Objective To study the effects of enriched environment on the capacity of learning and memory and expression of brain derived neurotrophie factor(BDNF) expression of CA1 hippocampus in Wistar rats. Methods 20 male Wistar rats were raised from weaning(21 days old) to young adulthoed(50 -60 days old) in either an enriched or normal environment. The ability of learning and memory was measured with the Morris water maze test, and the expression of BDNF of CA1 hippocampus neuron was detected by immunohistochemistry. Results During Morris water maze test, the mean latency in enriched environment group was significantly shorter than that in the normal control group(24.37±5.45)s Vs (31.28±5.39)s;the time taken to cross the target in enriched environment group was significantly more than that in the normal control group(3.38±0.79)Vs (2.21± 0.49);the swimming distance of platform quadrant increased in the enriched environment group, and the decreased gray intensity of BDNF expression in CA1 hippoeampus was found in the enriched environment group. Conclusions Enriched environment can improve the capacity of learning and memory of the rats, which were realized may be through BDNF pathway.

Objective To assess the clinical significance of three-dimensional speckle tracking imaging (3D-STI) in the evaluation of left ventricular myocardial function in breast cancer survivors with postoperative anthracycline-based chemotherapy. Methods A total of 51 breast cancer survivors with postoperative anthracycline-based chemotherapy were recruited and 31 female healthy volunteers as the controls.Conventional echocardiography and 3D-STI derived parameters [including global longitudinal strain (GLS) and global area strain (GAS)] were measured before and at 3 and 6 anthracycline-based chemotherapeutic cycles. The receiver operating characteristics (ROC) curve was constructed to determine optimal sensitivity and specificity of 3ID-STI derived parameters for the prediction of future cardiotoxicity. Results In comparison with the controls and baseline cases,GLS and GAS deteriorated significantly (P < 0.05 for both). No statistical difference in GCS and conventional parameters showed before and after the initiation of chemotherapy (P <0.05,respectively).The ROC curves showed that GAS as the best 3D-STI predictor of patients who develop cardiotoxicity during the follow-up.The area under ROC(AUC) of GAS was 0.894,and its optimal cut-off value was-28.4%,with a specificity of 88.0% and a sensitivity of 82.9%.AUC of GLS was 0.802,and its optimal cut-off value was-14.3%,with a specificity of 62.0% and a sensitivity of 85.4%.Conclusions Early decreases in GAS and GLS based on 3D-STI may allow the prediction of subsequent cardiotoxic development accurately in breast cancer survivors with postoperative anthracycline-based chemotherapy.

Objective:To summarize the oblique branches found in the transfer of anterolateral thigh flap (ALTF) and to handle the issues of oblique branch. Methods:Thirty patients who require surgery of ALTF transfer from May, 2017 to October, 2019 were enrolled. CTA examination was perfected prior surgery and the origin of ALTF vessels was preliminarily determined. During the surgery, Three-longitudinal-and-five-transverse methods were used to locate and design the flap. The ALTF was taken according to the flap design. Attention should be paid to the location of the oblique branch and the separation of the vessels of oblique branch to avoid a damage as much as possible. The vessels of oblique branch should be completely explored and separated. One or 2 vascular pedicles were cut according to whether the oblique branch vessels and the descending branch vessels were joined together during surgery. Clamping tests were carried out on the 2 vascular pedicles to determine an arterial blood supply. Super drainage of vascular pedicle veins according to the situation of blood circulation. After the surgery, routine treatment was carried out. Blood supply, skin temperature, swelling degree, exudation and survival of the flap were closely observed and regular follow-up was carried out. Results:Among the 30 ALTF examined by CTA, 13 patients were identified with oblique branch vessels before operation. During operation, 11 oblique branch vessels (The occurrencce rate was 36.6%)were found to enter the flap, and were completely preserved. Of the 11 identified oblique branch vessels, 8 had 2 vascular pedicles taken and the vascular pedicles were treated by venous super drainage technique. The postoperative blood supply of the flap was good; The skin temperature was closed to surrounding normal skin; Swelling of flap was minor and there was little seepage. The flaps all survived after surgery with stage one healing. Followed-up time was 3-32 (average 16.1) months. The recipient site healing was good, and the function and appearance were satisfactory. The joint movement at the donor site was normal, and there was no obvious loss of local sensation. Conclusion:More than one third of the oblique branches appear in this group. The oblique branch vessels should be preserved as much as possible to avoid issues in relation to the oblique branch. Reasonable handling of oblique branch is the key to the success of the surgery.

Objective To explore the effects of community-based comprehensive intervention on social function and quality of life of patients with bipolar disorder. Methods 244 bipolar disorder patients were assigned into control group (n=119) and intervention group (n=125). The control group accepted antipsychotics only, and the intervention group accepted community-based comprehensive intervention in addition. They were assessed with Social Disability Screening Schedule (SDSS) and Generic Quality of Life Inventory-74 (GQOLI-74) before and 6, 12 months after treatment. Results The SDSS score was lower (P<0.05) and the GQOLI-74 score was higher (P<0.05) in the intervention group than in the control group. Conclusion Community-based comprehensive intervention can improve the social function and quality of life of patients with bipolar disorder.

Objective To investigate the left ventricular (LV) performance in patients with essential hypertension(EH) by global performance index (GPI) based on three-dimensional speckle tracking imaging (3D-STI).Methods A total of 164 EH patients were enrolled and divided into four subgroups according to LV configuration:LV normal group (LVN,n =67),concentric cardiac remodeling group (CCR,n =49),concentric cardiac hypertrophy group (CCH,n =28) and eccentric cardiac hypertrophy group (ECH,n =20).Thirty age and sex matched healthy volunteers were enrolled as controls.3D-STI was performed to assess LV 3D global strain and LV torsion, then LV systolic dyssynchrony index (SDI) and global performance index (GPI) were calculated.Global strain parameters were investigated among the groups, and correlations with GPI were studied. Results GPI,global longitudinal strain(GLS),global area strain(GAS),global radial strain(GRS) and global 3D strain(G3DS) in EH groups were respectively lower than those in the normal controls, and there were statistical differences among the respective parameters (P <0.05,respectively).SDI in group CCH and ECH were significantly higher than that in normal controls (P < 0.05,respectively).In multivariate stepwise regression analysis,GAS was the most influential to GPI.Conclusions GPI based on 3D-STI may accurately reflect LV performance in patients with EH,and has potential applications in clinical practice.

Objective To observe the effect of core stability training combined with eye acupuncture on motor function in patients with hemiplegia after stroke. Methods Eighty patients with hemiplegia after stroke were randomly divided into a treatment group and a control group (40 cases in each). Both groups were given regular rehabilitation training, but the patients in the treatment group also were taught core stability training of their waist and abdominal muscle groups, their shoulder muscles, trunk and pelvis. They were also given eye acupuncture.A trunk control test (TCT) , Berg's balance scale (BBS), the modified Barthel index (MBI), functional ambulation categories (FACs) and the Fugl Meyer assessment scale (FMA) were used to assess motor function before and after treatment. Results At the end of 6 weeks of treatment, all of the measurements in both groups had significantly improved. The average TCT, BBS, MBI, FMA, and FAC scores of the treatment group significantly exceeded those of the control group. Conclusions Core stability training and eye acupuncture together can significantly improve the motor function of hemiplegic stroke survivors. The combination therapy is worthy of clinical popularization and application.
