1.Analysis of test results using residual reagents in electrochemiluminescence immunoassay analyzer

Objective To evaluate the detection efficiency of residual reagent used in electrochemiluminescence immunoassay an-alyzer .Methods Alpha fetoprotein (AFP) ,carcino-embryonic antigen(CEA) and total prostate specific antigen(TPSA) were de-tected by using remaining reagents .The precision and recovery rate of remaining reagents were analyzed .Referring to the document of NCCLS EP9-A2 ,8 clinic samples were detected everyday by residual reagent and new reagent respectively .The test results of to-tal 40 samples were recorded within 5 days .The test results derived from 2 kinds of reagents were analyzed comparatively and their bias was evaluated by using new reagent as a control method and residual reagent as experimental methods .Results The within-run and between-run coefficients of variation(CV) of the 3 items measured by residual reagents in low and high levels of quality control products met the related standard .The recovery rate was variable from 90% to 110% .The test rewults of the 2 kinds of reagents were positively correlated(r2 >0 .95) .Their anticipated biases were within allowed biases on the medical decision level of CEA ,AFP and T PSA .Conclusion Residual reagent of electrochemiluminescence immunoassay analyzer can meet the clinical practice needs , which also can ensure the quality of measurement and the reduction of the cost .
2.Curative effect of edaravone on acute severe brain injury

Objective To study curative effect of edaravone on acute severe brain injury through the observation of neuron-specific enolase( that is:NSE) changes.Methods 60 severe brain injury patients were randomly assigned into the treatment group (n=30 ) and the control group (n=30).Besides the routine treatment,edaravone was used in the treatment group for 7 days.The control group received the same treatment except edaravone.NSE were measured in the two groups before the treatment and at 1st,3rd,5th,7th day after the treatment.GCS was evaluated at 21 days.Results Compared the NSE before treatment and after three days,they had significant difference(P<0.05,also between the two groups and the GCS after 21 days.they had no significant adverse drug reactions during the two groups of patients with medication.Conclusion Edaravone for patients with severe brain injury has a better treatment effect,medication safety,fewer adverse reactions from the mechanism of NSE at acute phase.
3.Influence of edaravone therapy on serum levels of neuron-specific enolase in the patients of severe brain trauma

Objecfive To study the influence of edaravone therapy on semm levels of neuronspecific enolase (NSE)in the patients of severe brain trauma and assess the therapic effect of edaravone.Method Sixty patients of severe brain trauma were randomly assigned into the treatment group(n=30)and the control group(n=30).Besides the routine treatment in both groups.edaravone Was only used in the treatment group.NSE levels were measured in the two groups before treatment and at 1st,3rd,5th,7th day after treatment.Results NSE levels before treatment in treatment group and control group were(48.73±30.46)μg/L and (40.86±22.05)μg/L,respectively,there Was no statistical significance between two groups.NSE levels after treatment at 3rd,5th,7th day in the two groups were(34.16±15.08),(29.86±12.61),(29.93±14.77)μg/L and (27.42±10.07),(22.33±8.93),(21.17±9.67)μg/L,respectively,there were statistical significances between two groups and in the same group (P< 0.05 or < 0.01 ). With the time of therapy extended,NSE levels declined progressively. There were no significant adverse drug reactions during the treatment period. Conclusions The influence of edaravone therapy on serum levels of NSE in the patients of severe brain trauma is significant,edaravone for patients with severe brain trauma has a better curative effect ,medication safety,and fewer adverse drug reactions during acute phase.
4.The expression, and significance of PTA1 in patients with pregnancy-indnced hypertension syndrome

Objective To explore the relationship of the expression, regulation and adhesion of PTA1 in pregnancy induced hypertension(PIH). Methods The expression,regulation and adhesion of PTA1 on vascular en- dothelial cells incubated by sera from 10 PIH patients and 10 normal pregnant women repectively was detected by flow cytometry. Results The positive rate of PTA1 15.51% (24h) and 6.32 % (48h) in PIH group is significantly higher than that 7.81% (24h) and 4.72 % (48h) in normal pregnant woman group( P < 0.01 ). After the incubation with platelets,the positive rate of PTA1 6.64% (24h) and 4.13% (48h) in PIH group is lower than before(P< 0.05). The PTA1 expression 8.11% (24h) and 4.28% (48h) in PIH group is much lower after blocking with PTA1/IgG than that before(P < 0.01 ). Conclusion There are some abnormal stimulated factors in PIH patients, which activate vascular endothelial cells. PTA1 plays an important role in the adhesion of platelets to vascular en- dothelial cells, PTA1 is closely related to the progress of PIH which indicates that PTA1 directly or indirectly takes part in the pathophysiologic consequences of PIH.
5.The diagnostic value of cytokeratin 19 fragment in non small-cell lung cancer

Objective To discuss the diagnostic value of cytokeratin 19 fragment(CYFRA21-1) in non small-cell lung cancer (NSCLC) .Methods The serum CYFRA21-1 levels were tested in 1 046 healthy adults(healthy group) ,244 benign pulmonary dis-ease patients(benign disease group) and 90 NSCLC patients(NSCLC group) .The results were analyzed statistically .Results There was not significantly difference between man and women in healthy group for CYFRA21-1 level(P>0 .05) .The CYFRA21-1 level in more than 60 years cases was obviously higher than that in less than 18-40 years group and >40-60 group .The upper limit of serum CYFRA21-1 was 4 .00 ng/mL according 95% confidence interval of benign disease group .A cut-off value of 4 .70 ng/mL in NSCLC group was suggested when compared with receiver operating characteristic curve(ROC curve) ,and its sensitivity and speci-ficity were 62 .1% and 92 .6% respectively .Conclusion The diagnostic value for serum CYFRA21-1 in NSCLC was 4 .70 ng/mL .
6.Fungal infection in organ transplant patients.

PURPOSETo review the characteristics and evolution of the fungal spectrum, and the risk factors causing fungal infection, and to make progress in diagnosing fungal infection after organ transplantation.
DATA SOURCESAn English-language literature search (MEDLINE 1990 - 2000) and bibliographic review of textbooks and review articles.
STUDY SELECTIONTwenty-three articles were selected from the literature that specifically addressed the stated purpose.
RESULTSFungal infections in organ transplant patients were generally divided into two types: (1) disseminated primary or reactivation infection with one of the geographically restricted systemic mycoses; (2) opportunistic infection by fungal species that rarely cause invasive infection in normal hosts. The risk factors of fungal infection after a transplant can be evaluated and predicted according to the organ recipient's conditions before, during and after the transplant. Progress in early diagnostic methods during the past 10 years has mainly revolved around two aspects, culture and non-culture.
CONCLUSIONSIt is important to undertake a systemic evaluation on the condition of the organ recipient before, during and after a transplant; should any risk factor for fungal infection be suspected, diagnosis should be made as early as possible by employing mycological techniques including culture and non-culture methods.

7.Addition of Theophylline or Increasing the Dose of Inhaled Corticosteroid in Symptomatic Asthma: A Meta-Analysis of Randomized Controlled Trials.

PURPOSE: Low-dose theophylline has anti-inflammatory effects. The aim of this study was to evaluate the effects of adding theophylline compared with increasing the dose of inhaled corticosteroid (ICS) on symptomatic asthma. MATERIALS AND METHODS: The associated literature was acquired through deliberate searching and selected based on the established inclusion criteria for publications. The extracted data were further analyzed by a meta-analysis RESULTS: Four randomized, controlled, parallel studies were selected. Addition of theophylline produced a greater increase of forced expiratory volume in one second as %predicted (FEV1pred) by 2.49% [95% confidence interval (CI) 1.99-3.00; z = 9.70; p < 0.001], compared with increasing the dose of ICS. There was no difference between the two treatments in terms of peak expiratory flow (PEF). CONCLUSION: Addition of theophylline to ICS has similar therapeutic effects on improving lung function as increasing the dose of ICS in the treatment of symptomatic asthma.

8.Identification of Trichophyton rubrum by PCR fingerprinting.

OBJECTIVETo observe the unique DNA profile and the relationship between DNA profile and phenotype of Trichophyton rubrum,and establish an effective molecular method to identify T. rubrum.
METHODSThree primers, including (GACA)(4), (GTG)(5) and M13 core sequence (5'-GAGGGTG-GCGGTTCT-3'), were used to distinguish variations among 20 clinical isolates of T. rubrum and Trichophyton mentagrophytes.
RESULTSDifferent PCR-fingerprinting was seen between T. rubrum and T. mentagrophytes using three different primers. 2 stains of T. rubrum were identified among 6 supposed T. mentagrophytes.
CONCLUSIONST. rubrum can be distinguished using PCR, and (GACA)(4) is the most suitable primer.
