1.Detection of clostridium perfringens with qPCR in mouse models and a clinical case

Objective To detect of clostridium perfringens by qPCR in mouse models and a clinical case in order to offer early diagnosis.Methods 40 Kunming mice were randomly grouped and intramuscular injected clostridium perfringens type A in leg 0.1 ml(3.5 × 109cfu/ml or 3.5 × 108cfu/ml or 3.5× 107cfu/ml,diluted with saline),while control group was injected with 9% sodium chloride 0.1ml.The mouse models and a clinical case were detected by qPCR.Results The death rate of 3.5 × 109,3.5 × 108,3.5 × 107cfu/ml and the blank group were 90%,70%,10% and 0% after intramuscular injection for 72 h spectively.The mean Ct values among these groups were 21.21 ±2.69,28.45 ±2.74,32.49 ±2.87 and 0.00 ± 0.00(P < 0.05).The Ct values of the patient were 30.67 and 30.44.Conclusions Cclostridium perfringens could be successful identified with qPCR in mouse models when the mice still did not show any symptoms.
2.Segmentation of heart valves in DS CT image based on Contourlet transform

Objecthe Children's heart valves by Dual-Source Compater Tomography(DS CT)images were enhanced and segmented in order to reconstruct 3D construction of heart valves and make motion analysis based on this model.Method,Adaptive seed-filling algorithm is proposed in this paper to segment interested region of heart based on DS CT images.Contoudet transform is used to enhance and segment heart valve based on the part-segmented image.Result With these methods,DS CT images were SUCCESS fully pro-cessed and segmented in Matlab 7.0 and clear images of mitral valves were got.Condusions The adaptive seed-filling algorithm proposed in this paper and Contourlet transform are very useful for segmentation in dif-ferent situation.The experimental result in this paper proides useful reference for doctors in their research on the motion of mitral valves and planning on surrgical operation of mitTal valves.
3.Analysis of 3 527 Male Infertile Patients’Semen

Objective To study the influence factors of infertility by analysis of semen sample and reference for clinical treat-ment.Methods 3 527 cases of semen sample were collected from Jan 2012 to Jun 2014.All samples were analysed by SQA-V analyzer and compared with 80 cases of normal semen.Results There were 358 normal samples (10.2%)and 3 169 ab-normal samples (89.8%).Among the abnormal samples low sperm motility had the highest ratio (2.7%)while abnormal pH had the lowest ratio (61.5%).All the indexes had significance difference to normal sample expect pH value (t=0.065, P =0.969).Among them,the comparison of rate had statistical significance(χ2 =3.214~24.712,P <0.05).The compari-son of mean also had statistical significance(t=2.523~15.324,P <0.05).Conclusion Infertility male almost has abnormal index of semen volume,liquefaction time,sperm motility,sperm density,sperm morphology,sperm viability.Accurately sperm analysis can provide objective basis to clinical diagnosis and treatment.
4.Research progress of glucagon-like peptide-1 and its analogues on oxidative stress

Glucagon-like peptide-1(GLP-1), a polypeptide secreted by small intestinal L cells, has various effects including increasing insulin synthesis and secretion, suppressing appetite, and delaying gastric emptying. In addition to glucose control, GLP-1 has multiple functions in a variety of tissues and organs. A number of studies have shown that GLP-1 receptor agonists could treat a variety of chronic diseases, including diabetes, through antioxidant mechanisms. Based on oxidative stress, this paper summarizes the current progress in the synthesis & metabolism, pancreatic and extracantral effects, anti-oxidation effects of diabetes and its complications, aging, and neurological diseases of GLP-1, with an attempt to provide theoretical reference to researches on related oxidative stress mechanisms and development of new drug.
5.Clinical characteristics in gout patients with different body mass index

Objective Obesity is one of the risk factors for gout.The aim of the present study was to evaluate clinical characteristics of gout patients with different BMI.Methods A total of 5 104 patients with gout were enrolled and divided into three groups according to the BMI.The clinical information was collected and relevant biochemical indices were detected.SPSS software was applied for the statistical analyses.Results There were significant differences in the ratios of gender,regular exercise,hypertension,tophus,renal insufficiency,hyperlipidemia,impaired glucose metabolism,liver dysfunction among the three groups (all P<0.01).The onset age in overweight [45(36,55) years] and obese subjects [40(31,50)years] were earlier than that of the normal weight subjects [50 (38,61) years].Moreover,waist circumstances [103(99,108) cm and 94 (90,98) cm vs 87 (82,91) cm],systolic pressure [130 (120,145) mmHg (1 mmHg =0.133 kPa) and 130 (120,140) mmHg vs 128 (115,140) mmHg],diastolic pressure [90 (80,100) mmHg and 85 (80,92) mmHg vs 80 (79,90) mmHg],fasting blood glucose [5.77 (5.30,6.44) mmol/L and 5.65 (5.19,6.26) mmol/L vs 5.55 (5.10,6.15) mmol/L],TG [2.10 (1.46,3.04) mmol/L and1.88 (1.35,2.78) mmol/L vs 1.52 (1.07,2.39) mmol/L],TC [5.20 (4.55,5.93) mmol/L and 5.07 (4.46,5.75) mmol/L vs 4.95 (4.27,5.65) mmol/L],serum uric acid [483(418,552) μmol/L and461(395,524) μmol/L vs440 (368,517) μmol/L],ALT [30 (21,46) U/L and25 (18,36) U/L vs 21 (14,29) U/L],AST [21(17,28) U/L and 20 (17,26) U/L vs 20 (6,25) U/L],the number of joints involved [2 (1,3) joints and 2 (1,2) joints vs 1 (1,2) joints] in the overweight and obese groups were higher than those in the normal weight group (all P < 0.01).There were no statistical differences in family history,involvement of upper limb joints,kidney stones and coronary heart disease among the three groups (all P > 0.05).Conclusions Obesity is associated with an earlier age of gout onset.With the increase of BMI,the blood pressures,glucose,lipid,serum uric acid,liver transaminase levels and the number of involved joints increased gradually.Cautions should be taken in treating patients with different BMI.

AIM: To detect the expression level of telomere regulating factor tankyrase and its clinical significance in the origin and development of gastric adenocarcinoma. METHODS: Quantitative Real-time PCR was used to measure the expression of tankyrase mRNA in cancerous and normal adjacent tissues from 16 patients with gastric adenocarcinoma. Immunohistochemistry was used to detect tankyrase protein expressions in 37 gastric adenocarcinoma samples and 13 normal controls. 7.Analysis of risk factors for susceptibility of subcutaneous tophi in people with gout

Objective To discuss the risk factors for susceptibility of subcutaneous tophi with an aim to provide clinical evidence for the prevention and treatment of tophi.Methods A total of 5 321 cases of gout patients whose course of disease was less than 10 years were selected and divided into two groups according to whether a subcutaneous tophus was present. The clinical information was collected and relevant biochemical indices were detected.Results There were significant differences in the ratios of regular exercise, involvement of upper limb joints, combining renal insufficiency, kidney stone and coronary heart disease between the tophus group and the non-tophus group(P<0.05 or P<0.01). There were significant statistical differences in age, disease duration, body mass index, systolic blood pressure, diastolic blood pressure, serum uric acid, serum creatinine, creatinine clearance rate, the number of joints involved between the two groups[(51.80±14.58 vs 48.45±14.45)year,(5.61±3.20 vs 3.28±3.06)year,(26.50±3.39 vs 27.05±3.45)kg/m2,(138±20 vs 133±18)mmHg, 1 mmHg=0.133 kPa,(89±13 vs 87±12)mmHg,(486.66±121.96 vs 460.02±119.10) μmol/L,(112.11±62.97 vs 104.43±46.28) μmol/L, (86.46±37.99 vs 93.00±40.11 )ml/min,(2.89±1.86 vs 1.76±1.22) joints, all P<0.01]. There were no statistical differences in family history, gender, history of smoking and drinking, the means of waistline, fasting blood glucose, triglyceride, and total cholesterol between the two groups(all P>0.05). Further logistic regression analysis indicated that disease duration, a large number of joints involved, the involvement of upper limb joints, combining kidney stones, higher serum uric acid level, and higher diastolic pressure were risk factors of the early onset of subcutaneous tophi, while regular exercise and body mass index were protective factors.Conclusion Patients with long duration, high serum level of uric acid, a large number of joints involved, upper limb joint involved, kidney stones, and hypertension were more likely to develop subcutaneous tophi. These risk factors should be intervened actively in clinic.
8. Investigation report on education organizations status of doctor-patient communication courses in medical colleges and universities of China

In order to understand the educational organization status of doctor-patient communication courses, 114 medical colleges and universities of China were investigated, including course offerings, teaching methods and teacher condition. The survey showed that the ratio of offering doctor-patient communication courses was 64.9%, but only 51.3% courses were compulsory. The setting of class hours was arbitrary. The target objects were mainly concentrated on medical students before entering clinical practice and the large-class teaching was a main mode. The average number of teachers was three to four, half of whom were part-time teachers. Therefore, we suggest attaching great importance to doctor-patient communication course by setting it as a compulsory course, emphasizing the practicability of the course in teaching content, and properly handling the relationship between full-time teachers and part-time teachers.

Objective To investigate the treatment of radi us head fracture through internal fi xation.Methods 42cases of radius head fracture trea ted with internal fixation were foll owed up.Mason classification and Broberg and Morrey criteria were use d to analyze the correlation between the results and the factors of fractu re type and method of fixation.Results42cases were followed up for a mean time of 25months.Evaluation was done according to Broberg and Morrey score systems.The function recovered we ll in 76%of the cases.The results of minor plate group were better than th ose of the screw(P=0.01)or K wire group(P=0.04).The results of Mason typeⅡwere better than those of the typeⅣ(P=0.03).Conclusion Treatment of radius head fracture with internal fixation will improve the elbow function and has better result for Mason typeⅡ.Fixation with minor plate is better than with screw or K wi re.[
10.Synthesis and preliminary anti-diabetic activity evaluation of novel PEGylated GLP-1 receptor agonists

In order to obtain glucagon-like peptide-1(GLP-1)analogs which can sustainedly control the levels of glucose, 12 derivatives were designed and synthesized by coupling monomethoxy polyethylene glycol(mPEG, with average molecular weights of 350, 550 and 750)to GLP-1 analogs. Preliminary pharmacological activities showed that all compounds retained GLP-1 receptor agonist activities, and the hypoglycemic activity of compound I-12 was similar to those of Ex-4 and Liraglutide, suggesting I-12 could be a potential long-acting GLP-1 receptor agonist.
