Objective To compare the efficacy of laparoscopy versus traditional surgery for elderly patients undergoing nephrectomy.Methods Totally 134 elderly patients undergoing nephrectomy diagnosed by CT or PET-CT were divided into the laparoscopy group and the traditional surgery group (n=67,each).Clinical symptoms,operation time,bleeding volume,analgesic use,the duration of drainage,exhaust time,length of hospital stay,adverse reactions and death rate were recorded.All patients were followed-up for 2 years.Results There were significant differences in the bleeding volume,operation time,analgesic use,duration of drainage and exhaust time between the laparoscopy group and the traditional surgery group [(141.2 ± 23.3) ml vs.(175.7 ± 31.7) ml,(162.4±13.1) min vs.(177.6±13.1) min,(60.7±15.1) mgvs.(89.2±17.7) mg,(2.4±0.9) d vs.(4.0±1.1) d,t=2.92,5.21,15.94,10.5,respectively,all P＜0.05].The incidence of postoperation complications was lower in the laparoscopy group than in the traditional group [4 cases (6.0％) vs.15 cases (22.4％),x2=11.03,P=0.001].Tumor relapse was found in 2 cases in the laparoscopy group and 3 patients in the traditional group.1 patient died in laparoscopy group and 2 patients in traditional group,respectively.There were no significant differences in tumor relapse and death rate between groups (both P＞ 0.05).Conclusions Laparoscopy is safe and effective for elderly patients undergoing nephrectomy and it can reduce the complications and adverse reactions,which can be the first choice for elderly patients undergoing nephrectomy.

microRNA(miRNA)is a group of naturally occurring small non-coding RNA of about 22 nucleotides that have recently emerged as important regulators of gene expression at the posttranscriptional level.miRNAs relates closely with the progressive,diagnosis,treatment and prognosis in lung cancer,this article describes the miRNA in the early diagnosis of lung cancer and potential application value to elaborate.

Objective To study the feasibility of chelating resins A and B removing heavy metals from the extracts of Radix Isatidis.Methods Heavy-metal Removal experiment was performed on automatic control platform in industry mode.With dry ointment yield,comparability of HPLC and removal rate of heavy metals as the indexes,the effects of two kinds of Chelating resins in removing heavy metal were compared.Results After finishing heavy metal removal step,the loss rate of the dry-extract was lower than 7 %,and the heavy metal contents in dry-extract were lower than national limits.HPLC similarity of the extracts before and after removing heavy metals by resin A was higher than 0.97,but that by resin B was very low.Conclusion Chelating resin A is suitable for removing heavy metals from extracts of Radix Isatidis.

Objective To investigate indications of video-assisted thoracoscopic surgery in the treatment of esophageal carcinoma.Methods Video-assisted thoracoscopic surgery was performed in 11 patients with esophageal carcinorma from January 2005 to November 2005.The tumors were located at the upper segment in 2 patients,middle segment in 8 patients,and lower segment in 1 patient.The TNM staging results showed T_1N_0M_0 in 1 patient,T_2N_0M_0 in 2 patients,T_2N_1M_0 in 1 patient,and T_3N_0M_0 in 7 patients.Under thoracoscopy via thoracic and cervical incisions,the esophagus and tumor were disconnected and the intrathoracic lymph nodes were cleared through the right thorax.Then a median incision was made at the epigastrium.Through the incision the stomach was disconnected and intraabdominal lymph nodes were removed.Afterwards,esophagectomy and gastroesophagostomy were performed by way of the cervical incision.Results The operation time was 220~300 min(mean,265 min).The operative blood loss was 150~400 ml(mean,240 ml).The number of removed lymph nodes was 4~10(mean,6.4).The drainage tube was taken out at 2~3 days after operation,with a drainage volume of 200~500 ml(mean,350 ml).One patient developed a temporary left recurrent laryngeal nerve palsy that disappeared at 3 months after operation.Follow-up checkups for 4~15 months(mean,8 months) in the 11 patients showed no recurrence or metastasis of tumor.Conclusions Video-assisted thoracoscopic surgery can be adopted in patients with esophageal carcinoma at stage Ⅰ or Ⅱ,

Objective To observe the influence of heating method (or temperature control strategy) on the concentration of main active components in extracts from Radix Salviae Miltiorrhizae. Methods Common chromatographic peak area and the contents of Salvianolic acid B in three kinds of extracts obtained by 3 kinds of heating methods were compared. Results With the content of extracts extracted by the first heating method as the baseline, content of salvianolic acid B in extracts extracted by the second heating method was decreased 21.73 percent and that by the third method decreased 55.44 percent. Conclusion Quality of Salvianolic acid extracts can be affected greatly by temperature range and temperature control strategy. It is important to strictly control the heating method for the exaction of heat- sensitive active compounds from Chinese herbs.

Objective: The effect of separation rates on the quality and shelf life of TCM in the process of alcohol precipitation was discussed. Methods: According to the sedimentation rate, controlling process was chosen for different doses of medicine. Results: In the process of alcohol sedimentation, different centrifugal coefficient was needed with the variation of conditions, such as temperature, concentration of alcohol and centrifugal coefficient, etc. Conclusion: The control parameters of the process of alcohol precipitation affect the efficacy and shelf life directly in the processing of TCM production.

OBJECTIVE:To investigate influential factors of standardization of clinical research on therapeutic efficacy of Yao-tongning capsules,and to provide direction for reasonable and standard development of clinical research. METHODS:Retrieved from CJFD,VIP and Wanfang database,RCTs about Yaotongning capsules in the treatment of related disease were collected. After data extraction and quality evaluation,Meta-analysis was conducted by using Rev Man 5.3 statistical software. The effects of the di-alectical situation,treatment course and route of administration on the evaluation of therapeutic efficacy of Yaotongning capsules were compared;based on this,the concepts of index evidence-based value which reflected clinical standardization were put for-ward,and its effects on Yaotongning capsules were investigated. RESULTS:A total of 71 RCTs were included,involving 11009 patients. Results of Meta-analysis showed therapeutic efficacy of Yaotongning capsules was better than that of other drugs in dialecti-cal case(P=0.02),while therapeutic efficacy of Yaotongning capsules was inferior to that of other drugs in non-dialectical case(P<0.01);therapeutic efficacy of Yaotongning capsules was inferior to that of other drugs when treatment course was shorter than 30 days(P<0.05),while treatment course was longer than 30 days,there was no difference in therapeutic efficacy between Yaotongn-ing capsules and other drugs(P=0.99). When did not take medicine according to the instruction,therapeutic efficacy of Yaotongn-ing capsule was inferior to that of other drugs(P<0.05);when taking medicine according to the instruction,there was no differ-ence in therapeutic efficacy between Yaotongning capsules and other drugs(P=0.94). When evidence-based value was ≥5,thera-peutic efficacy of Yaotongning capsules was better than that of other drugs(P=0.03);when evidence-based value was equal to 4, there was no difference in therapeutic efficacy between Yaotongning capsules and other drugs(P=0.56);when evidence-based val-ue was ≤3,therapeutic efficacy of Yaotongning capsules was inferior to that of other drugs(P<0.05),with statistical significance. CONCLUSIONS:It is an important guarantee of playing therapeutic efficacy of Yaotongning capsules to regularly use drugs strictly according to the principle of dialectical treatment,and take drugs according to treatment course and instructions. Clinical trials should be standardized strictly in accordance with the instructions,so as to provide objective and fair clinical findings.

Objective To observe the clinical efficacy of Eureklin in the treatment of progressive cerebral infarction in young and middle-aged people outside the thrombolytic time window (the onset time was more than 6h).Methods From March 2016 to March 2019,120 young and middle-aged patients with progressive brain infarction who admitted to the neurology department of Chaonan Minsheng Hospital were selected and all patients were confirmed by cranial CT or MRI.They were divided into two groups by random drawing method.Clinical conventional therapy was applied in the control group (60 cases),and on the basis of conventional treatment,Eureklin intravenous drips was used in the treatment group (60 cases).Before and after treatment 14d,the national institutes of health stroke scale(NIHSS),daily living ability scale(ADL) were used to assessment of neurological impairment and Barthel index in patients.At the same time,the drug side effects and adverse reactions were observed.Results After treatment,the NIHSS score in the conventional treatment group[(3.4 ± 0.3)points] was lower than that in the Eureklin group[(4.1 ± 0.4) points],and the ADLscore in the conventional treatment group [(67.5 ± 24.8) points] was higher than that in the Eureklin group[(57.8± 26.7) points],the difference was statistically significant(t =0.014,0.003,all P ＜ 0.05).The total effective rate of the treatment group [83.33％ (50/60)] was higher than that of the control group [71.67％ (43/60)],the difference was statistically significant (x2 =3.596,P ＜ 0.05).At the same time,no serious adverse reactions were found in the Eureklin treatment group.Conclusion For young and middle-aged patients with progressive cerebral infarction outside the thrombolytic time window,the application of Eureklin can improve the patients' symptoms of neurological dysfunction,improve the ability of daily life,and without serious adverse events such as bleeding,which is worthy of clinical application.

Objective To investigate the value of neutrophil to lymphocyte ratio (NLR) in predicting in-stent restenosis (ISR) in patients with coronary heart disease. Methods We collected the data of 618 patients with coronary heart disease hospitalized in the Department of Cardiology in the First Affiliated Hospital of Kunming Medical University from January 2011 to June 2012. All selected patients underwent coronary angiography and stent implantation, and were divided into two groups according to the average size of NLR. The relationship between the neutrophil to lymphocyte ratio and coronary heart disease in-stent restenosis was investigated. Results There were statistically significant differences in the presence of diabetes, high density lipoprotein, low density lipoprotein, the percentage of neutrophils and lymphocytes in patients between two groups (P<0.05) . We found 13 patients (4.2%) and 32 patients (10.4%) with in-stent restenosis in the first group and the second group, respectively, and there was a statistically significant difference between two groups (P<0.01) . Conclusion Neutrophil to lymphocyte ratio is correlated with ISR,and has clinical value in predicting ISR.
