1.The efficacy of infiltrating injection of local anesthetic and oral NSAIDs for postoperative pain of total knee arthroplasty

To observe the efficacy of infiltrating injection of local anesthetic and oral NSAIDs for postoperative pain of total knee arthroplasty(TKA). 50 patients undergoing TKA were randomly assigned to two groups. The study group received intraoperative infiltrating injection of ropivacaine around knee and perioperative oral celecoxib. The control group received intravenous analgesia pump. The VAS scores in postoperative 3 days of the study group (4.2±0.6 ,4.6±0.8 ,4.6±1.1 ) were lower than the control group ( 6.5 ±0.7 ,6.4 ±1.0 ,6.1 ±0.9 ) . The differ-ence was statistically significant(P<0.05),and the dose of remedial morphine of the study group (10.6±7.7)mg was less than the control group (18.4±10.3) mg. The difference was statistically significant(P<0.05). Infiltra-ting injection of ropivacaine and oral celecoxib could effectively relieve postoperative pain of TKA which was better than intravenous analgesia pump.
2.Integration and Expression of BGL1 Gene from Saccharomy-copsis fibuligera in Industrial Saccharomyces cerevisiae

The recombinant plasmid pPIC-gpd-bgl-hyg was constructed, which contained GPD2 promotor and terminator from industrial yeasts Saccharomyces cerevisiae, ?-glucosidase gene (BGL1) from Sac-charomycopsis fibuligera and hyg from hygromycin as the selected marker. With the yeast’s high efficiency of homologous integrated, the BGL1 gene was successfully integrated into industrial yeasts S. cerevisiae. The recombined yeast could grow on the cultures with the cellobiose as a sole carbon source, and the ?-glucosidase activity achieved 0.764 U/mL after 48 hours’ cultivation. In the experiments of VHG ethanol fermentation, the cellobiose concentration in broth of recombined yeast was 80% lower than that of indus-trial yeast.
4.Clinical Observation on Wind-Dispersing and Cold-dispelling, Blood-activating and Stasis-removing Therapy for Cold-type Urticaria

[Objective] To observe the therapeutic effect of wind-dispersing and cold-dispelling, blood-activating and stasis-removing therapy for cold-type urticaria and to explore its possible mechamism. [Methods] Sixty-six cases were allocated to two groups by a simple random sampling method. Group A ( n = 36) was treated with wind-dispersing and cold-dispelling, blood-activating and stasis-removing herbal medicine including Herba Ephedrae, Ramulus Cinnamomi, Herba Schizonepetae, Radix Saposhnikoviae, Semen Armeniacae Amarum, Rhizoma Zingiberis Recens, Fructus Jujubae, Radix Salviae Miltiorrhizae, Radix Angelicae Sinensis, Caulis Spatholobi, Radix Paeoniae Rubra, etc. Group B (n =30) was treated with cetirizine hydrochloride tablets. The two groups were treated for two 20-day courses. One week after treatment, the therapeutic effect and the changes of hemorheological parameters before and after treatment were observed and compared with those in 38 healthy volunteers. [Results] The total effective rate was 80.6% in group A and 63.3% in group B, the difference being significant ( P 0.05) . [ Conclusion] Wind-dispersing and cold-dispelling, blood-activating and stasis-removing therapy is an important method for cold-type urticaria and the selected herbal medicines are effective. Its therapeutic mechanism may be related to the improvement of hemorheological parameters.
5.Clinical Observation on Therapy of Dredging Meridians for Prostatitis Type Ⅲ

[Objective] To investigate the effect of dredging meridians in improving prostatitis type Ⅲ. [Methods] Sixty cases of prostatitis were randomized into groups A and B, 30 in each group. The two groups were both treated with Keduohua Capsule, which was a ?-eceptor blocker, and group B was added with Guizhi Fuling Pill (GFP). The treatment lasted 12 weeks. Chronic prostatitis symptoms index issued by American National Institute of Health (NIH-CPSI) , white blood cells (WBC) count and the amount of lecithin body in prostatic secretion, and urinary flow rate were examined before and after treatment to evaluate the therapeutic effect of GFP and the safety of the combination of two medicines . Objective:To investigate the expression of amphiregulin in human endometrium during the menstrual cycle.Methods: Endometrial tissues were collected from the patients undergoing hysterectomy or endometrial biopsy.Real-time RT-PCR,in situ hybridization and immunohistochemistry were used to detect the expression characteristics of amphiregulin in human endometrium in proliferative and secretory phases.Results: Real-time RT-PCR showed the expression of amphiregulin mRNA in secretory phase was 32 times that in proliferative phase.The results from in situ hybridization and immunohistochemistry showed that amphiregulin was located in the cytoplasm and mainly expressed in the gland of endometrium.The expressions of amphiregulin mRNA in proliferative and secretory phases were 0.54?0.22 and 2.96?0.47(P

Summary
Cisplatin is an anti-tumor drug which is widely used for the treatment of various solid tumors. Unfortunately, seriousside-effects have affected patients, such as hearing loss. Up to now, there is no clear and effective measure to protect the cisplatin-induced ototoxicity in the clinical use of cisplatin studies indicated that autophagy may be involved in the whole process of cisplatin-induced hearing loss. In this review, the relationship between cisplatin ototoxicity and autophagy was reviewed. It is hoped that this study can provide reference for further study of cisplatin ototoxicity and intervention of autophagy with autophagy activator or inhibitor.

9.Six Cases of Pulmonary Cavity in Children

Objective To investigate the clinical feature,image of CT scan pulmonary,diagnosis and treatment response in children with pulmonary cavity,and discuss the method of diagnosis and the tactics of treatment for pulmonary cavity in children.Methods A retrospective study of 6 patients with pulmonary cavity,who were diagnosed and treated from Jul. 2003 to Oct. 2009 in Department of Pediatrics of the First Hospital Affiliated to China Medical University.The clinical manifestations,laboratory tests,image of CT scan pulmonary,microbiological evidence,diagnostic procedure and treatment response were collected and evaluated.Results Six patients all didn′t have history of lung di-sease,there were 4 boys and 2 girls,8-15 years old,average age was 10.5 years old.Two cases of them had unrelated pulmonary underlying diseases,1 case had hyperthyroidism,and the other had juvenile idiopathic arthritis and had complication of macrophage activation syndrome,the other 4 cases had no obvious history.All cases had fever (38-40 ℃),3 cases had cough and 1 case had chest pain.Staphylococcus aureus were cultured in 2 cases,no bacteria was cultured in other 4 cases;the count of white blood cell decreased in 2 cases and increased in 4 cases;C-reactive protein increased in 5 cases and was normal in 1 case;plasma IgE level increased in 2 cases and was normal in other 4 cases;plasma 1,3-beta-D-glucan of all 6 cases were negative.Pulmonary cavities were found in the first CT scan of the lungs in 5 cases and only 1 case of patient′s pulmonary cavities was found in the second CT scan of the lung.Five cases were diagnosed infective causes,1 case was diagnosed noninfectious cause,5 cases of infective causes had been treated with anti-microbial drugs for at least 1 week,1 case of noninfectious cause were treated with methylprednisolone cobined cyclosporin A for 2 weeks.Pulmonary CT scan was rechecked in all cases,and the state of the cases were improved before discharged from hospital.Conclusions The causes of pulmonary cavity in children are not only infective factors,but also some non-infective disease,especially some changes of image of pulmonary CT scan has diagnostic value,detailed past medical history and appropriate rechecking of chest radiographic check are very necessary for diagnosis,according to the result of microbial inspection and evaluation of treatment effect in time and then adjust the treatment protocols.
10.Ultrasound radiofrequency-data technique assessment of carotid: a reproducibility study

Objective To evaluate the reproducibility of quality intima-media thickness(QIMT) and quality arterial stiffness (QAS) technique in assessment of carotid under different measuring methods.Methods Between December 2012 and January 2014,carotid QIMT and QAS examinations were carried out in 30 health volunteers.QIMT and QAS indicators included IMT in QIMT and distensibility coefficient (DC),compliance coefficient(CC),stiffness index α(α),stiffness index β(β),pulse wave velocity(PWV) in QAS.The measurement employed unilateral/once,bilateral/twice,and unilateral/twice methods.Using intra observer and inter-observer variability,the reproducibility was compared between different QIMT and QAS indicators and measuring methods.Results Extremely high level of intra-observer reproducibility was found for both QIMT and QAS technique (ICC>0.8).QIMT also showed an excellent inter observer reproducibility (ICC>0.8).In contrast,the reproducibility of QAS technique varied in different indicators (PWV > β ≈ α > CC > DC) and method ( unilateral/once > bilateral/twice > unilateral/twice).Conclusions QIMT measurement was highly reproducible.Whereas the reproducibility of QAS technique varied in different indicators and methods.Due to low reproducibility,the study result did not support the clinical application of DC indicator and unilateral/once method.
