Objective To investigate the effect of chymase on the proliferation of rat cardiac fibroblasts (CFs) and the role of transforming growth factor-?1 (TGF-?_1).Methods Cultured CFs of neonatal SD rats were isolated by trypsinization.Cell number and DNA synthesis were evaluated by MTT assay (A_(490) value) and [~3H]-deoxythy- midine [~3H]-TdR incorporation.The mRNA expression of TGF-?_1 in CFs was determined by RT-PCR.Results Chymase increased CFs numbers and [~3H]-TdR incorporation in a dose-dependent manner.The A_(490) value of CFs stimulated by 15,30 and 60 ng/mL chymase was 0.263?0.033,0.348?0.031 and 0.387?0.026,respectively, which were all significantly higher than that of control (0.201?0.019,P

5.Prognostic Value of Initial Troponin Ⅰ Level in Non-cardiac Surgery Critically Ill Patients at Intensive Care Unit

Objective: To evaluate the prognostic value of initial troponin Ⅰ (TnI) level in non-cardiac surgery critically ill patients at surgical intensive care unit (ICU). Methods: We consecutively observed non-cardiac surgery critically ill patients admitted in surgical ICU of our hospital from 2015-01-01 to 2015-12-31. TnI level was measured at the early ICU admission, general clinical data, previous history, acute physiology and chronic health evaluation (APACHE) Ⅱ score and mortality were compared between the patients with elevated TnI and normal TnI; TnI level at the early ICU admission, general clinical data, previous history and APACHE Ⅱ score were compared between survival patients and deceased patients. Risk factors for ICU mortality were studied by multivariable regression analysis;predictive values for initial TnI level and APACHE Ⅱ score in mortality were assessed by receiver operating curve (ROC). Results: A total of 1 193 patients were enrolled and 159 (13.3%) of them had TnI elevation upon ICU admission. Compared with normal TnI patients, TnI elevated patients had the higher APACHE Ⅱ score [17.0 (14.0-21.0) vs 15.0 (13.0-18.0)] and ICU mortality (18.2% vs 2.6%), both P=0.000. There were 56 patients died and 1 137 survived, single and multivariable regression analysis indicated that age (OR=1.041), APACHE Ⅱ score (OR=1.218) and initial TnI level (OR=6.366) were the independent risk factors for ICU mortality, all P<0.05. AUC of ROC for predictive value of ICU mortality in APACHE Ⅱ score was 0.763, in initial TnI level was 0.778; their combination AUC was 0.803. Conclusion: Increased TnI level at the early ICU admission was an independent risk factor for ICU mortality in non-cardiac surgery critically ill patients which had predictive value for death.
6.Investigation and analysis of occult hepatitis B virus infection among voluntary blood donors in Lishui area.

OBJECTIVEOccult hepatitis B infection of voluntary blood donors has been plagued in the serum screening. Determined the OBI through the highly sensitive detection methods Nest-PCR among the blood donors, and then learned occult HBV infection and analysed the genotypes of this area.
METHODS10 080 serums of donors were determined respectively by the imported Abbott HBsAg kit and Beijing Wantai anti-HBc and anti-HBs reagents, obtained the gene and detected DNA sequences by the high sensitive Nest-PCR method.
RESULTSAmong 10 080 cases of unpaid blood donors, 108 cases were detected HBsAg positively by Abbott sensitivity kit (positive rate of 1.07%), 767 cases were anti-HBc single - positive (positive rate of 7.67%). 25 patients screened blood donors who tested negative for serum HBsAg and positive for HBV DNA in the 10 080 cases. Occult HBV infection incidence rate was 0.25%. 12 cases were HBV genotype C (48%), 13 cases were genotype B (52%), and no other genotypes. Genotype B has no statistically significant difference to genotype C (P > 0.05). Sequence analysis showed that 5 patients in the HBsAg epitope "a" (aa124 - aa147) have mutation (20%).
CONCLUSIONThe high proportion of occult hepatitis B infected among voluntary blood donors in our country. Also genotype and mutation was differences in different regions.

7.Application of protective appendicostomy after sphicter-preserving surgery for patients with low rectal carcinoma who are at high-risk of anastomotic leakage.

OBJECTIVETo explore the application of protective appendicostomy after sphicter-preserving surgery for patients with low rectal carcinoma who are at high-risk of anastomotic leakage.
METHODSClinical data of 74 low rectal cancer cases with high-risk anastomotic leakage undergoing laparoscope-assisted total mesorectal excision(TME) sphincter-preserving operation in our department from September 2013 to September 2014 were analyzed retrospectively. Patients were randomly divided into two groups: 36 patients received appendicostomy and catheter was removed 4 to 6 weeks after operation when sinus tract formation in abdominal wall was identified; 38 patients received traditional ileostomy and stoma closure was performed 3 to 6 months after operation.
RESULTSThe operation time was (149.2±9.4) min vs. (146.7±12.7) min, postoperative complication morbidity was 8.3%(3/36) vs. 13.2%(5/38), anastomotic leakage rate was 2.8%(1/38) vs. 2.6%(1/36), mean drainage volume of 1-week stoma was (203.2±76.9) ml vs. (195.8±76.5) ml, intestinal function recovery time was (25.5±5.6) h vs. (24.0±5.8) h in intubation colostomy group and ileostomy group respectively, and these differences were not significant (all P>0.05). While total hospital stay was shorter and cost was less in intubation colostomy group as compared to ileostomy group [(8.8±1.7) d vs. (18.0±1.7) d, (32 000±3000) yuan vs. (51 000±4000) yuan], and these differences were significant (all P<0.05).
CONCLUSIONFor low rectal cancer patients who are at high-risk of developing anastomotic leakage undergoing sphincter-preserving anterior resection, protective appendicostomy can decrease anastomotic leakage rate, avoid second stoma closure, shorten hospital stay and reduce hospital cost.

AIMTo study preventive and therapeutic effect of zinc sulfate on lung injury during superior mesenteric artery occlusion (SMAO) shock and their mechanism of action.
METHODSModel of rabbit SMAO shock was made. The effect of zinc sulfate on the malondialdehyde (MDA) in erythrocyte membrane and plasma, oxidase (XOD) in plasma, superoxide dismutase (SOD) in erythrocyte and MDA, SOD and pulmonary surfactant (PS) in lung tissues homogenate were observed.
RESULTSThe administration of zinc sulfate decreased MDA and XOD, prevented the reduction of SOD and PS, and alleviated lung injury.
CONCLUSIONIt is suggested that lung is injured during SMAO shock and zinc sulfate possesses preventive and therapeutic effect, through stabilized membrane.

9.Studies on the coumarins in the root of Zanthoxylum dimorphophyllum.

OBJECTIVETo study the chemical constituents of the dried roots of Zanthoxylum dimorphophyllumr. spinifolium and to find out the active constituents of the plant.
METHODModern chromatography was used to purify chemical constituents, and their structures were identified by various spectral methods.
RESULTFour compounds were isolated and identified as isopimpinellin (I), xanthoxyletin (II), 6-(2', 3'-dihydroxy-3'-methyl-butyl)-7-hydroxy-2H-1-benzopyran-2-one (III), 6-(2'-O-beta-D-glucopyranosyloxy, 3'-dihydroxy-3'-methybutyl)-7-hydroxy-2H-1-benzopyran-2-one (IV).
CONCLUSIONAll of the above compounds were isolated from the above mentioned plant for the first time.

10.Diagnostic value of ultrasound-guided core needle biopsy for soft tissue tumors.

OBJECTIVETo study the diagnostic value and pitfalls of ultrasound-guided core needle biopsy (CNB) of soft tissue tumors.
METHODSOne hundred and six cases of CNB specimens encountered during the period from 2007 to 2012 were enrolled into the study. The pathologic diagnosis using CNB was compared with that using surgical specimens. Diagnostic accuracy was analyzed using Chi-square test, with respect to the histologic pattern (such as spindle cell and myxoid), biologic behavior (benign versus malignant) and immunohistochemical results. The 59 cases of sarcoma were subdivided into three grades according to FNCLCC grading system.
RESULTSHistologic diagnosis could be made in 84.0% (89/106) cases. Thirteen cases were non-diagnostic on CNB. There were 4 cases on CNB showing diagnostic discrepancy with surgical specimens. Four cases of "benign lesions" on CNB found to be myxoid liposarcoma and lipoma-like liposarcoma upon resection. In general, myxoid pattern (9/17) seen on CNB showed less diagnostic correlation with surgical specimens, as compared to spindle cell and other histologic patterns (P < 0.01). The rate of diagnostic correlation was 79.7% (49/59) for the 59 cases of sarcoma studied, with grade 2 and grade 3 sarcoma showing better correlation (in contrast to 7/17 for grade 1 sarcoma) (P < 0.01). Comparative analysis showed no significant difference between benign/borderline tumors and sarcomas. The application of immunohistochemical study did not result in significant improvement in diagnostic accuracy on CNB.
CONCLUSIONSUltrasound-guided CNB is a reliable tool in pathologic diagnosis of soft tissue tumors and shows a high accuracy rate especially for high-grade sarcoma. Tumors with myxoid pattern, lipomatous tumors and grade 1 sarcomas are associated with lower diagnostic accuracy on CNB. Correlation with clinicoradiologic findings would also be helpful in diagnostic evaluation and surgical planning.
