2.Effect of Proprioception Disturbance on Activities of Daily Living after Stroke

ObjectiveTo observe the effect of proprioception disturbance on the activities of daily living (ADL) after stroke.Methods67 stroke patients were divided into group A (32 cases with proprioception disturbance) and group B (35 cases without proprioception disturbance) according to if having proprioception disturbance. All patients of two groups were treated with routine medicine and rehabilitation treatment. Modified Barthel Index (MBI) was used to assess ADL when admission and 8 weeks later.ResultsIn all 67 stroke patients, the incidence of proprioception disturbance was 48.6%. After 8 weeks treatment, MBI scores of both groups improved significantly (P<0.05~0.01) and the average scores of group A was obviously lower than that of group B (P<0.05).ConclusionProprioception disturbance has bad affect on recovery of ADL after stroke obviously. Examination for proprioception disturbance should be performed in early stage after stroke and rehabilitation training should be carried on as soon as possible.
3.Effect of curcumin on expression of p-ERK and p-CREB in spinal dorsal horn and dorsal root ganglion in type 2 diabetic neuropathic pain in rats

Objective To evaluate the effects of curcumin on the expression of phosphorylated extracellular signal-related kinase (p-ERK) and phosphorylated cAMP response element binding protein (p-CREB) in the spinal dorsal horn and dorsal root ganglion (DRG) in type 2 diabetic neuropathic pain (DNP) in rats.Methods Type 2 diabetes mellitus was induced by high-fat and high-sucrose diet and intraperitoneal streptozotocin (STZ) 35mg/kg,and confirmed by fasting blood glucose level≥ 16.7 mmol/L in male Sprague-Dawley rats.Type 2 DNP was confirmed by the mechanical withdrawal threshold (MWT) and thermal withdraw latency (TWL) measured on day 14 after STZ administration < 80% of the baseline value,and the rats with type 2 DNP were randomly divided into 3 groups (n =27 each):type 2 DNP group (group DNP),curcumin group (group Cur) and solvent control group (group SC).Curcumin and corn oil 100 mg/kg (25 mg/ml) were injected intraperitoneally once a day for 14consecutive days starting from 14 days after administration of streptozocin in Cur and SC groups,respectively.Another 27 normal rats were served as control group (group C) and were fed with common forage.MWT and TWL were measured at 3,7 and 14 days after curcumin injection (T1 3),and the lumbar segment 4-6 of the spinal cord and DRGs were removed at the same time for determination of the expression of p-ERK and p-CREB (by Western blot).Results Compared with group C,MWT was significantly decreased,TWL was shortened,and the expression of p-ERK and p-CREB in spinal dorsal horn and DRGs was up-regulated at T1-3 in DNP and SC groups,and at T1 in Cur group (P < 0.05).Compared with group DNP,MWT was significantly increased,TWL was prolonged,and the expression of p-ERK and p-CREB in spinal dorsal horn and DRGs was down-regulated at T2,3 in Cur group (P <0.05).There was no significant difference in the MWT,TWL and expression of p-ERK and p-CREB between DNP and SC groups (P > 0.05).Conclusion Curcumin can attenuate type 2 diabetic DNP by inhibiting up-regulation of the expression of p-ERK and p-CREB in the spinal dorsal horn and DRG in rats.
4.An experimental study of motor evoked potentials of the diaphragm elicited by transcranial electric stimulation

Objective To study the characteristics of the transcranial electric stimulation(TES)-elicited motor evoked potentials(MEP)recorded from different points of the diaphragm,and to determine the optimal record- ing site in the diaphragm.Methods One EEG electrode was placed subcutaneously in the midline of the skull as stimulation electrode and another EEG electrode was inserted into hard palate submucously as cathode in 30 male rats.And single square wave electrical pulse was used for stimulation.The stimulation intensity was 15 mA,the du- ration of the pulse was 0.2 ms,and the stimulating interval was 200 ms.The concentric needle electrode were insert- ed into the following eight sites:the medial,intermediate,lateral crura of the diaphragm,the junction of the posterior axilla line and the inferior border of the eleventh rib,the junction of the anterior axilla line and the inferior border of the ninth rib,the junction of the intermediate line of the clavicle and the inferior border of the eighth rib,the junction of the para-sternal line and the inferior border of the seventh rib,the pars sternalis.The MEP was recorded from each point,respectively.The optimal sites of the diaphragm MEP were found and recorded.Results The MEPs were re- corded from each of the 8 sites of the diaphragm in all the rats after TES.There was no statistically significant differ- ence among the latencies of every site.But the amplitude varied from 6.08?0.46 mV in maximum along the inferior border of the ninth rib at the anterior line of the axilla to the minimum of O.91?0.03 mV at the pars sternalis. Conclusion It is safer to insert the needle along the inferior border of the ninth rib at the anterior line of the axilla, because there was certain distant from the folding part of the pleura and the amplitude of the recorded wave at this point is maximal.
5.Surgical treatment of epilepsy induced by gray matter heterotopia

Objective To explore the relationship between gray matter heterotopia and epilepsy and assess the therapeutic effect of surgical intervention. Methods Six cases of gray matter heterotopia-induced epilepsy treated in our department between May, 2004 and May, 2006 were analyzed retrospectively for the clinical characteristics, surgical approaches, and the outcomes in the 2- to 4-year-long follow-up. Results All the patients received surgical interventions through different approaches, including resection of the heterotopic gray matter plus bipolar coagulation of the cortexes in 4 cases, heterotopic gray matter resection with bipolar coagulation of the cortexes and temporal lobectomy in 1 case, and exclusive bipolar coagulation of the cortexes in 1 case. Five patients were free of seizure attach and 1 patient showed significantly reduced seizure attack after the operation. Conclusion Surgical intervention can be effective for treatment of intractable epilepsy induced by gray matter heterotopia.
6.Changes of Level of Plasma Angiotensin Ⅱ and Cardiac Function after Captopril Treatment in Children with Acute Viral Myocarditis

Objective To explore the changes of plasma angiotensinⅡ (AngⅡ) and cardiac function,and the curative effect of children with acute viral myocarditis (VMC) treated with captopril(CAP).Methods Concentrations of plasma AngⅡ were measured with radio-immunity and cardiac function was detected by Doppler echocardiography for the VMC group (n=60) before and after treatment [the CAP group (n=30), the routine group (n=30) and the control group (n=30)].Results 1. The level of plasma AngⅡ significantly increased and the contractive and diastolic function obviously declined in children with acute VMC. There was a significant difference between VMC group and control group, with a significant correlation between the level of AngⅡand the contractive diastolic function.2. Compared with the level before treatment, the level of AngⅡ decreased and the contractive function obviously ameliorated in two groups; the diastolic function obviously ameliorated in the CAP group and did not ameliorate in the routine group after treatment. In CAP group the level of AngⅡ and the cardiac function significantly improved; there were statistical differences between the two groups after treatment.Conclusions 1.The increase of the plasma AngⅡ was an important factor for decrements of the contractive and diastolic function in acute viral myocarditis.2.It could decrease the concentration of plasma AngⅡ and ameliorate cardiac function in children with acute VMC treated with captopril,which was an effective therapy for acute VMC.

In this paper, based on the special influence of supramolecular chemistry on the basic theory of Chinese medicines ( CM) , the authors further analyzed the history of meridian tropism and natural origins of CM organisms and explained CM ingredients and the universal regularity of the automatic action of the supramolecular "imprinting templates" hole channel structure. After entering human bodies, CMs, as the aggregation of supramolecular "imprinting templates" , automatically seek supramolecular subjects that are matched with their "imprinting templates" in human meridians and organs for the purpose of self-recognition, self-organization, self-assembly and self-replication, so as to generate specific efficacy in meridians and organs, which is reflected as the meridian tropism phenomena at macro level. This regularity can be studied by in vitro and in vivo experimental studies. In vitro methods are mostly supra molecular structure analysis and kinetic and thermodynamic parameter calculation; Whereas in vivo methods are dominated by the analysis on object component distribution, chromatopharmacodynamic parameters and network chromatopharmacodynamic parameters; Particularly, the acupoint-medicine method can simplify to study the supramolecular subject-object relations. Consequently, CM's'meridian tropism reveals the universal regularity for interactions of macromolecular and micromolecular "imprinting templates" of subjects and objects in natural organisms. As the first barrier for the material base of the CM theory and breakthrough in the modernization of the basic CM theory, meridian tropism plays an important role in studies on basic theories of the basic CM theory.

8.Therapeutic effect of electroacupuncture combined with medicine on acute rheumatoid arthritis at active stage.

OBJECTIVETo explore a treatment program for increasing therapeutic effect on rheumatoid arthritis at active stage.
METHODSOne hundred and forty-six cases were randomly divided into treatment group (n = 74) and medicine control group (n = 72). The treatment group were treated by electroacupuncture at Quchi (LI 11), Hegu (LI 4), Yanglingquan (GB 34), etc. , combined with meloxicam, sulfasalazine and MTX. The control group treated by simple the Western medicines. Their therapeutic effects were compared.
RESULTSThe effective rate was 79.73% in the treatment group and 51.39% in the control group with a significant difference between the two groups (P< 0. 05).
CONCLUSIONElectroacupuncture combined with medicine has a better therapeutic effect than the simple medicine on rheumatoid arthritis at active stage.

9.Clinical observation of desirable metal stents in the treatment of achalasia

Objective To observe the clinical efficacy and investigate its safety of esophageal achalasia with retrievable metal stents. Methods Thirty cases for esophageal achalasia in our hospital were selected, and they were randomly divided into stent group and balloon group. The former were treated by retrievable metal stent, and the latter were treated endoscopic balloon-dilating. The dysphagia grade, clinical symptom scores and the situation of esophageal in-spection before treatment,after 1 month and 6 months of treatment, and the common complications were compared be-tween two groups. Results (1)The rusults of two groups in dysphagia grade and clinical symptom scores after 1 month and 6 months' treatment were obviously lower than those before therapy(P<0.05).(2)Two groups patients in dysphagia grade and clinical symptom scores after 1 month's treatment had no statistical difference (P>0.05),and after six months' treatment, the stent group patients' dysphagia grade and clinical symptom scores were lower than balloon group (P<0.05). (3)After treatment, the maximum diameter of the esophagus of two groups patients were significantly lower than before treatment (P<0.05), and the narrowest diameter of the cardia were significantly higher than before treatment (P<0.05).(4)The rates of common complications such as hemorrhage, perforation, pain, stent group were lower than the balloon group(P<0.05),but reflux stent group was higher than the balloon group(P<0.05). Conclusion The treatment of esophageal achalasia with endoscopic retrievable metal stent is simple, safe and effective, compared with balloon-dilating,its long-term efficacy is more excellent,and the complications are more lighter and less.
10.Catechin remits the inflammation in allergic asthma mice by suppressing NF-κB-TSLP signal pathway

Aim To explore the mechanism of catechin in inhibiting Th2-driven inflammation responses of allergic asthma mice.Methods Female BALB/c mice were chosen to establish an allergic asthma model by sensitized and excited with OVA.Then,the administered group was intervened with catechin by series of dose (75,150,300 mg · kg-1).The proportion of peripheral leukocyte cells was analysed,and pathological changes in lung tissues were observed by HE dyeing.The levels of TSLP,IL-5,IL-13 in lung tissues and OVA specific IgE in mouse serum were measured using ELISA kit.NF-κB p65,IκB protein expression levels in lung tissues were investigated using Western blot,and NF-κB p65 nuclear translocation was detected with immunofluorescent technique.Results Catechin could not only effectively reduce the proportion of peripheral leukocyte cells,but relieve the levels of TSLP,Th2 inflammatory factor IL-5 and IL-13,and the activation of NF-κB signal pathway in lung tissues of allergic asthma mice.Conclusions Catechin can effectively relieve Th2 inflammation in allergic asthma mice induced by allergen OVA.The possible mechanism may be that it could reduce expression of TSLP by inhibiting the NF-κB signal.
