1.Cause analysis and preventive measures for blood sample errors

Objective To study the reason and preventive measures of inconformity of the extraction appeared in the process of transfusion blood specimens with the patient's blood type.Methods The reasons of transfusion errors why extracting required blood type was not consistent with the patient's blood type examplesa in Zhengzhou transfusion of hospital from 2008 to 2012 were retrospectively analyzed.Results The experimental results showed that blood specimen inconformity was 49.60%,the error rate of blood extraction and blood infusion was 31.20%,infusion error only was 15.20%,blood type change after stem cell transfusion was 4.00%.Reasons of blood type change after stem cell transplantation to extract blood samples mainly included the blood center or blood did not accord with logo type blood stations provided (16.13%),blood use application form to fill in error(20.97%),check the wrong blood type (6.45%),blood samples taken(29.03%),the blood sample tag(27.42%).Conclusion In order to ensure the safety for clinical use must adopt measures to prevent resolutely put an end to a blood transfusion errors.
2.A clinical study of piezosurgery osteotomy in lateral sinus floor augmentation

Objective:To evaluate the clinical effect and safety of piezoelectric osteotomy in lateral sinus floor augmentation.Meth-ods:53 implants were placed in 30 sites after lateral sinus floor augmentation(LSFA)in 29 patients by piezoelectric osteotomy.47 implants were placed simultaneously with LSFA,6 were delayed.6-8 months after implantation,the final fixed prostheses were per-formed.The patients were followed-up at 3,6,and 12th months after restoration.Results:The mean residual bone height(RBH) was 4.46 mm before operation.The mean height of sinus elevation was 7.34 mm after operation.No membrane perforation occurred. Peri-implantitis,peri-implant mucositis and implant mobility were not found during follow-up.X-ray examination showed good os-seointegration in the interface between the implants and bones.The implant survival rate and the operation success rate were 100%. Conclusion:The application of piezosurgery is safe and reliable in lateral sinus floor augmentation.
3.Experimental investigation of the measurement accuracy of mitral annulus by real-time three-dimensional echocardiography

Objective To validate the accuracy of real-time three-dimensional echocardiography(RT3DE) in measuring spatial geometric parameters of mitral annulus.Methods Hearts in vivo of nine healthy hybrid canines were examined by RT 3DE.Three-dimensional full-volume images of mitral annuli were acquired.The annuli were annotated and measured by TomTec 4D MV-assessment software and MATLAB.The distance between anterolateral and posteromedial commissure (commissure-commissure, CC),septolateral distance (septal-lateral, SL) and height (H) of mitral annulus were measured.Parameters correlation coefficients of CC,SL and H derived by two methods were 0.83( P<0.01),0.78( P<0.05)well consistent with anatomic measurements, the mean differences of CC, SL and H between two methods were 0.22 cm,0.12 cm and-0.08 cm respectively.Conclusions RT-3DE is feasible in measuring mitral annulus, which provided a convenient, accurate and reliable new method for quantitative assessing annulus geometry configuration.
4.Efficacy of blood purification therapy on diuretic resistance in patients withnephrotic edema

Objective To investigate the efficacy and safety of blood purification therapy in the treatment of diuretic resistance in patients with nephrotic edema.Methods 40 cases of primary nephrotic syndrome with diuretic resistance in our hospital were randomly divided into hemodialysis (HD) group and continuous blood purification (CVVH) group and each group were 20 cases.All the patients were given basic treatment (lipid modulation,anticoagulation and glucocorticoid) and the observation time was 4 weeks.The changes of biochemical indexes (24 h urine protein,plasma albumin,serum urea nitrogen and serum creatinine) were observed before and after treatment in two groups.In addition,the complications of four groups were recorded.Results After blood purification treatment,urinary volume of two groups were significantly increased,the quantity of 24 h urinary protein and serum albumin were significantly improved,levels of serum urea nitrogen and serum creatinine were significantly decreased.The differences were statistically significant (P < 0.05 or P < 0.01).Compared with HD group,the urine volume increased more obviously in CVYH group after 2 weeks' treatment (P < 0.05).The same time,24 h urinary protein was reduced and plasma albumin was improved more significantly (P < 0.05),levcls of serum urea nitrogen and serum creatinine were not differ between the two groups (P > 0.05).There was no difference in urine volume and renal function between the two groups (P > 0.05) after 4 weeks' treatment.24 h urinary protein was significantly decreased and serum albumin was significantly higher than HD group (P < 0.05).There were no serious complications in the two groups.Conclusion Blood purification in the treatment of diuretic resistance in patients with nephrotic edema is safe and effective.Continuous blood purification treatment can effectively alleviate nephrotic edema with diuretic resistance,promote the recovcry of renal function and improve the prognosis of kidney disease.
5.Effects of Kuntai Capsule and hormone replacement therapy on cognitive function and mental symptoms of early postmenopausal women: a randomized controlled trial.

Background: Conjugated equine estrogen (CEE) treatment, a hormone replacement therapy, is restricted for use in perimenopausal and postmenopausal women because of security issues. Consequently, traditional Chinese herbal medicine has become an alternative choice for the patients with contraindications to hormone replacement therapy. Objective: To evaluate the efficacy and safety of Kuntai Capsule and CEE in treating cognitive function disorder and mental symptoms of early postmenopausal women. Design, setting, participants and interventions: A total of 57 cases of early postmenopausal women from Outpatient Department of West China Women and Children's Hospital were included. The subjects were randomly divided into two groups: Kuntai group with 28 cases and CEE group with 29 cases. The patients in Kuntai group received 6 g Kuntai capsules three times a day. The patients in CEE group received CEE 0.3 mg and 0.6 mg alternately once a day (average dose of 0.45 mg/d). The patients with intact uterus in CEE group were treated with 2 mg medroxyprogesterone acetate daily. Main outcome measures: In one-year treatment course, the recognition function and mental symptoms of each patient were investigated by questionnaires of Mini-Mental State Examination (MMSE), Kupperman, and quality of life (QOL) every three months. Both intention-to-treat (ITT) and per-protocol set (PPS) analyses were done. Results: The MMSE, Kupperman index and QOL scores at each time point were improved as compared with those before treatment (P<0.05), however there were no statistical differences between the two groups (P>0.05). The MMSE scores showed a tendency to escalate while mental symptoms investigated by Kupperman index and QOL scale showed a downtrend. No severe adverse effects occurred in the study phase and no statistical difference in incidence of side effects between the two groups was found except for vaginal bleeding. The incidence rates of vaginal bleeding in CEE and Kuntai groups were 39.3% and 11.1% respectively (P=0.029). Conclusion: Both Kuntai Capsule and CEE may contribute to maintain the cognitive function and ameliorate mental symptoms of early postmenopausal women.
6.A preliminary investigation of relations of fat mass and leptin to the bone mineral density in ovariectomic rats.

The present paper is aimed to investigate the correlation between the fat mass and the bone mass, and to investigate the effect of leptin on bone metabolism in female ovariectomic rats. Forty Wistar female rats, 6-month-old, were randomly divided into the sham surgical (Sham) group and ovariectomic (OVX) group. The body weight, the fat mass inside the abdominal cavity, the bone mineral density (BMD) were determined by DEXA method, and the concentration of leptin was measured by ELISA after operation. The Rats' body mass in ovariectomized group increased significantly (P < 0.05), while the fat mass in abdominal cavity was not increased obviously in ovariectomized group (P = 0.499). The concentration of leptin was no difference between the two groups (P = 0.166). Bone mineral content (BMC)/weight in ovariectomized group was decreased significantly than the sham group (P = 0. 003). The weight at 8th week post-operation was negative correlation with BMC/weight in Sham group and was positive with BMD in OVX group. The fat mass and the leptin concentration were negative correlation with BMC/ weight in Sham group. It was concluded that the fat mass and the leptin were raleted to the BMC/weight.

7.On the relationship between serum total adiponectin and insulin resistance in polycystic ovary syndrome.

It is the intent of this investigation to gain an insight into the relationship of serum total adiponectin with polycystic ovary syndrome (PCOS) and insulin resistance. Fifty-eight PCOS patients were enrolled (29 with high serum insulin level and 29 without), at the same time, 29 non-PCOS women with normal weight were included as control. The influencing factors of total adiponectin, PCOS and insulin resistance were analyzed. The serum total adiponectin of PCOS patients and all participants were found to be negatively related to waist hip ratio (r = -0.39, r = -0.36) and InHOMA-IR (r = -0.53, r = -0.45), respectively. Adiponectin was not a protective factor of PCOS (P > 0.1), but it was that of PCOS-insulin resistance (OR = 0.81; 95% CI: 0.67-0.97; P = 0.02). LH/FSH (OR = 1.51; 95% CI: 1.16-1.96; P = 0.01) and InHOMA-IR (OR = 1.26; 95% CI: 1.10-1.44; P = 0.01) were risk factors of PCOS, and waist hip ratio was that of PCOS-insulin resistance (OR = 8.57; 95% CI, 2.14-34.30, P = 0.01). Adiponectin might influence fasting insulin and InHOMA-IR (B = -0.22, P = 0.001; B = -0.02, P = 0.002). These data signify that adiponectin is not directly related with PCOS, but it is related with PCOS-HL Adiponectin might participate in the pathophysiologic mechanism of PCOS by influencing insulin sensitivity.

8.Correlation analysis between serum free testosterone and total testosterone in Chengdu females.

This paper is aimed to analyze the correlation between serum free testosterone (FT) and total testosterone (TT) to acquire a cutoff about using total testosterone to diagnose hyperandrogenism in Chengdu females. We investigated 1854 women by cluster sampling method, detected their serum FT levels and TT levels, scored relative items, analyzed the correlation and made the ROC curve to get a cutoff of TT levels. Serum FT had a linear correlation with serum TT (r = 0.597, r2 = 0.356, P < 0.001). The cutoff value was 0.635 ng/mL. The specificity and sensitivity were 76.3% and 77.24%, respectively. No correlation found between serum FT and Ferriman-Gallway Score (P = 0.392). Positive correlations were seen between serum FT and Plewig-Kligman Score (r = 0.137, P < 0.001), serum TT and Ferriman-Gallway Score (r = 0.069, P = 0.003) and serum TT and Plewig-Kligman Score (r = 0.092, P < 0.001). There is a linear correlation between serum FT and TT. We can diagnose hyperandrogenism according to the serum TT cutoff value (0.635 ng/mL). Its clinical symptoms are not paralleled with the biochemical test results.

9.The Effect of Nitrous Oxide and Isoflurane on the Total RNA Yield from the Cochlea of the Rats

The possible mechanism of inhalation anesthetics on the internal auditory impairment of the rat was investigated by determining the effect of nitrous oxide (N20) and isoflurane on the total RNA yield from the cochlea of the rats. Thirty healthy Wistar rats were randomly divided into 3 groups: group C (control group, n=10) with a 3-h unremitting inhalation of 50% O2 group N (ex-periment group, n= 10) with a continuous inhalation of 50% N2O+50% O2for 3 h, and group I (ex-periment group, n=10) with a 3-h sustained inhalation of 2.5% isoflurane. The TRIzol in combination with RNeasy was used to respectively extract the total RNA from cochlea of rats in the 3 groups. Spectrophotometry was used to detect total RNA yield and electrophoresis to detect the quality. The total RNA extracted from the cochlea of the rats in the groups C and N was 7.69 and 6.51 μg, respec- tively. There was a 15% decrease in the N group as compared with group C. The total RNA from the rats in the group I was 7.32 μg, and there was hardly any change in the group as compared with the group C. The value of A260/A280 in groups C, N and I was 2.07, 2.04 and 2.04, respectively, showing a very high RNA purity. The result of gel electrophoresis suggested that there was no degradation in the total RNA. It was suggested that the interference of N2O on the cochlear RNA yield might be one of the reasons which cause an injury of the ear. The isoflurane shows no harm on the heating.
10.Epidemiologic characteristics and a prognostic nomogram for patients with vulvar cancer: results from the Surveillance, Epidemiology, and End Results (SEER) program in the United States, 1975 to 2016

Objective:
To elucidate clinical characteristics and build a prognostic nomogram for patients with vulvar cancer.
Methods:
The study population was drawn from the Surveillance, Epidemiology, and End Results (SEER) database. Patients were randomly assigned to training and validation sets. Cox proportional hazards model and competing risk model were used to identify the prognostic parameters of overall survival (OS) and cancer-specific survival (CSS) to construct a nomogram. The nomogram was assessed by concordance index (C-index), area under the curve (AUC), calibration plot, and decision curve analysis (DCA).
Results:
A total of 20,716 patients were included in epidemiological analysis, of whom 7,025 patients were selected in survival analysis, including 4,215 and 2,810 in training and validation sets, respectively. The multivariate Cox model showed that the predictors for OS were age, marital status, histopathology, differentiation and tumor node metastasis (TNM) stages, whether to undergo surgery and chemotherapy. However, the predictors for CSS were age, race, differentiation and TNM stages, whether to undergo surgery and radiation. The C-index for OS and CSS in the training set were 0.76 and 0.80. The AUC in the training set for 1-, 3- and 5-year OS and CSS were 0.84, 0.81, 0.80 and 0.88, 0.85, 0.83, respectively, which was similar in the validation set. The calibration curves showed good agreement between prediction and actual observations. DCA revealed that the nomogram had a better discrimination than TNM stages.
Conclusions
The nomogram showed accurate prognostic prediction in OS and CSS for vulvar cancer, which could provide guidance to clinical practice.
