical features and mutation of myeloid differentiation factor 88 (MYD88) L265P in patients with diffuse large B-cell lymphoma (DLBCL) of central nervous system (CNS).Methods:The clinicopathological materials of 45 cases of DLBCL of CNS were retrospectively collected in Xuanwu Hospital, Capital Medical University from September 2014 to February 2017. The clinicopathological data were retrospectively analyzed, combined with immunohistochemistry, EB virus in situ hybridization, imaging and medical history. The mutation of MYD88 L265P gene was detected by pyrosequencing and its clinical significance was analyzed. Results:The age of the patients ranged from 42 to 82 years [(57.6±8.8) years], including 24 males and 21 females. Totally 93.3% (42/45) of the patients had supratentorial tumours, which were single or multiple. The cerebral hemisphere (31/45, 68.9%) was the most common involved site, and 21 cases (21/45, 46.7%) had multiple lesions. Histologically, DLBCL in the CNS showed diffuse infiltration of tumor tissue, some of which grew around blood vessels in a "sleeve" arrangement. CD 20 and CD 79a were diffusely and strongly positive. Thirty-nine cases (39/45, 86.7%) were non-germinal center B cell (non-GCB) subtype and 6 cases (6/45, 13.3%) were germinal center B cell (GCB) subtype. MYD88 L265P mutation was found in 64.4% (29/45) patients. There was statistically significant difference between non-GCB type (71.8%, 28/39) and GCB type DLBCL (1/6, P=0.017). Compared with the operation/biopsy group without chemotherapy, operation+chemotherapy, biopsy+chemotherapy, operation/biopsy+chemotherapy+stem cell transplantation can improve the survival and prognosis ( HR=0.05, 95% CI 0.01-0.33 , P=0.002; HR=0.04, 95% CI 0.01-0.36 , P=0.004; HR=0.01, 95% CI 0.00-0.17 , P=0.001; respectively). Conclusions:DLBCL of the CNS is aggressive tumor with poor prognosis, the clinical manifestations are complex and diverse, and the diagnosis is challenging. MYD88 L265P is a common and specific gene mutation in primary CNS lymphoma(PCNSL), which is of great significance in the diagnosis and treatment of lymphoma. The MYD88 L265P mutation was more frequently detected in non-GCB than GCB subtype. Chemotherapy can improve the survival rate of PCNSL patients. If chemotherapy achieves complete remission and autologous hematopoietic stem cell transplantation is performed, there may be a chance of long-term survival.
10.Efficacy and safety of transcatheter arterial chemoembolization for the treatment of locally recurrent breast cancer

Objective To investigate the efficacy and safety of transcatheter arterial chemoembolization(TACE)for the treatment of locally recurrent breast cancer.Methods The clinical data of 57 patients with locally recurrent breast cancer from January 2018 to December 2020 were retrospectively analyzed.Twenty-four patients(group A)received TACE using adriamycin 12 mg/m2 and paclitaxel 45 mg/m2,which was accomplished by local perfusion into the tumor target blood vessels with microcatheter catheterization,the embolization material was Embosphere microspheres,and the embolization endpoint was occlusion of the main stem of the target vessel.Other 33 patients who received systemic chemotherapy using adriamycin 40 mg/m2 and paclitaxel 135 mg/m2 in the same period were collected as group B.The 6-month disease control rate(DCR),progression-free survival(PFS)and overall survival(OS)were compared between the two groups.Results Successful TACE was accomplished in all the 24 patients of group A,with a technical success rate of 100%.In group A,the 6-month DCR was 87.50%,the median PFS was 12 months,and the median OS was 22 months.In group B,the 6-month DCR was 63.63%,the median PFS was 9 months,and the median OS was 20 months.The differences in the 6-month DCR and the median PFS between the two groups were statistically significant(P=0.04 and P=0.03 respectively),while no statistically significant difference in the median OS existed between the two groups(P=0.21).The incidence of post-embolization syndrome in group A was 75%(18/24),the clinical symptoms included chest wall pain and mild fever,which disappeared 3 days after symptomatic treatment such as pain-relief and antipyretic medication,and no TACE-related serious complications such as target vessel injury,ectopic embolization of embolization materials or chest wall necrosis occurred in all patients.All patients were followed up for a mean period of(19.47±4.96)months(range of 8-24 months)Conclusion For the treatment of locally recurrent breast cancer,TACE is superior to systemic chemotherapy in short-term efficacy.TACE carries no intervention-related serious complications.However,more studies need to be conducted before its long-term efficacy and safety can be clarified.(J Intervent Radiol,2024,33:280-284)
