1.Total hip revision and non-bone cement prosthesis: Review of 41 cases with 41 hips in one institute within 5 years

BACKGROUND: With follow-up prolonged, more patients received total hip replacement need to revision. Periprosthetic ostaolysi, accompanied by aspetic loosening, would be aggratate, which lead to artificial joint loosening, eventually, result in joint renovation. OBJECTIVE: To observe the therapeutic effect of non-bone cement prosthesis on joint renovation following total hip replacement. METHODS: Totally 41 patients (41 hips) received renovation following total hip replacement at the Shenyang Orthopaedics Hospital from 2004 to 2009 were selected. They were renovated by Circumflex type total hip joint and press fit type non-bone cement type total hip joints. Eight cases had acetabular deficiency, 17 cases had Gustillo Ⅰ-Ⅱ acetabular loosening, and 8 cases had Gustillo Ⅲ acetabular loosening. All these patients were received spiral mortar shaping of pure titanium directly or combined with particle bone graft. Eight patients had Gustillo Ⅳacetabular loosening combined with bone defects, who adopted particles to plant the bone, rebuilt the hip mortar by titanium mesh, and shaping with bone cement. Non-bone cements prosthesis or lengthened prosthesis, as well as bone graft, was selected individualized. The cerclage band could be used if necessary. RESULTS AND CONCLUSION: Eight patients received titanium mesh and bone graft could do un-bearing exercise, and the remained patients could perform bearing exercise at day 3 after renovation. In the 6-66 months follow-up, no prosthetic displacement or subsidence could be found. No patients need a second renovation. Harris scores were increased from 32.6 before operation to 88.1 after operation. X-ray film showed that the bone density was increased in parts of patients. The results demonstrated that hip renovation using non-bone cement prosthesis can received an excellent short-term therapeutic effect, but the long-term effect needs to be explored.
2.Effect of maximal exercise on energy metabolism

Objective The purpose of this study was to investigate the relationship between oxygen consumption (VO2) after recovery from incremental maximal exercise and the levels of the blood lactate (HLa), glucose and alanine metabolism. Method The VO2, HLa and plasma alanine were obtained using a modified version of Bruce' s protocol 30 min after exhaustion for 10 men. Result The VO2 after recovery has returned to the pre - exercise, but the HLa concentrations and plasma alanine were significantly increased from rest to maximal exercise, and were significantly decreased thereafter, but remained above resting values for 30 min after maximal stage. Conclusion we conclude that oxygen consumption after exhaustive maximal exercise is related to glucose resyntbesis through alanine metabolism, as compared with that from lactate metabolism.

5.Present and future for associating liver partition and portal vein ligation for staged hepatectomy

Compared to portal vein embolization and traditional two-stage hepatectomy,associating liver partition and portal vein ligation for staged hepatectomy (ALPPS) can induce the proliferation of liver rapidly,concomitantly with high incidence of complications and mortality in the perioperative period.The feasibility and safety have been improved gradually as the improvement of technology and the accumulation of experience.But it is still controversial on its efficacy for malignant tumors,especially with insufficiency of medium-and long-term out-comes.The mechanism of rapid proliferation induced by ALPPS needs more studies with further steps.
6.The effect of rhEPO on the expression of Bcl-2 and Caspase-3 in intestinal tract of newborn rats with necrotizing enterocolitis

Objectives To investigate the effect of rhEPO on newborn rats with necrotizing enterocolitis (NEC). Meth-ods Sixty newborn Sprague-Dewley (SD) rats at the age of 48 hours were randomly divided into 5 groups:control group, NEC group, and intervention groups 1, 2 and 3 treated with rhEPO. The rats were fed rat breast milk substitutes and stressed under hypoxia and cold exposure to establish NEC model. The rats with NEC were treated with different doses of rhEPO (0.1U/ml, 1U/ml and 10U/ml) in intervention groups. The expression of Bcl-2 and caspase-3 were measured by immunohistochemistry, and intestinal pathological changes were observed using HE staining. The value of positive expression was analyzed by IOD (integral optical density) image analysis system. Results Abdominal distention, decreased activity and unresponsiveness occurred in NEC rats 24 hours after stress exposure, and pale skin, decreased skin temperature and respiratory rhythm change were observed in severe cases. The symptoms appeared later and milder in three intervention groups. The NEC incidence of newborn rats was as followings:control group(0%), model group(60%), intervention group 1( 30%), intervention group 2(18.2%), intervention group 3(9.1%) and the difference was signiifcant between each group (P=0.008). The grades of intestinal injury, the expression of active caspase-3 and Bcl-2 were signiifcantly different among groups (P<0.01). Intestinal injury was the most severe and the expression level of active caspase-3 was the highest in NEC group. After rhEPO treatment, the intestinal injury and the production of active caspase-3 protein were decreased, and the expression of Bcl-2 was increased. Conclusions Oral rhEPO could decrease the ex-pression of intestinal active caspase 3, and increase the expression of Bcl-2. The protective effect of rhEPO on NEC is dosede-pendent.

8.Research advances in stem cell transplantation for the treatment of acute-on-chronic liver failure

Acute-on-chronic liver failure (ACLF) is a critical disease in clinical practice and has a mortality rate as high as 80%.At present,there are still no ideal therapeutic measures for ACLF.Stem cell transplantation has unique advantages in the treatment of ACLF;however,there are still some issues regarding the research on stem cell transplantation,which limit its clinical application.This article elaborates on conventional treatment methods,source of stem cells,transplantation approach,and related issues,in order to provide help for further studies.
9.Application of epidermal growth factor in corneal injury

Epidermal growth factor (EGF) is a very important peptide growth factor,which is not only involving in a variety of ocular cell mitosis,proliferation and differentiation,but also participates in mediating the signal transduction between cell and cell,cell and matrix interaction and other biological functions.Corneal injury is a common and frequently occurring disease in ophthalmology,and it would lead to blindness without appropriate treatment.Researches showed that EGF can promote the repair and regeneration of wounded cornea.In recent years,these studies advance from animal experiment stage to clinical stage and also from the morphology evaluation stage to the genetics study stage.This paper provided an overview of EGF concept,its biological effectiveness,its relationship to corneal injury and its clinical application.
10.Management of Application of Perioperative Prophylactic Antimicrobial in Orthopedic Patients

OBJECTIVE: To discuss the feasibility of application of perioperative prophylactic antibiotics in orthopedic patients. METHODS:Intervention, feasible regulations and rational medication scheme were formulated and present medical records were monitored and related information was feedback timely. Utilization of perioperative prophylactic antimicrobial was compared between before and after intervention. RESULTS: In the intervention group, hospitalization days, medical costs, cost and proportions of antibiotics were reduced; medication time and selection of antibiotics for all kinds of incision were standardized and the mean duration was shortened. The proportion of antibiotics used at 0.5~2 hours before operation was increased. The first and second generation cephalosporin was applied more and more. Combination of medication was decreased and rational use of antibiotics was significantly improved. CONCLUSION:Management of drug use is helpful to standardize the application of perioperative prophylactic antibiotics and improve clinical rational use of antibiotics.
