1.Application of evidence-based laboratory medicine in quality control of biochemical tests in Chinese peacekeeping secondary hospital of United Nations Interim Force in Lebanon

Objective To seek a operation method of quality control of biochemical tests in peacekeeping mission via perspective of evidence-based laboratory medicine .Methods Literatures related to quality control of biochemical tests in international peace-keeping were referred ,compared and discriminated .Combined with 5-year peacekeeping laboratory experience ,a set of feasible ap-proaches were worked out and a practice evaluation was performed in a task period .Results Under the supervision of the quality control system ,providing accurate and reliable test results to the clinical ,developing and implementing quality control of biochemical tests were economical ,practical and reliable ,and could guarantee the effectiveness of reports of biochemical test in peacekeeping medical institutions .Conclusion Evidence-based laboratory medicine is important for quality control of biochemical tests in peace-keeping medical institutions .
2.Change of glycogen synthase kinase-3 activity in lymphocytes in patients with mild cognition impairment

Objective To explore the change of glycogen synthase kinase-3(GSK-3) activity in lymphocytes in patients with mild cognition impairment(MCI).Methods The phosphorylation of GSK-3? at Ser 9 site and total level of GSK-3? in peripheral blood were measured by Western-blot in normal control group,one-year MCI group,two-year MCI group and three-year MCI group.Results The phosphorylation of GSK-3? at Ser 9 site were lower in 1-year MCI group,2-year MCI group and 3-year MCI group,compared with normal control group(all P0.05).Conclusion The activity of GSK-3 in lymphocytes in MCI patients is raised,which implies a subsidiary value of MCI diagnosis.

4.Study of serum endotoxin and CD_(14) gene expression in a model of cerebrogenic multiple organ dysfunction syndrome

Objective To investigate the changes of serum endotoxin and its receptor CD 14 gene expression in multiple organs in models of acute forebrain ischemia complicated with multiple organ dysfunction syndrome (MODS), and the pathogenesis of cerebrogenic multiple organ dysfunction syndrome (CMODS). Methods 54 Wistar rats were randomly divided into normal control group ( n=6), sham-operative group ( n=8) and forebrain ischemic group ( n=40). The rats in forebrain ischemic group were randomly divided into 5 subgroups: 12, 24, 36, 48, and 72 h (8 rats in each group). The contents of endotoxin in plasma were determined after models of acute forebrain infarction established. The area density and optical density of positive staining expressing CD 14mRNA in lung, liver, intestine and kidney were analyzed for the relative content of CD 14mRNA using in situ hybridization and CMIA medical image analysis system.Results Plasma endotoxin level was markedly high at 12 hours after acute forebrain ischemia, peaked at 24 hours and somewhat decreased at 72 hours. The CD 14mRNA expression in lung, liver, intestine, and kidney tissues increased after brain ischemia, reached the peak at 24~36 h, and decreased after 48 hours. 5.Calcium phosphate cement with rhVEGF and rhBMP-2 promotes revascularization of big section bone allograft in dogs

Objective To explore the ability of revascularization of big section deproteinized bone (DPB) allograft combining calcium phosphate cement (CPC) with recombinant human vascular endothelial growth factor (rhVEGF) and bone morphogenetic protein-2 (rhBMP-2). Methods Totally 36 adult hybrid dogs were inflicted to demi-articular defect models by exsecting 30 mm right femur inferior extremity near articular surface,and then randomly divided into 3 groups, group A, receiving material CPC/rhVEGF/rhBMP-2/DPB transplantation into the demi-articular defect, group B, material rhVEGF/rhBMP-2/DPB transplantation, and group C with simple material DPB transplantation. At 4, 8, 12 and 16 weeks after operation, vasculogenesis and new bone formation were assessed by X-ray examination, histological examination, transmission electron microscopy, vas capillare analysis by vaso-filling with ink and radionuclide bone imaging examination (ECT). 6.Assay and counter measure for the complications of the prosthetic disc nucleus-pulposus(PDN)replacement

[Objective]To analyze the reasons and counter treatment of complication after prosthetic disc nucleus implantation.[Method]From Feb 2002 to Aug 2004,80 patients of lumbar disc herniation were implanted with one single PDN device at our centre.65 patients of them came to the clinical,functional,and radiographic follow-up examinations,magnetic resonance images and follow-up records of all patients were reviewed carefully.They were divided into three groups.Group A:30 cases had at least 24 months follow-up data.Group B:23 cases had at least 36 months follow-up data.Group C:12 cases had at least 48 months follow-up data.[Result]After implantation,device migrations were noticed in 3 patients.24 cases experienced low back pain.In 24~48 month follow-up,the MR images of 39 patients had shown the abnormal signal pattern of the endplate where PDN-solo was implanted,even 32 of them had been found subsidence.In Group A,the height of intervertebral disc at the 24 month follow-up was decreased 4%.In Group B,at 36 month follow-up,it was decreased 12%.In Group C,at 48 month follow-up,it was decreased 18%.[Conclusion]After follow-up,the clinical effect of PDN is preferred.However,the complications of device dislocation,device subsidence remain problems that must be highlighted.This study suggested that the complications of PDN are serious.Therefore,the proper selecting patients for PDN replacement are crucial for good results.The indications for PDN must be restricted.

OBJECTIVE:
To investigate the clinical application value of cone-beam CT (CBCT) online correcting technology in volume modulated radiation therapy (VMRT) for nasopharyngeal carcinoma (NPC).
METHOD:
Fifty NPC patients applying head neck and shoulder thermoplastic body membrane fixing device were eligible for treatment VMRT, these patients would accept a couple of CBCT scanning by on board imager (OBI) in a fixed time each week after initial setup and after online correcting during the first three week for radiotherapy, CBCT images and DRR images constructed by CT simulation were carried out registration,which could calculate the setup errors of initial setup and after online correcting.
RESULT:
Fifty patients were accepted 150 scanning after initial setup and 150 scanning after online correcting respectively, the errors after initial setup were (-1.24 ± 1.25)mm in X direction, (1.19 ± 1.85)mm in Y direction, (1.49 ± 1.70) mm in Z direction. The setup errors after online correcting were (-0.13 ± 0.29)mm in X direction, (0.10 ± 0.47)mm in Y direction, (0.17 ± 0.36)mm in Z direction. The setup errors after online correcting were significantly lower than the errors after initial setup in X direction (P < 0.05), Y direction (P < 0.05), Z direction(P < 0.05). The M(PTV) value after online correcting were 0.46 mm, 0.53 mm and 0.59 mm in X, Y and Z directions respectively.
CONCLUSION
The use of CBCT online correcting technology can significantly reduce the setup errors of VMRT for NPC and improve the treatment effect.
