1.Transmission Route of Legionella pneumophila in Central Air Conditioning System

Objective To explore the transmission route of Legionella pneumophila in central air-conditioning system. Methods The environmental samples collected from four public places of Nanjing city were detected by nested-PCR and analyzed by phylogenetic analysis. Results The positive rate of Legionella pneumophila in sediment, cooling water, aerosol near cooling tower, dust in duct surface and indoor air were 100%(9/9),78.6% (11/14),25.4% (17/67),16.7% (2/12) and 17.3% (13/75), respectively. The nucleotide sequence similarities of Legionella pneumophila in cooling water and indoor air was more than 92%. Conclusion The main pollution source of Legionella pneumophila is cooling water and Legionella pneumophila can be transmitted to indoor air through central airconditioning system.
2.Expression of Glucocorticoid Receptor in Peripheral Blood Mononuclear Cells from Patients with Pemphigus

Objective To clarify whether the mRNA expression level of glucocorticoid receptor (GR) in the peripheral blood mononuclear cells (PBMCs) can predict the clinical response to glucocorticoid (GC) in patients with pemphigus. Methods Reverse-transcription polymerase chain reaction (RT-PCR) was applied to determine the mRNA expression of GR? and GR? in PBMCs from 30 patients with pemphigus and 30 healthy volunteers. Results GR? mRNA was detected in all patients and all healthy volunteers, and the expression of GR? mRNA in pemphigus patients was significantly lower than that in healthy volunteers (P = 0.044), but no significant difference was found between patients insensitive and sensitive to glucocorticoid. In contrast, GR? mRNA was detected in 10 of 12 patients insensitive to glucocorticoid, 3 of 18 patients sensitive to glucocorticoid, and 9 of 30 healthy Volunteers. The clinical manifestations of the disease include dysphagia, foreign body sensation in pharyngeal, retrosternal pain and regurgitation. Physical examination showed a sausage-shaped mass hanging outside the mouth, sometimes. CT scan demonstrated a benign placeholder in upper segment of esophagus. Surgery is the only way to achieve radical cure. Pathological examination: inflammtory fibroid polyp.

4.Diagnosis and endoscopic treatment of isolated sphenoid sinus disease.

OBJECTIVE:
This study was to investigate the clinical symptoms, characterization of imaging technique and the effect of endonasal endoscopic surgery.
METHOD:
Following 38 cases of isolated sphenoid sinus disease patients, among whom 35 cases treated with nasal sinus CT (including 5 cases of MRI at the same time), 3 cases undergoing sinus MRI, 1 cases going through the CT cisternography. All patients underwent endoscopic sphenoid otomy, of which 33 cases underwent nasal olfactory cleft approach, 5 cases anterior ethmoid sinus and posterior ethmoid sinus approach (Messerklinger technology).
RESULT:
All patients were followed up for more than half a year, of which 34 were recoverd and 4 were improved. No obvious complications came up during or after endoscopic surgery.
CONCLUSION
Isolated sphenoid sinus disease clinical symptoms are not typicaland without specialty; nasal examination had no positive sign, only with headache as the chief neurological symptom, often difficult to diagnose at an early stage. The CT and MRI are the best methods for the diagnosis of isolated sphenoid sinusitis. The nasal endoscopic operation is the preferred method for treatment of this disease.

5.Influence Factors of Drinking Water Quality of Rural Areas in Huaihe and Yangtze Region in Jiangsu Province

Objective To understand the hygienic situation of drinking water in the rural areas in the river system of the Yangtze and Huaihe in Jiangsu Province,and to provide the scientific evidence for improving the quality of drinking water. Methods From January to October in 2007,according to the guideline of drinking water quality monitoring in the rural areas,in the river system of the Yangtze and Huaihe in Jiangsu Province,in 53 counties (cities),the drinking water quality of the central water supply and the non-central water supply was investigated. Results From the result of comprehensive evaluation,the eligible rate of drinking water in the rural areas in the river system of the Yangtze and Huaihe in Jiangsu Province was 41.34%. As for residual chlorine the eligible rate was 49.64%,as for total coliform group it was 79.69%,as for total number of bacteria it was 88.78%,as for the turbidity it was 85.75%,as for fluoride it was 92.63%,as for the other indexes it was above 95%. Objectives:To investigate the clinical significance of the expression level of cytokeratin5/6/18 positive cells in peripheral blood cells from patients with breast carcinoma.Methods:The expression rates of ck5/6/18 positive cells in peripheral blood cells from 56 patients with breast carcinoma were measured and analyzed by flow cytometry.Results:The expression rates of ck5/6/18 positive cells in peripheral blood cells from 56 patients with breast carcinoma was 46.4% and there was significant differences compared with healthy volunteers and the patients with breast benign diseases.The positive rates increased significantly with the pathological staging and show no significant relationship with the ER and PR.Conclusion:The expression of ck5/6/18 positive cells in peripheral blood cells correlates significantly with the pathological staging of breast carcinoma and it could be an effective signal to clinical monitor and prognosis.
8.Effect of surgical intervention on the activity of peripheral blood T lymphocyte subsets and NK cell in children with obstructive sleep apnea hypopnea syndrome

OBJECTIVE To explore the condition of the cellular immune function of children with obstructive sleep apnea hypopnea syndrome(OSAHS) and its change after operation METHODS Eighty children diagnosed as OSAHS were included in this study. T lymphocyte subsets and NK cells of the blood were measured before operation and one month after operation respectively. Children without the related disorders or systemic disease were recruited as control group.RESULTS Before operation, CD4+T cell percentage, CD4+/CD8+ ratio and NK cell percentage in OSAHS children were significantly lower than that in the control group[(26.19±3.38)%vs (5.10±4.09)%, (0.90±0.19)%vs (1.61±0.52)%, (11.45±4.35)%vs (13.73±3.78)%,P<0.05], CD8+T cell percentage was significantly higher than that in the control group[(30.02±5.50)%vs (22.31±3.42) %,P<0.05]. The difference, between the two groups, in CD3+T cells was not statistically significant. After operation, the CD4+T cells, CD8+T cells, CD4+/CD8+ ratio, the percentage of NK cells and CD3+T cells showed no significance difference compared with that before operation (P>0.05). CONCLUSION OSAHS may damage the cellular immune function. The operation does not reduce the cellular immune function in the short term.
9.Predicting possibility of mortality in critically ill patients with neurological diseases by using Simplified Acute Physiology Score Ⅱ

Objective To evaluate feasibility and reliability of using Simplified Acute Physiology Score Ⅱ (SAPS Ⅱ)in predicting mortality in critically ill patients with neurological diseases.Methods All 653 patients hospitalized in neurological intensive care unit (N-ICU) from Jan 2005 to Dec 2007 were retrospectively studied.SAPSⅡ scores were scaled upon admission at 24,48 and 72 h,and possibility of hospital mortality (PHM) was calculated based on SAPS Ⅱ score.Neurological diseases diagnosis made initially at time of hospitalization was classified into five categories:intracranial hemorrhage,cerebral infarction,neurologic infection,neuromuscular diseases and other neurologic diseases.At each of four time points,the SAPSⅡscores were compared between the survival group and death group,and the relationship of SAPS Ⅱ score and prognosis were analyzed.The calibration of the SAPS Ⅱ were accessed with the Hosmer-Lemeshow goodness-of-fit chi-squared statistic and the discrimination with area under the receiver operating characteristic curve (AUROC).Multivariate logistic regression was used to analyze the relationship between disease category and prognosis.Results SAPS Ⅱ scores in the death group (46.53±12.22,47.28±13.84,48.58±14.18 and 49.06±14.61)at each time point were significantly higher than those in the survival group (34.70±11.78,30.28±12.24,29.79±12.36 and 29.69±12.96;t=11.12,14.02,14.43 and 13.49 at 0,24,48 and 72 h,respectively,P<0.01).Furthermore,univariate logistic regression analyses demonstrated that SAPS Ⅱ score was correlated with prognosis (OR=1.080,1.100,1.109,1.100 at 0,24,48 and 72 h,respectively,P<0.01).The scores at 48 and 72 h were more accurate in predicting mortality.SAPS Ⅱ had good calibration at each time points (x2=5.305,7.557,6.369 and 8.540,P>0.05),however,the consistency of expected mortality with observed mortality was satisfactory only at 48 and 72 h(82.6%,83.4%),so was the discrimination ( AUROC=0.825,0.847 respectively).There was no correlation between disease categories and outcome.Conclusion SAPS Ⅱ scoring,best evaluated at 48 and 72 h after hospitalization,can be used as a reliable predictor of probability of mortality in patients hospitalized in N-ICU and prediction can be applied in these patients with all different neurology diseases.
