1.Exploration of magnetic resonance urography technique
Chinese Medical Equipment Journal 2003;0(10):-
Objective To explore the clinical application and significance of Magnetic Resonance Urography. Methods T1WI and T2WI images in transverse section and T1WI images in coronary section were obtained. Then fast spiral echo sequence was used to get weighted T2WI images and the images were reconstructed with MIP(maximum intensity projection). Results Among the 20 cases,18 MRU images met the diagnostic need, and 2 cases failed because of poor preparation. Conclusion MRU is suitable for the patients who are inappropriate to do IVP(intravenous pyelography),such as urinary tract restricted and obstructed, old people, poor physical condition and iodine allergy,or the patients who did IVP but there were no contrast agent discerned.

Objective: To explore the application value of multi-slice spiral CT based on low dose technique in diagnosis of rib fracture. Methods: 58 patients with highly suspected rib fracture patients were examined by DR and multi-slice spiral CT. 3D reconstruction were finished after MSCT examination at the workstation, including MPR(multi-planar reconstruction), and volume rendering technique(VR). And the results were compared between DR and CT. Results:58 patients all successfully completed the chest X-ray film and multi-slice spiral CT examination. Multi-slice spiral CT image quality fully met the requirements of clinic. It can not only diagnose, but also display the number, location and morphology of rib fracture. 45 cases were diagnosed with chest DR rib fracture and suspected fracture, and the positive rate was 77.59%(45/58). 56 cases were definitely diagnosed with multi-slice spiral CT scanning and three-dimensional reconstruction rib fracture, and the positive rate was 96.55%. Conclusion: The patients can receive lower dose and the tube can be used for a longer time by low dose technique. 3D reconstruction techniques of multi-slice spiral CT not only improve the accuracy of diagnosis, but also display rib fracture morphology clearly, so its clinical value is much higher than DR.

Objective To study the technique of multi-slice spiral CT 3D angiography(CTA) and it's clinical appliction. Methods Multi-slice spiral CT angiography(CTA)in 71 cases were analyzed including intracranial vascular CTA 12 cases, thoracic vascular 9 cases, abdominal 23 cases, low extremity 7 cases, carotid 8 cases, renal artery 12 cases. Media contrast were used in 1.5~2ml/kg, the rate of injection were 2.5~3ml/s.Three ways of scan delay time were applied, that was bolus tracking, test bolus and calculating time. Data reconstruction were done with Real time 3D and Fly 3D and MPR in working-station.Results In intracranial CTA, there were intracranial aneurysm 6, AVM 1, normalities 5. On thoracic CTA, lung arterio-venous fistula in 3, lung arterious embolism in 1, thoracic aortic dissection in 3, and normalities in 2 were showed. In abdominal CTA, abdominal aortic aneurysms were 5, dissection were 15, normalities were 3. In renal arteries CTA, left renal artery stenosis were 3, right renal artery stenosis were 2, normalities were 7. In low extremity CTA, artifical vessel was 1, calcification in both femoral artery was 1; aneurysm in both femoral artery was 1,normalities were 4. In carotid artery, calcification in both sides was 1, normalities were 7.The vascular cavity, calcic spots and abdominal aortic dissection could be showed on CTA. Conclusion Multi-slice spiral CT angiography (CTA) can show 3D vascular structure in whole body, CTA is helpful in guiding vascular operation and stent transplantation

Objective To evaluate the diagnostic and staging value of orbital structures quantitative measurement with 3T mag-netic resonance imaging in patients with Graves’ophthalmopathy (GO).Methods Twenty-three GO patients (patient group)and eighteen healthy volunteers (contrast group)were enrolled.Quantitative measurement of orbital structures including exophthalmos values, fatty tissue thickness of the inner side of eye balls,cross-sectional areas of extraocular muscles and signal intensity ratios of extraocular muscles to the ipsilateral temporal muscles (SIR values),and they were compared between two groups.Correlations be-tween the quantitative values and clinical active score (CAS)were accessed.Receiver operating characteristic (ROC)analysis was performed to evaluate the most relevant quantitative parameter and its diagnostic value in discriminating active from inactive GO pa-tients.Results There were significant differences between the GO group and contrast group regarding to all the quantitative parame-ters (P <0.05).Strong correlation was found between SIR values and CAS (r =0.730,P <0.001 ).Significant difference of SIR was found between patients with active GO and inactive GO (P =0.002),and a cut off value of 3.25 might be the critical threshold value, with diagnostic sensitivity of 81.8% and specificity of 83.3% respectively. Conclusion Quantitative measurement of orbital structures with 3T MR imaging could provide assistant in diagnosing and staging of GO.

Objective To evaluate the diagnostic value of MSCT angiography and postprocessing techniques in the diagnosis of spontaneous isolated dissection of superior mesenteric artery(SIDSMA).Methods Twenty patients with SIDSMA performed with MSCT angiography were analyzed retrospectively. The volume data were transferred to the workstation for image postprocessing and performed with various postprocessing techniques such as multi-planar reconstruction(MPR),curved planar reconstruction(CPR), volume rendering(VR)and maximum intensity projection(MIP).Results The dissection length of SIDSMA was positively correlated with visual analogue scale(VAS).MPR,CPR and VR clearly showed the intimal flap, true and false lumen.MPR and CPR displayed more entries of dissection than VR.VR demonstrated the extension of the involved aorta and its branches, but cannot satisfactorily demonstrate the entry of SIDSMA.Conclusion MSCT angiography is of positive applicable value to the diagnosis of SIDSMA.MPR, CPR and VR may be optimal choice as post processing techniques for diagnosis of SIDSMA.
