2.Relationship between anti-oncogene mutation and microsatelite instability in gastrointestinal stromal tumor

By now,the mutations of C-KIT gene and PDGFRA have been regarded as the most important primary incidences.The latest results have revealed the fact that the abnormal expressions of p53,p16,p21 WAF-1,p27 and E-cadherin(cDH1) which were labeled as anti-oncogenes in most of human tumors,existed in the progression of gastrointestinal stromal tumor.Therefore,we put forward the theory that gene mutaion may lead to the instability of DNA that further promotes mutation of short tandem repeat in anti-oncogene coding region,on basis of which,that finally lead to the malignant progession of gastrointestinal stromal tumor by anti-oncogene mutation.
3.Effects of tetramethyl pyrazine (TMP) on noise induced hearing loss in hypobaric environment

Objective To evaluate the effect of tetramethyl pyrazine(TMP)on noise induced hearing loss (NIHL) in hypobaric environment. Methods Normal guinea pigs were randomly divided into two groups, in which the animals were exposed to noise for 1 day or 7 days, respectively. Each group was randomly divided into 3 subgroups: control, TMP-treatment and TMP-pretreatment groups. All animals were exposed to 110dB white noise in hypobaric chamber to repreduce a low pressure environment of 5500m altitude. The mechanism underlying TMP protection was studied by detecting the auditory brainstem response (ABR). Results ABR threshold shift in different degrees occurred in every group, especially in animals exposed to 4kHz. The threshold shifts increased in extent along with prolongation of exposure time. The longer they were exposed, the more ABR threshold shifts. ABR threshold shifts occurred more severely in control group compared with the two TMP-utilized groups. Conclusion TMP has certain protective effect on NIHL in a hypobaric environment.
4.Correlation between Signs of Living Body in Abdominal and Pelvic Cavities and Syndrome Typing of Chinese Medicine in Colorectal Cancer Patients.

OBJECTIVETo explore the correlation between signs of living body in abdominal and pelvic cavities and syndrome typing of Chinese medicine (CM) in colorectal cancer patients.
METHODSTotally 112 colorectal cancer patients undergoing open abdominal surgery or laporoscopic surgery were syndrome typed as five types, i.e., inner-accumulation of damp and heat, blockage of stasis and toxin, Pi-Shen yang deficiency, blood-qi deficiency, Gan-Shen yin deficiency. Signs of living body in abdominal and pelvic cavities were collected. The correlation between signs of living body in abdominal and pelvic cavities and syndrome typing of CM were analyzed.
RESULTSRed colorectal canals or mass were dominated in colorectal cancer patients with inner-accumulation of damp and heat syndrome. Dark purple colorectal canals or mass were dominated in colorectal cancer patients with blockage of stasis and toxin syndrome. Reddish colorectal canals or mass were dominated in colorectal cancer patients with blood-qi deficiency syndrome. Pale colorectal canals or mass were dominated in colorectal cancer patients with Pi-Shen yang deficiency syndrome. Whitish or red-white stripes were dominated in colorectal cancer patients with Gan-Shen yin deficiency syndrome. Dropsy colorectal canal was associated with Pi-Shen yang deficiency syndrome. Intracavitary effusion was often seen in colorectal cancer patients with inner-accumulation of damp and heat syndrome. The effusion was yellowish in less amount. Intracavitary adhesion was often seen in colorectal cancer patients with blockage of stasis and toxin syndrome. There was no correlation between the maximum diameter of mass or each syndrome type of CM.
CONCLUSIONThere existed correlation between signs of living body in abdominal and pelvic cavities and syndrome typing of CM, which could be taken as one of references for syndrome typing of colorectal cancer patients.

5.Therapeutic Observation of Acupunctureat Ghost Points for Anxiety and Depression in Methamphetamine-dependent Subjects

ObjectiveTo observe the effect of acupuncture at the ghost points on anxiety and depression in methamphetamine (MA)-dependent subjects.MethodSixty MA-dependent males were randomized into a control group, acupuncture groupⅠand acupuncture groupⅡ. The control group didn’t receive any intervention; inacupuncture groupⅠ, bilateral Neiguan (PC6), Shenmen (HT7), Sanyinjiao (SP6), and Jiaji points (EX-B2) were selected; Shuigou (GV26), Shangxing (GV23), Chengjiang (CV24), and bilateral Daling (PC7), Jianshi (PC5), Shenmai (BL62), and Quchi (LI11) were selected in acupuncture groupⅡ. For the three groups, treatment was given three times a week, totally for 12 times. The Hamilton Anxiety Scale (HAMA) and Hamilton Depression Scale (HAMD) were observed before treatment, and respectively after 1, 2, 3, and 4-week treatment.ResultAfter the first-week treatment, the HAMA and HAMD scores in acupuncture groupⅡwere significantly lower than that in the control group (P<0.05);afterthe second, third, and fourth week, the HAMA and HAMD scores in acupuncture groupⅠandⅡwere significantly lower than that in the control group (P<0.05); after the second and third week, the HAMD score in acupuncture groupⅡwas significantly lower thanthat in acupuncture groupⅠ(P<0.05). However, after 4-week treatment, there was no significant difference in comparing the HAMD score between the two acupuncture groups (P>0.05).ConclusionAcupuncture at the ghost points and at the effective points selected according to previous studies both can improve anxiety and depression state in MA-dependent subjects, but acupuncture at the ghost points can possibly produce a more significant effect in improving depression state in MA-dependent subjects in the midway of the treatment.
6.Clinical application and mechanism on the ghost points in the treatment of mental diseases.

In order to explore the clinical application and mechanism of the ghost points in the treatment of mental diseases, taking "the ghost points" and "mental diseases" as the theme and the key words, the literatures in recent 10 years on the treatment of mental diseases with the ghost points were collected from CNKI and Wanfang database and analyzed. It is discovered that the ghost points can treat schizophrenia, manic-depressive disorder, dementia, stroke, loss of consciousness, insomnia. anxiety, depression and hysteria and the efficacy is favorable. It is believed that the ghost points act on balancing yin and yang, regulating qi and blood, regaining consciousness and tranquilizing the mind and they achieve the very excellent effect on mental diseases. At present, the reports on this aspect are limited. Hence, it is required to promote the study on the clinical application and mechanism of the ghost points so as to enlarge the clinical indications of the ghost points and provide the powerful support on mechanism for the treatment of mental diseases.

7.Final test analysis and teaching reflection of clinical diagnostics

Objective To evaluate the quality of the examination paper of the theory of Clinical Diagnostics,to explore and reflect on the teaching methods,so as to improve the quality of teaching.Methods Finals results of Clinical Diagnostics including 40 clinical undergraduates of Capital Medical University were analyzed.SPSS 20.0 was used to make analysis of the frequency,means statistics and normality of the examination paper.Kuder Richardson/Cmnbacha formula,percentage unification methods and so on were used to calculate confidence,validity,difficulty and degrees of distinction.Acquisition of relevant knowledge was assessed according to score distribution,while test paper quality was evaluated based on indicators including confidence,validity,difficulty and degrees of distinction.Results Test scores of 40 students were between 61 to 96 (83.64 ± 8.07).The degree of confidence (γ) for choice questions and subjective questions was 0.65 and 0.59 respectively;The validity (V) was 0.27;The overall difficulty (P) of the examination was 0.84;The degrees of distinction (D) were between 0.16 to 0.30.And the total points losing rate was 16.36%.Conclusion The examination is of medium difficulty and good degree of distinction,but the teaching strategies still need further adjustment in order to improve the students' ability of flexible application of the basic knowledge.
8.Clinical application of non-mutilation Roux-en-Y digestive tract reconstruction after total gastrectomy

Objective:To investigate the efficacy of three different methods of digestive tract reconstruction after total gastrectomy.Methods:A total of 106 cases underwent the uncutted Roux-en-Y esophagojejunostomy with a jejunal pouch(group A,n=38) ,Roux-en-Y esophagojejunostomy with the J-type jejunal pouch(group B,n=31) ,and Roux-en-Y esophagojejunostomy with the Orr-type jejunal pouch(group C,n=37) respectively.The clinical data were retrospectively studied.The digestive tract reconstruction time,the post-operative complication incidence rate,postprandial symptoms,food-intake quantity,weight,serum nutritional parameters and emptying time of the gastricsubstitute were compared respectively.Results:The reconstruction time was the shortest in group A,which was significantly shorter than that in group B(P0.05) ,Group A was significantly lower than group B and C in the incidence of roux stasis syndrome.The Visick score of group A were superior to those of group B and C in 6 and 12 months after operation.The food intake gain of group A and B were superior to that of group C 6 and 12 months after operation.There were no significant differences among 3 groups in the term of weight,Hb and ALB loss and prognosis nutritional index(PNI) 6 months postoperatively.Weight and ALB loss in group A and B were lower than those in group C 12 months postoperatively,but PNI was higher in group A and B than those in group C.Emptying time of the gastric substitute was prolonged both in group A and B 12 months postoperatively.Conclusion:The uncutted Roux-en-Y esophagojejunostomy may act as an adoptable method of digestive tract reconstruction after total gastrectomy for gastric cancer.
9.Clinical analysis of cellular schwannoma in the gastric wall misdiagnosed as gastric stromal tumor

Cellular schwannoma is a rare benign tumor of the peripheral nerves.The tumor is consist of spindle cells which arranged in interwoven bundles,and lacking the mesh district and palisading structure.The tumor cells are abundant,with coarse chromatin,deeply stained nuclei and in multiforms.So the tumor may easily be misdiagnosed as various types of spindle cell sarcoma.Intraperitoneal cellular schwannoma may also be mistaken as gastrointestinal stromal tumors.In this article,the clinical characteristics,diagnosis and treatment of cellular schwannoma were analyzed based on the clinical data of a patient with cellular schwannoma in the gastric wall misdiagnosed as gastric stromal tumor.
