1.Clinical Research of CO2 Laser Resection of the Paraglottic Space and Arytenoid for the Treatment of Bilateral Vocal Cord Paralysis

[Summary] This paper reported 10 patients with bilateral vocal cord paralysis from March 2013 to October 2015.The paraglottic space and arytenoid were resected with CO 2 laser.The endotracheal intubation was removed at 3 months after surgery .The patients were followed up for 1-2 years.No dyspnea or eating difficulty was seen .Patient’ s voice was normal.The cavity mucous membrane was smooth .No complications such as granulation tissue growth occurred .
2.Expression of EMS1 protein and its clinical significance in laryngeal carcinoma

Objective: To investigate the expression of EMS1 in laryngeal carcinoma and its clinical signifi-cance. Method:The expression of EMS1 protein was measured in 40 samples of, 40 samples of para carcinoma tis-sues (which were near to cutting margin of laryngeal carcinoma tissuse over 0. 5 cm) ,and 20 samples of normal la-ryngeal mucosa as controls by Flow Cytometere( Epics-XL Ⅱ ). Results:The quantity and percentage of EMS1 pro-tein expression in laryngeal carcinoma tissues was significantly higher than those in para carcinoma and in normal laryngeal mucosa tissues respectively(P<0. 05). There was no significant expression difference between the para carcinoma tissues and normal laryngeal mucosa tissues. There were positive correlation between the expressions of EMS1 protein and metastasis, pathological grade and clinical stage in laryngeal carcinoma. But there were not rela-tionship with patients' clinical classification, tumor size, smoking history, age and sex. Conclusion: The high ex-pression of EMS1 may contribute to the carcinogenesis and development of laryngeal carcinoma. The expression of EMS1 protein is an important index of judging differentiation, infiltration, metastasis and staging of laryngeal car-cinoma.
3.Research Progress in She Medicine Fresh Herb Tea

Objective:To investigate the biological characteristics, wild resources distribution, chemical constituents, pharmacologi-cal effects and clinical application of She medicine fresh herb tea. Methods: The related literatures on fresh herb tea in CNKI, CBM, Wanfang and Vip database from 1989 to 2014 were sunmmrized and analyzed. Results:Fresh herb tea showed narrow distribution, and mainly contained volatile oils, alkaloids and flavonoids etc with the effects of antioxidant, immune regulation, antibiosis and so on. Con-clusion:The researches on the chemical composition of fresh herb tea provide reliable basis for the separation and extraction of specific chemical components, which also provide reliable support for the development of health products and other products containing fresh herb tea.

Infectious disease hospitals are obliged to cope with public health emergencies such as outbreak of infectious diseases.Such hospitals are required to make early detection,early containment,proper management,reasonable preplans,timely training,targeted drills,reasonable deployment of hospital resources,appropriate protection for hospital staff.General hospitals should also cope with prevention and control of infectious diseases to some extent,and work with infectious disease hospitals hand in hand to better cope with such outbreaks.
5.Expression and purification of JC virus VP2 fusion protein and preparation of its polyclonal antibody

Objective To obtain the antigen and antibody of JC virus(JCV)VP2.Methods The JCV VP2 gene were amplified from a cerebrospinal fluid sample by polymerase chain reaction (PCR)and confirmed by sequencing.Then,the gene was cloned into plasmid pET32a(+)to construct recombinant prokaryotic expression vector pET-32a(+)-VP2.The recombinant plasmid was transformed into the competent E.coli BL21.Induced with isopropyl-β-D-1-1 thiogalactopyranoside (IPTG),E.coli BL21 were subsequently crushed by ultrasound.The gene expression in the supernatant was analyzed by Western blot.Thereafter,the expressed protein was purified by isoeleetric point method.The polyclonal antibody against JCV VP2 protein was obtained from the BALB/c mouse immunized with the purified protein.Results The VP2 fusion protein was expressed in the E.coli BL21.The recombinant fusion protein was expressed by IPTG induetion with relative molecular mass of 58.5×10~3.Sodium dodecyl sulphate-polyacrylamide gel electrophoresis(SDSPAGE)analysis showed that the expression level was highter after 6-10 h of IPTG induction.The recombinant protein had good antigenicity which was confirmed by BALB/c mice immunized with the protein.Conclusions The successful expression and purification of VP2 fusion protein and the antibody will be valuable for the study on the biological function of VP2 and JCV epidemiologieal investigation.
6.Analysis of clinical factors for the efficacy of TPF in treating hypopharyngeal carcinoma.

OBJECTIVE:
To summarize the clinical effect of TPF regimen in the treatment of hypopharyngeal carcinoma and explore various clinical factors affecting treatment efficacy.
METHOD:
The clinical data of 20 cases with hypopharyngeal carcinoma, who received TPF treatment, were analyzed retrospectively. After two courses of chemotherapy, based on radiographic outcomes, next treatment plan was developed. To sum up the clinical information, including the clinical type, patterns of tumor growth, pathologic type, tumor stage, lymph node metastasis, age and so on. To analyze possible influencing factors affecting curative effect.
RESULT:
(1) After 20 cases with hypopharyngeal carcinoma received two courses of TPF treatment, the effect was evaluated. Objective response rate was 65%. (2) In patients with hypopharyngeal carcinoma, the efficacy of TPF therapy was significantly related to the clinical type, patterns of tumor growth and pathologic type; there was no statistical significance in tumor stage, lymph node metastasis and age.
CONCLUSION
According to the clinical type, patterns of tumor growth and pathologic type of hypopharyngeal carcinoma, resistance to chemotherapy in hypopharyngeal carcinoma can be assessed, which provides important basis for designing individualized treatment plan.

7.Clinical study of 15 children with hand foot and month disease and acute flaccid paralysis

Objective To discuss the clinical characteristics and prognosis of 15 children with hand foot and mouth disease (HFMD) and acute flaccid paralysis (AFP) who were admitted to Beijing Ditan Hospital during the outbreak of HFMD in 2008. Method The epidemiology, clinical manifestations, cerebrospinal fluid (CSF),magnetic resonance imaging and prognosis of 15 children with HFMD and AFP were retrospectively reviewed. The recovery of the patients' affected extremities were monitored for 4 weeks. Results The mean age of these patients was (22.47 ± 20.68) months (range: 5~72 months). Acute paralysis developed (3.47 ± 1.68) days after the onset of fever and progressed to maximum severity within (1~2) days. Poliomyelitis-like syndrome was observed in all cases. Of the 15 cases, 10 had monoplegia of lower limbs, two had paraplegia, one had monoplegia of upper limbs and two had quadriplegia. In these cases, the muscle power varied from level 0 to level 4, and six even showed no muscle power in their affected extremities. Thirteen cases developed neurologic complications (encephalitis, meningitis or ataxia) and three had transient urinary retention. Cerebrospinal MRI examination in eight cases showed hyperintense lesions on T2-weighted images, predominantly in the impaired anterior horn regions of the spinal cord (C2~C7 for cases with upper extremity impairments and T12~L1 for cases with lower extremity impairments), and displayed long T1 signals and long T2 signals. In addition, the midbrain, brain-stem or medulla was also involved in four cases who also contracted encephalitis or meningitis. The muscle strength in 11 patients with single lower extremity impairment showed improvements in the distal limb muscles within 4~8 days, and the other cases showed recovery 2~3 weeks later. Conclusions HFMD in combination with AFP most commonly occurs in children aged less than 2 years old. Acute paralysis develops during the early stage of infection and progresses to a maximum severity within 2 days. In most cases described here, paralysis occurred in a single lower extremity and recovered more rapidly than those with all four limbs affected or with single upper extremity impairment . MRI examination is particularly valuable for the diagnosis and prognosis of AFP because of its high sensitivity and accuracy.
8.Expression of DcR3 protein and its significance in laryngeal carcinoma.

OBJECTIVE:
To investigate the expression of DcR3 in laryngeal carcinoma and analyze the relation between DcR3 and clinical factors.
METHOD:
The expression of DcR3 protein in 41 laryngeal carcinoma tissues and 41 para-carcinoma tissues (to cutting margin > 0.5 cm) were measured by Flow Cytometer(Epics-XL II), 15 normal laryngeal mucosa tissues were served as controls.
RESULT:
(1) The quantitative and qualitative expression of DcR3 protein in laryngeal carcinoma tissues was obviously higher than those in para-carcinoma and normal laryngeal mucosa tissues, respectively (P < 0.05). There was no significant difference for the DcR3 protein expression between para-carcinoma and normal laryngeal mucosa tissues. (2) In laryngeal carcinoma, the expression of DcR3 protein was not significantly related to clinical classification, tumor size, smoking history, patients' age and sex but related to metastasis, pathological grade and clinical stage.
CONCLUSION
The high level of DcR3 expression may contribute to the carcinogenesis and development of laryngeal carcinoma. So it can be an important index for judging the differentiation, infiltration, metastasis and staging of laryngeal carcinoma.

(2) In laryngeal carcinoma, the expression of EMS1 and DcR3 protein was independent of patients' clinical classification, tumor size, smoking history, patients' age and sex but associated with tumor metastasis, pathological grade and clinical stage. (3) In laryngeal carcinoma, the expression of EMS1 was positively correlated with that of DcR3.
CONCLUSION
EMS1 was positively related to DcR3, which might play an important role in the carcinogenesis and development of laryngeal carcinoma by synergic effect.
