1.Clinical observation of effect of mifepristone combined with norethindrone in treating dysfunctional uterine bleeding of peri-menopause

Objective To analyze the efficiency of mifepristone combined with norethindrone in treating dysfunctional uterine bleeding of peri-menopause. Methods 56 patients with dysfunctional uterine bleeding of peri-menopause were randomly divided into two groups, the study group(30 cases) and the comparision group(26 cases). The patients in the study group were treated with mifepristone in small doses combined with norethindrone. And the patients in the comparision group were treated only with norethindrone. Before treating, all the patients were observed and measured symptom of dysfunctional uterine bleeding, blood routine,the function of liver and ren.the en-dometrial thickness and pelvic by ultrasonography. Through hysteroscopy or uterine curettage of diagnosis, en-dometriums were measured by pathology. Results 26 cases in the study group were evaluated as effective,2 cases as obviously effective,and 2 cases as no effective. In the comparision group, 10 patients were evalutated as effective,8 patients as obviously effective,8 patients as no effective. 3.Nursing cooperation in using lung inflation system for video-assisted thoracoscopic surgery for spontaneous pneumothorax

Objective To explore the nursing cooperation in use of lung inflation system for video-assisted thoracoscopic surgery (VATS) for spontaneous pneumothorax. Method The key nursing points in nursing 42 patients with spontaneous pneumothorax undergoing VATS were analyzed. Results The operations were all successful. One patient contracted lower left atelectasis but it was cured after the tracheal tube was re-adjusted. On day one, there were two cases of gas leakage during thoracic closed drainage:one was of diffuse pulmonary bulla, and the gas leakage stopped on the second day; In the other cases, the gas leakage stopped 2 days after the tube was withdrawn by 3cm probably because the drainage tube was inserted too deep at the beginning. 6-30 months follow-ups showed no recurrence of pneumothorax in the operated site. Conclusions The lung inflation system is safe during VATS for spontaneous pneumothorax. Careful preoperative preparation of the equipment and skillful nursing cooperation at all procedures are critical for the success of VATS.
4.Application of‘index’teaching method in practical teaching of medical imaging

Objective To investigate the teaching effect of‘index’teaching method used in practical teaching of the medical imaging.Methods A total of 178 medical undergraduates were selected this study,113 clinical medical students using‘index’teaching method were taken as experimental group while others 65 anesthesia medical students using traditional teaching method as control group.The teaching effect of‘index’teaching method was evaluated quantitatively and qualitatively by practical test and questionnaire survey.Results The test scors of experimental group were(80.3±16.5),obviously higher than those of(71.3±25.8)in control group(P<0.05),especially instudents with low scores.Conclusion The new teaching method had obvious advantages in culturing the thinking of imaging diagnosis and in improving the ability to solve clinical practical problems,the‘index’teaching method was more effective than traditional teaching method.
5.Application of case introduced method based on PACS system in medical imaging course

Objective To evaluate the teaching effects of case introduced method based on picture archiving and communication system (PACS) system in medical imaging course.Methods Totally 148 students (2009 grade) majored in five-year clinical medicine from Xiangya Medical College were divided into 10 groups; the 10 groups were taken randomly as control group (n=75) and experimental group (n=73).Control group used traditional teaching methods (film teaching) while experimental group used case introduced method based on PACS system.Effects of teaching between were evaluated by questionnaire and test.x2 test was used to compare students' theoretical test scores,with α level =0.05.Results Based on results of questionnaires,more students in experimental group than in control group hold that case introduced method based on PACS system can achieve the purpose of teaching,improve interests in learning,and strengthen the mastery of knowledge.Test scores showed that the percentage of students scored 90-100 and 80-89 (respectively 14%,75%) in experimental group was significantly higher than that in control group (respectively 5%,66%) (x2 =17.932,P=0.000).Conclusions Application of case introduced method based on PACS system in medical imaging course not only improve students' motivation to learn and their clinical ability,but also allow teachers to find and correct shortcomings of students timely,therefore,it is worth promoting.

7.The prevalence and charateristics of carotid atherosclerosis among healthy middle-aged people in community

Objective To investigate the prevalence and charateristics of carotid atherosclerosis among healthy middle-aged people of community. Methods Totally 1930 subjects aged 50 years and above were selected randomly from 10027 participants recruited during Phase 3 of the Guangzhou Biobank Cohort Study (GBCS).Common carotid artery intima-media thickness (CCA-IMT) and carotid plaques (CP) were measured by B-mode ultrasonography using ALT HDI 3000 mainframe with a high-resolution. Results The prevalences of CCA-IMT and CP in men were higher than women [41.5%(400/963)vs.16.2 (157/967),34.1% (328/967) vs.13.8% (133/967),both P<0.001].The prevalence of CCA-1MT was higher with age increasing in either men or women in groups of 50-yrs,60-yrs and 70-79 yrs (all P<0.001).The most of CP was cankerous spot (37.8%),secondly hard spot (24.9%),soft spot (22.1%) and the prevalence of flat spot was lowest (15.2%).The number of CP was not significantly different between the left and right side (51.0% and 49.0%).The most common location of CP was bifurcations of carotid arteries (61.7 %),next the common carotid artery (21.0%) and internal carotid artery (ICA) was lowest (17.3 %).The prevalence of CP was 44.4% (83/87) in women aged 50- yrs,it was higher than that in men 20.9% (108/517).However,the prevalence of CP was 79.1% (409/517) in men aged more than 60 yrs,higher than that in women (55.6%). Conclusions The carotid atherosclerosis is commonly encountered disease in the male elderly,and the most of CP was cankerous spot in bifurcations of carotid arteries
8.Awareness and control rates of diabetes and hyperlipidemia in hypertensive cerebral infraction patients

Objective To explore the prevalence of diabetes and hyperlipidemia in cerebral infraction patients with hypertension and look into the rates of awareness, treatment and control. Methods Blood pressure, blood glucose and blood lipids were measured in 238 cerebral infraction patients with hypertension. Meanwhile, the rate of awareness, treatment and control of hypertension, diabetes and hyperlipidemia were investigated by questionnaire. Results Among 79.41%of the patients had abnormal blood pressure and cerebral infaction on admission, the rates of awareness , treatment and control of hypertension were 74.79%, 67.23%, 20.59%. 10.11%of them never took antihypertensive drugs in spite of their awareness of hypertension. 32.35% of the patients were combined with diabetes, the rates of awareness, treatment and control were 79.22%, 62.34%, 12.99%respectively, and among them 21.31%never tookantidiabetic drugs in spite of awareness of diabetes. 81.51% of the patients were combined with hyperlipidemia, the rates of awareness, treatment and control were 23.20%, 6.70%, 1.55%respectively, and among them 71.11%never took lipid-lowering drugs with awareness of high blood lipid. Conclusions 79.41% of the incidence of cerebral infraction was related to abnormal blood pressure. 32.35%cerebral infarction patients with hyperlipdemioa are complicated with hypertension diabetes;81.51%are complicated with 28.57%cerebral infraction patients with hypertension were complicated with diabetes and hyperlipidemia as well, but the rate of awareness, treatment and control were relatively low. In order to improve the secondary prevention, we should pay more attention to the stroke risk factors, and help make the individualized medicine-taking plan to improve the control rate.
9. Clinical outcome of allogeneic hematopoietic stem cell transplantation with FLAG sequential busulfan/cyclophosphamide conditioning regimen for refractory/relapsed acute myeloid leukemia

Objective:
To investigate the therapeutic effects of allogeneic hematopoietic stem cell transplantation (allo-HSCT) with FLAG sequential busulfan/cyclophosphamide(Bu/Cy) conditioning regimen for refractory/relapsed acute myeloid leukemia.
Methods:
From February 2012 to June 2017, 21 patients with refractory/relapsed acute myeloid leukemia underwent allo-HSCT with FLAG sequential Bu/Cy conditioning regimen. Transplantation-related complications and clinical outcome were retrospectively analyzed.
Results:
After conditioning, no hepatic veno-occlusive disease (VOD) and grade Ⅲ hemorrhagic cystitis occurred. 76.2% (16/21) patients had fever with 4 septicemia. One patient died of septic shock before engraftment. Twenty patients achieved neutrophil engraftment with a median time of 13 days (range, 10 to 21 days). Seventeen patients achieved platelet engraftment with a median time of 18 days (range, 9 to 25 days). The cumulative incidence of acute graft-versus-host disease (aGVHD) was 39.5%, and 3 patients developed grade Ⅲ-Ⅳ aGVHD. Of 19 patients who survived more than 100 days after transplantation, 4 had local chronic graft-versus-host disease (cGVHD). Of 21 patients, the median survival time was 15 months (range, 0.5 to 67 months) post-transplantation. Transplantation-related mortality rate was 28.7%. Leukemia relapse occurred in 4 patients with a median time of 4 months (range, 3 to 8 months) after transplantation. The cumulative relapse rate at 1 year was 21.4%. The 1-year and 3-year overall survival (OS) rates were 60.7% and 54.9% respectively. Log-rank analysis revealed that bone marrow blasts ≥ 20% or extramedullary leukemia before transplantation, poor platelet engraftment and grade Ⅲ-Ⅳ aGVHD were significantly related to shortened OS (

Objective:To evaluate the efficacy and safety of mitoxantrone hydrochloride liposome injection in the treatment of peripheral T-cell lymphoma (PTCL) in a real-world setting.Methods:This was a real-world ambispective cohort study (MOMENT study) (Chinese clinical trial registry number: ChiCTR2200062067). Clinical data were collected from 198 patients who received mitoxantrone hydrochloride liposome injection as monotherapy or combination therapy at 37 hospitals from January 2022 to January 2023, including 166 patients in the retrospective cohort and 32 patients in the prospective cohort; 10 patients in the treatment-na?ve group and 188 patients in the relapsed/refractory group. Clinical characteristics, efficacy and adverse events were summarized, and the overall survival (OS) and progression-free survival (PFS) were analyzed.Results:All 198 patients were treated with mitoxantrone hydrochloride liposome injection for a median of 3 cycles (range 1-7 cycles); 28 cases were treated with mitoxantrone hydrochloride liposome injection as monotherapy, and 170 cases were treated with the combination regimen. Among 188 relapsed/refractory patients, 45 cases (23.9%) were in complete remission (CR), 82 cases (43.6%) were in partial remission (PR), and 28 cases (14.9%) were in disease stabilization (SD), and 33 cases (17.6%) were in disease progression (PD), with an objective remission rate (ORR) of 67.6% (127/188). Among 10 treatment-na?ve patients, 4 cases (40.0%) were in CR, 5 cases (50.0%) were in PR, and 1 case (10.0%) was in PD, with an ORR of 90.0% (9/10). The median follow-up time was 2.9 months (95% CI 2.4-3.7 months), and the median PFS and OS of patients in relapsed/refractory and treatment-na?ve groups were not reached. In relapsed/refractory patients, the difference in ORR between patients with different number of treatment lines of mitoxantrone hydrochloride liposome injection [ORR of the second-line, the third-line and ≥the forth-line treatment was 74.4% (67/90), 73.9% (34/46) and 50.0% (26/52)] was statistically significant ( P = 0.008). Of the 198 PTCL patients, 182 cases (91.9%) experienced at least 1 time of treatment-related adverse events, and the incidence rate of ≥grade 3 adverse events was 66.7% (132/198), which was mainly characterized by hematologic adverse events. The ≥ grade 3 hematologic adverse events mainly included decreased lymphocyte count, decreased neutrophil count, decreased white blood cell count, and anemia; non-hematologic adverse events were mostly grade 1-2, mainly including pigmentation disorders and upper respiratory tract infection. Conclusions:The use of mitoxantrone hydrochloride liposome injection-containing regimen in the treatment of PTCL has definite efficacy and is well tolerated, and it is a new therapeutic option for PTCL patients.
