1.Application value of duodenal suspension traction technology in laparoscopic duodenal anastomosis

Objective:To observe the application value of duodenal suspension traction technology in laparoscopic duodenal anastomosis.Methods:80 cases of congenital intestinal atresia and stenosis A children treated in our hospital from May 2013 to May 2015were selected by retrospective study law,and were divided into the control group (not used) group and the observation group (with group) according to their surgery whether duodenum suspension traction technology,40 cases in each group.All patients were laparoscopic surgical approach for the three holes.The general surgery situation the incidence of postoperative complications and associated laboratory parameters in the two groups were compared.Results:The operative time,the initial feeding time,the time return to normal diet and hospital stay in observation group were shorter than those in the control group,and the number of blood loss and cases converted to open surgery in the procedure was as well as was significantly less than the control group (P<0.05);Before surgery,the level of WBC,NEU%,and inflammatory cells factors as PCT,IL-6CPR in the two groups had no significant difference;five days after surgery,the level of WBC,NEU% and IL-6 in the two groups were all decreased,but the observation group decreased more significantly(P<0.05);observation group The rate of anastomotic leakage,anastomotic dehiscence,the incidence of adhesions,wound infection and other complications in observation group were significantly lower than the control group,the difference was statistically significance(P<0.05).Conclusion:The duodenum suspension traction technology in laparoscopic duodenal anastomosis,can reduce accidental injury,then reduce the operation rate,thus shortening the duration of surgery in children,the time required for the first time to eat and hospitalization time for the expansion of the surgical field,and which significantly can reduce the incidence of postoperative infection and other complications in children.

The properties of Chinese materia medica are believed to be the summarization of the effects of biological performance on the various body states. Systemic discussion of chemical-factor elements, body-condition elements, biological-performance elements and their interrelationships is needed for research into the properties of Chinese materia medica. Following the practical characteristics of Chinese medicine, the three-element mathematical model was formed by introducing some mathematical concepts and methods and was used to study the cold or hot property of Chinese medicine, and to investigate the difference in biological performances of the two properties.

Hydrocortisone-induced yang-deficiency animal model has now become the generally accepted model of yang deficiency. However, assessing the most appropriate dose of hydrocortisone is a long-term challenge. For analyzing the modeling dose, the authors have built several kinds of yang-deficiency models induced by hydrocortisone at different doses, and analyzed the experimental data with various mathematical statistical methods. In order to discuss the effects of the modeling dose on the basis of previous research, the authors introduced Ridit analysis.

It is suited for the application of Fuzzy mathematics in research of herbal properties. Objective To explore the problems and countermeasures during the implementation of after-sale service of the medical equipment.Methods The common problems and their causes during after-sale service were analyzed,and some countermeasures were proposed from the aspects of signing purchase contract,acceptance check,operation quality,overall evaluation,personnel and etc.Results Strengthened after-sale service contributed to enhancing the medical equipment in operation quality,medical safety and efficiency.Conclusion It is important to take corresponding measures during after-sale service to improve operation quality and decrease operation cost of the medical equipment.

9.Follow-up of endoscopic submucosal dissection for early esophageal cancer

Objective To investigate the intermediate and long-term efficacy of endoscopic submucosal dissection (ESD) for early esophageal cancer(EEC).Methods A total of 56 patients with EEC underwent ESD at Jiangsu Province People's Hospital between April 2010 and June 2015.Among the 56 cases,there were 39 cases of intramucosal cancer,17 cases of submucosal cancer.Intravascular cancer embolus was found in 2 patients.The en bloc and complete resection rates,the residual,local tumor recurrence and new occurrence rates of EEC after ESD were evaluated.The average follow-up time was 24.4 months,ranging from 1 to 62 months.Results The en bloc and complete resection rates were 92.9%(52) and 87.5% (49),respectively.Four patients were treated by additional esophagectomy.The cases of residual lesions,local tumor recurrence,new occurrence and second primary extra-esophageal cancer (gastric cancer) was 1 (1.8%),2 (3.6%),2 (3.6%) and 2 (3.6%),respectively.No additional surgical operations were performed in the 7 patients or no recurrence was found,and there was no death during the follow-up period.Conclusion ESD has the advantage of high complete resection rate,low residual and local tumor recurrence rate in treatment of EEC,and the intermediate and long-term outcomes are satisfactory.
10.Investigation of RET proto-oncogene mutation in patients with medullary thyroid carcinoma

Objective To detect the mutations of RET proto-oncogene in patients with medullary thyroid carcinoma (MTC). Methods Twelve patients with MTC confirmed by pathology and two clinically suspected patients were recruited. Total genomic DNA was extracted from peripheral blood for PCR. PCR products of exon 10, exon 11 and exon 16 of the RET proto-oncogene were purified and direct gene sequencing was performed. Results The genomic mutations were detected in 8 patients, in which direct gene sequencing analysis detected a mutation of TGC to CGC at codon 618 in exon 10 in 1 patient, a mutation of TGC to TAC at codon 634 in exon 11 in 2 cases, a mutation of TGC to CGC at codon 634 in exon 11 in 3 patients and a mutation of ATG to ACG at codon 918 in exon 16 in the remaiming 2 cases. Conclusion MTC can be diagnosed at gene level by direct gene sequencing analysis. It is possible to diagnose MTC before operation by means of molecular genetic analysis.
