7.Influence of follow-up intervention on disease control of patients with diabetes mellitus
Chinese Journal of Practical Nursing 2009;25(3):21-23
Objective To discuss the influence of follow-up intervention on disease control of pa-tients with diabetes mellitus. Methods 160 patients who received education about diabetes mellitus were divided into the intervention group and the control group randomly according to the date of dis-charge with 80 patients in each group. The intervention group was given regular follow-up visit (including telephone fol-low-up) which lasted 12 months. The control group received no regular foUow-up.The health behavior,self-management ability and alleviation state of cognition level, fast glucose, glycosylated hemoglobin were com-pared after intervention between the two groups by t test andχ2 test. Results The health behavior,self-management ability and alleviation state of cognition level in the intervention group were better than those of the control group. The blood glucose 6 and 12 months after discharge was higher than that at the time of dis-charge and that of the intervention group at the same time point. The glyeosylated hemoglobin of the interven-tion group was reduced 6 and 12 months after discharge compared with that at the time of dis-charge. Conclusions Out-of-hospital follow-up is an effective method to extend hospital health educa-tion to patients' home. It can instruct and supervise the patients to assume the responsibility of subsequent targets control,so it plays a positive role in the blood glucose control, improvement of life quality and promo-tion of psychological and physical health. It is worthy of wide popularization.
8.Serum levels of triggering receptor expressed on myeloid cells-1 and inflammatory factors in patients with acute pancreatitis and its clinical values
Chinese Journal of Pancreatology 2010;10(2):89-91
Objective To determine the serum levels of triggering receptor expressed on myeloid cells-1 (TREM-1), analyse its relationship with inflammatory factors, and explore its clinical values. Methods Serum concentrations of TREM-1, TNF-α, IL-8 and IL-1β in 19 patients with early acute pancreatitis (AP) were measured by enzyme-linked immunoadsorbent assay and the relationship among them was analyzed, and it was compared with those of 20 normal controls. Results In patients with early AP, serum levels of TNF-α, IL-8, IL-1β and TREM-1 were (52.26±12.98) pg/ml, (20.18±9.81) pg/ml, (35.57 ± 13.34) pg/ml and (83.09 ± 32.49) pg/ml;while they were (18.69 ± 6.42) pg/ml, (9.81 ± 4.80) pg/ml, (14.94 ± 5.16) pg/ml and (8.44±8.42) pg/ml in control group, and there was significant difference between the two groups (P < 0.01). TREM-1 was positively correlated with other inflammatory factors in patients with early AP, and it had a sensitivity of 77.8%, specificity of 70.0% for prognosis prediction, which was better than TNF-α, IL-8, IL-1β. Conclusions TREM-1 may influence the leveLs of inflammatory factors and to some extent predict the early prognosis of AP.

10.Changes in peripheral blood CD14dimCD16+ monocyte count and serum C- reactive protein level in elderly patients with chronic heart failure
Journal of Shanghai Jiaotong University(Medical Science) 2017;37(3):423-426
Objective · To observe the changes in peripheral blood CD14dimCD16+ monocyte count and serum C-reactive protein (CRP) level in elderly patients with chronic heart failure (CHF) and analyze the relationship between these changes and the cardiac function of patients. Methods · 42 elderly patients with CHF (the CHF group, consisting of 18 cases of NYHA cardiac function grade Ⅱ , 18 cases of NYHA cardiac function grade Ⅲ , and 6 cases of NYHA cardiac function grade Ⅳ) and 25 healthy elderly people (the control group) were enrolled. Peripheral blood CD14dimCD16+ monocyte count, serum CRP level, left ventricular ejection fraction (LVEF), and left ventricular end-diastolic diameter (LVEDD) were measured and statistical analysis was performed. Results · CD14dimCD16+ monocyte count and serum CRP level were significantly higher in the CHF group than in the control group (P<0.05). In the CHF group, the CD14dimCD16+ monocyte count, serum CRP level, and LVEDD of NYHA grade Ⅳ patients were significantly higher than those of NYHA grade Ⅱ patients, while their LVEF was remarkably lower than that of NYHA grade Ⅱ patients. The differences were statistically significant (P<0.05). Correlation analysis results showed that peripheral blood CD14dim CD16+ monocyte count and serum CRP level were positively correlated with LVEDD (r =0.37, r =0.45, P<0.05) and negatively correlated with LVEF (r =-0.56, r =-0.33, P<0.05) in the CHF group. Conclusion · The deterioration of elderly patients with CHF is associated with inflammation and the monocyte dysfunction may exist in the heart failure process. Peripheral blood CD14dim CD16+ monocyte count and serum CRP level are expected to be novel biomarkers that can reflect the severity of heart failure.
