1.Qualitative research in factors affecting the compliance of patients with chronic hepatitis B

Objective To explore the factors influencing the patients' treatment compliance infected with chronic hepatitis B virus, and provide some useful advices to improve patients compliance. Methods Data were collected through deep interviews with 9 patients and analyzed using a phenomenological hermeneutic method. Results Using generic analysis, the factors influencing patients' compliance fell in-to three themes: (1) The enforceability of the doctor's advice; (2)The factors of the patients their own, in-cluding the disease-related knowledge, the realization of how harmful the disease is, and past behavior habits; (3)The social factors, including economic situation, pressure of life, family care, and the supervision and guidance of health care workers. Conclusions In order to make the patients consciously adopt the doctor's advice, health workers should pay more attention to strengthen the enforceability of the doctor's ad-vice and health education, and focus on the impact of cognitive, psychological and social factors on pa-tients' compliance.
2.Study on intra- and interatrial asynchrony in aged patients with paroxysmal atrial fibrillation

Objective To evaluate intra- and interatrial asynchrony and its determinants in aged patients with paroxysmal atrial fibrillation (PAF) by using tissue Doppler imaging. Methods Ninty-one patients without PAF (control group, including 40 elder patients and 51 non-elder patients) and 52 aged patients with PAF were included. As to assessment of intra- and interatrial synchronicity, the atrioventricular plane were selected on the right atrial (RA) free wall, interatrial septum (IAS), and left atrial (LA) free wall. The time differences from the onset of the P wave to the onset of the A wave at the left atrium (P-LA), the IAS (P-IAS), and the right atrium (P-RA) were measured. Intra-atrial asynchrony was defined as the differences between P-IAS and P-RA (RA asynchrony) and between P-LA and P-IAS (LA asynchrony). Interatrial asynchrony was defined as the difference between P-LA and P-RA. Stepwise regression was made to determine the influencing factors for atrial asynchrony in aged patients with PAF. Results Compared with the control group, aged patients with PAF had significant LA and interatrial asynchrony (P<0.01). Multivariate stepwise regression demonstrated that systolic blood pressure (x2), age (x1) and left ventricular mass index (LVMI x5) entered the regression equation in aged patients with PAF (Y=-57.241+0.481 x1+0.223 x2+0.294 x5). Conclusions Aged patients with PAF have LA and interatrial asynchrony. LVH, aged and SBP are important factors leading to these asynchronies in the aged patients with PAF.
3.Effect ofω-3 polyunsaturated fatty acids on endothelial function in postmenopausal women with type 2 diabetes

Objective The incidence of cardiovascular and cerebrovascular diseases in postmenopausal women with type 2 diabetes is grim.The study was designed to explore the effect of ω-3 polyunsaturated fatty acids (PUFA) on endothelial function in postmenopausal women with type 2 diabetes. Methods 50 patients admitted to Dingli Medical College of Wenzhou Medical Univer-sity from March 2014 to October 2014 were divided into group A and Group B by random number table .Cross-design of two stages ( I, II) was applied in the investigation .At stage I(3 months ahead of the experiment ), Group A took oral ω-3 PUFA while Group B took placebo .At stage II ( 3 months after the experiment ) , Group B was given oral ω-3 PUFA, while Group A was given placebo .T1 and T3 time was the beginning of the stage I and stage II experiment , while T2 and T4 time was the end of stage I and stage II experiment .At the beginning and end of each stage , detection was made on LDL-C, TG, IL-6, plasminogen activator inhibitor-1 (PAI-1) and endothelium-dependent flow-mediated vasodilation (FMD). Results After the intervention on Group A at stage I , FDM at T2 time was significantly increased compared with that at T 1 time([7.23 ±3.28]% vs [3.62 ±2.13]%, P<0.05), while all the other indexes at T2 time decreased significantly in comparison with T1 time: LDL-C ([2.85 ±0.47]mmol/L vs [3.36 ±0.57] mmol/L), TG([2.41 ±1.06]mmol/L vs [2.96 ±1.12] mmol/L), IL-6([2.83 ± 0.30]ng/L vs [3.42 ±0.32]ng/L), PAI-1 ([7.23 ±3.28]ng/L vs [3.62 ±2.13]ng/L) (P<0.05).After receiving ω-3 PUFA intervention on Group B at stage II , FDM at T4 time was significantly increased compared with that at T 3 time([6.88 ±2.06]% vs [3.60 ±2.18]%, P<0.05), while all the other indexes at T4 time decreased significantly in comparison with T3 time: LDL-C ([3.26 ±0.77]mmol/L vs [3.63 ±0.73] mmol/L), TG([2.28 ±0.94]mmol/L vs [2.77 ±1.25] mmol/L), IL-6([2.91 ± 0.48]ng/L vs [3.30 ±0.52]ng/L), PAI-1 ([45.7 ±24.4]ng/L vs [56.3 ±24.4]ng/L) (P<0.05).Two-period crossover design analysis of variance showed that there was significant difference effect on LDL -C(F=2.754, P=0.019), TG(F=3.115, P=0.011), IL-6(F=1.825, P=0.032), PAI-1(F=2.324, P=0.023) and FMD(F=3.784, P=0.006)between ω-3 PUFA and placebo . Conclusion ω-3 PUFA can improve endothelial function in postmenopausal women with type 2 diabetes , which is of great significance for the primary prevention of cardiovascular disease .
4.Effect of different solvents on extraction of effective components from Ligusticum chuanxiong.

OBJECTIVETo compare the effect of different solvents such as water, ethanol, methanol, ethyl acetate and petroleum ether on extraction of 10 effective components from Ligusticum chuanxiong and component characteristics of corresponding extracts.
METHODUltrasonic assisted solvent extraction and high performance liquid chromatography quantitative analysis were adopted to determine effective components. CAPCELL PAK C18 column (4.6 mm x 150 mm, 5 microm) was adopted. The mobile phase was methanol-0. 5% HAc for gradient elute. The detection wavelength was 280 nm. The column temperature was 30 degrees C. The flow rate was 0. 7 mL x min(-1). The sample size was 10 microL.
RESULTMethanol or ethanol showed no significant difference in extraction of ferulic acid, hydroxyphthalide, alkylphthalide and diligustilide. Ethyl acetate displayed relatively low extraction ratios in hydroxyphthalide and ferulic acid. Water and petroleum ether showed relatively low extraction ratios in all four effective components, and water extracts showed different component characteristics.
CONCLUSIONEthanol and methanol are the most suitable solvents to extract four effective components from L. chuanxiong.
