1.The diagnosis and surgical treatment of hepatolithiasis associated with cholangiocarcinoma

Objective To explore into the relationship between the hepatolithiasis and the intrahepatic cholangiocarcinoma, and summarize the experience of early diagnosis and surgical treatment of these disease. Methods A retrospective clinical analysis was made in 28 cases of hepatolithiasis complicated by intrahepatic cholangiocarcinoma from September 1998 to December 2008. Results It was found that the incidence of cholangiocarcinoma in hepatolithiasis was 5.9%. The correct rate of preoperative ultrasonic diagnosis was 57.15% ( 16/28 ) ,and that of CT diagnosis was 46.43 % ( 13/28), and that of MRCP diagnosis was 40.00% (6/15). 12 cases were radically resected,12 cases were treated by palliative therapy, and 4 cases were examined only with biopsy, and all cases were followed up. The 1-,2-,and 3-year survival rate in radical surgery were 83.34% ,58.34% ,25.00% respectively. And the 1-year,and 2-year survival rate in palliative surgery group were 66.67% ,8.33% respectively. And all the cases examined only with biosy died in 6 months after the biosy. Conclusion Cholangiocarcinoma is related to hepatolithiasis. In patients of hepatolithiasis who were older than 40 years and have a long history of recurrent cholangitis, weight-loss in a short period, progressive jaundice, or intractable abdominal pain,the possibility of accompanying cholangiocarcinoma should be considered. The key of improving the therapeutic effectiveness was early diagnosis ,early treatment and striving for radical operation.
2.Application of perioperative enteral nutrition support in patients with gastrointestinal cancer

Objective To study the application method and effect of perioperative enteral nutrition support in patients with gastrointestinal cancer.Methods 82 patients with gastrointestinal tumor were randomly divided into two groups.41 cases in the control group were given intravenous nutrition.41 patients in the study group were given enteral nutrition support 7d before operation,outage on the day of surgery,postoperation consistently given enteral nutrition support for 7d.The preoperative nutritional status,immune index,postoperative complications,the length of time and hospitalization expenses were compared between the two groups.Results Preoperative 1d,7d,the ALB,TRF,TP, PAB levels in the study group had no statistically significant differences compared with admission (all P >0.05).7d after operation,the TRF,PAB of the study group were (1.71 ±0.17)g/L,(174.67 ±22.81)mg/L,which of the con-trol group were (1.46 ±1.46)g/L,(142.73 ±27.32)mg/L,the differences were statistically significant (t =2.662, 2.917,all P <0.05).3d,7d after operation,the CRP,IL -6 and TNF - levels in the two groups increased signifi-cantly,compared with preoperation the differences were statistically significant (P <0.05).3d,7d after operation,the team CRP levels of the study group were (121.5 ±15.6)mg/L,(55.17 ±7.62)mg/L respectively,IL -6 levels were (81.42 ±13.56)pg/L,(56.14 ±9.73)pg/L and TNF -levels were (67.25 ±17.23)pg/L,(63.23 ±11.80)pg/L;In the control group,3d,7d after operation,CRP levels were (161.3 ±20.7)mg/L,(99.26 ±20.74)mg/L,IL -6 levels were (103.62 ±21.54)pg/L,(77.43 ±18.26)pg/L,the TNF -levels were (79.01 ±16.9)pg/L,(68.25 ± 12.48)pg/L,the differences between the two groups were statistically significant (P <0.05).The incidence rate of complications of the study group was 2.44%,which of the control group was 12.20% (χ2 =2.717,P <0.05 ). Conclusion Gastrointestinal tumor patients with malnutrition,perioperative enteral nutrition support can promote the synthesis of protein metabolism,relieve excessive postoperative systemic inflammatory reaction,reduce the risk of surgery.
3.Improvement of memory in healthy elderly subjects by Shentai Tea Polyphenols

Objective:To investigate the effect of Shentai Tea Polyphenols on memory and its underlying mechanism in healthy elderly adults.Methods:According to the randomized, double blind and placebo-controlled prospective experimental design, 240 healthy middle-aged or elderly subjects with normal objective cognition were recruited in our hospital from April to December 2019 through advertising, and they were randomly divided into an experimental group ( n=120) and a control group ( n=120). Oral administration of Shentai Tea Polyphenols or placebo capsules (lasting for 90 d, 2 times/d, 2 capsules/time) was given to subjects in the two groups. The neuropsychological scale scores and near-infrared brain imaging data of all subjects were collected before and after intervention, and the mean time from baseline data collection before intervention to follow-up data collection after medication was controlled within 6 months. Results:In patients from the experimental group, as compared with those before intervention, scores of delayed recall and recognition memory in the auditory verbal learning test (Huashan version) and Boston naming test, scores of Montreal cognitive assessment scale (basis version) were significantly higher, and scores of Hamilton anxiety scale, shape trails test A and rapid-eye-movement sleep behavior disorder screening questionnaire were significantly lower after intervention ( P<0.05); however, only scores of delayed recall in the auditory verbal learning test (Huashan version) were significantly increased in the control group ( P<0.05). A total of 23 pairs of differences of brain function connection before and after intervention in the experimental group were significantly higher than those in the control group ( P<0.05), mainly involving the function connection of forehead peak network (FPN), and function connection of FPN-default network, function connection of FPN-somatosensory movement network, function connection of FPN-dorsal attention network and function connection of FPN-visual network, as well as the function connection of default network and somatosensory movement network. Conclusion:Shentai Tea Polyphenols can improve cognitive performances including memory, language and executive function, anxiety mood and sleep quality in healthy middle aged or elderly subjects by affecting the functional connections of the networks in the brain.
