BACKGROUND: The Topping-off technique, which combines lumbar fusion with the dynamic internal fixation system (Coflex), can not only reduce the pressure, but also protect the adjacent segments. There is no relevant mechanical analysis performed on the rationality of the application of Topping-off technique to young patients with the need for fusion on the lumbosacral region and adjacent degenerated segments. OBJECTIVE: To establish a finite element model of Topping-off surgery on the lumbosacral junction and to analyze the biomechanical changes of the adjacent segments and the range of motion trend of the lumbar spine. METHODS: A healthy young male volunteer with no previous history of low back pain or congenital malformations was randomly selected for thin-slice CT scanning after signed the informed consent. The image information was imported into the computer and the whole lumbar spine model as the healthy group model was established by analyzing the image information through Mimics, Geomagic Studio 12.0, HyperMesh and Abaqus successively. After verifying the effectiveness of the model, the moderate degeneration model of intervertebral disc was established by changing the material properties of L4-s1 discs on the basis of the healthy model, and the fusion model and Topping-off model were respectively established on the basis of the degeneration model. After applying 400 N compressive load and 10 N-m momentum to the four groups of models, the variation trends of range of motion from L2 to L5 and the stress changes of L4/L5 intervertebral disc, nucleus pulposus and facet joints were calculated respectively. RESULTS AND CONCLUSION: (1) Compared with the degeneration model, the lumbar range of motion of Topping-off model and fusion model decreased, and the Topping-off model decreased more significantly than the fusion model. (2) The range of motion of fusion model L4-L5 increased significantly and the range of motion of L2/L3 and L3/L4 segments did not change significantly. Compared with the degeneration model, the L4-L5 range of motion of Topping-off model decreased, and range of motion of the L2/L3 and L3/L4 levels increased to some extent in the flexion and extension positions. (3) Compared with the degeneration model, the stress on the disc, nucleus pulposus and facet joint of the fusion model L4-L5 increased in four positions of flexion, extension, rotation and bending, while the fiber stress on the Topping-off model decreased significantly in all four positions. (4) These results suggest that Topping-off technology can not only reduce the stress on the upper adjacent degenerative intervertebral disc, nucleus pulposus and facet joints, but also reduce the hyperactivity of the adjacent segments and increase the range of motion of other upper segments, thereby compensating the lumbar spine mobility and delaying the degeneration of upper adjacent segments.

A novel solid-phase extraction (SPE) adsorbent for simultaneous extraction of atrazine (ATZ) and its metabolites, deisopropylatrazine (DIA) and deethylatrazine (DEA) from environmental water samples was prepared. Polyacrylonitrile nanofibers (PAN NFs) mat was prepared via electrospinning, and was further functionalized to obtain polypyrrole modified polyacrylonitrile nanofibers (PPy-PAN NFs) mat, hydrazine modified polyacrylonitrile nanofibers (NH2-PAN NFs) mat and carboxyl modified polyacrylonitrile (COOH-PAN NFs) mat. The results showed that the adsorption capacity of COOH-PAN NFs mat was better than other three NFs mats in both static (2.0 mg/g) and dynamic (0.19 mg/g) experiments. Meanwhile, the runoff ratios of COOH-PAN NFs mat were the lowest (less than 30.0％) in the adsorption of three analytes, especially for high polar analytes, which showed that the hydrogen bond between carboxyl groups and analytes was the main interactive force. A combination of mat-based SPE and high performance liquid chromatography-diode array detection was further established for determination of 3 analytes in environmental water samples. The recoveries were 81.4％-120.3％ and the limits of detection were 0.12 ng/mL for DIA, 0.09 ng/mL for DEA and ATZ, respectively.

This study aimed at investigating the inhibitory effects and the anti-tumor mechanisms of co-adminis-tration of fusion proteins mGM-CSF/βhCG ( GC ) and hVEGF121/βhCG ( VC ) on RM-1 prostatic cancer and B16 F10 melanoma in the C57 BL/6 J mouse model. Two recombinant stains containing pET-28 a-mGM-CSF-X10-βhCGCTP37 and pET-28 a-VEGF-M2-X10-βhCG-CTP37 were induced by lactose to express fusion proteins. The fusion proteins were separated and purified to prepare the anti-tumor protein vaccines ( VC protein vaccine and GC protein vaccine) , which were then mixed to prepare a combined protein vaccine named VGC protein vac-cine. The prostatic cancer and melanoma tumor-bearing mice C57 BL/6 J were immunized with described vac-cines, then the growth of each tumor was measured;splenocyte proliferation of immunized mice was detected;and the cytotoxic effects of the vaccine on tumor cells were tested. After that, the in vivo concentrations of IFN-γ and anti-hVEGF antibodies were investigated by ELISA. The difference between each experimental group and normal saline group ( NS) was statistically significant in both tumor-bearing mouse models ( P <0. 05) respectively. Besides, VGC group exhibited significantly better anti-tumor effect compared with the GC and VC groups with the anti-tumor rate ( 41. 7 ± 0. 83)% and ( 46. 4 ± 1. 27)% for prostatic cancer and melanoma tumor, respectively. The co-administration of the two proteins, VC and GC, could inhibit the growth of RM-1 prostatic tumor and B16F10 melanoma effectively via anti-tumor immunity and anti-tumor angiogenesis.

Objective To establish a porcine model of penetrating-suture pancreaticojejunostomy.Methods Ten domestic pigs were selected,and after general anesthesia they underwent laparotomy and the pancreas was visualized.Then the pancreas was dissected at the level of superior mesenteric vascular,and the proximal pancreatic stump was sewed up.The anastomosis between the distal pancreatic stump and the intestinal wall adopted penetrating-suture pancreaticojejunostomy; the digestive tract was reconstructed by Roux-en-Y.Results The anastomosis with penetrating-suture pancreaticojejunostomy was successful in ten domestic pigs.The mean pancreatic stump diameter was 2.5 cm,and the mean pancreatic duct diameter was 1.5 mm,the mean time for operation was 1.0 ～ 2.5 h,and the average time of pancreaticojejunostomy was 8 minutes.The mean blood loss was 25 ml.After operation,diarrhea occurred in 2 pigs and wound infection occurred in 1 pig,and all were cured with appropriate management.No pig died intra-operatively,and no pancreatic fistula or death occurred after operation.Conclusions A porcine model of penetrating-suture pancreaticojejunostomy is successfully established.

Objective To evaluate the efficacy of unilateral atlanto-axial transpedicle screw fixation plus iliac bone graft for treatment of unstable atlas fractures combined with unilateral pedicle dysplasia or comminuted fractures.Methods A retrospective case control study was made on 44 patients with unstable atlas fractures surgically treated between January 2012 to June 2016.Unilateral atlanto-axial transpedicle screw fixation combined with iliac bone graft was performed for 22 patients combined with unilateral pedicle dysplasia or comminuted fractures in Group A[15 males,seven females;(37.5 ± 13.4)years],and bilateral atlanto-axial transpedicle screw fixation combined with iliac bone graft was performed for 22 patients without unilateral pedicle dysplasia or comminuted fractures in Group B [14 males,eight females;(38.1 ± 13.3)years].Between-group differences were compared concerning operation time,intraoperative blood loss,length of hospital stay,success rate of screw placement,postoperative atlantoaxial stability,surgery-related complications,visual analog scale (VAS),Japanese orthopedic association score (JOA) and bone fusion.Results Mean duration of follow-up was 28.4 months (range,14-48 months).In Group A,operation time was (123.4 ± 18.2) min,blood loss was (218.5 ± 80.2) ml,hospital stay was (7.1 ± 1.0)d,success rate of screw placement was 100％,postoperative atlanto-axial stability of all patients was obtained,and no complications occurred.In Group B,operation time was (173.4 ± 12.4) min,blood loss was (318.2 ± 61.7) ml,hospital stay was (7.2 ± 0.8) d,success rate of screw placement was 100％,postoperative atlanto-axial stability of all patients was obtained,and no complications occurred.There were significant differences in operation time and blood loss between the two groups (P ＜0.01),while not in hospital stay,success rate,postoperative atlant-oaxial stability,complication incidence,VAS and JOA (P ＞ 0.05).Conclusion Both treatments are effective,but unilateral atlanto-axial transpedicle screw fixation combined with iliac bone graft is associated with relatively shorter operation time and less blood loss and hence is considered as a better choice for treatment of unstable atlas fractures.

Objective:To evaluate the application of a 3D printed positioner for hip rotation center in total hip arthroplasty.Methods:From August 2015 to December 2017, 14 patients were treated by unilateral total hip arthroplasty at Department Ⅰ of Joint Surgery, Orthopaedics Hospital of Zhengzhou for avascular necrosis of femoral head or femoral neck fracture. They were 8 males and 6 females with an average age of 51.8 years (from 37 to 65 years). All their surgeries were assisted by a 3D printed positioner for hip rotation center. Postoperatively, the abduction and anteversion of acetabular cup and the hip rotation center were measured, the agreement between the hip rotation center of the affected side (O2) and the anatomical rotation center of the healthy side (O1) was assessed, and the function of the affected hip and complications were recorded at the last follow-up.Results:The 14 patients were followed up for an average of 18 months (from 6 to 24 months). The postoperative ordinates of O2 and O1 were respectively 19.36 mm±3.61 mm and 18.33 mm±3.41 mm while their abscissas 39.93 mm±2.97 mm and 39.99 mm±3.16 mm, indicating agreement between O2 and O1 ( P>0.05). The postoperative abduction and anteversion of the cup were within a normal range (39.3°±3.2°and 14.6°±1.2°, respectively). The last follow-up showed that their preoperative Harris hip scores (42.3±3.2) were significantly improved (94.3±4.7) ( t=2.873, P=0.002) and that 13 cases were rated as excellent and one as good. X-ray follow-up showed good position of their acetabular prostheses, no loosening of the cups or femoral prostheses, and no heterotopic ossification. Conclusion:A 3D printed positioner for hip rotation center can effectively assist reconstruction of the hip rotation center and accurate implantation of acetabular prosthesis in total hip arthroplasty.

Objective:To evaluate the application of 3D printed osteotomy template in total knee arthroplasty (TKA) for valgus knee.Methods:A retrospective analysis was performed of the 21 patients with valgus knee who had undergone TKA at Department Ⅰ of Joint Surgery, Orthopaedics Hospital of Zhengzhou from March 2016 to February 2018. They were 5 males and 16 females, aged from 62 to 78 years (average, 66.7 years). TKA was performed with the assistance of a 3D printed personalized osteotomy template for each patient. The efficacy of the personalized osteotomy templates was evaluated by comparing the femorotibial angle, knee range of motion, Hospital for Special Surgery (HSS) knee scores of the patients between preoperation and postoperation.Results:The 21 patients were followed up for a mean duration of 16.3 months(range, from 10 months to 4 years). All incisions healed by the first intention. The last follow-ups showed that their preoperative femorotibial angle 17.6°±5.7° was improved significantly to 6.3°±1.2°, their preoperative knee range of motion 71.3°±5.8° increased significantly to 102.4°±7.5°, and their preoperative HSS scores 38.2±5.7 increased significantly to 87.5±3.5 ( P<0.001). Their radiographic evaluation at the last follow-up showed stable prosthetic components with no implant loosing or osteolysis. Conclusion:A personalized 3D printed osteotomy template can be used in TKA for valgus knee, leading to accurate osteotomy, effective correction of mechanical alignment and satisfactory clinical outcomes.

The construction of clinical ontology of traditional Chinese medicine (TCM) is one of the important components of TCM internationalization.Among them,the study of clinical term entity has been quite successful.But the research on semantic relation is still lacking.This paper presented a method based on the combination of clustering and syntax pattern to study the semantic relations between TCM conceptual entities.By extracting the feature words around the entity,K-means was used as the clustering algorithm to perform the first round of clustering for all corpora.Based on results of the first round of clustering,the longest common subsequence was extracted in the same cluster and generalized as syntax pattern.According to the sentence after manual adjustment,it was automatically judged that each sentence in the corpus has the most suitable syntax pattern of semantic relations,and the second round of clustering is characterized by the syntax pattern.The result was the final clustering result.The experimental results showed that the accuracy of this method was 88.23％ for the classification of semantic relations in corpus.

Objective:To explore the relationship between serum urea nitrogen to creatinine ratio (UCR) and cardiac function in elderly patients with severe pneumonia.Methods:A prospective selection of 100 elderly patients with severe pneumonia admitted to Group Wanbei General Hospital of Wanbei Coal Power from May 2020 to April 2023 was conducted as the case group, and an additional 50 patients who underwent health examinations in the hospital during the same period were selected as the control group. Serum urea nitrogen (BUN), creatinine (Cr), UCR value, myocardial injury indexes: cardiac troponin I (cTnI), N-terminal pro-brain natriuretic peptide (NT-ProBNP), creatine kinase (CK), creatine kinase isoenzyme MB (CK-MB), and cardiac function indexes: left ventricular ejection fraction (LVEF), cardiac output (CO), stroke volume (SV) and cardiac output index (CI) were detected in the two groups. The patients in the case group were divided into two subgroups based on the occurrence of heart failure during hospitalization: the heart failure subgroup and the normal heart function subgroup. The serum UCR, myocardial injury and cardiac function indexes were compared between the case group and the control group. The general data, myocardial injury and cardiac function indexes were compared between the heart failure group and the normal heart function group. The relationship between serum UCR and cardiac function in patients with severe pneumonia was analyzed by bivariate Pearson correlation. Logistic regression model was used to analyze the influencing factors of cardiac dysfunction in patients with severe pneumonia, and using receiver operating characteristic (ROC) curve to analyze the value of serum UCR in predicting cardiac dysfunction in patients with severe pneumonia.Results:The values of BUN, UCR, cTnI, NT-ProBNP, CK and CK-MB in the case group were higher than those in the control group: (8.72 ± 1.14) μmol/L vs. (6.41 ± 0.76) μmol/L, 125.00 ± 19.75 vs. 86.12 ± 12.02, 0.04 (0.03, 0.05) μg/L vs. 0.04 (0.03, 0.05) μg/L, (185.49 ± 20.59) ng/L vs. (147.76 ± 20.85) ng/L, (104.78 ± 14.98) U/L vs. (99.33 ± 15.07) U/L, (31.59 ± 6.23) U/L vs. (29.13 ± 5.76) U/L. The values of Cr, LVEF, CO, SV and CI in the observation group were lower than those in the control group: (70.22 ± 5.76) μmol/L vs. (74.75 ± 5.12) μmol/L, (59.72 ± 2.41)% vs. (61.78 ± 2.16)%, (3.93 ± 0.43) L/min vs. (4.53 ± 0.62) L/min, (59.82 ± 6.12) ml vs. (62.23 ± 7.22) ml, (2.95 ± 0.30) L/(min·m 2) vs. (3.06 ± 0.33) L/(min·m 2), with a statistical significant difference ( P<0.05). In the case group, 21 patients had heart failure during hospitalization, accounting for 21.00%. The pneumonia severity index score on admission in the heart failure subgroup was higher than that in the normal heart function subgroup: (172.76 ± 9.18) points vs. (168.24 ± 8.81) points. The serum BUN and UCR levels were higher than those in the normal heart function subgroup: (9.51 ± 0.79) mmol/L vs. (8.51 ± 1.13) mmol/L, 141.62 ± 9.89 vs. 120.59 ± 19.39. The serum Cr level was lower than that in the normal heart function subgroup: (67.26 ± 5.34) μmol/L vs. (71.00 ± 5.65) μmol/L, with a significant statistical difference ( P<0.05). There was no significant statistical difference in other data between the two subgroups ( P>0.05). The bivariate Pearson correlation analysis showed that serum UCR was positively correlated with cardiac function indicators cTnI, NT-ProBNP, CK and CK-MB levels in severe pneumonia ( r = 0.40, 0.27, 0.32 and 0.33; P<0.05), and negatively correlated with LVEF, CO, SV and CI levels ( r = - 0.37, - 0.21, - 0.25 and - 0.21; P<0.05). Univariate and multivariate Logistic regression analysis showed that the occurrence of cardiac dysfunction in elderly patients with severe pneumonia may be related to the pneumonia severity index score on admission and the abnormal expression of serum BUN, UCR and Cr levels ( P<0.05). ROC curve analysis found that the area under the curve for predicting the occurrence of cardiac dysfunction during hospitalization in severe pneumonia patients with serum UCR on admission was 0.85 (95% CI 0.77 to 0.92), which had certain predictive value. Conclusions:Elderly patients with severe pneumonia are accompanied by a certain degree of elevated serum UCR levels, and the higher the serum UCR level, the more severe the cardiac function damage and the greater the risk of cardiac dysfunction.

Objective:To investigate the efficacy of two-way needle suture technique (TNST) in the minimally invasive repair of acute closed Achilles tendon rupture.Methods:From June 2019 to June 2021, 26 patients with acute closed Achilles tendon rupture were treated at Zhengzhou Orthopedic Hospital. They were 20 males and 6 females, with a mean age of 28 (23, 31) years. The rupture end was (4.2±1.3) cm away from the calcaneal insertion, and the interval from injury to operation 4.3 (2.0, 5.0) d. Preoperative MRI examinations revealed in all the patients closed Achilles tendon rupture which was to be repaired by TNST. The operation time, incision length, incidence of complications, ankle dorsiflexion and plantar flexion were recorded. The Arner-Lindholm scoring was used to evaluate the clinical efficacy.Results:The operation time was (20.0±5.0) min and the incision length (2.5±0.4) cm. Postoperatively, all incisions healed by the first stage, with no complications like incision infection, skin edge necrosis, deep vein thrombosis at lower limbs, injury to the sural nerve, or re-rupture of the Achilles tendon. All patients were followed up for (12.0±6.0) months. At the last follow-up, the patients walked normally, their incisions healed well, the continuity of the Achilles tendon was good by palpation, their heel lift was strong, and all their activities were restored to the levels before rupture of the Achilles tendon. The ankle dorsiflexion was 22.6°±3.7° and the plantar flexion 25.3°±3.7°, According to the Arner-Lindholm evaluation, the clinical efficacy was rated as excellent in 25 cases and as good in 1 case, giving an excellent and good rate of 100% (26/26).Conclusion:In the minimally invasive repair of acute closed Achilles tendon rupture, TNST shows the advantages of limited surgical invasion, a low incidence of postoperative complications, and reliable curative effects.
