2.Clinical Efficacy of Prostaglandin E_1 Used Early for Patients after Renal Transplantation

OBJECTIVE:To observe the clinical efficacy of prostaglandin E1 used early in patients after renal transplantation. METHODS:93 renal transplant recipients (treatment group) were treated with PGE1(20 ?g?d-1) within 2 weeks after operation,and another 85 renal transplant recipients (control group) were not given PGE1 therapy in the same period. Urine volume,serum creatinine (Scr),blood flow resistance-indexes (RI) under Doppler color ultrasound,incidences of acute rejection (AR) and delayed graft function (DGF) and the 1 year patient/graft survival rates were compared between the two groups. 3.Efficacy of Early Use of Rapamycin for Patients after Renal Transplantation and the Complications

OBJECTIVE:To evaluate the efficacy and safety of early use of rapamycin in renal transplant recipients.METHODS:A total of 58 patients undergoing renal transplantation(with kidneys came from corpses)were assigned to receive cyclosporine A(CsA)plus rapamycin(RPM)and prednisone(trial group,n=28)or CsA plus mycophenolate(MMF)and prednisone(control group,n=30).The clinical efficacy at 6 months after renal transplantation,the complications and the adverse drug reactions of the two groups were observed,meanwhile,biochemical indicators including serum blood lipid level and hepatic and renal functions were monitored to analyze the effects of two immunosuppressive schemes on acute rejection of the transplanted kidneys and the survival rates of patients/kidneys.RESULTS:The transplanted kidneys in both group all survived with function.There was no significant difference between the two groups in the incidence of acute rejection(4/28(14.3%)for trial group vs.5/30(16.7%)for control group).The abnormal serum lipid level,infection of incisional wound,diarrhea and hypoleukemia were noted in 11 cases(39.3%),4 cases(14.3%),3 cases(10.7%)and 1 case(3.6%)for the trial group vs.3 cases(10.0%),2 cases(6.7%),8 cases(26.7%)and 5 cases(16.7%)for the control group.CONCLUSION:The early use of triple immunosuppressive therapy(CsA + RPM+ Pred)showed more potent immunosuppressive efficacy but fewer acute rejections as compared the control group(CsA + MMF + Pred).The main side effects of the trial group(CsA +RPM+ Pred)manifested as increased serum lipid level and infection of incisional wound,however,the incidences of diarrhea and hypoleukemia were significantly lower than in the control group.
4.Research progress and challenge of the stem cell therapy for intervertebral disc regeneration

BACKGROUND: Interverbral disc degeneration (IDD) is the major cause of low back pain, which is considered as the pathological basis of intervertebral disc herniation and other degenerative diseases that severely affect the patients' quality of life. At present, neither conservative treatment nor operative treatment can reverse or inhibit the pathological development of IDD.OBJECTIVE: To summarize the progress of stem cell therapy for IDD, and to further analyze its challenge and potential solutions.METHODS: Relevant studies in PubMed database published before September 2016 were retrieved. Studies about stem cell therapy in IDD in vitro and vivo as well clinical trials were enrolled to analyze its existing problems.RESULTS AND CONCLUSION: (1) Stem cell therapy provides a new strategy for IDD treatment. So far, a large number of studies in vitro and vivo as well as clinical trials have reported the remarkable effect of stem cell therapy. (2) However, problems such as how to deal with microenvironment of interverbral disc, choose proper stem cells and master indications will be a challenge in the future.
5.Effects of parenting style and behavior on children with allergic asthma

Objective To explore the effect of parenting style and behavior on children with allergic asthma.Methods From July to December in 2016,a total of 60 children with allergic asthma,who were from Jiangsu Province Hospital of Traditional Chinese Medicine and Jiangsu Research Institute of Traditional Chinese Medicine,were selected as the asthma group.Another 60 normal children from kindergarten and primary school were selected as the healthy group.Achenbach child behavior checklist (CBCL) and parenting style questionnaire were used to evaluate the behavior and parenting style of the two groups.The Fractional exhaled nitric oxide (FeNO) of the two groups were also collected.Results (1) The scores of dotage,autocracy,inconsistence of parenting style in asthma group ((14.95±4.95),(22.23±4.69),(13.43±3.80))were higher than those in healthy group((11.68±3.69),(16.47±4.16),(11.07±3.32))and the differences were significant (all P<0.05).The scores of democracy,indulgence in asthma group ((38.92 ± 5.37),(18.12± 4.54)) were lower than those in healthy group((41.35±3.62),(19.88±3.41)),and the differences were statistically significant(all P<0.05).(2) The scores of attention,socialization,aggression and withdrawal of child behavior in asthma group ((7.35±3.55),(3.85±2.17),(11.57±5.30),(3.40±2.65))were higher than those in healthy group((3.38±2.81),(2.52± 1.52),(6.98±4.77),(2.02± 1.63)),the differences were statistically significant(all P<0.01).There was no significant difference between the two groups in the dimension of discipline(P>0.05).(3) The FeNO index of asthma group((32.27± 14.27) × 10-9 mol/L) was higher than that of healthy group ((11.10± 1.88) × 10-9 mol/L) and the differences were statistically significant (P<0.01).(4)Logistic regression analysis showed that the attention(B=0.38,OR=1.47,95% CI =1.27-1.70,P<0.01),dotage(B=0.19,OR=1.20,95% CI =1.06-1.37,P<0.01) and autocracy(B=0.24,OR=1.27,95% CI =1.14-1.42,P<0.01)had significant effects on allergic asthma.Conclusion Attention,dotage and autocracy are the risk factors to influence the control level of allergic asthma.This should arouse the attention of clinicians and parents.
6.Signal transducer and activator of transcription 1 and matrix metalloproteinase 3's genetic expression and their clinical significance on urothelial tumor after renal transplantation

Objective To investigate the signal transducer and activator of transcription 1(STAT1) and matrix metalloproteinase 3(MMP3)′s genetic expressions and their clinical significance on urothelial carcinoma after renal transplantation. Methods Fifty-one patients with urothelial carcinoma were recruited in this study. Sixteen of them who had renal transplant were in the experimental group and 35 of them without renal transplant were in the control group. All the cases had been proved postoperatively having transitional cell carcinoma by histopathological study. The human genome oligo arrays were used to analyze the gene expression spectrum of urothelial carcinoma after transplantation, aiming the STAT1 and MMP3's expression. Real time RT-PCR and immunohistochemical staining were used to compare the differences in the 2 groups. Results The experimental group showed that there were 35 genes up-regulated compared with the control group. Of them, 23had known gene function or partly known, and 12 had unknown gene function. There were 76 genes down-regulated. Of them, 46 had known gene function or partly known, and 30 had unknown gene function. After pathway analysis of the differentially expressed genes, there were 23 groups of pathways which had significant differences (P<0.05), referring to the aspects of immunosuppressive and tumor growth. The levels of STAT1 and MMP3 expressions had significant differences between the 2groups(P<0.05)as well. Conclusions The differential expression of urothelial tumor genes is obvious between patient who has had renal transplant and who has not. There are many aspects that are related to the tumor's growth like signaling pathways regulating proliferation, apoptosis of tumor cells, tumor angiogenesis and the tumor metastasis potential. STAT1 and MMP3 maybe become the targets of chemoprevention for post-transplantation urothelial carcinoma.
7.Post-transplant lymphoproliferative disorders following renal transplantation: 4 cases report and literatures review

Objective To study the clinicopathologic features of post-transplant lymphoproliferative disorders (PTLD) following renal transplantation.Methods Four cases of PTLD following renal transplantation were studied and relevant literatures were reviewed.Results All the 4 cases had received 3-drug-immunosuppression after transplantation.The duration between renal transplantation and diagnosis of PTLD was 5-112 months,averagely 34 months.The patients were suffered from infective monocytic hyperplasialike PTLD,plasmacytic hyperplasialike PTLD,polymorphic PTLD and monomorphic PTLD respectively in morphology and had no specific symptoms.All the patients received therapy with dosage reduction of immunosuppressants and some received rituximab or chemotherapy.The case of monomorphic PTLD died in a short time after diagnosis.Conclusion PTLD is a lymphoproliferative disease with distinctive morphologic and clinical characteristics.The main treatments include the dosage reduction of immunosuppressive agents,biotherapy and chemotherapy.The prognosis of monomorphic PTLD is poor.
8.Effect of cyclosporine on the genetic expression of signal transducer and activator of transcription 1 in bladder cancer of rats and its clinical significance

Objective To investigate the effect of cyclosporine on signal transducer and activator of transcription 1 (STAT1) genetic expression on bladder cancer in rats induced by BBN and its clinical significance.Methods Twenty SD rats were divide into experimental group or control group randomly.Ten samples of SD rats bladder cancer induced with BBN and cyclosporine simultaneously as experimental group,and 10 samples of SD rats bladder cancer induced with BBN only as control.Real time RT-PCR and immunohistochemistry stain were used to detect STAT1 mRNA and protein level expressions of bladder cancer in rats respectively.Results The STAT1 mRNA median expression fold was 4.5 (2.1-6.6) in experimental group and 5.6 (3.4-8.5) in control group.The STAT1 protein expression were 5 cases with (-),3 cases (+),2 cases (++) in experimental group and 0 case (-),5 cascs (+),5 cases (++) in control group.The expression of STAT1 mRNA and protein level of bladder cancer between experimental group and control group were both significant different (P < 0.05).Conclusions Cyclosporine may stimulate the growth and development of bladder cancer through changing expression of some genes like STATI,and STAT1 maybe become one of the targets of chemoprevention for post-transplantation bladder cancer.
9.A quantitative analysis of factors affecting BK viral DNA loads in early renal transplant recipients

ObjectiveTo investigate DNA loads and risk factors of BK virus infection in renal transplant recipients.MethodsWe developed a real-time PCR assay to quantitate BK virus loads in 80 patients receiving renal transplantation in our center,and correlation between the BK virus load and clinical course was analyzed.BK virus loads were measured in urine and plasma. Epidemiological features and risk factors of BK virus infection were analyzed.ResultsThe positive rate of BKV viruria and viremia in 80 renal recipients was 37.5% (30/80) and 8.75% (7/80),respectively.BKV loads were higher in renal allograft recipients whose age was more than 50 years old.BKV loads were observed in urine and plasma (compared with group whose age was less than 50 years,P=0.017 and 0.05,respectively).BKV DNA copies were higher in group Tac than that in group CSA (P<0.05),and the peak of BKV load in serum appeared at14th and10th month after transplantation,respectively,but the peak in urine was ahead of that in serum,appeared at 2nd and 8th month,respectively.ConclusionSerial measurement of BKV viral loads by quantitative PCR is a useful tool in monitoring the course of BK virus infection.The ages of recipients (>50 years) and using Tac + MPA can reactivate BK virus and then result in BKVAN in renal transplant recipients. Intensive BKV monitoring is necessary for these recipients.
