1.THE ASSOCIATION OF ACE GENE INSERTION/DELETION POLYMORPHISM WITH SERUM ACE LEVEL AND HYPERTENSION IN THE ELDERLY

Objective To investigate angiotensin converting enzyme (ACE) gene insertion/deletion (I/D) polymorphism in patients with hypertension in the elderly and it's relation with serum ACE level among Chinese population.Methods Polymorphism of ACE gene was categorized into three genotype:two deletion alleles (genotype DD),heterozygous alleles (genotype ID),and two insertion alleles (genotype II).Polymerase chain reaction(PCR) was used to examine ACE gene polymorphism.Compared the distribution of three ACE genotypes frequencies and D allele frequency in 56 patients with hypertension in the elderly and 40 healthy subjects,whose serum ACE levels were measured.Results The frequency of D allele was higher in the hypertension group (0.61) than in healthy subjects (0.46) (P<0.05).A significant difference in serum ACE level was observed between the hypertensives and healthy (31.0±11.1) U/L and (25.1±10.0) U/L respectively,P<0.05.The highest level of serum ACE activity was in genotype DD,and the lowest level in genotype II,and intermediate in genotype ID.A significant difference in serum ACE level was observed among the genotype DD and ID,and among the genotype DD and II.Conclusion ACE gene I/D polymorphism was associated with serum ACE level.D allele might be a genetic risk factor for hypertension in the elderly.

3.The therapeutic efficacy of coronary heart disease intervention and effect on vascular endothelial growth factor and C-reactive protein

Objective To investigate the therapeutic efficacy of coronary heart disease intervention and effect on vascular endothelial growth factor(VEGF) and C-reactive protein(CRP) levels.Methods 127 cases with angina and myocardial infarct were done PCI(PTCA/PTCA percutaneous transcoronary angioplasty+stenting),the therapeutic efficacy was observed,and perioperative VEGF and CRP levels were determined.Results 146 brackets cases were placed in 127 cases,all brackets were successfully inserted into target vascular lesion sites;2 cases with two rami pathological changes given up PTCA because guide wire couldn't pass anterior descending branch narrow part;4 cases took place no re-flow phenomenon,operation achievement ratio was 95.3%.The levels of post-operative CRP increased 142.9%,but the levels of post-operative VEGF decreased 31.9%.Conclusion The therapeutic efficacy PCI on coronary heart disease is satisfactory monitoring CRP and VEGF in blood serum could be helpful to understand the therapeutic efficacy and inflammation degree.
4.Clinical analysis of dynamic electrocardiogram in 186 cases of premature ventricualr contractions

5.Analysis of the effect of psychological intervention combined with Levocarnitine on nutrition indicators and inflammatory state in maintenance hemodialysis patients

Objective To investigate the effect of L-carnitine combined with psychological intervention on nutritional status and inflammatory status in maintenance hemodialysis patients. Methods 75 cases of maintenance hemodialysis patients treated in our hospital from September 2016 to December 2016 were randomly divided into control group(n=37) and study group(n=38)by single and double number method (hospital visit order). The control group in the hemodialysis treatment combined with Levocarnitine; the study group in the use of L-carnitine combined with psychological intervention. Results There were no significant differences in ALB, Hb, BMI, IL-6 and CRP between the two groups before treatment. After the corresponding treatment, the improvement of ALB, Hb, BMI, IL-6, CRP and other inflammatory indexes and nutritional indexes in the study group were better than those in the control group(P<0.05). Conclusion The application of L-carnitine in maintenance hemodialysis patients combined with psychological intervention can significantly improve the body's Micro inflammatory status and nutrition indicators, has positive significance to protect the patients' quality of life and physical and mental health.
6.Long-term results of treatment of 30 patients with primary intracranial germinoma

Objective To report long term results of 30 patients with primary intracranial germinoma and evaluate the treatment policy. Methods After the tumor of these 30 patients was completely or partially removed they were routinely given radiation therapy with the conventional extenal irradiation. Of these 30 patients, 28 received prophylactic whole brain irradiation followed by the cone down technique to the local site to an effective dose except 2 patients who were treated locally without prophylactic irradiation.Nineteen patients received prophylactic spinal cord irradiation.Results The overall 5 ,10 , 15 ,19 year survival rates were 93.1%, 87.6% ,87.6% and 68.2%,respectively.The two patients who did not receive prophylactic whole brain irradiation (1 patient) or too low a dose of that (1 patient) developed metastasis in the brain and spinal cord were given a second course of radiotherapy. One survived for 17 years and the other is living now over 10 years. The overall response rate (CR + PR) was 95%.Conclusions While surgery is indicated to reduce the tumor volume, shunt the hydrocrania and obtain a pathologic diagnosis, radiotherapy is invaluable to cure germinoma. Prophylactic whole brain irradiation of 26~30 Gy is necessary and the curative local dose should be 50~55 Gy. Prophylactic spinal cord irradiation is not indicated routinely except some appropriate patients. For patients younger than 14 years, shielding of the pituitary is necessary while the prophylactic whole brain is being irradiated. The tumor dose at the sella turcica region should be limited to 56 Gy, and the spinal cord need not be irradiated prophylactically.

8.Effects of removing laryngeal mask airway in anaesthetized state on emergence agitation after sevoflurane anaesthesia in children

Objective To investigate the effect of removing laryngeal mask airway (LMA) in anaesthetized state on emergence agitation after sevoflurane anaesthesia in children.Methods Ninety children with subumbilical operation,ASA Ⅰ or Ⅱ grade,were divided into three groups by random digits table with 30 cases each:ET-A group (endotracheal tube and extubation while awake),ET-D group (endotracheal tube and deep extubation) and LMA-D group (insertion of LMA and deep removal).The incidence of preoperative emotional bad,anaesthesia time,sevoflurane administration time,emergence agitation,the numbers of treated with propofol and/or fentanyl and postoperative anaesthesia care unit(PACU)retention time were measured.Results There was no significant difference in incidence of preoperative emotional bad,sevoflurane administration time and anaesthesia time among the three groups (P > 0.05).The incidence of emergence agitation,the numbers of treated with propofol and/or fentanyl and PACU retention time in LMA-D group were significantly lower than those in ET-A group [26.7%(8/30) vs.66.7%(20/30),8cases vs.20 cases and (23.9 ± 4.9) min vs.(32.9 ± 5.8) min,P < 0.01].Conclusion Insertion of LMA and removing LMA in anaesthetized state can decreased emergence agitation in children after sevoflurane anaesthesia,especially suitable for the children who do not suitable for drug treatment.
9.Value of neutrophil-lymphocyte count ratio in diagnosing bloodstream infection

Objective To explore the diagnostic value of five infection markers in bloodstream infection.Methods Randomly selected 110 bloodstream infection patients with positive blood cultures and 30 bacterial infection patients with negative blood cultures.Blood was simultaneously drawn with blood cultures;the complete blood count and C-reactive protein (CRP) levels were measured.The white blood cell count (WBC),neutrophil count (NEU),lymphocyte count (LMY),CRP level and neutrophil-lymphocyte count ratio (NLCR) were compared between the two groups.Results The levels of WBC,NEU,NLCR and CRP in bloodstream infection group were significantly higher than those in control group (P<0.05),while LYM was significantly lower than that in control group (P< 0.05).Among these five infection markers,the area under the receiver operating characteristic curve (ROC-AUC) was the highest for NLCR (0.808) and LMY (0.756);when the cutoff value for NLCR was >9.33,sensitivity was 63.6%,specificity was 93.3%;and the cutoff value for LYM was ≤0.97,sensitivity was 58.2%,specificity was 86.7%.Furthermore,the NLCR of patients with gram-negative bloodstream infection was higher than those in patients with gram-positive bloodstream infection.NLCR showed important clinical significance in distinguishing strains of different bloodstream infections.Conclusions NLCR is the better predictors than routine parameters in diagnosing bloodstream infection.
10.Application of fresh whole blood in comparison and bias estimation between different hematocyte analyzers

Objective To compare the detection results and estimate the bias of two hematocyte analyzers of different brands . Methods Sysmex XE‐2100 hematocyte analyzer and Abbott CD‐1700 hematocyte analyzer were chosen to be the reference instru‐ment and comparison instrument ,respectively .40 cases of fresh whole blood samples were collected for the detection of WBC ,RBC , HGB ,HCT ,MCV ,MCH ,MCHC ,and PLT respectively by the two instruments .The correlation coefficient (r) ,regression equa‐tions and bias were calculated and compared to determine the comparability of the two instruments .Results The precisions of two hematocyte analyzers were satisfactory .When fresh whole blood was used as calibrators ,the results of all eight items of the two in‐struments had good correlations (r>0 .975) ,and the relative bias was acceptable .Conclusion The results of two hematocyte ana‐lyzers are comparable .With fresh whole blood using in the comparison test between different hematocyte analyzers ,systematic er‐rors can be discovered in time .
